    A review of the surgical conversion rate and independent management of spinal extended scope practitioners in a secondary care setting

    INTRODUCTION Spinal orthopaedic triage aims to reduce unnecessary referrals to surgical consultants, thereby reducing waiting times to be seen by a surgeon and to surgical intervention. This paper presents an evaluation of a spinal orthopaedic triage service in the third largest spinal unit in the UK. METHODS A retrospective service evaluation spanning 2012 to 2014 was undertaken by members of the extended scope practitioner (ESP) team to evaluate the ESPs’ ability to manage patient care independently and triage surgical referrals appropriately. Data collected included rates of independent management, referral rates for surgical consideration and conversion to surgery. Patient satisfaction rates were evaluated retrospectively from questionnaires given to 5% of discharged patients. RESULTS A total of 2,651 patients were seen. The vast majority (92%) of all referrals seen by ESPs were managed independently. Only 8% required either a discussion with a surgeon to confirm management or for surgical review. Of the latter, 81% were considered to be suitable surgical referrals. A 99% satisfaction rate was reported by discharged patients. CONCLUSIONS ESP services in a specialist spinal service are effective in managing spinal conditions conservatively and identifying surgical candidates appropriately. Further research is needed to confirm ESPs’ diagnostic accuracy, patient outcomes and cost effectiveness

    The Infrared Array Camera (IRAC) for the Spitzer Space Telescope

    The Infrared Array Camera (IRAC) is one of three focal plane instruments in the Spitzer Space Telescope. IRAC is a four-channel camera that obtains simultaneous broad-band images at 3.6, 4.5, 5.8, and 8.0 microns. Two nearly adjacent 5.2x5.2 arcmin fields of view in the focal plane are viewed by the four channels in pairs (3.6 and 5.8 microns; 4.5 and 8 microns). All four detector arrays in the camera are 256x256 pixels in size, with the two shorter wavelength channels using InSb and the two longer wavelength channels using Si:As IBC detectors. IRAC is a powerful survey instrument because of its high sensitivity, large field of view, and four-color imaging. This paper summarizes the in-flight scientific, technical, and operational performance of IRAC.Comment: 7 pages, 3 figures. Accepted for publication in the ApJS.     Characterizing the Magnitude of and Risk Factors for Functional Limb Lengthening in Patients Undergoing Primary Total Knee Arthroplasty

    Background: There is little data on the magnitude and factors for functional leg lengthening after primary total knee arthroplasty (TKA). Questions/Purpose: We sought to determine the incidence of and risk factors for functional leg lengthening after primary TKA. Methods: We retrospectively reviewed consecutive unilateral primary TKAs at a single institution from 2015 to 2018. Of the 782 TKAs included, 430 (55%) were performed in women; the mean age was 66 years, and the mean body mass index was 29 kg/m2. Preoperatively, 541 (69%) knees were varus deformities and 223 (29%) were valgus deformities. Hip to ankle biplanar radiographs were obtained preoperatively and 6 weeks postoperatively for all patients. Two independent researchers measured leg length, coronal plane deformity, lateral knee flexion angle, and overall mechanical alignment on all preoperative and postoperative radiographs. Results: The mean overall ipsilateral functional leg lengthening was 7.0 mm. Seven hundred knees (90%) were overall functionally lengthened, including 462 (59%) knees lengthened \u3e5 mm and 250 (31%) knees lengthened 10 mm or more. A valgus deformity and coronal plane deformity of 10° or more were significant risk factors for increased functional lengthening. Patients with severe valgus deformities (\u3e10°) had the largest amount of functional lengthening, at a mean of 13.5 mm. Conclusion: After primary TKA, 90% of limbs are functionally lengthened, including roughly one-third over a centimeter. Valgus knee deformities and severe deformities (\u3e10°) were significant risk factors for increased limb lengthening

    Postoperative outcomes of total knee arthroplasty compared to unicompartmental knee arthroplasty: A matched comparison

    Background: The purpose of this study was to evaluate early postoperative outcomes in patients following UKA (unicompartmental knee arthroplasty) compared to a matched cohort of TKA (total knee arthroplasty) patients. Methods: Patients who met radiographic criteria for a medial UKA who underwent either a TKA or UKA at a single institution were matched based on age, gender, and BMI. Results: One hundredy and fifty UKA in 138 patients and 150 TKA in 148 patients were included in this retrospective analysis. Mean age was 62.6 ± 9 years and 65.2 ± 9 years in the UKA and TKA groups respectively (p = .01). Patients who underwent UKA had significantly less pain at two and six weeks postoperatively compared to TKA patients with mean Numeric Pain Rating Scale (NPRS) scores of 3.7 ± 1.1 vs. 7.8 ± 1.2, p < .001 and 2.6 ± 1.3 vs. 4.6 ± 1.6, p < .001 respectively. Knee Society Scores (KSS) were higher in the UKA group at six weeks and two years postoperative (86.5 ± 2.8 vs. 81.4 ± 3.6, p < .001 and 89.5 ± 2.4 vs. 84.5 ± 3.3, p < .001 respectively). Return to work was faster in the UKA group (mean 20.6 ± 7.89 vs. 38.6 ± 6.23 days, p < .001). The UKA group also had higher mean Forgotten Joint Scores of 90.5 ± 3.6 vs. 79.5 ± 9.5 (p < .001). Conclusions: Patients with primarily medial compartment OA who underwent UKA had less postoperative pain, earlier return to work, and higher KSS compared to a matched group who underwent TKA
