Leczenie chorego z pękniętym tętniakiem aorty brzusznej i z nieoperacyjnym rakiem jelita grubego - opis przypadku i przegląd piśmiennictwa
Jednoczasowe występowanie u pacjenta choroby nowotworowej i pękniętego tętniaka aorty brzusznej
w praktyce klinicznej występuje rzadko. Ze względu na coraz większą liczbę zachorowań na nowotwory,
coraz lepsze i bardziej wyrafinowane badania diagnostyczne, a także wzrost w populacji liczby osób powyżej
65. roku życia, lekarze coraz częściej w swojej praktyce będą spotykać się z pacjentami, u których jednoczasowo
występuje tętniak aorty brzusznej i choroba nowotworowa. Leczenie takich chorych wciąż budzi duże
kontrowersje.
W niniejszej pracy przedstawiono przypadek leczenia chorego z pękniętym tętniakiem aorty brzusznej
i nieoperacyjnym rakiem jelita grubego. Poruszono także problematykę etyczną i prawną związaną z postępowaniem
lekarza w takich przypadkach.It is very unusual to encounter in clinical practice a simultaneous neoplasm and a ruptured abdominal aortic
aneurysm in one patient. Because of the growing number of patients suffering from neoplasms, better and
more precise diagnostic procedures, and a growing population of people over 65 years old, doctors will, more
often than not, meet patients with simultaneous abdominal aortic aneurysm and neoplasm disease. The
treatment of such cases gives rise to a great deal of controversy.
In this paper, the treatment of a patient with a ruptured abdominal aortic aneurysm and with unoperative
colon carcinoma is presented together with the ethical and legal problems related to medical procedures in
such cases
Plasma levels of NT-pro-brain natriuretic peptide in patients with overt and subclinical hyperthyroidism and hypothyroidism
Background: Several studies have assessed natriuretic peptides in patients with thyroid disorders, and these studies have provided contrasting
results. This difference may be partially explained by the presence of concomitant disorders of the cardiovascular system in participants.
Material and methods: The study included 101 patients free of any cardiovascular disorder, who, on the basis of plasma levels of TSH
and thyroid hormones, were divided into patients with overt hyperthyroidism, patients with subclinical hyperthyroidism, patients with
overt hypothyroidism, patients with subclinical hypothyroidism, and control subjects with normal thyroid profile. Hyperthyroidism was
induced either by nodular thyroid disease or by Graves’ disease, while hypothyroidism was secondary to autoimmune thyroiditis or surgery.
Results: Compared to control subjects, hyperthyroid patients were characterised by higher plasma levels of NT-pro-BNP. This increase
was particularly pronounced in cases of overt disease. On the other hand, neither clinical nor subclinical hypothyroidism was associated
with any significant changes in this peptide. Plasma levels of NT-pro-BNP did not differ between patients with Graves’ disease and toxic
nodular goitre nor between patients with autoimmune hypothyroidism and hypothyroidism secondary to thyroidectomy. Only L-thyroxine
substitutions, but not hyperthyroidism treatment, caused changes in plasma concentration of NT-pro-BNP.
Conclusions: Hyperthyroidism and hypothyroidism induce changes of the plasma concentration of NT-pro-BNP. Although both exogenous
L-thyroxine and antithyroid drugs restored thyroid function, only the former drug changed plasma NT-pro-BNP content. The thyrometabolic
state of a patient should always be taken into consideration when NT-pro-BNP is assessed as a marker of cardiac dysfunction.
(Pol J Endocrinol 2011; 62 (6): 523–528)Wstęp: W dotychczas przeprowadzonych badaniach oceniano stężenia peptydów natriuretycznych u osób z chorobami tarczycy,
dostarczyły one jednak rozbieżnych wyników. Różnice te można częściowo tłumaczyć obecnością współistniejących schorzeń układu
sercowo-naczyniowego.
Materiał i metody: Badaniem objęto 101 pacjentów, u których nie stwierdzono schorzeń układu sercowo-naczyniowego. Na podstawie
wyników oznaczeń stężeń w osoczu TSH i wolnych hormonów tarczycy uczestników badania podzielono na chorych z klinicznie jawną
nadczynnością tarczycy, chorych z subkliniczną nadczynnością tarczycy, pacjentów z klinicznie jawną niedoczynnością tarczycy, pacjentów
z subkliniczną niedoczynnością tarczycy i osoby bez zaburzeń funkcji gruczołu tarczowego. U podłoża nadczynności tarczycy leżała choroba
Gravesa-Basedowa lub wole guzowate, podczas gdy niedoczynność miała charakter autoimmunologiczny lub była konsekwencją tyreoidektomii.
Wyniki: W porównaniu z grupą kontrolną u osób z nadczynnością tarczycy stwierdzono wyższe stężenia w osoczu NT-pro-BNP.
Wzrost ten był szczególnie wyrażony w przypadku klinicznie jawnej nadczynności tarczycy. W klinicznie jawnej oraz w subklinicznej
niedoczynności tarczycy nie obserwowano zmienionych stężeń tego peptydu. Stężenie NT-pro-BNP w osoczu nie różniło się pomiędzy
osobami z chorobą Gravesa-Basedowa i chorymi z wolem guzkowym nadczynnym, jak również pomiędzy osobami z niedoczynnością
tarczycy o podłożu autoimmunologicznym i pozabiegowym. Obserwowano, iż jedynie leczenie niedoczynności tarczycy wiązało się ze
zmianami stężeń badanego białka we krwi.
Wnioski: Nadczynność i niedoczynność tarczycy w różny sposób wpływają na stężenie NT-pro-BNP w osoczu krwi. Mimo iż zarówno
egzogenna L-tyroksyna, jak i leczenie tyreostatykiem przywracają prawidłową funkcję tarczycy, jedynie L-tyroksyna wpływa na stężenie
NT-pro-BNP. Stan tyreometaboliczny chorego powinien być zawsze uwzględniany przy ocenie stężenia NT-pro-BNP jako markera dysfunkcji

Peptydy natriuretyczne: ich znaczenie w diagnostyce i terapii
Cardiac natriuretic peptide hormones, atrial natriuretic peptide (ANP) and B-type natriuretic peptide (BNP), are synthesized
and secreted by the heart, producing several biological effects, such as natriuresis, vasorelaxation and hypotension.
During the last decade these peptides, especially BNP, have received increasing attention as potential markers of cardiovascular
disease. Their measurements can be used to diagnose heart failure, including diastolic dysfunction, and using them
has been shown to save money. BNP levels can enable the differentation between dyspnoic patients secondary to ventricular
dysfunction and subjects with primary respiratory disorders. Moreover, there is good evidence that natriuretic peptides
may have a diagnostic role in arterial hypertension, acute coronary syndromes, pulmonary hypertension, some valvular
heart disease and some disorders affecting other systems (diabetes or thyroid disorders). In this paper we disscuss the
clinical utility of assessment of natriuretic peptide hormones in the diagnosis of various clinical conditions and their use as
pharmacological agents.

(BNP, brain natriuretic peptide) są wytwarzane i wydzielanie przez serce i wykazują ważne efekty biologiczne,
takie jak: działanie natriuretyczne, diuretyczne oraz hipotensyjne. W ostatnich latach peptydy te, szczególnie BNP, wzbudzają
coraz większe zainteresowanie jako potencjalne markery schorzeń układu sercowo-naczyniowego. Pomiary ich stężeń
mogą ułatwić rozpoznanie niewydolności serca, w tym niewydolności rozkurczowej, przy umiarkowanym nakładzie
kosztów. Oznaczenie BNP umożliwia różnicowanie między dusznością spowodowaną niewydolnością serca i dusznością
z przyczyn pierwotnie płucnych. Ponadto istnieją dowody, że zbadanie stężeń peptydów natriuretycznych może mieć
znaczenie w diagnostyce: nadciśnienia tętniczego, ostrych zespołów wieńcowych, nadciśnienia płucnego, niektórych wad
zastawkowych serca oraz schorzeń innych układów (cukrzyca, choroby tarczycy). Celem niniejszej pracy było podsumowanie
wiedzy na temat przydatności oceny stężeń peptydów natriuretycznych w różnych jednostkach klinicznych i ich
możliwego zastosowania w terapii
Thyroid hormones and the cardiomyocytes
W niniejszej pracy przedstawiono aktualny stan wiedzy na
temat wewnątrzkomórkowych mechanizmów działania
hormonów tarczycy w kardiomiocytach. Wiele klinicznych
objawów chorób tarczycy zależy od zmiany hemodynamiki
układu sercowo-naczyniowego pod wpływem działania
hormonów tarczycy. Trójjodotyronina zmienia stan hemodynamiczny
głównie przez swój wpływ na ekspresję genów
komórek mięśnia sercowego. Geny te kodują w sercu
zarówno białka strukturalne, jak i regulatorowe (łańcuchy
ciężkie miozyny, aktywowaną wapniem ATP-azę retikulum
sarkoplazmatycznego, fosfolamban). Upośledzona kurczliwość
serca w niedoczynności tarczycy przypomina zmiany
towarzyszące niewydolności serca, co wskazuje, że zastosowanie hormonów tarczycy w niewydolności krążenia
może poprawiać czynność serca.The authors present the current knowledge on the intracellular
mechanisms of thyroid hormone action in the cardiomyocytes.
Many of the clinical manifestations of thyroid
diseases are due to the ability of thyroid hormone to alter
cardiovascular hemodynamics. Triiodothyronine affects
the hemodynamic state mainly by its influence on the
expression of cardiomyocyte genes. These genes encode
both structural and regulatory proteins in the heart (myosin
heavy chains, sarcoplasmic reticulum calcium-activated
ATP-ase, phospholamban). The impaired myocardium
contractile activity in hypothyreosis reminds findings in
heart failure and may warrant further exploration of therapeutic approaches using thyroid hormone to improve
cardiac function in heart failure
Insulinoma - diagnostyka i leczenie
Insulinomas are the most common functioning endocrine tumors of pancreas. Approximately 10% are multiple, less than
10% can be malignant and 5-10% associated with the MEN-1 syndrome. Insulinomas are the most common cause of hypoglycemia
resulting from endogenous hyperinsulinism. The aim of this lecture is to present the up-to-date information
concerning the prevalence, diagnosis and treatment of insulinoma.Insulinoma to najczęściej rozpoznawany hormonalnie czynny guz trzustki. Około 10% z nich występuje w postaci mnogiej,

Accuracy Analysis of a Wireless Indoor Positioning System Using Geodetic Methods
Ubisense RTLS is one of the Indoor positioning systems using an Ultra Wide Band. AOA and TDOA methods are used as a principle of positioning. The accuracy of positioning depends primarily on the accuracy of determined angles and distance differences. The paper presents the results of accuracy research which includes a theoretical accuracy prediction and a practical test. Theoretical accuracy was calculated for two variants of system components geometry, assuming the parameters declared by the system manufacturer. Total station measurements were taken as a reference during the practical test. The results of the analysis are presented in a graphical form. A sample implementation (MagMaster) developed by Globema is presented in the final part of the paper
Trimming flow, plasticity, and mechanical properties by cubic silsesquioxane chemistry
Abstract In this work, the possibility of managing the rheological and mechanical parameters of composites based on PLA with the use of cubic structures of organofunctional spherosilicates was verified. To accurately observe the effect of various organosilicon modifier substitutions on changes in composites’ properties, we synthesized and used monofunctional octasubstituted derivatives as reference systems. The OSS/PLA systems were tested with concentrations of 0.1–2.5% (w/w) using extrusion to obtain a filament with a diameter of 1.75 mm. The printed samples underwent comprehensive tests including microscopic (SEM–EDS, optical microscope), rheological, thermal (TG, DSC, HDT), mechanical (impact and strength) as well as water contact angle tests. The work is interdisciplinary in nature and combines elements of organosilicon synthesis, materials engineering, and materials processing and characterization technology
Novel Multifunctional Spherosilicate-Based Coupling Agents for Improved Bond Strength and Quality in Restorative Dentistry
The aim of this study was to investigate the restorative connections of composite materials after fracture, under controlled conditions of treating the materials with novel, spherosilicate-based (SS) primers bearing both methacryl (MA) and trimethoxysilyl (TMOS) groups. The chemistry of methacrylate group insertion and reactive groups hydrolysis has been studied with the aid of 1H NMR (Nuclear Magnetic Resonance) spectroscopy. The light-cured resin composites were repaired by activating the connection site with the obtained primers and, for comparison, a silane (methacryloxypropyltrimethoxysilane, MATMOS) as a conventional coupling agent bearing the same reactive groups. The resistance of such a joint was tested in a three-point bending test after 24 h and 28 days period of sample conditioning. The effect of bond application was also studied, showing that spherosilicate-based primers may be used more effectively than MATMOS for two-step (primer-composite) restorative process, while for silane, the three-step process with bond application is crucial for satisfactory joint quality. The joint failure mode was determined by microscopic analysis and it was found that SS-4MA-4TMOS and SS-2MA-6TMOS application resulted in mostly composite, and not joint, failure. After 28 days of conditioning, the flexural strength of the joint repaired with SS-4MA-4TMOS was at 94% of the neat, solid material under the same procedure. However, the strength of the neat composite was observed to decline during the conditioning process by ~30%. The joint behavior was explained on the basis of the gradual hydrolysis effect (the greatest decrease being observed for silane)
Novel Silsesquioxane-Derived Boronate Esters—Synthesis and Thermal Properties
The functionalization of mono- and octahydrospherosilicate with vinylboranes and allylboranes via hydrosilylation reaction in the presence of a Karstedt’s platinum (0) catalyst is presented. This is the catalytic route to obtain a new class of silsesquioxanes containing boron atoms in their structure in high yields (>90%) and with satisfactory selectivity. The obtained compounds were fully characterized by spectroscopic (1H, 13C, 29Si NMR) and spectrometric methods (MALDI-TOF-MS), as well as thermal analysis (TGA). The obtained compounds were subjected to thermal tests, characterizing the processes of melting, thermal evaporation, sublimation and thermal decomposition
Natural and Synthetic Polymer Fillers for Applications in 3D Printing—FDM Technology Area
This publication summarises the current state of knowledge and technology on the possibilities and limitations of using mineral and synthetic fillers in the field of 3D printing of thermoplastics. FDM technology can be perceived as a miniaturised variation of conventional extrusion processing (a microextrusion process). However, scaling the process down has an undoubtful drawback of significantly reducing the extrudate diameter (often by a factor of ≈20–30). Therefore, the results produced under conventional extrusion processing cannot be simply translated to processes run with the application of FDM technology. With that in mind, discussing the latest findings in composite materials preparation and application in FDM 3D printing was necessary
