It shares how one family, the Marklyns, established The Share Fund, a participatory giving model that is not funded with endowed funds, but by their deliberate, annual redistribution of their assets to their mission.     Is Oral Rivaroxaban Safe and Effective in the Treatment of Patients with Symptomatic DVT?

    OBJECTIVE: The objective of this selective EBM review is to determine whether or not oral rivaroxaban is safe and effective in the treatment of patients with symptomatic DVT. STUDY DESIGN: Review of three English language primary randomized controlled trial studies published from 2007-2011. DATA SOURCES: Three randomized controlled trials studying the effectiveness and safety of rivaroxaban in the treatment of patients with symptomatic DVT in comparison to enoxaparin and placebo. OUTCOMES MEASURED: Outcomes measured were episodes of recurrent DVT and bleeding. Patients were monitored for the proposed treatment period and seen at fixed intervals that were matching for the rivaroxaban and comparison groups. At this time a checklist was used to gather information on symptoms and signs of recurrent venous thromboembolism, bleeding, and adverse events.RESULTS: The articles showed rivaroxaban is safe and effective in the treatment of patients with deep venous thrombosis. The primary efficacy outcome in all was episodes of recurrent DVT. The principle safety outcome in all studies was clinically relevant major and non-major bleeding. CONCLUSIONS: This review supports the use of rivaroxaban in the treatment of patients with symptomatic deep venous thrombosis. More information is needed for the use of the drug in special populations such as cancer patients

    The Versatility of Epidemiology: Association of Chronic Diseases to Age-Related Hearing Loss and the Risk of Cancer Within a Community Exposed to Gasoline

    This dissertation demonstrates the versatility of epidemiology in public health research. The association between hearing sensitivity and diabetes, cardiovascular disease (CVD), and their risk factors was examined in a population of 2,049 adults within the Health, Aging, and Body Composition Study (mean age 77.5 ± 2.8 years; 37% black). CVD and diabetes may contribute to age-related hearing loss by affecting blood flow within the inner ear via macro- and micro-vascular changes. Clinical CVD was not associated with hearing sensitivity however; subclinical CVD measures were moderately associated with poorer auditory function in females. After controlling for age, race, and site, CVD risk factors positively associated with worse mid-frequency hearing thresholds in males were weight, insulin, glucose, triglycerides, and smoking and in females were heart rate and glucose. Risk factors associated with worse high frequency thresholds were weight, insulin, triglycerides, and smoking in males and heart rate, glucose, and smoking in females. Diabetes was associated with mid-frequency hearing loss upon adjustment for common hearing loss risk factors (OR=1.60; 95%CI: 1.26-2.02). The metabolic syndrome was associated with mid-frequency hearing loss in whites prior to excluding diabetics. These results suggest that diabetes, in conjunction with CVD, contributes to age-related hearing loss, particularly strial presbycusis, and independent of common hearing loss risk factors. Given the high prevalence of hearing impairment among older adults, the identification of potentially modifiable risk factors for age-related hearing loss is of public health significance. Epidemiology can also be utilized in more applied settings. A retrospective cohort study was conducted to determine if residents affected by an underground gasoline spill in Hazle Township/Hazleton, Pennsylvania were at increased risk for cancer from 1990-2000. A total of 663 individuals representing 275 households comprised the study population. Age-adjusted standard incidence ratios (SIRs) were calculated using Pennsylvania rates to determine expected numbers. The age-adjusted leukemia SIR for the gasoline affected area was 4.40 (95%CI: 1.09-10.24). These results suggest a possible association between chronic low-level benzene exposure and increased risk for leukemia in the residents living near the spill site. In Arabidopsis thaliana, there are nine members in the OPT gene family that are thought to be involved in peptide transport. Spacial and temporal expression of AtOPT6 correlates with transport of peptides in the major sink tissues indicating that this transporter may be involved in long distance transport of peptides to provide organic nitrogen to the developing plant organs. Over-expression of AtOPT6 leads to cadmium hyper-sensitivity and higher accumulation of cadmium and phytochelatins in root tissues. opt6 mutant plants exhibited less sensitivity to virulent pathogen Psuedomonas syringae pv tomato DC3000 and showed minimal chlorosis in leaves. The differential in bacterial growth in opt6 mutant and wild-type plants was abolished when infected with Pst DC3000 COR- strain, indicating a possible role of AtOPT6 in transport of bacterial phytotoxin coronatine. In addition, opt6 mutant plants showed less susceptibility when infected with both cyst and root-knot nematode. Expression of AtOPT6 increased during early stages of both cyst and root-knot nematode infection in and around the developing feeding sites. AtOPT6 mediated transport of various Arabidopsis CLAVATA3/ESR-like (CLE-like) and nematode secreted peptides when expressed in Xenopus. Collectively, these data suggest that AtOPT6 may transport nutrients into the feeding site or AtOPT6 transports plant CLEs or nematode secreted peptides into the infected root cells to induce root differentiation.Includes bibliographical reference

    Hypernatremia in exclusively breastfed term neonates

    Background: Hypernatremia is known to occur in exclusively breastfed neonates in the first few days of life, and its clinical presentation is usually nonspecific. Early identification and prompt treatment of this condition are necessary to prevent morbidity and mortality. Objective: The objective of the study was to evaluate the clinical features, causative factors, complications, and outcome of hypernatremia in exclusively breastfed term newborns. Materials and Methods: A prospective study of exclusively breastfed healthy term neonates with hypernatremia (serum sodium ≥150 mg/dl) was carried out from March 2016 to February 2017. The presenting symptoms, clinical signs, birth and feeding history, and laboratory investigations of the subjects were noted and analyzed. Results: Hypernatremia was noted in 35 (1.6%) of the total term neonates. The condition was more common in babies delivered by cesarean section (34.3%), firstborn neonates (74.3%), and during the summer months (74.3%). Mean age of presentation was 4.06±1.43 days. The most common findings were weight loss ˃7 % from the birth weight (74.3%), feeding problems (71.4%), and decreased frequency of urine (60%). Serum sodium levels ranged from 150.5 to 187 mEq/dl. Acute kidney injury was noted in 4 (11.4%) cases. All the patients were discharged, and there was no mortality. Conclusion: Hypernatremia is a common condition in exclusively breastfed neonates. It occurs due to inadequate intake of breast milk by the neonate and is exaggerated in hot environment. Daily weighing of the neonates and monitoring the frequency of urine are important for early detection. Addressing feeding problems early is a simple yet effective measure to prevent this condition

    Probiotic properties of exopolysaccharide producing lactic acid bacteria isolated from vegetables and traditional Indian fermented foods

    A total of 203 lactic acid bacteria (LAB) isolated from vegetables and traditional fermented food products of India such as idli batter, dhokla batter and dahi were screened for exopolysaccharide (EPS) production. Based on the amount of EPS produced, 17 LAB isolates were selected for biochemical and genetic characterization using 16S rRNA gene sequencing. The isolates were belonging to the genera Lactobacillus, Weissella and Pediococcus. In vitro examination was performed to evaluate their probiotic potential. Resistance to low pH and 0.3 % bile salts was studied. Bile salt hydrolase (BSH) activity, susceptibility to various antibiotics and antimicrobial activity against the non pathogenic E. coli K12 were also investigated. The results showed that the isolates could not grow in the presence of oxbile however some survived exposure to it for 2.5 h. Some isolates were able to grow in the presence of sodium taurocholate and showed considerable antimicrobial activity against E. coli K12. Five isolates were showing BSH activity which is reported for the genus Weissella for the first time.
