    A munkanélküliségi viszonyok alakulása a Gyöngyösi járásban, a 2008-as világgazdasági válságot követően

    A 2008-ban kitört világgazdasági válság végigsöpört a világ fejlett hatalmainak nagy részén, megrendítve a gazdaságukat. Magyarországra is komoly hatásai voltak, visszaesette az ipar, a GDP és természetesen a foglalkoztatás is. Magyarországon a válság kitörése utáni hónapokban már csökkeni kezdett a munkanélküliség, amely azonban 2010-re javulni látszott. 2011-ben azonban újabb visszaesés következett. A legnehezebb helyzetben a kelet — magyarországi és a dél — dunántúli régiók voltak. Különösen nehéz helyzetbe kerültek a 25 év alattiak, az 50 év fölöttiek és a 8 általánost végzettek. Vizsgálatunk kiterjedt a Gyöngyösi járás területére, amely Kelet-Magyarországon található. A járásban a regisztrált álláskeresők számának alakulása a vizsgált 7 év alatt elérte sőt kedvezőbb értékeket mutatott 2013-ban, mint a válság kitörése előtti évben. Vizsgálatunkban szerepel továbbá a járás munkanélküliségi rátájának összehasonlítása a megyei és az országos munkanélküliségi rátával

    Módszereket dolgoztunk ki oligo-szachariddal glikozilált N-glikopeptidek szintéziséremind konvergens, mind építőkő módszerrel. Védőcsoportkombinációkat optimalizáltunk O-glikopeptidek szintézisére, valamint optimalizáltuk a kapcsolási reakciót a glikán és a védett aminosav között. Számos foszfopeptidet és sejtpenetráló peptidet szintetizáltunk, részben fluoreszcensen jelzett formában is. Új foldamer molekulát (N-amino prolin) állítottunk elő és építettünk be béta-aminosav oligomerekbe. Megvizsgáltuk a kapott oligomerek térszerkezeti és stabilitásviszonyait. | We improved the applicable methods for the synthesis of N-glycopeptides including convergent and building block approach. The protecting group combinations for the synthesis of O-glycopeptides was optimized and we improved the coupling conditions for the reaction of the glycosyl donor and acceptor. Numerous phospho and cell-penetrating peptide were synthesized and a part of them was fluorescently labelled. A new foldamer molecule (N-amino proline) was prepared and incorporated into beta-amino acid oligomers. The conformation and the stability of the new oligomers was investigated

    Rational Design of α-Helix-Stabilized Exendin-4 Analogues

    Exendin-4 (Ex4) is a potent glucagon-like peptide-1 receptor agonist, a drug regulating the plasma glucose level of patients suffering from type 2 diabetes. The molecule’s poor solubility and its readiness to form aggregates increase the likelihood of unwanted side effects. Therefore, we designed Ex4 analogues with improved structural characteristics and better water solubility. Rational design was started from the parent 20-amino acid, well-folded Trp cage (TC) miniprotein and involved the step-by-step N-terminal elongation of the TC head, resulting in the 39-amino acid Ex4 analogue, E19. Helical propensity coupled to tertiary structure compactness was monitored and quantitatively analyzed by electronic circular dichroism and nuclear magnetic resonance (NMR) spectroscopy for the 14 peptides of different lengths. Both 15N relaxation- and diffusion-ordered NMR measurements were established to investigate the inherent mobility and self-association propensity of Ex4 and E19. Our designed E19 molecule has the same tertiary structure as Ex4 but is more helical than Ex4 under all studied conditions; it is less prone to oligomerization and has preserved biological activity. These conditions make E19 a perfect lead compound for further drug discovery. We believe that this structural study improves our understanding of the relationship between local molecular features and global physicochemical properties such as water solubility and could help in the development of more potent Ex4 analogues with improved pharmacokinetic properties

    Helicobacter pylori Infection Is Associated With Carotid Intima and Media Thickening : A Systematic Review and Meta-Analysis

    Background Helicobacter pylori (H. pylori) infection affects ≈4.4 billion people worldwide. Several studies suggest that this pathogen impacts the digestive system, causing diverse and severe conditions, and results in extragastrointestinal disorders like vascular diseases. Our study aims to examine the association between H. pylori infection and carotid intima-media thickness. Methods and Results Electronic databases (MEDLINE, Embase, CENTRAL, Web of Science, and Scopus) were searched for studies, comparing the thickness of the carotid intima-media in H. pylori-infected and noninfected individuals listed until October 20, 2020. Statistical analyses were performed using the random effects meta-analysis of model of weighted mean differences with the corresponding 95% CI using the DerSimonian and Laird method. The protocol was registered in advance in PROSPERO (International Prospective Register of Systematic Reviews; CRD42021224485). Thirteen studies were found meeting inclusion criteria for our systematic review and meta-analysis, presenting data on the thickness of the carotid intima-media considering the presence of H. pylori infection. Altogether, 2298 individuals' data were included (1360 H. pylori positive, 938 negative). The overall carotid intima-media thickness was significantly larger among infected patients compared with uninfected participants (weighted mean difference: 0.07 mm; 95% CI, 0.02-0.12; P=0.004; I2=91.1%; P<0.001). In case of the right common carotid artery, the intima-media thickening was found to be significant as well (weighted mean difference, 0.08 mm; 95% CI, 0.02-0.13, P=0.007; I2=85.1%; P<0.001), while it showed no significance in the left common carotid artery (weighted mean difference, 0.12 mm; 95% CI, -0.05 to 0.28, P=0.176; I2=97.4%; P<0.001). Conclusions H. pylori infection is associated with increased carotid intima-media thickness. Therefore, the infection may indirectly contribute to the development of major vascular events

    Meta-Analysis of the Long Term Success Rate of Different Interventions in Benign Biliary Strictures.

    BACKGROUND: Benign biliary stricture is a rare condition and the majority of the cases are caused by operative trauma or chronic inflammation based on various etiology. Although the initial results of endoscopic, percutaneous and surgical treatment are impressive, no comparison about long term stricture resolution is available. AIMS: The goal of this study was to compare the long term disease free survival in benign biliary strictures with various etiology after surgery, percutaneous transhepatic-and endoscopic treatment. METHODS: PubMed, Embase, and Cochrane Library were searched by computer and manually for published studies. The investigators selected the publications according to the inclusion and exclusion criteria, processed the data and assessed the quality of the selected studies. Meta-analysis of data of 24 publications was performed to compare long term disease free survival of different treatment groups. RESULTS: Compared the subgroups surgery resulted in the highest long term stricture resolution rate, followed by the percutaneous transhepatic treatment, the multiple plastic stent insertion and covered self-expanding metal stents (SEMS), however the difference was not significant. All compared methods are significantly superior to the single plastic stent placement. Long term stricture resolution rate irrespectively of any therapy is still not more than 84%. CONCLUSIONS: In summary, the use of single plastic stent is not recommended. Further randomized studies and innovative technical development are required for improving the treatment of benign biliary strictures

    Early prediction of acute necrotizing pancreatitis by artificial intelligence : a prospective cohort-analysis of 2387 cases

    Pancreatic necrosis is a consistent prognostic factor in acute pancreatitis (AP). However, the clinical scores currently in use are either too complicated or require data that are unavailable on admission or lack sufficient predictive value. We therefore aimed to develop a tool to aid in necrosis prediction. The XGBoost machine learning algorithm processed data from 2387 patients with AP. The confidence of the model was estimated by a bootstrapping method and interpreted via the 10th and the 90th percentiles of the prediction scores. Shapley Additive exPlanations (SHAP) values were calculated to quantify the contribution of each variable provided. Finally, the model was implemented as an online application using the Streamlit Python-based framework. The XGBoost classifier provided an AUC value of 0.757. Glucose, C-reactive protein, alkaline phosphatase, gender and total white blood cell count have the most impact on prediction based on the SHAP values. The relationship between the size of the training dataset and model performance shows that prediction performance can be improved. This study combines necrosis prediction and artificial intelligence. The predictive potential of this model is comparable to the current clinical scoring systems and has several advantages over them

    Hypoalbuminemia affects one third of acute pancreatitis patients and is independently associated with severity and mortality

    The incidence and medical costs of acute pancreatitis (AP) are on the rise, and severe cases still have a 30% mortality rate. We aimed to evaluate hypoalbuminemia as a risk factor and the prognostic value of human serum albumin in AP. Data from 2461 patients were extracted from the international, prospective, multicentre AP registry operated by the Hungarian Pancreatic Study Group. Data from patients with albumin measurement in the first 48 h (n = 1149) and anytime during hospitalization (n = 1272) were analysed. Multivariate binary logistic regression and Receiver Operator Characteristic curve analysis were used. The prevalence of hypoalbuminemia (< 35 g/L) was 19% on admission and 35.7% during hospitalization. Hypoalbuminemia dose-dependently increased the risk of severity, mortality, local complications and organ failure and is associated with longer hospital stay. The predictive value of hypoalbuminemia on admission was poor for severity and mortality. Severe hypoalbuminemia (< 25 g/L) represented an independent risk factor for severity (OR 48.761; CI 25.276-98.908) and mortality (OR 16.83; CI 8.32-35.13). Albumin loss during AP was strongly associated with severity (p < 0.001) and mortality (p = 0.002). Hypoalbuminemia represents an independent risk factor for severity and mortality in AP, and it shows a dose-dependent relationship with local complications, organ failure and length of stay.Peer reviewe
