    Spatial analysis of Kawaguchi City crime data

    In this paper, we built some models of the spatial data, and evaluate those, using the crime data of Kawaguchi city, Saitama prefecture in Japan. Compute the Moran’I statistics of five crime data related to thefts in the city, the value of sneakthief take the aggregation. So, we pay attention to the sneak, and make a model to estimate the incidence of the events. As a regression, we select the old-age index among regional indexes. Applying a simple regression(SR), a spatial autoregressive model(SAR), a geographically weighted regression(GWR) , we evaluate these models. In GWR model, it is interesting that there are some sections in which the sign of the coefficient takes the opposite versus SR and SAR models.     Background: Pulmonary alveolar proteinosis (PAP) is a rare disorder characterised by abundant alveolar accumulation of surfactant lipoproteins. Serum levels of KL-6, high molecular weight human MUC1 mucin, are increased in the majority of patients with PAP. The prognostic significance of KL-6 in PAP is still unknown. Aim of the study was to evaluate whether serum KL-6 levels correlate with the outcome of the disease. Patients and methods: From 2006 to 2012, we prospectively studied 33 patients with primary autoimmune PAP. We measured serum KL-6 levels by ELISA (Eisai, Tokyo, Japan), and evaluated the correlation between initial KL-6 levels and clinical variables. Disease progression was defined as deterioration of symptoms, and/or lung function, and/or chest imaging. Main results: The initial serum KL-6 levels were significantly correlated with the baseline PaO2, A-aDO(2), DLCO, VC and TLC (p=0.042, 0.012, 0.012, 0.02 and 0.013, respectively). The change over time of serum KL-6 correlated with the change over time of DLCO (p=0.017). The initial serum KL-6 levels were significantly higher in patients with disease progression than in those with remission (p<0.001). At a cut-off level of 1526 U/mL, the initial serum KL-6 level predicted disease progression (Se 81\%, Sp 94\%). At a cut-off level of 2157 U/mL, the initial serum KL-6 predicted the necessity of repeated whole lung lavage (Se 83\%, Sp 96\%). In the multivariate analysis, the initial serum level of KL-6 was the strongest predictor of disease progression (HR 9.41, p=0.008). Conclusions: Serum KL-6 seems to predict outcome in PAP

    Structures of [Mg·(H2O)1,2]+ and [Al·(H2O)1,2]+ ions studied by infrared photodissociation spectroscopy: Evidence of [HO-Al-H]+ ion core structure in [Al·(H2O)2]+

    Infrared spectra of [Mg·(H2O)1,2]+ and [Al·(H2O)1,2]+ are measured in the OH stretching region (3200-3800 cm-1). The spectra show the symmetric and asymmetric OH stretching bands of water molecules that are directly bound to the metal ions through metal-oxygen intermolecular bonds. In addition to these bands, the [Al·(H2O)2]+ ion has another band at 3714 cm-1. This band is assigned to the free OH stretching vibration of the [HO-Al-H]+ ion; the aluminum ion is inserted into the O-H bond of one water molecule in [Al·(H 2O)2]+

    Pediatric cardiorespiratory failure successfully managed with venoarterial-venous extracorporeal membrane oxygenation: a case report

    Background: Venoarterial-venous extracorporeal membrane oxygenation (VAV ECMO) configuration is a combined procedure of extracorporeal membrane oxygenation (ECMO). The proportion of cardiac and respiratory support can be controlled by adjusting arterial and venous return. Therefore, VAV ECMO can be applicable as a bridging therapy in the transition from venoarterial (VA) to venovenous (VV) ECMO. Case presentation: We present an 11-year-old girl with chemotherapy-induced myocarditis requiring extracorporeal cardiorespiratory support. She showed progressive hypotension, tachycardia, hyperlactemia, and tachypnea under support of catecholamines. Echocardiography showed severe left ventricular hypokinesis with an ejection fraction of 30 %. She was placed on VA ECMO with a drainage catheter from the right femoral vein (19.5 Fr) and a return catheter to the right femoral artery (16.5 Fr). Extracorporeal circulation was initiated at a blood flow of 2.0 L/min (59 mL/kg/min). On day 31, although cardiac function had improved, persistent pulmonary failure made weaning from VA ECMO difficult. We planned transition from VA ECMO to VAV ECMO to ensure gradual tapering of extracorporeal cardiac support while evaluating cardiopulmonary function. An additional return cannula (13.5 Fr) was inserted from the right internal jugular vein, which was connected to the circuit branch from the original returning cannula. We then gradually shifted the blood from the femoral artery to the right internal jugular vein over 24 h. She was successfully switched from VA to VV ECMO via VAV ECMO. Conclusions: VAV ECMO might be an option in ensuring oxygenation to the coronary circulation and allowing time to adequately evaluate cardiac function during transition from VA to VV ECMO. Further investigations using larger cohorts are necessary to validate the efficacy of VAV ECMO as a bridging therapy in the transition from VA to VV ECMO.This work was supported by a JSPS KAKENHI Grant (Number JP 16K09541)
