A multimedia package for patient understanding and rehabilitation of non-contact anterior cruciate ligament injuries

Non-contact anterior cruciate ligament (ACL) injury is one of the most common ligament injuries in the body. Many patients’ receive graft surgery to repair the damage, but have to undertake an extensive period of rehabilitation. However, non-compliance and lack of understanding of the injury, healing process and rehabilitation means patient’s return to activities before effective structural integrity of the graft has been reached. When clinicians educate the patient, to encourage compliance with treatment and rehabilitation, the only tools that are currently widely in use are static plastic models, line diagrams and pamphlets. As modern technology grows in use in anatomical education, we have developed a unique educational and training package for patient’s to use in gaining a better understanding of their injury and treatment plan. We have combined cadaveric dissections of the knee (and captured with high resolution digital images) with reconstructed 3D modules from the Visible Human dataset, computer generated animations, and images to produce a multimedia package, which can be used to educate the patient in their knee anatomy, the injury, the healing process and their rehabilitation, and how this links into key stages of improving graft integrity. It is hoped that this will improve patient compliance with their rehabilitation programme, and better long-term prognosis in returning to normal or near-normal activities. Feedback from healthcare professionals about this package has been positive and encouraging for its long-term use

Defining the Costs of an Outbreak of Karnal Bunt of Wheat

In determining the economic impact of a possible outbreak of the quarantinable wheat disease Karnal Bunt, an examination was made of the detailed components of the costs involved. The costs were classified as: (a) Direct costs (yield and quality losses); (b) Reaction costs (export bans, quality down-grading, seed industry costs); and (c) Control costs (quarantine zones, fungicides, spore destruction). Issues in the Design of a Pilot Concept-Based Query Interface for the Neuroinformatics Information Framework

This paper describes a pilot query interface that has been constructed to help us explore a "concept-based" approach for searching the Neuroscience Information Framework (NIF). The query interface is concept-based in the sense that the search terms submitted through the interface are selected from a standardized vocabulary of terms (concepts) that are structured in the form of an ontology. The NIF contains three primary resources: the NIF Resource Registry, the NIF Document Archive, and the NIF Database Mediator. These NIF resources are very different in their nature and therefore pose challenges when designing a single interface from which searches can be automatically launched against all three resources simultaneously. The paper first discusses briefly several background issues involving the use of standardized biomedical vocabularies in biomedical information retrieval, and then presents a detailed example that illustrates how the pilot concept-based query interface operates. Topological Signals of Singularities in Ricci Flow

We implement methods from computational homology to obtain a topological signal of singularity formation in a selection of geometries evolved numerically by Ricci flow. Our approach, based on persistent homology, produces precise, quantitative measures describing the behavior of an entire collection of data across a discrete sample of times. We analyze the topological signals of geometric criticality obtained numerically from the application of persistent homology to models manifesting singularities under Ricci flow. The results we obtain for these numerical models suggest that the topological signals distinguish global singularity formation (collapse to a round point) from local singularity formation (neckpinch). Finally, we discuss the interpretation and implication of these results and future applications.Comment: 24 pages, 14 figure

Use of a Community Trail Among New and Habitual Exercisers: A Preliminary Assessment

INTRODUCTION: We evaluated physical activity patterns and trail use among new and habitually active exercisers using onsite trail interviews. METHODS: Using a cross-sectional study design, 414 adults who accessed two new trails that bisect a rural community of 26,809 residents were interviewed during the first summer of the trails' official operation (2001). The trails comprise 12 miles of level and paved surface and run parallel to adjacent water sheds, businesses, and neighborhoods. Recent trail activity patterns were obtained, including the following: frequency of use, mode of activity, duration, distance traveled on trail, access points, time of day used, use of exercise companions, and distance traveled to get to trail. Perceived enablers and barriers related to trail use were also obtained. Data were compared between newly adopted exercisers (new exercisers) and individuals active prior to development of the trails (habitually active exercisers). RESULTS: Twenty-three percent of the trail users were new exercisers. New exercisers were more dependent on the trails as a primary outlet for physical activity than were habitually active exercisers (P < .001). New exercisers traveled shorter distances to access the trails and rated convenience as a primary reason for using them. Both safety and terrain issues emerged as enablers for trail use, and unsafe conditions emerged as a concern among new exercisers. CONCLUSION: A community trail may be an important vehicle for promoting physically active lifestyles. However, new exercisers must overcome issues of proximal and safe access from residential areas in addition to other safety concerns to achieve regular physical activity

It remained for the Gr&t War and conditions following in its train to bring before that tribunal almost the first really important controversies relating to freedom of press and of speech. The case of U. S. ex rel. Milwaukee Social Democratic Publishing Company, Plaintiff in Error, v. Postmaster-General Albert S. Burleson, decided March 7, 192i, is the- latest of a series of notable cases concerning this important matter. Space charge behavior in thick oil-impregnated pressboard under HVDC stresses

The space charge accumulation can affect the dielectric performance of oilimpregnated pressboard in convertor transformers. In this paper, the space charge behaviour in 1mm thick impregnated pressboards is investigated by the means of pulsed electroacoustic method. The dried pressboards are separately impregnated in fresh oil and service aged oil as comparison. The space charge distributions are investigated under ±15 kV/mm and ±25 kV/mm at room temperature. The homocharge injection can be observed for all the samples, which is much more significant in the aged oil impregnated samples. The amount of injected charges and the depth of theinjected charges accumulation in the pressboard bulk are closely related to the oil properties, amplitude and polarity of the applied voltages. The service aged oil showsthe dominant effects on the local electric field distortion, about 48% enhanced in the aged oil impregnated samples while about 15% enhanced in the fresh oil impregnated samples. Handedness and schizotypy in a Japanese sample: an association masked by cultural effects on hand usage

Previous research has shown a robust association between schizotypy and mixed/ambiguous-handedness, but little is known about the universality of this relationship outside Western cultures. The present paper examines this issue in a sample of 413 Japanese students administered (in Japan) the Annett handedness questionnaire and a schizotypy scale (STA). Conventional analyses of current hand preference, using several indices derived from the Annett scale, mostly failed to replicate previous findings. However, there was a significant tendency for greater use of either hand in highly schizotypal males. Furthermore, a significant association between schizotypy and non-right-handedness was found—again only in males—after correcting for the effects of early switching of hand usage, presumed to be due to cultural pressure against left-handedness in Japanese society. These results were found to be highly convergent with findings previously reported for clinical schizophrenia

Belimumab : a technological advance for systemic lupus erythematosus patients? Report of a systematic review and meta-analysis

Objectives: To undertake a systematic review and meta-analysis to investigate clinical effectiveness of belimumab for patients with systemic lupus erythematosus (SLE) and antinuclear and/or anti-double-stranded DNA (dsDNA) autoantibodies. Methods: We searched eight electronic databases and reference lists for randomised controlled trials (RCTs) of belimumab against placebo or best supportive care. Quality assessment and random effects meta-analysis were undertaken. Design: A meta-analysis of RCTs. Participants: 2133 SLE patients. Primary and secondary outcome measures: SLE Responder Index (SRI) at week 52. Results: Three double-blind placebo-controlled RCTs (L02, BLISS-52 BLISS-76) investigated 2133 SLE patients. BLISS-52 and BLISS-76 trials recruited patients with antinuclear and/or anti-dsDNA autoantibodies and demonstrated belimumab effectiveness for the SRI at week 52. Ethnicity and geographical location of participants varied considerably between BLISS trials. Although tests for statistical heterogeneity were negative, BLISS-52 results were systematically more favourable for all measured outcomes. Meta-analysis of pooled 52-week SRI BLISS results showed benefit for belimumab (OR 1.63, 95% CI 1.27 to 2.09). By week 76, the primary SRI outcome in BLISS-76 was not statistically significant (OR 1.31, 95% CI 0.919 to 1.855)
