The efficiency of corrective therapy among patients with mediobasal and lateral temporal epilepsy in interictal period

The research of neurological electroencephalographic (EEG) and Doppler with application of functional tests in 78 patients mediobasal (MTE) and 42 lateral (LTE) temporal lobe epilepsy through 1,3,6 months after corrective therapy using cavinton and mexidol was carried out. It was found a decrease in seizure frequency by 50% and more in 92,5% of patients ТЕ, including full control seizures - 23,1% of patients MTE and 57,1% - LTE. A decreasing of index of paroxysmal activity in the EEG recordings and positive dynamics of cerebrovascular reserve was noticed. The persistent clinical-neurophysiological effect had a place among patients ТЕ of the both groups during 4 months and prevailed among patients LTE.Проведено исследование неврологическое, электроэнцефалографическое (ЭЗГ) и допплерографическое с применением функциональных проб у 78 пациентов медиобазальной (МВЗ) и 42 - латеральной (ЛВЭ) височной эпилепсией через 1, 3, б месяцев после корректирующей терапии с использованием кавинтона и мексидола. У 92,5% больных ВЭ выявлено снижение частоты приступов на 50% и более, в том числе полный контроль над припадками у 23,1% пациентов МВЭ и 57,1 % - ЛВЗ. Установлено уменьшение индекса пароксизмальной активности при записи ЭЭГ и положительная динамика цереброваскулярного резерва. Стойкий клинико-нейрофизиологический эффект имел место у пациентов ВЭ обеих групп в течение 4 месяцев и преобладал у пациентов ЛВЗ

The dynamics of clinical, electroencephalographic, hemodynamic indexes among the patients after the intergrated treatment

The dynamics of clinical electroencephalographic (EEG) and hemodynamic in-dexes according to the data of transcranial doppleography (TCD) after the treatment in a month with including of cavinton and mexidol among 66 patients with temporal epilepsy (ТЕ) with rare (I) and frequent (II) attacks at the age of 30,5+9,3 years was studied. The raise of the aspmetry coefficient of blood circulation middle velocity in middle (MBA),front (FBA) and backbone (BA) brain arteries with prevalence among the patients of the second group-on BA and BBA (p<0,05) among patients with temple epilepsy was established. After the combined therapy the complete control over the attacks was achieved among 58,8% and 31,3% patients, the reduction of the attack number more than 75% is observed among 35,2% and 46,9% patients with temple epilepsy of the first and the second group on BBA and BA (p<0,05). According to the data EEG the raise of alpha-activity and the reduction of paroxysmal activity (p<0,05) is recorded, however the index of paroxysmal activity among the patients in the second group remained increased (p<0,05).The normalization of brain circulation aspm etry index among the patients of the first group is revealed; the aspmetry index on BBA and BA (p<0,05) among patients of the second group was being kept. The revealed positive dynamics of studied index among patients with ТЕ motivates including vessel and antioxidant therapy in epilepsy treatment.Изучена динамика клинических, электроэнцефалографических (ЭЗГ) и ге-модинамических показателей по данным транскраниальной допплерографии (ТКД) через месяц после лечения с включением кавинтона и мексидола у 66 больных височной эпилепсией (ВЗ) с редкими (I) и частыми (II группа) припадками в возрасте 30,5 ± 9,3 лет. У пациентов ВЭ установлено повышение коэффициента асимметрии средней скорости кровотока по средней (СМА), передней (ПМА) и позвоночной (ПА) мозговым артериям с преобладанием у больных II группы - по ПА и ЗМА (р< 0,05). После комплексной терапии полный контроль над припадками достигнут у 58,8% и 31,3%, сокращение числа приступов более чем на 75% - у 35,2% и 46,9% пациентов ВЭ I и II групп соответственно (р<0,05). По данным ЭЗГ зарегистрировано увеличение альфа-активности и снижения пароксизмальной активности (р<0,05), однако у больных II группы индекс пароксизмальной активности оставался повышенным (р<0,05). У пациентов I группы выявлена нормализация показателей асимметрии мозгового кровотока, у пациентов II группы сохранялась асимметрия показателей по ЗМА и ПА (р<0,05). Выявленная положительная динамика изученных показателей у больных ВЭ обосновывает включение со-судистой и антиоксидантной терапии в лечение эпилепсии

It is spent electroencephalographic (EEG) research, the computer analysis with definition of indexes of the basic rhythms, the vegetative status under the program of variability of a rhythm of heart is studied, functional classes (FC) on D.I. Zhemajtite are defined. At all patients changed types EEG prevailed. It is noted low FC at patients with I, II, V types ЭЭГ. Correlation of type EEG with level FC is revealed: at patients with V type EEG prevailed IV and V, with I and II types EEG — higher (II, III) FC. The high index of an alpha rhythm correlates with high II FC, and a beta rhythm with III FC. Decrease in adaptable mechanisms of vegetative regulation defines necessity of medicament correction.Обследовано две группы по 35 пациентов в возрасте от 32 до 52 лет, перенесших легкую боевую черепно-мозговую травму. Проведено электроэнцефалографическое (ЭЭГ) исследование, компьютерный анализ с определением индексов основных ритмов, изучен вегетативный статус по программе вариабельности ритма сердца, определены функциональные классы (ФК) по Д.И. Жемайтите. У всех больных преобладали измененные типы ЭЭГ. Отмечен низкий ФК у пациентов с I, II, V типами ЭЭГ. Выявлена корреляция типа ЭЭГ с уровнем ФК: у больных с V типом ЭЭГ преобладали IV и V, с I и II типами ЭЭГ — более высокие (II, III) ФК. Высокий индекс альфа-ритма коррелирует с высоким (II) ФК, а бета-ритма — с III ФК. Снижение адаптационных механизмов вегетативной регуляции определяет необходимость медикаментозной коррекции

Headaches and its dependence from different factors in patients with temporal epilepsy at interictal period

56 patients with temporal epilepsy (TE) at the age of (30.5 ± 10.0) years under the duration of the disease — (17.5 ± 10.7) years are examined. The examination included the anamnesis, somatic and neurological statuses, ultrasonic medical examination of brain vessels, the magneto-resonance tomography (MRI) of brain. Headaches (HA) among 80.4% patients with TE is revealed and its dependence from different factors is studied. The absence of gender differences, the dependence from the type TE, frequency of attacks and the duration of the disease among patients TE with HA are noticed, however the burdened neurological and obstetrical anamnesis, the presence HA among relatives, changes on data MRI of the brain, pathology of vertebral-basilar basin, the reduction and asymmetry of the index of the vessel resistance are observed. This amends the algorithm of the examination and treatment of patients with temporal epilepsy
