    The changing role of axillary treatment in breast cancer: Who will remain at risk for developing arm morbidity in the future?

    Primary aim is to give an overview of changes in axillary staging and treatment of breast cancer patients. Secondly, we aim to identify patients with a high arm/shoulder morbidity risk, and describe a strategy to improve early detection and treatment. Recent and initiated studies on axillary staging and treatment were evaluated and clustered for clinically node negative and clinically node positive breast cancer patients, together with studies on pathology, detection and (surgical) prevention and treatment of lymphedema. For clinically node negative patients, the indication for axillary lymph node dissection in sentinel node positive patients is fading. On the contrary, clinically node positive patients are routinely subjected to an axillary lymph node dissection, in combination with other therapies associated with an increased lymphedema risk, such as mastectomy, adjuvant radiation- and (taxane-based) chemotherapy. Techniques for prevention, early detection and (surgical) treatment of lymphedema are being developed. Axillary staging and treatment in breast cancer patients with a clinically node negative status will become less invasive, thereby reducing the incidence of morbidity. Nevertheless, in patients with a clinically node positive status, aggressive treatment will still be required for oncologic control. For these patients, a surveillance program should be implemented in order to apply (curative) surgical treatment for lymphedema

    Foram avaliadas a aplicabilidade da cirurgia artroscópica na substituição do ligamento cruzado cranial (LCC) e o efeito da fisioterapia pós-operatória, por meio de exames clínicos, radiográficos e ultrassonográficos, em 16 cães sem raça definida, machos, com pesos entre 19,2 e 26,3kg, submetidos à ruptura experimental do LCC e subsequente substituição deste ligamento pelo enxerto autógeno da fascia lata. Os animais foram distribuídos em dois grupos de oito cada: no grupo I os cães foram submetidos ao programa de fisioterapia pós-operatória e no grupo II à imobilização temporária do membro. Por meio dos exames citados, não foram verificadas diferenças significativas entre os dois grupos, e ambos demonstraram sinais de processo articular degenerativo. A função do membro foi avaliada com auxílio da plataforma de força, e os animais do grupo I apresentaram melhor apoio do membro operado durante o período de reabilitação. Conclui-se que a cirurgia artroscópica é uma técnica eficaz para substituição do LCC em cães, apesar de não impedir o desenvolvimento da doença articular degenerativa, e que a fisioterapia permite recuperação mais rápida no apoio do membro durante o período de reabilitação.<br>The reconstruction of experimentally ruptured cranial cruciate ligament (CCL) under arthroscopic guidance and the effects of an early postoperative rehabilitation program by clinical, radiographic, and ultrasonographic exams were evaluated. Sixteen male mongrel dogs weighing from 19.2 to 26.3kg had the CCL experimentally ruptured and the stifle joint was stabilized by the use of an intracapsular arthroscopical technique with fascia lata as an autogenous graft. Eight dogs were included in a postoperative physiotherapy group and the other eight in a temporary immobilization group. From serial clinical, radiographic, and ultrasonographic examinations, no differences between groups were observed, although all dogs had showed signs of degenerative joint disease. Limb function was determined after surgery using force platform analysis, and the animals from physiotherapy group had significantly better results during rehabilitation period. It can be concluded that arthroscopic surgery for reconstruction of the cranial cruciate ligament is an efficient method to be used in dogs, in spite of not preventing the degenerative joint disease, and the physiotherapy has beneficial effects on early limb function during the rehabilitation period

    Avaliação do efeito da clorpromazina sobre a função renal de cães submetidos à isquemia e reperfusão

    A isquemia renal está presente em diferentes situações como em cirurgias renais, vasculares e no transplante renal. O objetivo deste trabalho foi avaliar a integridade e a função renal de cães submetidos à isquemia e reperfusão com ou sem aplicação de clorpromazina. Para tanto foram utilizados 12 cães distribuídos aleatoriamente em dois grupos de seis indivíduos: grupo A com isquemia e reperfusão sem tratamento por clorpromazina e o grupo B com isquemia e reperfusão tratados previamente com clorpromazina. De cada cão foi coletado sangue e urina antes da isquemia, no inicio da reperfusão, após 120 minutos de reperfusão e semanalmente até 28º dia pós-cirúrgico para verificar possíveis efeitos tardios da isquemia/reperfusão. Avaliações da integridade e função renal foram feitas por exame físico, concentração sérica de ureia e creatinina e determinação da GGT urinária. A avaliação da relação proteína urinária/creatinina urinária (PU/CU) e atividade da GGT urinária são exames mais sensíveis para detectar lesão tubular aguda que o exame de urina de rotina, uma vez que estas variáveis apresentaram alteração mais precocemente. Não houve ação protetora da clorpromazina conforme constatado por meio da urinálise, dosagens séricas de ureia e creatinina, excreção urinária de GGT e PU/CU.<br>Renal ischemia may occur in different situations such as vascular or renal surgery and also in renal transplantation. This study evaluates renal function in dogs submitted to ischemia and reperfusion after chlorpromazine application. Twelve adult mongrel dogs were distributed into two groups with six animals each. Group A was composed of dogs submitted to renal ischemia and reperfusion without previous administration of chlorpromazine. Group B was composed of dogs with renal ischemia and reperfusion previously treated with chlorpromazine. In order to evaluate the possible ischemia/reperfusion late effects, blood and urine samples were sampled in four different times: Before ischemia, early stages of reperfusion, 120 minutes after reperfusion, and every week until 28th day postsurgery. Renal function was evaluated by clinical examination, serum urea and creatinine levels and urinary GGT activity. PU/CU and GGT urinary activity were more sensitive in detecting acute tubular injury than routine urine examination because these variables showed earlier changes. Based on urinalysis, urea and creatinine serum levels plus urinary excretion of GGT and PU/CU, no evidences of protective action of chlorpromazine were observed

    Fifty six specimens of the hybrid "tambacu" (Piaractus mesopotamicus Holmberg, 1887 male x Colossoma macropomum Cuvier, 1818 female) were collected from fishfarm of Guariba, São Paulo, to evaluate their haematology. Fishes presented 400.0 to 3,100.0 g total weight and 20.0 to 52.0 cm total length. Haemoglobin, haematocrit, mean corpuscular haemoglobin content (MCHC) and percentage of defense blood cells including leucocytes and thrombocytes, were studied. Statistical analysis showed positive correlation (P<0.01) between haematocrit, MCHC and haemoglobin rate. Nevertheless, thrombocytes and lymphocytes showed negative correlation (P<0.01)
