Canagliflozin and renal outcomes in type 2 diabetes and nephropathy
BACKGROUND Type 2 diabetes mellitus is the leading cause of kidney failure worldwide, but few effective long-term treatments are available. In cardiovascular trials of inhibitors of sodium–glucose cotransporter 2 (SGLT2), exploratory results have suggested that such drugs may improve renal outcomes in patients with type 2 diabetes. METHODS In this double-blind, randomized trial, we assigned patients with type 2 diabetes and albuminuric chronic kidney disease to receive canagliflozin, an oral SGLT2 inhibitor, at a dose of 100 mg daily or placebo. All the patients had an estimated glomerular filtration rate (GFR) of 30 to <90 ml per minute per 1.73 m2 of body-surface area and albuminuria (ratio of albumin [mg] to creatinine [g], >300 to 5000) and were treated with renin–angiotensin system blockade. The primary outcome was a composite of end-stage kidney disease (dialysis, transplantation, or a sustained estimated GFR of <15 ml per minute per 1.73 m2), a doubling of the serum creatinine level, or death from renal or cardiovascular causes. Prespecified secondary outcomes were tested hierarchically. RESULTS The trial was stopped early after a planned interim analysis on the recommendation of the data and safety monitoring committee. At that time, 4401 patients had undergone randomization, with a median follow-up of 2.62 years. The relative risk of the primary outcome was 30% lower in the canagliflozin group than in the placebo group, with event rates of 43.2 and 61.2 per 1000 patient-years, respectively (hazard ratio, 0.70; 95% confidence interval [CI], 0.59 to 0.82; P=0.00001). The relative risk of the renal-specific composite of end-stage kidney disease, a doubling of the creatinine level, or death from renal causes was lower by 34% (hazard ratio, 0.66; 95% CI, 0.53 to 0.81; P<0.001), and the relative risk of end-stage kidney disease was lower by 32% (hazard ratio, 0.68; 95% CI, 0.54 to 0.86; P=0.002). The canagliflozin group also had a lower risk of cardiovascular death, myocardial infarction, or stroke (hazard ratio, 0.80; 95% CI, 0.67 to 0.95; P=0.01) and hospitalization for heart failure (hazard ratio, 0.61; 95% CI, 0.47 to 0.80; P<0.001). There were no significant differences in rates of amputation or fracture. CONCLUSIONS In patients with type 2 diabetes and kidney disease, the risk of kidney failure and cardiovascular events was lower in the canagliflozin group than in the placebo group at a median follow-up of 2.62 years

Search for eccentric black hole coalescences during the third observing run of LIGO and Virgo
Despite the growing number of confident binary black hole coalescences observed through gravitational waves so far, the astrophysical origin of these binaries remains uncertain. Orbital eccentricity is one of the clearest tracers of binary formation channels. Identifying binary eccentricity, however, remains challenging due to the limited availability of gravitational waveforms that include effects of eccentricity. Here, we present observational results for a waveform-independent search sensitive to eccentric black hole coalescences, covering the third observing run (O3) of the LIGO and Virgo detectors. We identified no new high-significance candidates beyond those that were already identified with searches focusing on quasi-circular binaries. We determine the sensitivity of our search to high-mass (total mass M>70 M⊙) binaries covering eccentricities up to 0.3 at 15 Hz orbital frequency, and use this to compare model predictions to search results. Assuming all detections are indeed quasi-circular, for our fiducial population model, we place an upper limit for the merger rate density of high-mass binaries with eccentricities 0<e≤0.3 at 0.33 Gpc−3 yr−1 at 90\% confidence level
Ultralight vector dark matter search using data from the KAGRA O3GK run
Among the various candidates for dark matter (DM), ultralight vector DM can be probed by laser interferometric gravitational wave detectors through the measurement of oscillating length changes in the arm cavities. In this context, KAGRA has a unique feature due to differing compositions of its mirrors, enhancing the signal of vector DM in the length change in the auxiliary channels. Here we present the result of a search for U(1)B−L gauge boson DM using the KAGRA data from auxiliary length channels during the first joint observation run together with GEO600. By applying our search pipeline, which takes into account the stochastic nature of ultralight DM, upper bounds on the coupling strength between the U(1)B−L gauge boson and ordinary matter are obtained for a range of DM masses. While our constraints are less stringent than those derived from previous experiments, this study demonstrates the applicability of our method to the lower-mass vector DM search, which is made difficult in this measurement by the short observation time compared to the auto-correlation time scale of DM

La atención a la diversidad desde el agrupamiento flexible de alumnos
Investigar el principio de atención a la diversidad. Aportar un instrumento de análisis. Aportar un modelo de intervención en las prácticas de agrupamiento flexible. Profesores de educación infantil y primaria de Barcelona. Centro educativo de primaria con alumnos de entre 6 y 14 años.. En una primera parte se hace una introducción al tema. En una segunda parte se desarrolla el tema de la escuela como institución sociocultural. En una tercera parte se desarrollo el paso de los principios a las prácticas. En una cuarta parte se desarrolla la investigación llevada a cabo. En una quinta parte se puede encontrar la bibliografía. Falta de formación y de recursos del profesorado para llevar a cabo los objetivos. No se tiene una visión de la actitud que debe tener el profesorado entre la diversidad de alumnado. Preocupación del profesorado por la dificultad para atender la gran variedad de diferencias individuales de los alumnos. Evaluar la práctica educativa sobre los agrupamientos flexibles de alumnos que se realizan en un centro público de enseñanza primaria. 323 profesores de centros de educación infantil y primaria de carácter público y privado concertado de diferentes zonas y situación geográfica que o formaban parte de un grupo de asistentes a cursos de formación en temas curriculares y organizativos relacionados con los nuevos planteamientos de la Reforma o respondían desde sus centros al cuestionario QUAETAE que les fue entregado. El trabajo se presenta en tres partes. La primera trabaja el tema de la escuela como institución sociocultural, la escuela comprensiva, la atención a la diversidad de alumnos como principio educativo y en el nuevo planteamiento de la Reforma. En la segunda parte traslada los principios a las prácticas, trabajando la innovación y la formación y los agrupamientos flexibles de alumnos como modalidad para atender la diversidad. La tercera parte comprende la investigación con un modelo de intervención en el tratamiento de la diversidad hacia un centro reformado. Ideograma. La organización de este tipo de agrupamientos requiere considerar la capacidad de flexibilidad del centro como institución tanto desde la perspectiva organizativa como desde la curricular o la relacional, si se quieren mantener los principios en los que se basan, habrá que plantearse no sólo los aspectos organizativos referentes a número de grupos, espacios físicos, horarios, o cantidad de profesores o de alumnos por grupo sino que, fundamentalmente, habrá que plantearse como llevar a cabo la flexibilidad en los aspectos curriculares: transición de un grupo a otro, periodos de adaptación, de consolidación o interacciones sociales.CataluñaES

Resumen tomado de la publicaciónReflexiona en torno al valor formativo de las creencias en la formación del profesorado, tanto inicial como permanente y a las ataduras que pueden suponer para el aprendizaje y el cambio. Se enmarca dentro de la investigación sobre el pensamiento del profesor y se focaliza en la construcción de conocimiento profesional a partir de la relación formación-innovación desde una perspectiva ecológica del conocimiento. Define el concepto de creencia como parte de una dimensión personal afectiva y emocional, íntimamente ligada a la propia cultura que se manifiesta en el ambiente. Argumenta la necesidad de considerar contenido de formación esta dimensión con la finalidad de facilitar la autonomía y la autorregulación del conocimiento profesional. Resumen tomado de la publicaciónEl presente estudio forma parte de una investigación más amplia basada en la integración de tres conceptos: investigación, formación e innovación que busca la mejora de la calidad de la enseñanza a partir del estudio de las estrategias didácticas. Los modelos basados en la mera reflexión no se han mostrado eficaces para el cambio docente, así como tampoco los que se han quedado en la mera práctica descontextualizada del lugar de trabajo. Se intentan integrar los conceptos de generación del conocimiento, formación e innovación; se define el concepto de estrategia didáctica; y se describe asimismo el modelo holodinámico de análisis en el contexto del aula. Además, se exponen los indicadores que se toman en cuenta en el estudio de casos utilizando como soporte informativo el cine comercial. El cine formativo es una estrategia que el grupo GAD de la Universidad de Barcelona utiliza desde hace diez años, tanto en jornadas anuales como en las aulas.MadridES
