A 120-years history of the Dermatovenerology Chair of SibirianStateMedicalUniversityis presented. The contribution of the scientists into the theory and practice in dermatovenerology and traning of specialists is described. ОПТИЧЕСКИЙ СПЕКТР ОТРАЖЕНИЯ КОЖИ ПРИ ВИТИЛИГО

Vitiligo is a disease with unknown etiology, which is characterized by skin depigmentation areas due to melanocyte death. Lack of clear understanding of pathogenic disease processes justifies the difficulty of choosing efficient treatment methods. Repigmentation is slow and not always complete. One of the key indicators in practical dermatology is objective assessment of skin cover condition for further diagnostics, definition of therapy and dynamics of the disease.The purpose of this study is to assess skin reflection spectrum in depigmentation areas, marginal layer and visible healthy skin before and after treatment of patients with vitiligo.21 patients having vitiligo were under observation; the patients were treated in the hospital of skin and venereal diseases of the Siberian State Medical University. The authors applied noninvasive method of spectrophotometry for the first time in order to study skin reflection spectrum of patients with vitiligo. Noninvasive medical spectrophotometry is based on dependence factor of integral optical properties of biological tissues and fluids in the conditions of homeostasis and in case of pathology. In order to assess the efficiency of skin disease treatment the patients were exposed to skin spectrophotometry before the beginning of the treatment and after the treatment itself with further measurements of the reflection spectrum integral criteria. The measurements were made in depigmentation area, on the boundary with healthy skin and in remote from depigmentation area of healthy skin.Obtained results indicate the difference of skin reflection spectra in the areas under investigation. Moreover, positive dynamics of the treatment associated with the decrease in the value of reflection spectrum integral criteria in the diseased area. There were no statistically important differences of the integral criteria, calculated based on measurement results of reflection spectra on the boundary of depigmentation area before and after treatment. Most likely this is related to active processes of repigmentation in the skin in the course of the treatment.For detailed study of identified phenomena there is a need for thorough study and expansion of the list of dermatologic diseases.Витилиго является заболеванием с неизвестной этиологией, характеризующимся очагами депигментации кожи в связи с гибелью меланоцитов. Отсутствие четких представлений о патогенетических механизмах заболевания объясняет затруднения в подборе эффективных методов лечения. Репигментация протекает медленно и не всегда бывает полной. Одним из важных показателей в практической дерматологии является объективная оценка состояния кожных покровов для диагностики, определение эффективности проводимой терапии и динамики заболевания.Целью настоящего исследования была оценка спектра отражения кожи в очагах депигментации, краевой зоне и видимо здоровой кожи до и после лечения у пациентов с витилиго.Под наблюдением находился 21 пациент с витилиго, получавший лечение по данному заболеванию в клинике кожных и венерических болезней Сибирского государственного медицинского университета (г. Томск). Авторами впервые был применен неинвазивный метод спектрофотометрии с целью изучения спектра отражения кожи у больных витилиго. Неинвазивная медицинская спектрофотометрия основывается на факте зависимости интегральных оптических свойств биологических тканей и жидкостей в условиях гомеостаза и при патологии. Для оценки эффективности лечения кожного процесса пациентам перед началом курса терапии и после окончания лечения проводилась спектрофотомерия кожи с последующим вычислением интегрального критерия спектра отражения. Измерения проводили в очаге депигментации, на границе со здоровой кожей и в отдаленном от очага депигментации здоровом участке кожи.Полученные результаты свидетельствуют о различии спектров отражения кожи в исследуемых зонах. Кроме того, положительная динамика лечения ассоциировалась с уменьшением значения интегрального критерия спектра отражения в очаге поражения. Статистически значимые различия интегрального критерия, рассчитанного по результатам измерения спектров отражения на границе очага депигментации до и после лечения, а также на границе витилигинозного очага и здоровой кожи после лечения отсутствовали. Вероятно, это связано с активно протекающими процессами репигментации в коже на фоне проводимого лечения.Для детального изучения обнаруженных закономерностей необходимо проведение развернутого исследования и расширения перечня дерматологических заболеваний

The comparative evaluation of modern imaging methods gives evidence of great opportunities of both MRT and US in the detection of structural alterations of joints at psoriatic arthritis. The key role is assigned to ultrasonography which has proved to be highly sensitive in the disclosure of peripheral joint synovitis and allows the follow-up during the treatment course.Сравнительная оценка современных методов визуализации свидетельствует о больших возможностях как магнитнорезонансной томографии, так и ультразвукового исследования в обнаружении структурных изменений суставов при псориатическом артрите. Особая роль отводится ультразвуковому исследованию, которое обладает высокой чувствительностью в выявлении синовитов в периферических суставах, позволяет оценивать динамику заболевания на фоне лечения
