БЛИЖАЙШИЕ И СРЕДНЕСРОЧНЫЕ РЕЗУЛЬТАТЫ ЭНДОПРОТЕЗИРОВАНИЯ КОЛЕННОГО СУСТАВА ИМПЛАНТАТАМИ ZIMMER NexGen CR И LPS
The article presents the experience gained by orthopedic unit of Saratov Regional Clinical Hospital at knee joint replacement with modern implants. We analyzed use of Zimmer NexGen CR and LPC implants for initial total knee joint replacement in 285 patients. The monitoring continued for 5 years. Good and excellent results were observed in 92% of cases. Pyogenic complications were observed in 2.3% of cases. The article describes the methods of surgical intervention and post-surgical rehabilitation. We consider our experience with Zimmer NexGen LPS and CR knee-joint implants as positive and prospective for further use.Представлен опыт ортопедического отделения областной клинической больницы г. Саратова по эндопротезированию коленного сустава с использованием современных имплантатов. Проведен анализ применения эндопротезов Zimmer NexGen CR и LPC при первичном тотальном эндопротезировании коленного сустава у 285 пациентов. Средняя продолжительность наблюдения составила 5 лет. Хорошие и отличные результаты получены у 92% больных, осложнения гнойного характера – у 2,3%. Описана методика операции и послеоперационной реабилитации

СРЕДНЕСРОЧНЫЕ РЕЗУЛЬТАТЫ ПЕРВИЧНОГО ЭНДОПРОТЕЗИРОВАНИЯ ТАЗОБЕДРЕННОГО СУСТАВА ИМПЛАНТАТАМИ ФИРМЫ «ZIMMER» ЦЕМЕНТНОЙ И БЕСЦЕМЕНТНОЙ ФИКСАЦИИ
We observed 503 patients with various pathologies of hip joint. 555 interventions with total hip joint replacement were conducted. 52 patients were operated on both sides. Total number of male patients was 219; the number of female patients was 284. Age of patients varied from 18 to 82. 24 patients were under 25 y.o.; 117 patients were 26–40 y.o.; 232 patients were 41-60 y.o.; 130 patients were older than 60. Cement-free restoration of implants took place in 196 cases. Trilogy + VerSys ET combination was used in 156 cases; Trilogy + Alloclassic combination was applied in 40 cases. Hybrid restoration (ZCA + VerSys ET) was used on 297 patients; completely cement restoration (ZCA + CPT) was applied in 62 cases.Под нашим наблюдением находилось 503 больных с различной патологией тазобедренного сустава, которым выполнено 555 операций тотального эндопротезирования тазобедренного сустава. 52 пациента оперированы с двух сторон. Мужчин было 219, женщин – 284. Возраст больных – от 18 до 82 лет, из них моложе 25 лет – 24; от 26 до 40 лет – 117; от 41 года до 60 лет – 232; старше 60 лет – 130 человек. Бесцементная фиксация имплантатов иcпользована в 196 случаях, из них комбинация Trilogy + VerSys ET – в 156, Trilogy + Alloclassic – в 40. Гибридная фиксация (ZCA + VerSys ET) использовалась у 297 пациентов; полностью цементная (ZCA + CPT) – у 62. Первичное тотальное эндопротезирование тазобедренного сустава эндопротезами Zimmer позволяет в короткие сроки получить положительный и долгосрочный эффект при тяжелых заболеваниях и травмах тазобедренного сустава
Solving or resolving global tomographic models with spherical wavelets, and the scale and sparsity of seismic heterogeneity
International audienceWe propose a class of spherical wavelet bases for the analysis of geophysical models and for the tomographic inversion of global seismic data. Its multiresolution character allows for modelling with an effective spatial resolution that varies with position within the Earth. Our procedure is numerically efficient and can be implemented with parallel computing. We discuss two possible types of discrete wavelet transforms in the angular dimension of the cubed sphere. We describe benefits and drawbacks of these constructions and apply them to analyse the information in two published seismic wave speed models of the mantle, using the statistics of wavelet coefficients across scales. The localization and sparsity properties of wavelet bases allow finding a sparse solution to inverse problems by iterative minimization of a combination of the ℓ2 norm of the data residuals and the ℓ1 norm of the model wavelet coefficients. By validation with realistic synthetic experiments we illustrate the likely gains from our new approach in future inversions of finite-frequency seismic dat
