    Effectiveness of Mannheim peritonitis index in predicting the morbidity and mortality of patients with hollow viscous perforation

    Background: Peritonitis due to hollow viscous perforation is common cause of emergency surgery in India. Despite advances in surgical skills, antimicrobial agents and supportive care the morbidity and mortality of secondary peritonitis remains high. Prognosis and outcome of it depends on many factors which includes patient related factors, disease specific factors and management. Categorizing patients into different risk groups would help in management. Mannheim peritonitis index (MPI) is based on measuring simple clinical parameters in case of hollow viscous perforation. This study was conducted to know efficacy of MPI for predicting morbidity and mortality in hollow viscous perforation.Methods: 50 patients with peritonitis undergoing surgical treatment at Dr. D.Y. Patil medical college, Nerul were included in study. Demographic, clinical data, surgical treatment, outcome were documented and analysed.Results: The morbidity and mortality rate were higher in patients with MPI >29. Surgical site infections were most common complication in patients with MPI 21. More ICU stay was required in in patients with MPI <21.Conclusions: MPI which is very easy to calculate is simple and effective in predicting morbidity and mortality in patients with hollow viscous perforation

    Kikuchi Fujimoto lymphadenitis- an uncommon entity in the surgical outpatients: a case report

    Kikuchi Fujimoto lymphadenitis is a benign self-limiting disease with subacute necrotizing lymphadenopathy of unknown cause. The clinical, histopathological and immunohistochemical features point to viral etiology hence delaying diagnosis commonly. Clinicians and pathologists awareness of this disorder may prevent misdiagnosis and inappropriate treatment. We describe here a case of a young woman admitted under our care with fever and cervical lymphadenopathy. Cervical lymph node biopsy revealed the rare clinical disorder of Kikuchi Fujimoto lymphadenitis which is not thought of as a primary cause of lymphadenopathy in the Indian subcontinent where tuberculosis is widely prevalent

Its occurrence in females under the age of 20 years is not frequent. Moreover, its presence in the adolescent population has not been well documented. With the advent of high frequency ultrasonography and magnetic resonance imaging it is now possible to diagnose such cases at an early stage. Asymptomatic uterine fibroids can be left untreated as long as they are monitored closely. However, larger tumors usually require surgical treatment. Medical management by prescribing gonadotropin-releasing hormone analogue has been used to suppress estrogen production, thereby reducing the size of existing myomas to make them amenable to laparoscopic surgery. Selective embolization of uterine artery has been used successfully in the treatment of uterine fibroids. We came across a similar case of a large leiomyoma in a sexually inactive adolescent girl. She presented with a lump in her abdomen and without a history of menstrual disturbances or intake of oral contraceptives.Keywords: leiomyoma of uterus, fibroid uteru

    Encysted hydrocele of the cord presenting as a groin swelling in an adult: a case report

    The hydrocele of the cord presents as a translucent swelling in the inguinal canal or scrotum, separately above the testis. It is more commonly seen in childhood and is rare in adults. It comprises of two varieties based on their communication with the peritoneal cavity. The encysted hydrocele of the cord does not communicate with the peritoneal cavity above or the tunica vaginalis below. The other variety known as the funicular hydrocele communicates with the peritoneal cavity above but does not communicate with the tunica vaginalis below. This case report presents the case of a 22 year old young male with an inguinal swelling. Clinical differential diagnoses were of inguinal lymphadenitis, encysted hydrocele of the cord and irreducible hernia. The aim of this report is to highlight the role of ultrasonogram in early diagnosis and surgical excision as mainstay of treatment

    This review paper mainly focuses on different types of solar still and highlights the passive solar still. It considers advanced modifications in the design and development of materials, single- and multieffect solar stills with augmentation of different materials, energy-absorbing insulators, and mechanisms of heat and mass transfer to improve the loss of heat and enhance the productivity of solar stills. The cost–benefit analysis along with the progressive advancement of solar stills is the major contribution of this review. To increase the output of a solar still, the application of advanced modifications is a promising tool, and it is anticipated that more improvements will shortly be added in this area with significant modifications in the design of solar stills
