    Modern science has made tremendous progress and acquired deep knowledge about Micro-organisms. But still infections caused by these, still ranks very high as a cause of death in the world. Even though there are number of Antibiotics present and new are being developed, the Micro-organisms are developing resistance towards these Antibiotics, hence there is need to find out a better medicine which can contradict all the Micro-organisms. In Ayurveda various formulations are mentioned and well documented, those are having the antimicrobial effect. This is the time to re-establish the effect of antimicrobial agents by proving it clinically or experimentally. Their effect will be evaluated by comparing with the standard drugs (Ciprofloxacin, Ampicillin,) which are in use now. The formulation taken for the study is Bhagottar Gutika. Its antimicrobial study will be done experimentally by adopting cup-plate diffusion method. The study does not require any investigation or intervention to be conducted on patients or animal

    Background: Konkan area in Maharashtra, India is rich in Biodiversity and known for its Natural Beauty. At the same time geographical difficulties, poor socio-economical status, superstitial and religious beliefs are some important factors contributing to more Psycho-social problems in this area including Physical Health. We focused in this Study to find out Psychological Problems, Health issues and planning future activities for School going adolescent girls.Methods: The Study was carried out on total no. Of 1290 School going adolescent girls of age group 11-20 years, over the period of six months. The following Psychological and Physical parameters were considered. Mental Status Examination, Higher Mental Functions, Personality, Decision Making capacity. Hemoglobin status, Body Mass Index and Health related factors of Adolescent girls.Results: According to Psychological assessment78.46% had abnormal expression, poor hygiene, low decision making. 72.47% were underweight and 47.22% were anemic.Conclusions: More Active measures need to be taken for care of Psychological and Physical health. Awareness Programme needs to implement for Students, Parents, Teachers and Public.

    We present the case of a 13-year-old boy with a congenital anterior urethral diverticulum. This is a rare condition in males which can lead to obstructive lower urinary tract symptoms and urosepsis. Diagnosis is by urethroscopy and radiological imaging. Surgical treatment can be open or endoscopic. Long-term followup is required to check for reoccurrence of the obstruction

    Effect of Vitamin D supplementation in under-5 children with pneumonia: A randomized controlled trial

    Objective: The objective of the study was to study whether Vitamin D supplementation in under-5 children presenting with pneumonia and severe pneumonia reduces its duration and recurrences. Study Design: This study was designed as a double-blind, randomized, placebo-controlled trial. Setting: Pediatric unit of a teaching institute. Methods: A total of 80 children aged between 2 months and 5 years with the diagnosis of pneumonia and severe pneumonia (as per the WHO definition) admitted over a period of 1 year were included in the study. Children with features of rickets, severe malnutrition, asthma, any underlying medical disorders, and if received Vitamin D supplementation over the past 12 months were excluded from the study. Children were randomized into two groups. Intervention group received 300,000 IU (international units) of Vitamin D (1 ml), and the control group received 1 ml of sterile water as a placebo along with antibiotics and supportive care. Children were monitored for the resolution of symptoms. The two groups were comparable for baseline demographic, socioeconomic, clinical, and laboratory parameters. All the children were followed up for 3 months after discharge for any repeat episodes of pneumonia. Results: Time to resolution of symptoms (fever, tachypnea, and chest retractions) was not significant (3.6

    Toxicity to immune checkpoint inhibitors presenting as pulmonary arterial vasculopathy and rapidly progressing right ventricular dysfunction

    Introduction: Immune Checkpoint Inhibitors (ICIs) are antitumor drugs associated with a number of serious immune-related adverse events (IRAEs). ICIs enhance anti-tumor immunity, thereby  energized  patient's immune system to fight cancer. IRAEs may affect functions of various organs, including heart, and may lead to morbidity and, to some  extent  mortality. Left ventricle (LV) myocarditis with dysfunction is a known side effect of this class of drugs. However, right ventricle (RV) myocarditis and pulmonary vasculitis are an unknown entity and has not been previously reported.  Here, we present the first case of  IRAEs causing selective RV involvement with dysfunctions, attributed to immune checkpoint inhibitors described till date in medical literature.Presentation of Case: A 58-year male presented  with history of low-grade fever and  weight loss. On palpation, he had diffuse cervical lymphadenopathy. Histopathology evaluation of  lymph node revealed  metastatic lesions of Renal Cell Carcinoma (RCC).Conclusion: Fatal cardiovascular adverse events can occur as a side effect of ICI. The combination of RV myocarditis with progressive pulmonary hypertension is fatal. Treatment with high dose corticosteroids and immunomodulators may help in patient survival. Physicians treating patients with ICIs should be aware of their lethal cardiotoxic side effects  to reduce adverse cardiac outcomes. Because the number of patients exposed to this new immune therapy is expected to increase remarkably in the near future, our study encourages further work to define guidelines for cardiovascular monitoring and management

    METHOD DEVELOPMENT AND VALIDATION OF SELECTIVE AND HIGHLY SENSITIVE METHOD FOR DETERMINATION OF APIXABAN IN HUMAN PLASMA USING LIQUID CHROMATOGRAPHY-TANDEM MASS SPECTROMETRY

    Objective: The present research work aims to develop and validate a selective and highly sensitive method for the determination of apixaban in human plasma using liquid chromatography-tandem mass spectrometry (LC-MS/MS). Methods: 200 µl of sodium heparin plasma samples were acidified and clean-up was performed by using solid-phase extraction (SPE). Apixaban 13C D3 was used as an internal standard (deuterated) to lower the relative matrix effects and a single step SPE was employed for sample clean up. 10 µl of SPE eluent was loaded onto Hypersil Beta Basic C18, 100×4.6 mm, 5 µ column for highly selective chromatographic separation using an isocratic mobile phase. 2 mmol ammonium acetate in water and acetonitrile were delivered by using a quaternary low-pressure gradient pump without premixing at a minimum flow rate of 0.50 ml/min. Results: LC-MS/MS method was successfully developed and validated to demonstrate the lowest detection limit of 0.05 ng/ml and a linear dynamic range from 1-250 ng/ml with r2&gt;0.99. Method development and validation results proved that the method is selective and highly sensitive for the determination of apixaban in human plasma using LC-MS/MS. Conclusion: Current method can be applied for both therapeutic drug monitoring (TDM) and pharmacokinetic (PK) study analysis

    NUMERICAL SIMULATION OF SHIP NAVIGATION IN ROUGH SEAS BASED ON ECMWF DATA

    Recently, several changes have been observed in the Earth’s environment. This is also applicable to the ocean environment. The concept of weather routing has been applied for ship navigation for a long time. Many service providers offer weather routing service with the availability of high-quality satellite data. Unfortunately, not much information is available in the public domain as to how much the recent change in the weather pattern has affected ship navigation. The purpose of this paper is to fill this information gap. We investigate the influence of recent changes in the ocean environment on ship navigation. Weather data from ECMWF, namely ERA-Interim, is used for this purpose. The ECMWF data for the last 27 years is analysed. We compute the statistical characteristics of this data for the first 10 years, last 10 years, and 27 years. The statistical characteristics of the data are determined based on “summer” and “winter” zones as defined by international maritime regulations. Six different worldwide commercial ship routes are selected covering all the ocean regions. Navigation on great ellipse with waypoint is considered. MMG type ship manoeuvring model for 3 different ship types (DTMB 5415, PCC, VLCC) is used. The added resistance due to wave, wind and the effort of keeping the ship on the desired course using autopilot in the rough ocean environment is included in the MMG model. The fuel consumption and the duration of each one of the voyage are computed. Based on the analysis and simulation results it is shown that: (i) The mean wave height, wave period, and wind speed has increased in some ocean zones and decreased in other ocean zones. If any change has occurred, it is uniform for both seasons (summer and winter). (ii) In which ocean regions there is a perceptible change in fuel consumption, average ship speed and voyage time due to the changes in the weather pattern. (iii) The changing weather pattern in different ocean zones affects each ship type differently
