    Imagining the Multilayered Cultural Landscape: A Template from the Columbia Plateau of North America

    Cultural landscapes encompass a diversity of manifestations of the interaction between humankind and the natural environment occurring in both space and time. In some instances where the human occupation of a specific area or region encompass a continuous and extended timeframe, successive cultural layers yield contrasting and disparate landscapes and heritage values. This “layering” of past cultural landscapes often leads to conflicting modern-day land, cultural resource management, and heritage value issues. A case study is presented from the Hanford Site in south-central Washington state, USA, where the natural landscape comprises prehistoric Native American, historic ethnographic, and historic period non-Indian evidence from over 10,000 years of occupation and use that clearly separate into several culturally and chronologically defined and overlapping cultural landscapes, which can be visualized as layered entities occurring on the same physical space

    This report summarizes the conclusions and recommendations resulting from 1991-92 work in the Department of Defense (DoD), Legacy Resource Management Program\u27s Education, Public Awareness, and Recreation Task Area. Following a discussion of the Task Area objectives and methods, conclusions and recommendations are presented that cover both natural and cultural resources. Several natural resources issues are examined, including authority and access, resources and activities, mandated access, Native American access, public awareness, legal liabilities, and non-DoD Federal management policies.     Research Priorities in the Secondary Prevention of Atrial Fibrillation: A National Heart, Lung, and Blood Institute Virtual Workshop Report

    There has been sustained focus on the secondary prevention of coronary heart disease and heart failure; yet, apart from stroke prevention, the evidence base for the secondary prevention of atrial fibrillation (AF) recurrence, AF progression, and AF-related complications is modest. Although there are multiple observational studies, there are few large, robust, randomized trials providing definitive effective approaches for the secondary prevention of AF. Given the increasing incidence and prevalence of AF nationally and internationally, the AF field needs transformative research and a commitment to evidenced-based secondary prevention strategies. We report on a National Heart, Lung, and Blood Institute virtual workshop directed at identifying knowledge gaps and research opportunities in the secondary prevention of AF. Once AF has been detected, lifestyle changes and novel models of care delivery may contribute to the prevention of AF recurrence, AF progression, and AF-related complications. Although benefits seen in small subgroups, cohort studies, and selected randomized trials are impressive, the widespread effectiveness of AF secondary prevention strategies remains unknown, calling for development of scalable interventions suitable for diverse populations and for identification of subpopulations who may particularly benefit from intensive management. We identified critical research questions for 6 topics relevant to the secondary prevention of AF: (1) weight loss; (2) alcohol intake, smoking cessation, and diet; (3) cardiac rehabilitation; (4) approaches to sleep disorders; (5) integrated, team-based care; and (6) nonanticoagulant pharmacotherapy.     Point-of-Care Technologies for Precision Cardiovascular Care and Clinical Research

    Point-of-care technologies (POC or POCT) are enabling innovative cardiovascular diagnostics that promise to improve patient care across diverse clinical settings. The National Heart, Lung, and Blood Institute convened a working group to discuss POCT in cardiovascular medicine. The multidisciplinary working group, which included clinicians, scientists, engineers, device manufacturers, regulatory officials, and program staff, reviewed the state of the POCT field; discussed opportunities for POCT to improve cardiovascular care, realize the promise of precision medicine, and advance the clinical research enterprise; and identified barriers facing translation and integration of POCT with existing clinical systems. A POCT development roadmap emerged to guide multidisciplinary teams of biomarker scientists, technologists, health care providers, and clinical trialists as they: 1) formulate needs assessments; 2) define device design specifications; 3) develop component technologies and integrated systems; 4) perform iterative pilot testing; and 5) conduct rigorous prospective clinical testing to ensure that POCT solutions have substantial effects on cardiovascular care
