    How Would Children Register Their Own Births? Insights from a Survey of Students Regarding Birth Registration Knowledge and Policy Suggestions in Kenya

    Birth registration and obtaining physical birth certificates impose major challenges in developing countries, with impact on child and community health, education, planning, and all levels of development. However despite initiatives, universal registration is elusive, leading to calls for new approaches to understanding the decisions of parents. In this paper, we report results of a survey of students in grades six to eight (age ~12-16) in an under-registered area of Kenya regarding their own understanding of registration issues and their suggestions for improvement. These students were selected because they themselves were also nearing the age for high school enrollment/entrance examinations, which specifically requires possession of a birth certificate. This assessment was also a companion to our previous representative survey of adults in the same Kenyan region, allowing for parent-child comparison. Results supported previous research, showing that only 43% had birth certificates. At the same time, despite these low totals, students were themselves quite aware of registration factors and purposes. The students also made quite prescient sources for understanding their households\u27 motivations, with many of their suggestions-for focus on communication of pragmatic benefits, or automatic measures shifting responsibility from parents-mirroring our own previous suggestions, and showing a level of pragmatism not witnessed when surveying their parents. This paper therefore adds evidence to the discussion of registration policy planning. More generally, it also builds on an important trend regarding the treatment of children as stakeholders and important sources of information, and raising an intriguing new avenue for future research

    Community perceptions and response to flood risks in Nyando District, Western Kenya

    In Kenya, the ability of local people to resist the impact of disasters has not been given adequate attention. A descriptive cross sectional study sought to investigate community perceptions and responses to flood risks in low and high risk areas of the Nyando District, Western Kenya. A total of 528 households, six government officials and have project managers of Community Based Organizations (CBOs) and Non Governmental Organizations (NGOs) were interviewed. Additionally, seven Focus Group Discussions(FGDs) involving three women, two male and two teacher groups were conducted. Data were analysed using the Statistical Package for the Social Sciences (SPSS) Program. The Chi-square test was used to determine associations and di'erences between variables. In the study, 83% of the respondents were aware of Traditional Flood Knowledge (TFK) and 80% acknowledged its use. Perception of the risk is influenced by several variables, most notably past experience of major floods and having survived them. Residents in the high risk areas had signfficantly higher levels of awareness and use of traditional flood knowledge. they were more aware of the nature of the flood related health risks they were exposed to and appeared better prepared for future flood risk. They were, however, more dependent on external aid.     Matching Development of Point-of-Care Diagnostic Tests to the Local Context: A Case Study of Visceral Leishmaniasis in Kenya and Uganda

    The rapid growth of point-of-care (POC) diagnostic tests necessitates a clear vision of when, where, and why a new POC diagnostic test needs to be developed and how it can be used in a way that matches a local health care context. Here, we present an innovative approach toward developing a concept target product profile (CTPP), which is a new mapping tool that helps researchers match a new diagnostic test to a specific local health care context early in the research and development process. As a case study, we focus on the diagnosis of visceral leishmaniasis (VL) in rural resource-limited regions of Kenya and Uganda. Our stepwise approach integrates elements of design thinking and uses a combination of literature reviews and field research for a context analysis of local health care systems and practices. We then use visual thinking in the form of Gigamaps and patient journeys to identify use case scenarios and to present our findings from the field research to key stakeholders. The use case scenarios describe the diagnostic scope of a new POC test based on the feasibility of the new test, the local need, and the contextual fit. For our case study of VL, we identify 2 valuable use case scenarios, namely test-of-cure and screening and confirmation, and we formulate a CTPP.
