Evaluation of phenytoin plasma levels in patients from Hospital William Allen (HWAT), Social Security Fund (CCSS)
La monitorización terapéutica de fármacos (MTF) es una herramienta válida para optimizar la farmacoterapia ,permitiendo el ajuste en la dosificación de medicamentos,tomando en cuenta las características individuales de cada paciente. El uso de la MTF ha sido indicado en el caso de falta de respuesta terapéutica o presencia de efectos adversos a dosis terapéuticas, ya sea por interacciones entre fármacos, presencia de particularidades genéticas concernientes a polimorfismos metabólicos y condiciones fisiológicas especiales, como sucede con los pacientes pediátricos y geriátricos. El tratamiento de pacientes con epilepsia, es una de las áreas donde el MTF ha hecho muchas contribuciones significativas y algunas de las razones que justifican su aplicación son: a) Que las crisis
convulsivas ocurren en intervalos irregulares, lo que hace difícil el establecer una dosis óptima para su
tratamiento; b) que los síntomas clínicos y los signos tóxicos pueden ser difíciles de detectar e interpretar
y c) que los marcadores fisiológicos intermediarios de esta patología no están claramente definidos. Quality assessment in the extemporaneous pediatric preparation of powder papers containing crushed tablets of spironolactone or amiodarone
To elaborate the pediatric medicines the specifics of this population
must be considered. Powder papers as extemporaneous formulations
may be an option to provide dosing flexibility. The main objective of
this research was to evaluate the quality of powder papers in its manual
preparation stages and during the 60 days of storage. The results shown
that a smaller than expected mass of tablets was distributed for the 40
papers at each formulation. The precision in the distribution performed
by the crushed and pulverized tablet preparatory assistant reflected
relative standard deviations ranging from 9 to 15%. On the start day,
the uniformity of mass showed an inaccuracy reflected in several
values outside the expected range, for all formulations. The accuracy
of the tablets powder mass during storage at 60 days showed a ±10% of difference respect
basal mass for papers with spironolactone 6,25 mg and the 15 and 75 mg amiodarone
hydrochloride papers. For 10 mg of amiodarone hydrochloride formulation, random errors
occurred outside the expected ranges at two sampling times. Powder adherence percentage
was between the 10 and 20%. In conclusion, the preparation of four formulations in powder
paper showed some errors which resulted in a significant loss of dose. A standardized
operating procedure for the preparation of powder papers should be available taking into
account the risk points of the manufacturing processes. It is advisable to enable the galenical
section in hospital pharmacies and to have legislation regulating extemporaneous

Review of the fractionation of medicines available in solid formulations (tablets)
Not always people receive the just dose of active ingredient required for his health problem. To obtain success in pharmacology treatment, sometimes the physicians prescribe drugs in a dose that is impossible obtain in the market because this specific strength does not exist in the pharmaceutical form that they proposed. This is the case of some tablets, which need to be divided to achieve this objective. Several reasons are to justify the need to split pills, since there exist some consequences of this act in different contexts. Laboratories produce pharmaceutical forms with Good Manufacturing Practices, but not all medications are designed to be split. Dividing a scored tablet into two parts, it is no guarantee to find accurately the half dose in each fraction. Evaluation of the fractionation of ten medicines in tablets under three methods in Costa Rica
To obtain the correct dose it was sometimes necessary to split tablets.
Some organizations have developed guides in order to verify the
quality of the tablets with functional score and their fragments. The
objective of this work was to evaluate the fractionation of tablets in
halves, performed with three methods and applied to ten medicines
used in pathologies of epidemiological relevance in Costa Rica. For
each medicine, 60 participants fractionated a tablet once with splitter,
knife and hand. The errors produced in the fractionation was
determined. The tablet breaking force and the uniformity of mass of
whole tablets were verified and the test of uniformity of mass and the
percentage of loss of mass was applicate to tablets halves. The results
showed that 98% of the whole tablets were in the expected range of
Percentage deviation of mass average. No pattern linked the breaking force with the results of
tablets halves. A high number of errors in the fractionation were associated with the knife.
About tablets halves, the splitter showed the better accuracy for split, and the good results in
percentages of loss of mass were evidenced for the fractionation with hand. The fractionation
showed a global waste of 46% of 1,800 tablets available for the study. Atenolol and warfarin
sodium have the higher levels of waste, which draw attention due to their high consumption
associated with to the epidemiological profile of population. These findings should not be

Development and validation of procedures for determining losartán potassium in therapeutic equivalence studies
Objetivos:
Desarrollar y validar dos procedimientos analíticos para una aplicación en estudios de
equivalencia terapéutica de tabletas de losartán potásico.
Métodos:
Se desarrolló un procedimiento por cromatografía líquida de alta resolución para cuantificar
losartán potásico en estudios in vitro (perfiles de disolución y uniformidad de contenido) y otro
procedimiento bioanalítico.
Las condiciones cromatográficas fueron: inyección de 100 L, fase estacionaria de
octadecilsilano, fase móvil de buffer fosfatos (pH 3,0)/acetonitrilo (50:50, v/v) a 0,8 mL/min y
detección a 225 nm.
Los protocolos de validación se basaron en los lineamientos de la Conferencia Internacional de
Armonización, la Organización Mundial de la Salud, la Administración de Drogas y Alimentos y
la Agencia de Medicinas Europea.Objectives:
Two analytical procedures were developed and validated for use in therapeutic equivalence
studies of losartan potassium tablets.
Methods:
A procedure for the in vitro studies (dissolution profiles and content uniformity of tablets)
proposed the development of a method for high resolution liquid chromatography whose
conditions were: injection of 100 μL, an octadecylsilane stationary phase, phosphate buffer (pH
3.0), acetonitrile (50:50, v/v) as mobile phase with a flow rate of 0.8 mL/min and a detection
wavelength of 225 nm. The setting of bioanalitycal procedure involved the solid phase extraction
to 1 mL of human plasma.
Validation protocols were based on the guidelines of the International Conferences of

Farmacogenética: hacia la individualización de la terapia farmacológica en Costa Rica Pharmacogenetics: toward the individualization of pharmacologic therapy in Costa Rica
La ‘era posgenómica’ ha impactado las ciencias biológicas y biomédicas desde la identificación de un significativo componente de variabilidad genética interindividual. La Farmacología no escapa a esta realidad, y esta ciencia, en conjunto con la Toxicología, ya desde hace varios años ve desarrollados campos como la Farmacogenética y la Farmacogenómica. Dichas disciplinas estudian la influencia de la variabilidad genética de la población respecto a la respuesta que se tiene ante el contacto con los fármacos y las sustancias tóxicas. Las variantes genéticas, reflejadas en el polimorfismo de los genes, tienen implicaciones en el manejo de xenobióticos en el organismo y además determinan las respuestas aumentadas, normales o disminuidas durante la administración de algunos fármacos. Muchos genes se relacionan con reacciones adversas y fallas terapéuticas de fármacos administrados a dosis predeterminadas y siguiendo protocolos establecidos. Es por esto que las herramientas de caracterización genética molecular se aplican en muchos países con el fin de adaptar estos protocolos a cada individuo, es decir, personalizar la terapia farmacológica, donde las dosificaciones son evaluadas de acuerdo con las características genéticas de la persona, para evitar o prevenir reacciones adversas en individuos predispuestos. Costa Rica es un país que hace grandes esfuerzos para brindar una atención médica de primer mundo a sus habitantes, sin embargo, el campo de la Farmacogenética aún no se ha desarrollado en nuestro medio. No obstante, el Centro Nacional Innovaciones Biotecnológicas financia un proyecto pionero junto con la Universidad de Costa Rica, para desarrollar la aplicación de esta disciplina en el país. Pharmacology has not escaped this influence. This discipline, together with toxicology, has seen the development of areas such as pharmacogenetics and pharmacogenomics for several years now. Different polymorphisms (genetic variants) in genes involved in the handling of xenobiotics in the body are able to determine an exaggerated, normal or reduced response to a certain dose of any drug. Many genes have been associated with poor response to pre-determined dosing protocols or with hypersensitivity reactions. Because of this, molecular genetics characterization tools are applied in many countries to adapt this protocol individually, i.e. a personalized pharmacology, where dosages are evaluated according to the genetic determinants a person carries in order to avoid or prevent adverse reactions in predisposed individuals. Costa Rica is a country that makes great efforts to give first-world medical care to its residents, but the field of pharmacogenetics has not been developed in our medium yet. Notwithstanding, the National Center for Biotechnology Innovations is currently funding a pioneering joint project with the University of Costa Rica in order to develop the application of this discipline in the country. This article presents the biological basis and clinical utility of pharmacogenetics, as well as the details regarding the efforts that are being made to develop this discipline in Costa Rica

Cuando se habla de farmacogenética o polimorfismos es importante buscar el eslabón que vincule la ciencia básica con la práctica clínica. Existen muchos factores que pueden mediar en la respuesta medicamentosa tales como: la edad, función orgánica y metabólica del paciente, presencia de tratamientos concomitantes, posibles interacciones medicamentosas e inclusive el tipo de patología. El conocer las características genéticas individuales de la población, a partir de una serie de exámenes o procedimientos, facilitaría optimizar y ajustar dosis para una respuesta medicamentosa dada y/o tomar las previsiones necesarias con respecto al comportamiento fisiopatológico del paciente. Quality verification of six medicines in wholes and halves tablets, re-packaged in individual plastic compartments during sixty days
Plastic boxes with individual compartments are often used to improve
medication compliance and only a few studies have been done on the
stability of the tablet halves after placing them on these devices. In this
investigation, wholes and halves tablets of furosemide,
hydrochlorothiazide, spironolactone, warfarin sodium and atenolol and
levothyroxine sodium were re-packaged and storing for sixty days
under normal environmental conditions. The physical characteristics
were observed and the quality attributes were verified. There were no
changes in physical characteristics of all products. All products, except
halves tablets of atenolol, showed accuracy according the test of
uniformity of mass. The comparisons of dissolution profiles between
whole tablets and tablets halves were adequate for hydrochlorothiazide and atenolol, but not
for formulations of furosemide, spironolactone and warfarin sodium. Levothyroxine sodium
showed a different dissolution behavior than expected. Whole tablets of atenolol do not meet
specifications for uniformity of contents, neither the halves tablets of atenolol and
spironolactone. According to the results, it is possible to re-package wholes tablets of
furosemide, hydrochlorothiazide, spironolactone and warfarin sodium; but it is not advisable
to maintain this practice for whole tablets of atenolol and levothyroxine sodium, neither for
the halves of the rest of the products, except hydrochlorothiazide. These findings may
become opportunities for improvement for pharmaceutical laboratories, as well as become a
study topic for practitioners to advise the use of multi-compartment compliance aids, finally, these results should not be extrapolated to drugs, manufacturers, lots and conditions other

Equivalencia terapéutica de tabletas de levofloxacino del mercado costarricense por comparación de perfiles de disolución
El presente trabajo corresponde a un estudio in vitro de equivalencia terapéutica e intercambiabilidad de tres medicamentos multiorigen de tabletas de liberación inmediata de levofloxacino del mercado costarricense. Método: Se siguieron los criterios del informe 7 de la Organización Mundial de la Salud (OMS). Los medicamentos se analizaron mediante un método de espectroscopía ultravioleta desarrollado y validado en nuestro laboratorio. Resultados: Se demostró que los productos analizados de dos laboratorios, designados por A y B, se comportaron con una muy rápida disolución (no menos de un 85 % disuelto del etiquetado en 15 minutos) En cambio, un tercer producto de un laboratorio, denominado C, mostró una rápida disolución (no menos del 85 % disuelto del etiquetado a los 30 minutos).
