    Household food insecurity and childhood overweight in Jamaica and Québec: a gender-based analysis

    Background - Childhood overweight is not restricted to developed countries: a number of lower- and middle-income countries are struggling with the double burden of underweight and overweight. Another public health problem that concerns both developing and, to a lesser extent, developed countries is food insecurity. This study presents a comparative gender-based analysis of the association between household food insecurity and overweight among 10-to-11-year-old children living in the Canadian province of Québec and in the country of Jamaica. // Methods - Analyses were performed using data from the 2008 round of the Québec Longitudinal Study of Child Development and the Jamaica Youth Risk and Resiliency Behaviour Survey of 2007. Cross-sectional data were obtained from 1190 10-year old children in Québec and 1674 10-11-year-old children in Jamaica. Body mass index was derived using anthropometric measurements and overweight was defined using Cole's age- and sex-specific criteria. Questionnaires were used to collect data on food insecurity. The associations were examined using chi-square tests and multivariate regression models were used to estimate odds ratios (OR) and 95% confidence intervals. // Results - The prevalence of overweight was 26% and 11% (p < 0.001) in the Québec and Jamaican samples, respectively. In Québec, the adjusted odds ratio for being overweight was 3.03 (95% CI: 1.8-5.0) among children living in food-insecure households, in comparison to children living in food-secure households. Furthermore, girls who lived in food-insecure households had odds of 4.99 (95% CI: 2.4-10.5) for being overweight in comparison to girls who lived in food-secure households; no such differences were observed among boys. In Jamaica, children who lived in food-insecure households had significantly lower odds (OR 0.65, 95% CI: 0.4-0.9) for being overweight in comparison to children living in food-secure households. No gender differences were observed in the relationship between food-insecurity and overweight/obesity among Jamaican children. // Conclusions - Public health interventions which aim to stem the epidemic of overweight/obesity should consider gender differences and other family factors associated with overweight/obesity in both developed and developing countries

    Bulkamid (PAHG) in mixed urinary incontinence: What is the outcome?

    INTRODUCTION AND HYPOTHESIS Mixed urinary incontinence (MUI), defined as mixed symptoms of stress urinary incontinence (SUI) and overactive bladder (OAB), is a difficult entity if conservative treatment has failed. Cure rates are low compared with SUI, particularly the OAB component, may deteriorate after sling insertion. Bulking agents pose an appealing alternative for the treatment of MUI. They have shown beneficial effect in small case studies, but larger series are lacking. The aim of this prospective study was an analysis of treatment efficacy and safety profile of the bulking agent, Bulkamid, in female patients with MUI. METHODS One hundred fifty-four women with MUI symptoms (components of SUI/OAB within the limits of 60-40% either way) received bulking therapy with polyacrylamide hydrogel (Bulkamid). Patients were followed-up 3 months postoperatively. Primary outcome was the domain Incontinence impact on the King's Health Questionnaire (KHQ). Secondary outcomes were the other KHQ domains, visual analog scale (VAS), and International Continence Society (ICS) standardized pad weight test as objective measurement of incontinence. RESULTS Statistically significant improvements were found for all KHQ domains, pad weight test, and the visual analog scale (VAS) before and after bulking. Overall complication rate was 13%. CONCLUSIONS This study has shown improvement in MUI after bulking therapy according to both subjective and objective outcomes. We can advocate bulking therapy for treating MUI, as it is simple and safe and shows both objective and subjective improvement and relief. Long-term results (up to 1 year) are awaited

    A modified QuickDASH-9 provides a valid outcome instrument for upper limb function

    Background. The 30-item Disabilities Arm Shoulder and Hand (DASH) questionnaire was introduced to facilitate assessment of upper limb functional limitations. To improve practicality and eliminate item redundancy a modified instrument was needed. The 11-item QuickDASH was developed to fulfil these requirements and translated into several languages. However, prospective investigations of psychometric and practical characteristics are limited. No published study investigated readability or used concurrent validation with a standardized upper limb criterion measure. The validity of the QuickDASH has been questioned as the results for factor structure are conflicting, and the English-language version has not yet had factor structure reported. A shortened 9-item version, the QuickDASH-9, that addresses these issues is proposed. Methods. This two-stage observational study assessed the psychometric and practical characteristics of the QuickDASH and the extracted QuickDASH-9. The Upper Limb Functional Index (ULFI) was the criterion standard in both stages. Stage 1, calibration, reanalyzed extracted QuickDASH and QuickDASH-9 responses from a previous prospective study, by the authors, of the 30-item DASH (n = 137). Stage 2, prospective validation, investigated the QuickDASH through repeated measures in consecutive upper limb musculoskeletal participants' consulting for physical therapy in Australia (n = 67). The QuickDASH and extracted QuickDASH-9 data from both stages was analyzed and compared for psychometric properties, practical characteristics and factor structure. Results. The proposed QuickDASH-9 had a unidimensional structure, high reliability (ICC 2:1, r = 0.92), internal consistency (alpha = 0.93) and responsiveness (ES = 1.05). It correlated highly with both the DASH (r = 0.97), QuickDASH (r = 0.99) and ULFI criterion (r = 0.85). QuickDASH-9 missing responses reduced to 3.5% from 26% in the QuickDASH. Completion and scoring time was 134 ± 56 seconds and required a computational aid. The QuickDASH demonstrated a bidimensional structure making it invalid. The QuickDASH-9 summary performance was measured on the 'Measurement of Outcome Measures' at 88% and on the 'Bot' clinimetric scale at 75%. Conclusions. The proposed QuickDASH-9 had a unidimensional structure and similar psychometric precision to the full-length DASH with improved practicality and completion time. The QuickDASH was invalid as its bidimensional structure made a single summated score inappropriate. The QuickDASH-9 offers a future direction for ongoing use of the QuickDASH concept. © 2009 Gabel et al; licensee BioMed Central Ltd

    Introduction to aeroacoustics and self-sustained oscillations of internal flows

    \u3cp\u3eAfter a review of basic equations of fluid dynamics, the Aeroacoustic analogy of Lighthill is derived. This analogy describes the sound field generated by a complex flow from the point of view of a listener immerged in a uniform stagnant fluid. The concept of monopole, dipole and quadrupole are introduced. The scaling of the sound power generated by a subsonic free jet is explained, providing an example of the use of the integral formulation of the analogy. The influence of the Doppler Effect on the radiation of sound by a moving source is explained. By considering the noise generated by a free jet in a bubbly liquid, we illustrate the importance of the choice of the aeroacoustic variable in an aeroacoustic analogy. This provides some insight into the usefulness of alternative formulations, such as the Vortex Sound Theory. The energy corrolary of Howe based on the Vortex Sound Theory appears to be the most suitable theory to understand various aspects of self-sustained oscillation due to the coupling of vortex shedding with acoustic standing waves in a resonator. This approach is used to analyse the convective energy losses at an open pipe termination, human whistling, flow instabilities in diffusers, pulsations in pipe systems with deep closed side branches and the whistling of corrugated pipes.\u3c/p\u3

    A Review and Empirical Evaluation of Feature Weighting Methods for a Class of Lazy Learning Algorithms

    Many lazy learning algorithms are derivatives of the k-nearest neighbor (k-NN) classifier, which uses a distance function to generate predictions from stored instances. Several studies have shown that k-NN&apos;s performance is highly sensitive to the definition of its distance function. Many k-NN variants have been proposed to reduce this sensitivity by parameterizing the distance function with feature weights. However, these variants have not been categorized nor empirically compared. This paper reviews a class of weight-setting methods for lazy learning algorithms. We introduce a framework for distinguishing these methods and empirically compare them. We observed four trends from our experiments and conducted further studies to highlight them. Our results suggest that methods which use performance feedback to assign weight settings demonstrated three advantages over other methods: they require less pre-processing, perform better in the presence of interacting features, and generally require less training data to learn good settings. We also found that continuous weighting methods tend to outperform feature selection algorithms for tasks where some features are useful but less important than others
