    Pharmacological correction of formation of connective tissue with mesh hernia repair in experiment

    The use of foreign materials in modern herniology, on the one hand, reduces the number of recurrences, and on the other - increases the risk of several complications: profuse exudation and possible infection in the postoperative wound, extensive cicatricial deformation of a zone of operations, persistent pain syndrome and possible reduced fertility in men. In the experiment 30 rats were studied influence of drugs «mesogel®» and «iodopiron» in the course of wound process during operations using mesh explant. Histological methods were evaluated the usefulness of the formation of connective tissue, degree of involvement in cicatricial process is located in the area of operation of tissues and structures. Compared with the control group of animals is set to a relatively low density and fewer collagen fibers.     HORMONEMEDIATED REGULATION OF CYTOKINES IN PATIENTS WITH ADENOMYOSIS AND UNSUCCESSFUL IVF ATTEMPTS

    Aim. The study was to evaluate the effect of cytokine dysfunctions on the endometrial implantation potential in patients with adenomyosis and unsuccessful IVF attempts and the possibility of hormone-mediated regulation of the cytokine balance at the pre-gravitational stage. Materials and methods. The study included 90 patients with infertility, an unsuccessful attempt of IVF in the history and diffuse adenomyosis of 1 or 2 degrees. Patients were randomized into two groups: the 1st group consisted of 44 patients (group 1) − no pre-education training was conducted. The second group − 46 patients − pregravid preparation of dienogest (2 mg daily for 3 months before the IVF cycle). Control group (group 3) − 30 healthy fertile women. To assess the implantation potential of the endometrium, initially, and after pre-gravity training, the concentration of LIF, IL1в, IL-6, IL-10, RAIL in cervical mucus was determined.Results. Analysis of the indices in patients with adenomyosis showed a decrease in their implantation potential of the endometrium. It has been revealed that hormone-modulating therapy with dienogest partially neutralizes cytokine regulation defects and leads to a certain increase in the effectiveness of IVF programs.Conclusion. Shows expediency of including dienogest in the pregravide preparation plan for patients with adenomyosis, but for full immunocorrection in pregravidual training in women with adenomyosis 1 and 2 The degree of additional directional immunotropic therapy is probably necessary

    SALIVARY THE LEVEL OF PROINFLAMMATORY CYTOKINES IN CHILDREN WITH AUTOIMMUNE DIABETES MELLITUS IN DIFFERENT PHASES OF COMPENSATION ENDOCRINOPATHY

    Aim. To assess the dynamics of the level of proinflammatory cytokines and their receptors in mixed saliva in patients with autoimmune diabetes mellitus during the period of the occlusal occlusion at various stages of disease compensation and to determine the possibility of applying these values in the early diagnosis and control of the effectiveness of endocrinopathy.Materials and methods. The material of laboratory-diagnostic and clinical studies was the results of examination of 93 children during the period of the change of occlusion, which were divided into two groups. The comparison group consisted of children without endocrine pathology. The main group consisted of children diagnosed with type 1 diabetes mellitus, which were divided into two subgroups depending on the degree of endocrinopathy compensation. The level of pro-inflammatory cytokines and their receptors in the unstimulated oral liquid was carried out by the solid-phase enzyme-linked immunosorbent assay using the «Vector-Best»» and «Cytokine» reagent kits.Results. With the compensated form of autoimmune diabetes mellitus, there is an overregulation of regulatory mechanisms with an imbalance in the level of soluble receptors initiating the realization of the proinflammatory properties of these cytokines. The decompensated form of autoimmune diabetes is caused by an absolute increase in virtually all pro-inflammatory cytokines in the oral fluid, with an even more pronounced imbalance in their soluble receptors.Conclusion. In parallel with the therapeutic measures concerning the underlying disease in children with autoimmune diabetes mellitus the expediency of conducting a comprehensive dental examination with subsequent medical observation of doctors of a stomatology profile. In connection with a direct correlation be-tween the degree of activity of caries process and increase the severity of endocrinopathy, treatment and preventive measures in children with autoimmune diabetes should be regularly monitored and conducted with utmost care

    The effectiveness of scientific development and research (NDR) in the agricultural sector depends on many factors, among which those that determine the safety of the process itself play a special role. The work examines the requirements for ensuring safe and harmless working conditions during scientific research and development, indicating the main legislative and other normative legal acts in the relevant areas. Depending on the features of the NDR (fundamental, applied, experimental) and the stages of their implementation, the venue, conditions, duration of work, equipment and engineering networks, personnel, etc. are determined, which necessitates the use of certain methods, methods and means of ensuring labor safety based current regulatory legal acts on labor protection, sanitation and occupational hygiene, fire safety, man-made safety and civil protection. In this work, we examine the conditions for conducting the NDR and analyze the current regulatory documents and scientific works on ensuring the safety of research, with the aim of preventing the number of accidents in the agricultural sector. An important role in ensuring healthy and safe working conditions, preservation of life and health of employees in scientific institutions is played by the labor protection service. The main tasks of the labor protection service include: implementation of scientific developments and rational proposals that increase labor safety; organize all types of briefings and preventive measures aimed at eliminating harmful and dangerous production factors and other cases of threat to the life or health of employees; carry out inspections of employees' compliance with regulatory legal acts on labor protection; draw up, with the participation of the direct supervisor, lists of professions, positions and types of work, for which instructions on labor protection (safety) should be developed, provide assistance during their development; to inform employees about the basic requirements of laws, other regulatory and legal acts and acts on labor protection that are in effect within the scope of scientific research.Ефективність наукових розробок та досліджень (НДР) в АПК залежить від багатьох чинників, серед яких особливу роль відіграють ті, що зумовлюють безпечність самого процесу. В роботі розглянуті вимоги щодо гарантування безпечних і нешкідливих умов праці під час проведення наукових досліджень та розробок із зазначенням основних законодавчих та інших нормативно-правових актів за відповідними напрямами. Залежно від особливостей НДР (фундаментальних, прикладних, експериментальних) та етапів їх виконання визначаються місце проведення, умови, тривалість виконання робіт, устаткування та інженерні мережі, персонал та ін., що зумовлює необхідність застосування певних методів, способів та засобів гарантування безпеки праці на основі чинних нормативно-правових актів з питань охорони праці, санітарії та гігієни праці, пожежної безпеки, техногенної безпеки та цивільного захисту. В даній роботі ми досліджуємо умови проведення НДР і аналізу чинних нормативних документів та наукових праць щодо надання безпечності досліджень з метою запобігання кількості нещасних випадків в АПК. Важливу роль щодо забезпечення здорових і безпечних умов праці, збереження життя та здоров’я працівників в наукових закладах відіграє служба охорони праці. Основні завдання служби охорони праці включають в себе: впровадження наукових розробок і раціональних пропозицій що підвищують безпеку праці; організація проведення всіх видів інструктажів та профілактичних заходів, спрямованих на усунення шкідливих і небезпечних виробничих факторів та інших випадків загрози життю або здоров’ю працівників; проведення перевірки дотримання працівниками нормативно-правових актів з охорони праці; складання за участю безпосереднього керівника робіт, перелік професій, посад і видів робіт, щодо яких повинні бути розроблені інструкції з охорони (безпеки) праці, надання допомогу під час їх розроблення; інформування працівників про основні вимоги законів, інші нормативно-правові акти та акти з охорони праці, що діють у межах наукових досліджень

    The values of the coagulation, overall and fibrinolysis potentials were estimated by the method of the global potential of Blomback M., as well as the values of concentrations of molecular markers of the hemostasis system: soluble fibrin (sf), D-dimer, fibrinogen (Fg) and protein C (88 patients, 52 of them men, 36 women). It was shown that hemostasis system activity in women plasma is higher than that in men plasma. The division of patients into 3 groups, depending on the concentration of sf: less than normal – sf ≤ 3, about norm – 3 4 μg/ml, allowed establishing the growth of the parameters of both the hemostatic potential and concentrations of molecular markers in accordance with concentration of sf in the groups of patients. Paerson’s correlation analysis of the relationship between the parameters of the hemostasis potential and concentrations of molecular markers revealed an increase in the correlation relationship to the strong and very strong between the parameters of coagulation, fibrinolysis and protein C systems with an increase in the concentration of soluble fibrin in plasma of patients

    Evaluation of regulatory-adaptive status of pregnant women with type ii diabetes at 38-40 weeks of gestation

    Observations were performed on 20 healthy pregnant women and 84 women with diabetes on 38-40 weeks of gestation. All pregnant conducted obstetric examination, determined the glycemic profile, urinalysis, glucose, ketone bodies, acetone. The regulatory-adaptive status was evaluated on a sample of cardio-respiratory synchronism. Registration of uterine activity, along with the movements of the fetus and the fetal heart rate was monitored by external cardiotocography. To determine the functional state of the fetus were used ultrasound. It was found that among 34 pregnant women 9 of them had incorrect diagnosis compensated diabetes, which had been disproved by values of regulatory-adaptive status indicated decompensated diabetes. Later the diagnosis had been proved. Thus, the question of delivery of pregnant women with diabetes should not only be on the level of blood glucose, glycated hemoglobin, fetal biophysical profile data, but also on the regulatory-adaptive status which is determined by the parameters of the sample cardiorespiratory synchronism

    The purpose of this work is to identify the relationship between concentration of progesterone in blood and stress healthy women. Material and methods. 52 healthy women aged 18 - 45 years were observed during the luteal phases of the menstrual cycle, undergoing examination in clinic “Yekaterininskaya”, in which ones progesterone was determined by immunochemiluminescent analysis of venous blood. Regulatory-adaptive status of the subjects was assessed by parameters of the sample cardiorespiratory synchronism before and after the stressor tests “Arithmetic account”. The level of stress resistance all observed the women were divided into three groups. In the high level of stress resistance - in the blood progesterone 7.0 - 21.0 ng / mL, with a moderate - 4.0 - 6.8 ng / ml and low - 1.7 ng - 3,8 / mL Conclusion. These data indicate that between kontsenentratsiey progesterone in the blood of healthy women and stress there is a connection: the more normative importance of progesterone in the blood, the higher the level of stress
