    Implications of the spatial variability of infiltration-water chemistry for the investigation of a karst aquifer: a field study at Milandre test site, Swiss Jura

    The Milandre test site is an ideal karstic aquifer for studying the spatial heterogeneity of groundwater chemistry. Numerous observation points can be sampled: the spring, the underground river and its tributaries, and boreholes at different depths. The main causes of the spatial variability of the chemical parameters are: nature and localisation of the input, the structure of the infiltration zone, chemical reactions (transit time vs. reaction kinetics) and mixing of different waters. Physico-chemical data on springs discharging from the karstic system represent the sum of this spatial heterogeneity. Therefore, it is difficult to interpret the global-chemical response with a simple mixing model of the aquifer subsystems (runoff, matrix reservoir, epikarst). Chemical constituents related to agricultural inputs show important seasonal variations (coefficient of variation approximately 15%) and parameters linked to rainfall (δ18O) and to the aquifer (Ca2+, HCO3 −) present variations of less than 5%.     A purely geometric distance to the binary star Atlas, a member of the Pleiades

    We present radial velocity and new interferometric measurements of the double star Atlas, which permit, with the addition of published interferometric data, to precisely derive the orbital parameters of the binary system and the masses of the components. The derived semi-major axis, compared with its measured angular size, allows to determine a distance to Atlas of 132+-4 pc in a purely geometrical way. Under the assumption that the location of Atlas is representative of the average distance of the cluster, we confirm the distance value generally obtained through main sequence fitting, in contradiction with the early Hipparcos result (118.3+-3.5 pc).Comment: 5 pages, 3 figures, accepted for publication in A&A Letter

    From Hipparcos to Gaia

    The measurement of the positions, distances, motions and luminosities of stars represents the foundations of modern astronomical knowledge. Launched at the end of the eighties, the ESA Hipparcos satellite was the first space mission dedicated to such measurements. Hipparcos improved position accuracies by a factor of 100 compared to typical ground-based results and provided astrometric and photometric multi-epoch observations of 118,000 stars over the entire sky. The impact of Hipparcos on astrophysics has been extremely valuable and diverse. Building on this important European success, the ESA Gaia cornerstone mission promises an even more impressive advance. Compared to Hipparcos, it will bring a gain of a factor 50 to 100 in position accuracy and of a factor of 10,000 in star number, collecting photometric, spectrophotometric and spectroscopic data for one billion celestial objects. During its 5-year flight, Gaia will measure objects repeatedly, up to a few hundred times, providing an unprecedented database to study the variability of all types of celestial objects. Gaia will bring outstanding contributions, directly or indirectly, to most fields of research in astrophysics, such as the study of our Galaxy and of its stellar constituents, the search for planets outside the solar system.Comment: 6 pages.     The Spectrum projection package: improvements in estimating mortality, ART needs, PMTCT impact and uncertainty bounds

    BACKGROUND: The approach to national and global estimates of HIV/AIDS used by UNAIDS starts with estimates of adult HIV prevalence prepared from surveillance data using either the Estimation and Projection Package (EPP) or the Workbook. Time trends of prevalence are transferred to Spectrum to estimate the consequences of the HIV/AIDS epidemic, including the number of people living with HIV, new infections, AIDS deaths, AIDS orphans, treatment needs and the impact of treatment on survival. METHODS: The UNAIDS Reference Group on Estimates, Modelling and Projections regularly reviews new data and information needs and recommends updates to the methodology and assumptions used in Spectrum. The latest update to Spectrum was used in the 2007 round of global estimates. RESULTS: Several new features have been added to Spectrum in the past two years. The structure of the population was reorganised to track populations by HIV status and treatment status. Mortality estimates were improved by the adoption of new approaches to estimating non-AIDS mortality by single age, and the use of new information on survival with HIV in non-treated cohorts and on the survival of patients on antiretroviral treatment (ART). A more detailed treatment of mother-to-child transmission of HIV now provides more prophylaxis and infant feeding options. New procedures were implemented to estimate the uncertainty around each of the key outputs. CONCLUSIONS: The latest update to the Spectrum program is intended to incorporate the latest research findings and provide new outputs needed by national and international planners

    OBJECTIVE The incidence of surgical site infections may be underreported if the data are not routinely validated for accuracy. Our goal was to investigate the communicated SSI rate from a large network of Swiss hospitals compared with the results from on-site surveillance quality audits. DESIGN Retrospective cohort study. PATIENTS In total, 81,957 knee and hip prosthetic arthroplasties from 125 hospitals and 33,315 colorectal surgeries from 110 hospitals were included in the study. METHODS Hospitals had at least 2 external audits to assess the surveillance quality. The 50-point standardized score per audit summarizes quantitative and qualitative information from both structured interviews and a random selection of patient records. We calculated the mean National Healthcare Safety Network (NHSN) risk index adjusted infection rates in both surgery groups. RESULTS The median NHSN adjusted infection rate per hospital was 1.0% (interquartile range [IQR], 0.6%-1.5%) with median audit score of 37 (IQR, 33-42) for knee and hip arthroplasty, and 12.7% (IQR, 9.0%-16.6%), with median audit score 38 (IQR, 35-42) for colorectal surgeries. We observed a wide range of SSI rates and surveillance quality, with discernible clustering for public and private hospitals, and both lower infection rates and audit scores for private hospitals. Infection rates increased with audit scores for knee and hip arthroplasty (P value for the slope = .002), and this was also the case for planned (P = .002), and unplanned (P = .02) colorectal surgeries. CONCLUSIONS Surveillance systems without routine evaluation of validity may underestimate the true incidence of SSIs. Audit quality should be taken into account when interpreting SSI rates, perhaps by adjusting infection rates for those hospitals with lower audit scores

    QTc interval and resting heart rate as long-term predictors of mortality in type 1 and type 2 diabetes mellitus: a 23-year follow-up

    Aims/hypothesis: We evaluated the association of QT interval corrected for heart rate (QTc) and resting heart rate (rHR) with mortality (all-causes, cardiovascular, cardiac, and ischaemic heart disease) in subjects with type 1 and type 2 diabetes. Methods: We followed 523 diabetic patients (221 with type 1 diabetes, 302 with type 2 diabetes) who were recruited between 1974 and 1977 in Switzerland for the WHO Multinational Study of Vascular Disease in Diabetes. Duration of follow-up was 22.6 ± 0.6years. Causes of death were obtained from death certificates, hospital records, post-mortem reports, and additional information given by treating physicians. Results: In subjects with type 1 diabetes QTc, but not rHR, was associated with an increased risk of: (1) all-cause mortality (hazard ratio [HR] 1.10 per 10ms increase in QTc, 95% CI 1.02-1.20, p = 0.011); (2) mortality due to cardiovascular (HR 1.15, 1.02-1.31, p = 0.024); and (3) mortality due to cardiac disease (HR 1.19, 1.03-1.36, p = 0.016). Findings for subjects with type 2 diabetes were different: rHR, but not QTc was associated with mortality due to: (1) all causes (HR 1.31 per 10 beats per min, 95% CI 1.15-1.50, p < 0.001); (2) cardiovascular disease (HR 1.43, 1.18-1.73, p < 0.001); (3) cardiac disease (HR 1.45, 1.19-1.76, p < 0.001); and (4) ischaemic heart disease (HR 1.52, 1.21-1.90, p < 0.001). Effect modification of QTc by type 1 and rHR by type 2 diabetes was statistically significant (p < 0.05 for all terms of interaction).     The shell structure underlying shape changes in neutron-rich nuclei near N=28 has been investigated by a novel application of the transient field technique to measure the first-excited state g factors in 38S and 40S produced as fast radioactive beams. There is a fine balance between proton and neutron contributions to the magnetic moments in both nuclei.     Fragmentation reactions with intermediate-energy heavy-ion beams exhibit a wide range of reaction mechanisms, ranging from direct reactions to statistical processes. We examine this transition by measuring the relative population of excited states in several sd-shell nuclei produced by fragmentation with the number of removed nucleons ranging from two to sixteen. The two-nucleon removal is consistent with a non-dissipative process whereas the removal of more than five nucleons appears to be mainly statistical.Comment: 5 pages, 6 figure

    Shell structure underlying the evolution of quadrupole collectivity in S-38 and S-40 probed by transient-field g-factor measurements on fast radioactive beams

    The shell structure underlying shape changes in neutron-rich nuclei between N=20 and N=28 has been investigated by a novel application of the transient field technique to measure the first-excited state g factors in S-38 and S-40 produced as fast radioactive beams. Details of the new methodology are presented. In both S-38 and S-40 there is a fine balance between the proton and neutron contributions to the magnetic moments. Shell model calculations which describe the level schemes and quadrupole properties of these nuclei also give a satisfactory explanation of the g factors. In S-38 the g factor is extremely sensitive to the occupation of the neutron p3/2 orbit above the N=28 shell gap as occupation of this orbit strongly affects the proton configuration. The g factor of deformed S-40 does not resemble that of a conventional collective nucleus because spin contributions are more important than usual.Comment: 10 pages, 36 figures, accepted for publication in Physical Review
