    Histereskopik submüköz myom rezeksiyonunun, menstrual dönemde kullanılan Ped Sayısı üzerine Etkisi

    Aim: To research the effect of hysteroscopic resection of submucosal myoma (HRSM) on the amount of pads used during the menstrual period in a premenopausal patient with menometrorrhagia caused by submucosal fibroids. Material and Method: A total of 27 women with submucosal fibroids were included in this prospective study using the Pictorial Blood Assessment Chart (PBAC) to evaluate quantity of menorrhagia. Bleeding evaluation and number of pads used by patients in the 6th month after HRSM were recorded. Results: The average age of patients included in the study was 41 +/- 5.1. Of 27 patients 20 (74%) had one. 4 (14%) had two and 3 (12%) had three or more submucosal myomas. The diameter of removed fibroids were 17 (63%) less than 3 cm, 6 (22%) between 3 and 5 cm and 4 (15%) were above 5 cm. In the postoperative 6th month on the day of heaviest menstrual bleeding average number of pads used dropped to 3.8 compared to 9.1 in the preoperative period (p<0.05). Discussion: HRSM performed due to menometrorrhagia in the premenopausal period significantly reduces the number of pads used and the number of days of bleeding

    Endometrioma Cerrahisinin Ovaryan Rezerv ve Ovaryan Kan Akımı üzerine Olan Etkisi

    Aim:To investigate the effectiveness of laparoscopic endometriosis surgery on ovarian reserve and ovarian blood flow index. Material and Method: Twenty-six patients were evaluated prospectively whom had laparoscopic surgery due to endometrioma in third care education and research hospital. Before and after surgical procedure all patients FSH, LH and E2 levels were measured in their 3rd menstrual period day. Periodically cysts and ovarian volume as well as ovarian artery resistive index measured by using transvaginal Doppler sonography. Results:Twenty-six patients' mean age was 25, 6 +/- 4, 5 (20-36 years). When the compared to changes of preoperative and postoperative values of hormonal parameters FSH (p:0,455) and LH (p:0,672) were insignificant and E2 (p:0,024) was significant respectively. While ovarian volume was decreasing significantly (p:0,001), Ovarian Artery RI parameter was not changed (p:0,587). Discussion:Our study was showed that while laparoscopic surgical treatment of ovarian endometriomas significantly decrease ovarian volume, but hormonal parameters and ovarian blood flow were not affected

    Primary cardiac myxomas: report of 28 cases and review the literature

    Purpose: Although primary cardiac tumors are rare, cardiac myxoma is the most common seen intracardiac tumor This study summarizes our surgical experience with cardiac myxomas. Methods: Twenty-eight cases of cardiac myxoma that were surgically treated in our center between January 1990 and March 2010 were retrospectively reviewed. In all patients the New York Heart Association (NYHA) functional classification was used to assess the functional status, and C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) levels were measured before and after surgery. Results: Nineteen patients were female and 9 male. The mean age was 53.3 +/- 12.7 years (range 4-73). The most common symptom was dyspnea (86%). Two patients presented with peripheral embolism. In preoperative assessment of functional status, 24 of the patients were in NYHA Class II, 2 of them were in Class land the other 2 patients were in Class III. All patients were in NYHA class fin the early postoperative period up to long-term follow up. The mean preoperative CRP and ESR levels were 47.3 +/- 14.6 mg/l (range 30-79) and 52.8 +/- 21.4 mm/h (range 28-82), respectively. One month after surgery, CRP and ESR levels returned back to normal values. Postoperative CRP and ESR levels were 8.4 +/- 2.6 mg/l (range 1-12) and 7.9 +/- 4.8 mm/h (range 3-18), respectively. Conclusion: Myxomas should be considered in all patients with embolism. After surgical resection of cardiac myxomas, the functional status of patients improve and CRP and ESR levels become normal

    Vascular complications in orthopedic surgery are not frequent, but they can be severe and occasionally limb-threatening. While spontaneous complications are not directly dependent on the surgical procedure, iatrogenic complications are those actively caused by surgical maneuvers or by instrumentation. Although vascular injury during placement of a cerclage wire is feared by all surgeons, its occurrence is rare and we have been able to find only one reported case describing an injury to the femoral artery and vein. This case report describes the surgical management of occlusions of the popliteal artery and vein following a cerclage wiring procedure of the distal femur during a total knee replacement procedure. Surgical treatment was accomplished by resection of the entrapped artery and vein segments and reattachment with end to end anastomosis. The orthopedic surgeon must be aware of prevalent risk of vascular injury during knee surgery and be diligent and methodological about performing preoperative and postoperative vascular examination in these patients
