Evaluarea metodelor de reabilitare a pacienților cu atrofie severă a maxilarului superior
Background. Insufficient bone volume is difficult to manage in rehabilitating patients with maxillary
atrophy. The currently available solutions are: increasing the bone amount followed by standard
implantation or the alternative method by using zygomatic implants without preimplantation bone
growth. Objective of the study. Comparative evaluation of standard versus alternative methods of
rehabilitation in patients with severe maxillary atrophy. Material and Methods. Study was axed on 12
patients, aged 32 - 67 years, mean age 54.5 years, divided into 2 groups. Patients in group I (7) were
rehabilitated by zygomatic implants without bone grafting, and patients in group 2 (5) were rehabilitated
by bone grafting, with delayed implantation. Patient data was processed in Sidexis 4.2 and Microsoft
Excel Results. In group I, 4.42 implants / patient were used, 1 intervention was performed under general
anesthesia, 1 under local anesthesia with intravenous sedation and 5 under local anesthesia only. The
patients underwent a single surgery with an average duration of 134 minutes and the functional
rehabilitation of the patients lasted an average of 6.28 days. In group II, 8.4 implants / patient were used,
all 5 bone grafting procedures were performed under general anesthesia, and subsequent interventions
were performed under local anesthesia, the average summary time of surgery was 385 minutes. The
functional rehabilitation lasted on average 12.8 months. Conclusion. The study demonstrated the
effectiveness of both methods, but showed that the alternative rehabilitation method can be performed
in a shorter time, with a smaller number of implants, mostly under local anesthesia, in a single surgery
with a shorter duration.
Introducere. Insuficiența ofertei osoase reprezintă o dificultate în reabilitarea pacienților cu atrofie
maxilară. Actualmente, soluțiile disponibile sunt: creșterea ofertei osoase cu implantare standard
ulterioară sau metoda alternativă prin utilizarea implanturilor zigomatice fără creștere osoasă
preimplantară. Scopul lucrării. Evaluarea comparativă a metodelor standarde versus alternarive de
reabilitare a pacienților cu atrofie severă maxilară. Material și Metode. Studiul a cuprins 12 pacienți,
cu vârsta 32 - 67 ani, vârsta medie 54,5 ani, repartizați în 2 loturi. Pacienții lotului I (7), au fost reabiliați
prin intermediul implanturilor zigomatice fără creștere osoasă preimplantară, iar pacienții lotului 2 (5)
au fost reabilitați prin creșterea osoasă, cu implantare amânată. Datele pacienților au fost procesate în
programul Sidexis 4.2 și Microsoft Excel. Rezultate. În lotul I, s-au utilizat 4,42 implanturi/pacient, 1
intervenție a fost efectuată sub anestezie generală, 1 sub anestezie loco-regională cu potențiere
intravenos, 5 sub anestezie loco-regională. Pacienții au fost supuși unei singure intervenții chirurgicale
cu o durată medie de 134 de minute, iar reabilitarea funcțională a pacienților a durat în mediu 6,28 zile.
În lotul II, s-au utilizat 8,4 implanturi/pacient, toate 5 intervenții de creștere osoasă au fost realizate sub
anestezie generală, iar intervențiile ulterioare au fost realizate sub anestezie locală, timpul sumar mediu
al intervențiilor chirurgicale a constituit 385 de minute. Reabilitarea funcțională a durat în mediu 12,8
luni. Concluzii. Studiul a demonstrat eficiența ambelor metode, însă a evidențiat că metoda de
reabilitare alternativă poate fi realizată într-un timp mai scurt, cu un număr mai mic de implanturi,
preponderent sub anestezie locală, într-o singură intervenție chirurgicală cu o durată mai mică
Therapeutic aproach in periimplantitis
Rezumat.
Periimplantita este întâlnită frecvent şi
prezintă o maladie polietiologică. Actualmente tratamentul acesteia este o provocare,
algoritmele de tratament fiind deocamdată
neclare. Scopul studiului: Evaluarea factorilor etiologici a complicaţiilor în reabilitarea
implanto-protetică şi analiza comparativă a
metodelor de management disponibile. Materiale şi metode: În studiu au fost incluşi 47 de
pacienţi şi 108 implante. Parametrii de interes
au fost: sexul, vârsta pacienţilor, numărul implantelor afectate, localizarea periimplantitei
după localizarea dinţilor şi repartizarea pe
maxilare, posibili factori etiologici, repartizarea defectelor după Schwarz, nivelul rezorbţiei osoase, tratamentul aplicat. Rezultate: Din
34 de implante tratate, s-a putut observa o
ameliorare în 6 cazuri, iar din cele 72 de implante extrase s-au obţinut condiţii favorabile
în toate cazurile. Prioritatea este acordată păstrării maxime de os şi posibilităţii replantării
ulterioare. Tratamentul chirurgical radical în
defectele extinse manifestă o predictibilitate
mai mare decât cel conservativ.Summary.
Peri–implantitis is a frequent a polyetiological disease. Currently its treatment is a
challenge, the treatment algorithms still being unclear. Purpose of the study: Evaluation
of etiological factors of complications in implant–prosthetic rehabilitation and comparative analysis of available management methods. Materials and methods: 47 patients and
108 implants were included in the study. The
parameters of interest were: gender, age of the
patients, number of affected implants, spreading of peri–implantitis according to the teeth
position and distribution between the jaws,
possible etiological factors, distribution of defects according to Schwarz classification, level
of bone resorption, applied treatment. Results:
Among 34 treated implants, an improvement
was achieved in 6 cases, among 72 extracted
implants favourable conditions were obtained
in all cases. Priority is given to maximum bone
preservation and the possibility of subsequent
replantation. Radical surgical treatment in
extensive defects shows greater predictability
than conservative treatment
Periimplantita: explantarea versus regenerarea periimplantară

masticatory and aesthetic disorders. Depending on the severity of the condition can be
addressed by conservative surgical or radical treatment. Objective of the study. Comparative analysis of
conservative surgical methods (implant preservation) versus radical (explantation). Material and
Methods. Were studied 2 patients who addressed to the SRL "Omni Dent" with periimplantitis. The
manifestation, treatment, evolution in the patient RI m/63 years with periimplantitis at Im 2.5 and the
patient TM w/33 years at Im 3.2 were compared. Analysis criteria: extension of bone defect, appearance
of soft tissues, tissue regeneration, implanto-prosthetic rehabilitation duration. Results. RI patient had
changes in color and appearance of peri-implant soft tissues (hyperemia, swelling, purulent discharge),
peri-implant radiolucency of the entire implant surface; TM patient had the same clinical signs limited to
½ implant length. The RI patient underwent explantation with reimplantation in the neighboring region
2.4 after 4 months after another 4 months prosthetic reabilitation. No complications in postoperative tissue
regeneration were observed. The TM patient was resorted to keeping the implant with curettage,
implantoplasty, bone addition. No complications in the prosthetic rehabilitation were found exept the
exposure of the crestal part of the implant. Conclusion. Early addressing would prevent the development
of complications. The trivial loss of the peri-implant tissues can be recovered with the preservation of the
implant; in the extended defects the optimal solution is the explantation with the subsequent implantprosthetic
rehabilitation.Introducere. Periimplantita este o complicație în reabilitarea implanto protetică care provoacă dereglări
morfo-funcționale, masticatorii și estetice. În funcție de gravitatea afecțiunii, poate fi abordată prin
tratament conservativ chirurgical sau radical. Scopul lucrării. Analiza comparativă a metodelor
conservativ chirurgicale (păstrarea implantului) versus radicale (explantarea). Material și Metode. S-au
studiat 2 pacienți, care s-au adresat la clinica SRL Omni Dent cu periimplantită. S-au comparat
manifestarea, tratamentul, evoluția la pacientul RI b/ 63 ani cu periimplantită la Im 2.5 si pacientul TM f/
33 ani la Im 3.2. Criteriile de analiză au fost: extinderea defectului osos, aspectul tesuturilor moi,
regenerarea tisulară, timpul de reabilitare implanto-protetică. Rezultate. La pacientul RI au fost modificări
de culoare și aspect al țesuturilor moi periimplantare (hiperemie, edem, eliminari purulente) şi
radiotransparență periimplantară pe toata suprafața implantului; la pacientul TM au fost aceleași semne
clinice, însă limitate la ½ lungime implant. La pacientul RI s-a efectuat explantarea cu reimplantarea în
regiunea învecinată 2.4 peste 4 luni, cu protezare peste alte 4 luni. Regenerarea tisulară postoperatorie fără
complicații. La pacientul TM s-a recurs la păstrarea implantului cu chiuretaj, implantoplastie, adiție osoasă.
Reabilitarea protetică urmată ulterior fară complicații însă, cu expunerea porțiunii crestale a implantului.
Concluzii. Adresarea la timp ar preîntampina dezvoltarea complicațiilor cu pierderea implantelor.
Pierderea neînsemnată a țesuturilor periimplantare poate fi recuperată cu păstrarea implantului, în defectele
extinse soluția optimală este explantarea cu reabilitarea implanto-protetică ulterioară
Dental extraction - imminent solution in crown-root fracture – case report

Pharmacy, Chisinau, Republic of Moldova, The 8th International Medical Congress for Students and Young Doctors, September 24-26, 2020Background. Complicated crown-root fractures represent 5-8% of all dento-periodontal cases
of trauma, some being on the borderline between therapeutic conservative and radical surgical
treatment. Despite being the last option in any treatment method, tooth extraction serves in
some cases as the only solution to improve the clinical situation.
Case report. Patient B.A. addressed 11 months ago with the following complains: pain and
tooth 21 mobility as a result of a trauma produced at home. Initially the goal was to preserve
the tooth, so we opted for and performed endodontic treatment and rigid immobilization. The
patient was monitored in time, but came again with the same accusations, and more than that,
with the presence of fistula with periodic purulent eliminations in the projection of the tooth
21 apex. By the clinical examination and the analysis of the complementary radioimaging
(OPG, CBCT), the diagnosis established was: Crown-root fracture of the tooth 21. Chronic
granulomatous periodontitis of the tooth 21 associated with fistula at the level of the apex
projection. As a result, the extraction of both the coronal and root fragments was performed,
the post-extraction alveolar socket was examined, noting the absence of the vestibular wall
(type 3 alveolar socket). The creation of the Vascularized Interpositional Periostal Connective
Tissue flap (VIP-CT) from the palate (single incision technique – Zuhr), allowed to omit the
use of other types of bone grafts to cover and restore the bone defect of the vestibular wall.
After the suture of the post-extractional alveolar socket and of the palatal flap, a Maryland
temporary bridge was made. Subsequently, dental implantation was planned using the virtual
surgery technique, with the creation of the template for guided insertion of the implants
according to the "fully guided" method. The simulation of the surgery was performed on the
3D printed composite models, at the same time making the provisional crown. After the period
of 10 months since the tooth extraction, type 4 postponed implantation was performed with the
use of the connective tissue graft taken from the tuberosity, followed by the immediate loading
of the implant with the screw retained temporary crown.
Conclusions. A good result was achieved, the dental extraction proving to be in this case the
safest and most effective method of treatment, and the regeneration potential of the body
allowed to rectify the defect even without the use of additional biomaterials

implantelor zigomatice cu analiza eficacităţii
metodei. Prin prisma unui caz clinic de reabilitare implanto-protetică după protocolul
„Quad Zygoma“, pacienta A.M., cu vîrsta 58
ani, la care s-a stabilit diagnosticul „Edentaţie parţială subtotală cl. I Kennedy. Atrofie
osoasă clasa D Misch în regiunea anterioară şi
SA-4 Misch în sectoarele posterioare ale maxilei. Parodontită cronică generalizată forma
medie-avansată“, a fost demonstrată eficienţa

THE USE OF PALATAL GRAFT IN THE TREATMENT OF PERI-IMPLANT DEFECTS

În urma intervenției de implantare dentară, datorită unor particularități individuale, exista riscul apariției defectelor de țesut moale în jurul implantului. Acestea ulterior pot provoca consecințe atât estetice, cât și funcționale. În literatură, au fost descrise diverse metode de tratament, una din ele fiind adiția de țesut moale cu grefă palatinală. Scopul. Aprecierea eficacității tratamentului de plastie gingivală peri-implantar utilizând grefa palatinală. Materiale și metode. Studiul dat a fost realizat pe un eșantion de 10 pacienți cu vârste cuprinse între 34-63 de ani care, necesitând efectuarea tratamentului de plastie/ augmentare gingivală, s-au prezentat în cadrul clinicii stomatologice „Omni Dent” în perioada anilor 2022-2023. În urma examenului clinic și paraclinic, s-a recurs la intervenția propriu-zisă de prelevare a grefei palatine, atașarea pe conformator și suturarea acesteia în zonele recipiente. Rezultate. Procedura de plastie gingivala cu grefă de pe palat a rezultat în obținerea semnificativă de țesut keratinizat peri-implantar. Astfel, grosimea gingiei în jurul implantului pre-operator reprezenta valori medii de la 3mm la 6mm, iar lățimea vestibulo-orală în mediu 8mm. Post-operator, s-a observat o creștere însemnată a țesutului moale peri-implantar, cu 6 mm mai mult în sens vestibulo-oral și cu aproximativ 5 mm adaos în grosime, echivalent a 40% mai mult țesut moale comparativ situației inițiale. Concluzii. În urma analizelor efectuate, s-a constatat o creștere semnificativă în lățime cât și în grosime a țesutului moale peri-implantar.Introduction. Following dental implant surgery, due to individual particularities, there is a risk of soft tissue defects around the implant. These can subsequently cause both aesthetic and functional consequences. Various treatment methods have been described in the literature, one of which is soft tissue addition with palatal grafting. Purpose. To assess the efficacy of peri-implant gingival plasty treatment using palatal grafting. Materials and methods. This study was performed on a sample of 10 patients aged 34-63 years who, needing gingival plasty/augmentation treatment, visited dental clinic ,,Omni Dent” in the period 2022-2023. Following clinical and paraclinical examination, the procedure was carried out to harvest the palatal graft, attach it to the conformer and suture it in the recipient areas. Results. The gingival graft-palatal placement procedure resulted in significant peri-implant keratinized tissue gain. Thus, gingival thickness around the preoperative implant represented mean values from 3mm to 6mm, and vestibulo-oral width averaged 8mm. Postoperatively, a significant increase in peri-implant soft tissue was observed, with 6 mm more in the vestibulo-oral dimension and about 5 mm more in thickness, equivalent to 40% more soft tissue compared to the initial situation. Conclusions. Based on the performig analyses, a significant increase in both width and thickness of peri-implant soft tissue was observed
