Falls prevention in community care: 10 years on
Background: A million older people living in Australia receive community care services each year due to experiencing functional or mental health difficulties. This group may be at greater risk of falling than similar-aged people not receiving services. However, there is limited falls prevention research for this population. Purpose: The aim of this study was to identify the falls prevalence rates of older people from 10 Australian community care organizations and compare current falls prevention data to a study 10 years prior that utilized the same 10 organizations. This study also identified factors associated with falling for this population. Patients and methods: This is a cross-sectional descriptive study, in which 5,338 questionnaires were mailed to a random sample of community care recipients aged $65 years. Results: A total of 1,991 questionnaires were returned (37.3%), with 47.7% of respondents having fallen in the previous year, and 32.7% in the month prior to completing the questionnaire, similar to 10 years prior. Community care clients had a 50% higher falls rate than that reported for similar-aged people not receiving services, and this remained unchanged over the last 10 years. Eighty-six per cent of fallers had fallen once or twice, and 60% reported being injured. Thirty-six per cent of respondents reported not being able to get up independently, and only 27.4% of fallers were referred to a falls prevention program (significantly fewer than 10 years ago; 95% CI: 0.821â6.366, p=0.01). Balance issues (odds ratio [OR]: 2.06, 95% CI: 1.288â3.290, p=0.003) and perceived risk of falling in the future being âdefiniteâ (OR: 6.42, 95% CI: 1.890â21.808, p=0.003) or âunsureâ (OR: 3.31, 95% CI: 1.144â9.544, p=0.027) were risk factors associated with falling. In contrast, individuals referred to a falls prevention intervention had a 47% reduced likelihood of having fallen (95% CI: 0.281â0.988, p=0.046). Conclusion: Community care clients should have their falls risk routinely assessed, and at-risk individuals be offered falls prevention advice and referral to fall prevention programs
Can community care workers deliver a falls prevention exercise program? A feasibility study
Background: Almost half of older people receiving community care fall each year and this rate has not improved in the last decade. Falls prevention programs targeted at this group are uncommon, and expensively delivered by university trained allied health professionals. Purpose: To investigate the feasibility of community care workers delivering a falls prevention exercise program to older clients, at low or medium risk of falling, as part of an existing service provision. Patients and methods: Community care workers from 10 community care organizations participated in the training for, and delivery to their clients of, an 8-week evidence-based falls prevention exercise program. Community care workers included assessment staff (responsible for identifying the need for community care services through completing an assessment) and support workers (responsible for providing support in the home). Clients were surveyed anonymously at the completion of the intervention and workers participated in a semi-structured interview. Results: Twenty-five community care workers participated in the study. The falls prevention program was delivered to 29 clients, with an average age of 82.7 (SD: 8.72) years and consisting of 65.5% female. The intervention was delivered safely with no adverse events recorded, and the eligibility and assessment tools were completed by the majority of community care workers (93.1%). Assessment staff found it difficult to find time to deliver the intervention. Support workers were able to complete the intervention within their current service delivery period, with the initial assessment taking a small amount of additional time. Support workers reported enjoying the additional responsibility afforded by delivering the falls prevention program and seeing changes in their clients. The majority of clients (82%) reported enjoying the exercises, with 59% reporting that they felt it made a positive change in their health. Cl ients completed the exercises on average 4.8 (SD: 2.2) days per week. Conclusion: Community care workers who have completed appropriate training are able to deliver a falls prevention exercise program to their clients as part of their current services. Further research is required to determine whether the program reduces the rate of falls for community care clients and whether integration of a falls prevention program into an existing service is cost-effective
Decoherence at zero temperature
Most discussions of decoherence in the literature consider the
high-temperature regime but it is also known that, in the presence of
dissipation, decoherence can occur even at zero temperature. Whereas most
previous investigations all assumed initial decoupling of the quantum system
and bath, we consider that the system and environment are entangled at all
times. Here, we discuss decoherence for a free particle in an initial
Schr\"{o}dinger cat state. Memory effects are incorporated by use of the single
relaxation time model (since the oft-used Ohmic model does not give physically
correct results)
Wigner Distribution Function Approach to Dissipative Problems in Quantum Mechanics with emphasis on Decoherence and Measurement Theory
We first review the usefulness of the Wigner distribution functions (WDF),
associated with Lindblad and pre-master equations, for analyzing a host of
problems in Quantum Optics where dissipation plays a major role, an arena where
weak coupling and long-time approximations are valid. However, we also show
their limitations for the discussion of decoherence, which is generally a
short-time phenomenon with decay rates typically much smaller than typical
dissipative decay rates. We discuss two approaches to the problem both of which
use a quantum Langevin equation (QLE) as a starting-point: (a) use of a reduced
WDF but in the context of an exact master equation (b) use of a WDF for the
complete system corresponding to entanglement at all times

LISSAT Analysis of a Generic Centrifuge Enrichment Plant
The U.S. Department of Energy (DOE) is interested in developing tools and methods for use in designing and evaluating safeguards systems for current and future plants in the nuclear power fuel cycle. The DOE is engaging several DOE National Laboratories in efforts applied to safeguards for chemical conversion plants and gaseous centrifuge enrichment plants. As part of the development, Lawrence Livermore National Laboratory has developed an integrated safeguards system analysis tool (LISSAT). This tool provides modeling and analysis of facility and safeguards operations, generation of diversion paths, and evaluation of safeguards system effectiveness. The constituent elements of diversion scenarios, including material extraction and concealment measures, are structured using directed graphs (digraphs) and fault trees. Statistical analysis evaluates the effectiveness of measurement verification plans and randomly timed inspections. Time domain simulations analyze significant scenarios, especially those involving alternate time ordering of events or issues of timeliness. Such simulations can provide additional information to the fault tree analysis and can help identify the range of normal operations and, by extension, identify additional plant operational signatures of diversions. LISSAT analyses can be used to compare the diversion-detection probabilities for individual safeguards technologies and to inform overall strategy implementations for present and future plants. Additionally, LISSAT can be the basis for a rigorous cost-effectiveness analysis of safeguards and design options. This paper will describe the results of a LISSAT analysis of a generic centrifuge enrichment plant. The paper will describe the diversion scenarios analyzed and the effectiveness of various safeguards systems alternatives

Modeling Efforts to Aid in the Prediction of Process Enrichment Levels with the Intent of Identifying Potential Material Diversion
As part of an ongoing effort at Lawrence Livermore National Laboratory (LLNL) to enhance analytical models that simulate enrichment and conversion facilities, efforts are underway to develop routines to estimate the total gamma-ray flux and that of specific lines around process piping containing UF{sub 6}. The intent of the simulation modeling effort is to aid in the identification of possible areas where material diversion could occur, as input to an overall safeguards strategy. The operation of an enrichment facility for the production of low enriched uranium (LEU) presents certain proliferation concerns, including both the possibility of diversion of LEU and the potential for producing material enriched to higher-than-declared, weapons-usable levels. Safeguards applied by the International Atomic Energy Agency (IAEA) are designed to provide assurance against diversion or misuse. Among the measures being considered for use is the measurement of radiation fields at various locations in the cascade hall. Our prior efforts in this area have focused on developing a model to predict neutron fields and how they would change during diversion of misuse. The neutron models indicated that while neutron detection useful in monitoring feed and product containers, it was not useful for monitoring process lines. Our current effort is aimed at developing algorithms that provide estimates of the gamma radiation field outside any process line for the purpose of determining the most effective locations for placing in-plant gamma-monitoring equipment. These algorithms could also be modified to provide both dose and spectral information and, ultimately, detector responses that could be physically measured at various points on the process line. Such information could be used to optimize detector locations in support of real-time on-site monitoring to determine the enrichment levels within a process stream. The results of parametric analyses to establish expected variations for several different process streams and configurations are presented. The benefits and issues associated with both passive and active interrogation measurement techniques are also being explored
Coherent Pair Production by Photons in the 20-170 GeV Energy Range Incident on Crystals and Birefringence
The cross section for coherent pair production by linearly polarised photons
in the 20-170 GeV energy range was measured for photon aligned incidence on
ultra-high quality diamond and germanium crystals. The theoretical description
of coherent bremsstrahlung and coherent pair production phenomena is an area of
active theoretical debate and development. However, under our experimental
conditions, the theory predicted the combined cross section and polarisation
experimental observables very well indeed. In macroscopic terms, our experiment
measured a birefringence effect in pair production in a crystal. This study of
this effect also constituted a measurement of the energy dependent linear
polarisation of photons produced by coherent bremsstrahlung in aligned
crystals. New technologies for manipulating high energy photon beams can be
realised based on an improved understanding of QED phenomena at these energies.
In particular, this experiment demonstrates an efficient new polarimetry
technique. The pair production measurements were done using two independent
methods simultaneously. The more complex method using a magnet spectrometer
showed that the simpler method using a multiplicity detector was also viable.Comment: 10 pages, 13 figures, 1 table, REVTeX4 two column, Version for

Linear to Circular Polarisation Conversion using Birefringent Properties of Aligned Crystals for Multi-GeV Photons
We present the first experimental results on the use of a thick aligned Si
crystal acting as a quarter wave plate to induce a degree of circular
polarisation in a high energy linearly polarised photon beam. The linearly
polarised photon beam is produced from coherent bremsstrahlung radiation by 178
GeV unpolarised electrons incident on an aligned Si crystal, acting as a
radiator. The linear polarisation of the photon beam is characterised by
measuring the asymmetry in electron-positron pair production in a Ge crystal,
for different crystal orientations. The Ge crystal therefore acts as an
analyser. The birefringence phenomenon, which converts the linear polarisation
to circular polarisation, is observed by letting the linearly polarised photons
beam pass through a thick Si quarter wave plate crystal, and then measuring the
asymmetry in electron-positron pair production again for a selection of
relative angles between the crystallographic planes of the radiator, analyser
and quarter wave plate. The systematics of the difference between the measured
asymmetries with and without the quarter wave plate are predicted by theory to
reveal an evolution in the Stokes parameters from which the appearance of a
circularly polarised component in the photon beam can be demonstrated. The
measured magnitude of the circularly polarised component was consistent with
the theoretical predictions, and therefore is in indication of the existence of

Results on the Coherent Interaction of High Energy Electrons and Photons in Oriented Single Crystals
The CERN-NA-59 experiment examined a wide range of electromagnetic processes
for multi-GeV electrons and photons interacting with oriented single crystals.
The various types of crystals and their orientations were used for producing
photon beams and for converting and measuring their polarisation.
The radiation emitted by 178 GeV unpolarised electrons incident on a 1.5 cm
thick Si crystal oriented in the Coherent Bremsstrahlung (CB) and the
String-of-Strings (SOS) modes was used to obtain multi-GeV linearly polarised
photon beams.
A new crystal polarimetry technique was established for measuring the linear
polarisation of the photon beam. The polarimeter is based on the dependence of
the Coherent Pair Production (CPP) cross section in oriented single crystals on
the direction of the photon polarisation with respect to the crystal plane.
Both a 1 mm thick single crystal of Germanium and a 4 mm thick multi-tile set
of synthetic Diamond crystals were used as analyzers of the linear
polarisation.
A birefringence phenomenon, the conversion of the linear polarisation of the
photon beam into circular polarisation, was observed. This was achieved by
letting the linearly polarised photon beam pass through a 10 cm thick Silicon
single crystal that acted as a "quarter wave plate" (QWP) as suggested by N.

Global identification helps increase identity integration among Turkish gay men
Globalisation provides novel contexts for individuals to express and transform their identities in ways that may not be available in their local cultures. For gay men living in cultures where traditional masculinity norms prescribe heterosexuality and the rejection of homosexuality, gay-male identity is inherently threatened. However, adopting an identity as a âglobal citizenâ may increase the compatibility between gay and male identities, and hence augment well-being. We conducted an experiment with a community sample of 220 gay men in Turkey, manipulating pro- and anti-globalisation world views. Priming with pro-globalisation world views increased peopleâs identification as global citizens, and thus indirectly led to higher gay-male identity integration. Identity integration, in turn, predicted higher subjective well-being. This study brings the first experimental evidence on the link between global identification and gay-male identity integration. Beyond its local focus on the cultural context of Turkey, it highlights the importance of an intersectional approach to studying social identities by showing how the compatibility of two social identities can be increased by adopting a third social identity
