    Identification of vertebral deformities in the Polish population by morphometric X-ray absorptiometry - results of the EPOLOS study

    Introduction: The aim of the study was the determination of the prevalence of asymptomatic vertebral deformities in healthy persons of the Polish population, based on morphometric X-ray absorptiometry (MXA), and comparison of the results with data from literature, obtained by other techniques. Material and methods: The study involved 829 persons, including 520 women and 309 men, aged 18-79 years, untreated for osteoporosis before. The Th4 to L4 vertebrae were examined. Lateral scans of the thoracic-lumbar spine were made by an Expert-XL densitometer. Sixpoint digitization was used to calculate the anterior (Ha), central (Hc), and posterior (Hp) height of the Th4-L4 vertebral bodies. The vertebrae were defined as having prevalent deformities when at least one ratio value (Ha/Hp, Hc/Hp, Hp/Hp up, or Hp/Hp low) fell 3 SDs below or even more than the reference mean of that ratio at any vertebral level. Results: The analysis was performed on 9629 vertebrae, of which 167 (1.75%), evaluated as deformed and considered as fractures, were observed in 113 patients (13.63 % of the examined patients). In 81 persons (74% of the patients with fractures; 9.7% of the studied population), single fractures were demonstrated, while in 28 persons, multiple deformities prevailed. Fractures occurred in 108 women (20.7% of the examined women) and 42 men (13.5% of the examined men). The highest incidence of deformities was observed in women over 55 years of age. First-degree deformities dominated. Deformities of the Th8 and Th6 vertebrae were most frequently observed. Conclusions: 1. Using MXA, it was found that in the Polish population deformities of vertebrae are common, as was demonstrated in X-ray morphometric studies in the European Vertebral Observation Study (EVOS). 2. Densitometric morphometry, as a non-invasive technique, may become a useful tool in the diagnostics of vertebral fractures.Wstęp: Celem badania było określenie częstości występowania bezobjawowych zniekształceń trzonów kręgowych u zdrowych osób populacji polskiej przy wykorzystaniu morfometrii densytometrycznej (MXA) i porównanie wyników z danymi z piśmiennictwa uzyskanymi innymi technikami. Materiał i metody: Badaniu poddano 829 osób, w tym 520 kobiet i 309 mężczyzn w wieku 18-79 lat, dotąd nieleczonych z powodu osteoporozy. Uwidoczniono kręgi od Th4 do L4. Boczne skany kręgosłupa piersiowo-lędźwiowego wykonano aparatem Expert-XL. Oznaczono sześć punktów w celu wyznaczenia przedniej (Ha), środkowej (Hc) i tylnej (Hp) wysokości trzonów kręgowych Th4-L4. Kręg uznawano za zniekształcony, jeśli którakolwiek z wartości Ha/Hp, Hc/Hp lub Hp/Hp kręgu następnego lub Hp/Hp kręgu poprzedniego była niższa niż 3 odchylenia standardowego od średniej wartości referencyjnej. Wyniki: Analizie poddano 9629 kręgów. Jako zniekształcone, uznane za złamane oceniono 167 kręgów, występowały one u 113 pacjentów (13,63% badanych osób). U 81 osób (74% osób ze złamaniami; 9,7% populacji badanej) wykazano pojedyncze złamanie, a u 28 osób występowały mnogie deformacje. Złamania występowały u 108 kobiet (20,7%) i 42 mężczyzn (13,5%). Dominowały zniekształcenia pierwszego stopnia. Największa częstość deformacji występowała w grupie kobiet po 55. roku życia. Dominowały zniekształcenia pierwszego stopnia. Najczęściej obserwowano deformacje kręgu Th8 i Th6. Wnioski: 1. Stosując metodę MXA, potwierdzono częste występowanie deformacji trzonów kręgowych w populacji polskiej. Dane te są zbliżone do wyników uzyskanych w badaniach morfometrii rentgenowskiej w badaniach EVOS. 2. Morfometria densytometryczna jako badanie nieinwazyjne może stać się przydatnym narzędziem w diagnostyce złamań trzonów kręgowych

    Vitamin D Toxicity–A Clinical Perspective

    Confusion, apathy, recurrent vomiting, abdominal pain, polyuria, polydipsia, and dehydration are the most often noted clinical symptoms of vitamin D toxicity (VDT; also called vitamin D intoxication or hypervitaminosis D). VDT and its clinical manifestation, severe hypercalcemia, are related to excessive long-term intake of vitamin D, malfunctions of the vitamin D metabolic pathway, or the existence of coincident disease that produces the active vitamin D metabolite locally. Although VDT is rare, the health effects can be serious if it is not promptly identified. Many forms of exogenous (iatrogenic) and endogenous VDT exist. Exogenous VDT is usually caused by the inadvertent or improper intake of extremely high doses of pharmacological preparations of vitamin D and is associated with hypercalcemia. Serum 25-hydroxyvitamin D [25(OH)D] concentrations higher than 150 ng/ml (375 nmol/l) are the hallmark of VDT due to vitamin D overdosing. Endogenous VDT may develop from excessive production of an active vitamin D metabolite – 1,25(OH)2D in granulomatous disorders and in some lymphomas or from the reduced degradation of that metabolite in idiopathic infantile hypercalcemia. Endogenous VDT may also develop from an excessive production of 25(OH)D and 1,25(OH)2D in congenital disorders, such as Williams–Beuren syndrome. Laboratory testing during routine clinical examinations may reveal asymptomatic hypercalcemia caused by the intake of vitamin D even in doses recommended for the general population and considered safe. That phenomenon, called hypersensitivity to vitamin D, reflects dysregulated vitamin D metabolism. Researchers have proposed many processes to explain VDT. Those processes include elevated activity of 1α-hydroxylase or inhibited activity of 24-hydroxylase, both leading to increased concentration of 1,25(OH)D; increased number of vitamin D receptors; and saturation of the capacity of vitamin D binding protein. Increased public awareness of vitamin D–related health benefits might increase the risk of VDT due to self-administration of vitamin D in doses higher then recommended for age and body weight or even higher than the established upper limit intake values.     Background: Plant-based diets (PBDs) are increasingly recommended for human and planetary health. However, comprehensive evidence on the health effects of PBDs in children remains incomplete, particularly in vegans. Objectives: To quantify differences in body composition, cardiovascular risk, and micronutrient status of vegetarian and vegan children relative to omnivores and to estimate prevalence of abnormal micronutrient and cholesterol status in each group. Methods: In a cross-sectional study, Polish children aged 5–10 y (63 vegetarian, 52 vegan, 72 matched omnivores) were assessed using anthropometry, deuterium dilution, DXA, and carotid ultrasound. Fasting blood samples, dietary intake, and accelerometry data were collected. Results: All results are reported relative to omnivores. Vegetarians had lower gluteofemoral adiposity but similar total fat and lean mass. Vegans had lower fat indices in all regions but similar lean mass. Both groups had lower bone mineral content (BMC). The difference for vegetarians attenuated after accounting for body size but remained in vegans (total body minus the head: –3.7%; 95% CI: –7.0, –0.4; lumbar spine: –5.6%; 95% CI: –10.6, –0.5). Vegetarians had lower total cholesterol, HDL, and serum B-12 and 25-hydroxyvitamin D [25(OH)D] without supplementation but higher glucose, VLDL, and triglycerides. Vegans were shorter and had lower total LDL (–24 mg/dL; 95% CI: –35.2, –12.9) and HDL (–12.2 mg/dL; 95% CI: –17.3, –7.1), high-sensitivity C-reactive protein, iron status, and serum B-12 (–217.6 pmol/L; 95% CI: –305.7, –129.5) and 25(OH)D without supplementation but higher homocysteine and mean corpuscular volume. Vitamin B-12 deficiency, iron-deficiency anemia, low ferritin, and low HDL were more prevalent in vegans, who also had the lowest prevalence of high LDL. Supplementation resolved low B-12 and 25(OH)D concentrations. Conclusions: Vegan diets were associated with a healthier cardiovascular risk profile but also with increased risk of nutritional deficiencies and lower BMC and height. Vegetarians showed less pronounced nutritional deficiencies but, unexpectedly, a less favorable cardiometabolic risk profile. Further research may help maximize the benefits of PBDs in children

    Daily sunshine hours as determinant of 25-hydroxyvitamin D concentration among diabetic cardiac patients who experienced myocardial infarction hospitalized due to acute coronary syndrome: a cross-sectional study

    Wstęp. Niedobór witaminy D jest problemem ogólnoświatowym o różnych konsekwencjach zdrowotnych. Witamina D może obniżać ryzyko niewydolności serca, jednak dowody świadczące o skuteczności suplementacji witaminą D na utrzymanie zdrowia układu sercowo-naczyniowego są sprzeczne z powodu braku odpowiedniej liczby i jakości badań klinicznych. Przyczyny braku jednoznacznych efektów potwierdzających pozytywny wpływ suplementacji witaminy D mogą być co najmniej trzy: 1) suplementacja zbyt małą ilością witaminy D lub 2) brak włączenia do badania tylko populacji z ciężkim niedoborem witaminy D, lub 3) czas trwania suplementacji. Celem pracy była charakterystyka grupy pacjentów kardiologicznych, u których stwierdzono w poprzednich badaniach najniższe stężenia 25-hydroksywitaminy D [25(OH)D].Materiał i metody. Analizie poddano wyniki 92 chorych kardiologicznych z cukrzycą w wieku 41–89 lat, którzy przeżyli zawał serca z istotnymi zmianami w tętnicach wieńcowych, hospitalizowanych z powodu ostrego zespołu wieńcowego, mieszkających w Warszawie.Wyniki. Mediana stężenia 25(OH)D w badanej populacji wyniosła 11 ng/ml (zakres: 4–28 ng/ml). Jedynym istotnym determinantem stężenia 25(OH)D był okres badania; stężenie było wyższe latem niż zimą.Wnioski. Leczenie witaminą D u polskich pacjentów kardiologicznych w celu osiągnięcia optymalnego stężenia, tj. 30 ng/ml (75 nmol/l), wydaje się konieczne i powinno być jak najszybciej wdrożone.Introduction. Vitamin D deficiency is a worldwide problem with a variety of health consequences. Vitamin D may reduce the risk of heart failure, however, evidence of the impact of vitamin D treatment on maintenance of cardiovascular health (i.e., preventing cardiovascular diseases) is conflicting due to lack of support from clinical trials. The reason for the failure of clinical trials to confirm an effect of vitamin D supplementation could be at least threefold: 1) too little vitamin D given to the participants or 2) lack of inclusion of only severely vitamin D-deficient populations or 3) study duration. The aim of this study was to characterize a group of cardiac patients who presented the lowest concentrations of 25-hydroxyvitamin D [25(OH)D]. Material and methods. Results of 92 diabetic cardiac patients aged between 41 and 89 years who experienced myocardial infarction, with significant coronary arteries changes, hospitalized due to acute coronary syndrome living in Warsaw were analyzed. Results. Patients presented median 25(OH)D concentration value of 11 ng/mL (range: 4–28 ng/mL). The only significant determinant of 25(OH)D concentration was the date of examination, with higher concentrations in summer than in winter. Conclusions. Vitamin D treatment in Polish cardiac patients aimed at reaching the optimal level of 30 ng/mL (75 nmol/L) seems to be necessary and implemented as soon as possible

    Wstęp: Wyniki badań z ostatnich lat dokumentują wiele korzyści wynikających z działania witaminy D na organizm człowieka na wszystkichetapach jego życia. Większość badań epidemiologicznych sugeruje, że niedobór witaminy D jest powszechny wśród mieszkańców EuropyŚrodkowej. Naturalną konsekwencją tej sytuacji jest konieczność ciągłego uświadamiania społeczeństwu oraz środowisku medycznemu,jaką rolę odgrywa witamina D w rozwoju i funkcjonowaniu organizmu ludzkiego.Metody: Na podstawie przeglądu danych literaturowych Polski Zespół Wielodyscyplinarny opracował tezy dotyczące zasad suplementacjiwitaminą D, które przesłano do członków Komitetu Naukowego konferencji „Witamina D — minimum, maksimum, optimum”,19–20 Październik, 2012, Warszawa. W trakcie powyższej konferencji z udziałem 550 delegatów oraz Ekspertów różnych dziedzin medycynyomówiono i przedyskutowano propozycje wytycznych suplementacji witaminą D populacji Europy Środkowej.Wyniki: W efekcie przeprowadzonych dyskusji Zespół Ekspertów opracował wytyczne suplementacji witaminą D dla wszystkich grupwiekowych populacji Europy Środkowej. Określono również kryteria diagnostyczne charakteryzujące stan zaopatrzenia organizmu w witaminę D: deficyt witaminy D ustalono jako stężenie 25(OH)D < 20 ng/mL (< 50 nmol/L)], suboptymalne zaopatrzenie jako stężenie25(OH)D wynoszące 20–30 ng/mL (50–75 nmol/L), a stężenie 30–50 ng/mL (75–125 nmol/L) uznano za docelowe dla zapewnienia efektuplejotropowego witaminy D.Wnioski: Poprawa obecnego stanu zaopatrzenia witaminy D w grupach dzieci, młodzieży, osób aktywnych zawodowo i seniorówpowinna zostać włączona do priorytetów polityki zdrowotnej społeczeństw Europy Środkowej.Introduction: Adequate Vitamin D intake and its concentration in serum are important for bone health and calcium–phosphate metabolismas well as for optimal function of many organs and tissues. Documented trends in lifestyle, nutritional habits and physical activityappear to be associated with moderate or severe Vitamin D deficits resulting in health problems. Most epidemiological studies suggest thatVitamin D deficiency is prevalent among Central European populations. Concern about this problem led to the organising of a conferencefocused on overcoming Vitamin D deficiency.Methods: After reviewing the epidemiological evidence and relevant literature, a Polish multidisciplinary group formulated theses onrecommendations for Vitamin D screening and supplementation in the general population. These theses were subsequently sent to ScientificCommittee members of the ‘Vitamin D — minimum, maximum, optimum’ conference for evaluation based on a ten-point scale.With 550 international attendees, the meeting ‘Vitamin D — minimum, maximum, optimum’ was held on October 19–20, 2012 in Warsaw(Poland). Most recent scientific evidence of both skeletal and non-skeletal effects of Vitamin D as well as the results of panellists’ votingwere reviewed and discussed during eight plenary sessions and two workshops.Results: Based on many polemical discussions, including post-conference networking, the key opinion leaders established ranges ofserum 25-hydroxyVitamin D concentration indicating Vitamin D deficiency [< 20 ng/mL (< 50 nmol/L)], suboptimal status [20–30 ng/mL(50–75 nmol/L)], and target concentration for optimal Vitamin D effects [30–50 ng/mL (75–125 nmol/L)]. General practical guidelines regardingsupplementation and updated recommendations for prophylactic Vitamin D intakes in Central European neonates, infants, childrenand adolescents as well as in adults (including recommendations for pregnant and breastfeeding women and the elderly) were developed.Conclusions: Improving the Vitamin D status of children, adolescents, adults and the elderly must be included in the priorities of physicians,healthcare professionals and healthcare regulating bodies. The present paper offers elaborated consensus on supplementationguidance and population strategies for Vitamin D in Central Europe

    Serum RANKL, osteoprotegerin (OPG), and RANKL/OPG ratio in nephrotic children

    Receptor activator of NF-kB ligand (RANKL) and osteoprotegerin (OPG) play key roles in the pathogenesis of glucocorticoid-induced osteoporosis (GIO). The aim of our study was to determine whether the cumulative glucocorticoid dose (CGCS) in children with idiopathic nephrotic syndrome (INS) has any effect on the concentration of serum RANKL and OPG and the RANKL/OPG ratio. The study population consisted of 90 children with INS, aged 3–20 years, who were treated with GCS. These children were divided into two groups according to the CGCS: low (L) <1 g/kg body weight (BW) and high (H) ≥1 g/kg BW, respectively. The control group (C) consisted of 70 healthy children. RANKL concentration was observed to be significantly higher and OPG significantly lower in INS children than in the reference group: 0.21 (range 0.01–1.36) versus 0.15 (0–1.42) pmol/l (p < 0.05), respectively, and 3.76 (1.01–7.25) versus 3.92 (2.39–10.23) pmol/l (p < 0.05), respectively. The RANKL/OPG ratio was significantly higher in INS children (p < 0.01). The concentration of RANKL, similar to the RANKL/OPG ratio, was significantly higher in Group H children than in Group L children: 0.46 (0.02–1.36 ) versus 0.19 (0.01–1.25) (p < 0.01) and 0.14 (0.01–0.71) versus 0.05 (0.002–0.37) (p < 0.01), respectively. The concentration of OPG was similar in both groups. There was a positive correlation between CGCS and the concentration of sRANKL as well as the RANKL/OPG ratio (in both cases r = 0.33, p < 0.05). Based on these results, we suggest that long-term exposure to GCS results in a dose-dependent increase in serum RANKL concentration and the RANKL/OPG ratio, but not in the level of serum OPG

    For most of September and October 2005, the Polish news media were busy covering the parliamentary and the presidential elections in that country. Beginning two weeks apart from one another, with the presidential run-off election following two weeks later, these overlapping campaigns became the most important media and political events of the year. Their conjunction was an occurrence expected to happen once in 20 years because of Poland’s five-year presidential term and a four-year parliamentary term. For the first time since 1989, the result was that the President, the upper house of the parliament (Senat) and the lower house (Sejm) of the parliament are now controlled by the same party, Law and Justice (PiS). For the first time since Solidarity swept both elections, the Polish electorate has also made a definite turn to the right, voting for a political party that supports radical change, the symbolic setting up of a Fourth Republic which will be a morally superior country in contrast to the third Republic, the independent Polish state established after the Solidarity revolution when Poland was the first country in the former Soviet Block to end communism.     Machine Learning in Recognition of Basic Pulmonary Pathologies

    Nowadays, during the diagnosis process, the doctor is able to obtain access to much information describing the patient&rsquo;s condition using appropriate tools. However, there are always two sides to the coin. The doctor has certain limitations regarding the amount of data they can process at once. Information technology comes to the rescue, which with the help of computers is able to quickly and effectively separate important information from redundant information and support the doctor in making a diagnosis. In this work, a decision-making system was created to diagnose common lung pathologies in digital radiography images. Here, we consider four basic pulmonary diseases: pneumothorax, pneumonia, pulmonary consolidation, and lung lesions. Our objective is to develop a new automatic detection method of lung pathologies on chest X-ray radiographs using python programming language and its libraries. The approach uses solutions in the field of artificial intelligence, such as deep learning, convolutional neural network and segmentation to make a diagnosis that aims to help the radiologist at work. In the first sections, this work describes the fundamentals of the present form of diagnosis, a proposal to improve this process, the method of operation of the algorithms used, data acquisition, segmentation and processing methods. Then, the results of the operation of four different models and their implementation in a practical window program were presented. The best model, which detects pulmonary consolidation, achieves accuracy higher than 91%, which is a satisfactory result because they are not intended to replace radiologists but to improve their work. In the future, this type of program can be further developed by adding models that recognize other conditions
