    Stochastic Models on Time to Recruitment in a Two Grade Manpower System using Different Policies of Recruitment

    In this paper a two grade organization in which depletion of manpower occurs due to its policy decisions is considered. Two mathematical models are constructed employing two different univariate recruitment policies, based on shock model approach. The mean and variance of the time to recruitment are obtained for both the models under different conditions. The analytical results are numerically illustrated and relevant conclusions are presented

    Study on outcome of N-acetylcysteine in rodenticide poisoning cases in a rural tertiary care teaching hospital

    Background: Rodenticide is one of the pesticides and heterogeneous substances used to kill rats. Rodenticide poisoning is the major health problem in Asian countries, particularly mode of self-poisoning, prevalent in India. It causes hepatotoxicity and no antidote has been found. N-acetylcysteine (NAC) is a powerful antioxidant and used in the treatment of acetaminophen-induced hepatotoxicity.  The aim of the study is to find the outcome of use of NAC in rodenticide poisoning.  The main objectives of this study were to determine the role of NAC in liver impairment rodenticide poisoning patients.   Methods: A record based observational study was conducted for the period of 3 months from August 2019 to October 2019 at Rajah Muthiah Medical College and Hospital, Annamalai University, Annamalai Nagar, Tamil Nadu. Data were collected according to the proforma designed and results were analyzed using standard methods.Results: Out of 50 patients, 21 (42%) were males and 29 (58%) were females. Age group between 21-30 years contributes the highest 20 (40%). Majority of type of rodenticide compound consumed by patients were paste 29 (58%) and less than four hours 34 (68%) patients were admitted. Patients with abnormal liver functions were tested, serum glutamic oxaloacetic transaminase/ aspartate aminotransferase -12 patients, 7 treated with NAC. serum glutamic pyruvic transaminase/ alanine aminotransferase - 9 patients, 2 treated with NAC. 37 (74%) were recovered out of 50 (100%), among them 28 (56%) treated with NAC.Conclusions: In liver impairment rodenticide poisoning patients, early use of NAC shows a significant result and the rate of recovery is also high

    ATTITUDE TOWARDS GENERIC FORMULATIONS USAGE: NARROWING THE GAP BETWEEN PHARMACIST AND PHYSICIANS

    Objective: The thought of providing the best quality of medicines at cheaper costs made the governments to think of generic drug substitutions in order to minimize the economic burden. This study was designed to investigate the attitude of physicians and pharmacists towards generic medicines and thus to reduce the gap between them.Methods: This is a simple, prospective, cross-sectional, comparative study conducted for a period of 3 mo. 100 Subjects (50 pharmacists and 50 physicians) were included in this study. Subjects who ever graduated with the pharmacy degree and working as pharmacists, as well as clinical practitioners, were included in this study. Data collected using a validated questionnaire.Results: 60% of physicians and 80% of pharmacists were confident enough in dispensing generic products. 56% of physicians agree that pharmacists play a vital role in providing assistance on the use of generic medicines. 70% of physicians and 90% of pharmacists agreed that the generic medicines are of less expensive.Conclusion: Our Study concludes that pharmacist's shows a higher positive response towards the usage of generic products than physicians. Generic drugs typically cost 30% to 60% less than their brand products. In addition, patients taking generic drugs seem to be more willing to continue therapy

    We introduce a large class of mappings called proximal Górnicki mappings in metric spaces, which includes Górnicki mappings, enriched Kannan mappings, enriched Chatterjea mappings, and enriched mappings. We prove the existence of the best proximity points in metric spaces and partial metric spaces. Moreover, we utilize appropriate examples to illustrate our results, and we verify the convergence behavior. As an application of our result, we prove the existence and uniqueness of a solution for the variational inequality problems. The obtained results generalize the existing results in the literature

    Pilot scale microbial production and optimization of Serratia peptidase from Serratia marcescens

    Serratia peptidase is active proteolytic enzyme which has the potential of cleaving peptide bond.  Present investigation deals about the Microbial production of serratia peptidase using Serratia marcescens in small scale fermentor. Batch fermentor has been run continuously throughout the night to analyze the production of protein as well as kinetics. Culture broth was maintained at 150rpm for 72 hrs. Protein sample was isolated by centrifuging at 3000rpm for 10mints. The result revealed that Serratia marcescens showed the enormous production of protein in fed batch fermentor compared to the small scale level.  Different substrates were been used for the production of enzyme. Among all cysteine showed the better activity as 2 units/ml of enzyme. Enzymatic assay of Serratia peptidase was done at different time interval of crude broth. Enzyme activity showed that maximum at 40ºC for 72hrs. It was observed that 0.65 units/ml of enzyme. Fed batch pilot scale production of Serratia peptidase was done at 0.5%cystein and 700rpm for 48hrs of run time.Â

    Functional outcome of platelet rich plasma on lateral epicondylitis of elbow

    Background: The study was conducted with the aim to explore the efficacy of platelet rich plasma in sixty patients who presented to us with lateral epicondylitis of elbow where conservative management has failed.Methods: Between 2013 and 2015, 60 patients (M: F- 20: 40) with lateral epicondylitis of elbow were included. All patients presented to us with lateral epicondylitis of elbow where conservative management has failed. All patients are treated with platelet rich plasma injection. Serial follow-up were done at 1, 6 and 12 month.Results: All sixty patients had significant improvement before and after platelet rich plasma injection. The mean VAS score and the Mayo score suggests significant improvement in pain and elbow function following platelet rich plasma treatment. Conclusions: Platelet rich plasma improves pain and elbow function in patients suffering from lateral epicondylitis where conservative management has failed. Platelet rich plasma treatment may decrease the overall time for healing, and thereby decreasing the overall need for surgical intervention

    A recurrent case of pyruvate dehydrogenase complex deficiency

    Pyruvate dehydrogenase complex deficiency is an inherited inborn error of metabolism causing lactic acidosis and several neurological symptoms. Its incidence and prevalence are not known. Here we report about a child with global developmental delay, central hypotonia and dyskinesia. Sanger sequencing was done and found to have homozygous nonsense mutation in exon 4 of PDHX gene causing lactic acidosis. In the next pregnancy selective Sanger variant analysis was carried out and the fetus was also found to be affected with the same genetic defect. Hence medical termination of Pregnancy was carried out. We conclude that early selective genetic testing will prevent further affected births

    Prostate cancer immunotherapy: Improving clinical outcomes with a multi-pronged approach

    Cancer immunotherapy has gained traction in recent years owing to remarkable tumor clearance in some patients. Despite the notable success of immune checkpoint blockade (ICB) in multiple malignancies, engagement of the immune system for targeted prostate cancer (PCa) therapy is still in its infancy. Multiple factors contribute to limited response, including the heterogeneity of PCa, the cold tumor microenvironment, and a low number of neoantigens. Significant effort is being invested in improving immune-based PCa therapies. This review is a summary of the status of immunotherapy in treating PCa, with a discussion of multiple immune modalities, including vaccines, adoptively transferred T cells, and bispecific T cell engagers, some of which are undergoing clinical trials. In addition, this review also focuses on emerging mechanism-based small-molecule tyrosine kinase inhibitors with immune modulatory properties that, either as single agents or in combination with other immunotherapies, have the potential to improve clinical outcomes
