Expression analysis of microRNAs in patients with hypertrophic cardiomyopathy
microRNAs (miRNAs) are small noncoding RNAs with a crucial role in the post-transcriptional regulation of gene expression. Several studies have shown that miRNAs are involved in the pathogenesis of cardiovascular diseases. Therefore, the potential use of miRNAs as biomarkers or targets for new therapies for a variety of cardiovascular diseases are areas of tremendous interest. The aim of this study was to measure plasma levels of miR-146a, miR-29a, miR-181c, miR-199a, miR-208a και miR-208b in patients with hypertrophic cardiomyopathy and normal controls and to correlate possible alterations in plasma miRNA levels with certain clinical and laboratory data from those patients. miR-146a-5p levels were analyzed in plasma samples from 50 patients with hypertrophic cardiomyopathy and 30 controls using Real time qPCR and specific TaqMan assays. Relative quantitation was performed using the 2-ΔΔCt method. The results of this study have shown that plasma levels of miR-146a was increased in patients with hypertrophic cardiomyopathy. Additionally, plasma levels of miR-146a correlate significantly with the presence of left ventricle outflow tract obstruction. The case of miR-29a underscores that hemolysis is a factor that could lead to pre-analytical errors in miRNA determination in plasma.Τα microRNAs (miRNA) αποτελούν μικρά μη κωδικά μόρια RNA με σημαντικό ρόλο στη ρύθμιση της γονιδιακής έκφρασης σε μετα-μεταγραφικό επίπεδο. Πρόσφατα δεδομένα αναδεικνύουν ότι τα miRNAs εμπλέκονται στην παθογένεση αρκετών νοσημάτων του καρδιαγγειακού συστήματος. Στα πλαίσια αυτά υπάρχει έντονο ερευνητικό ενδιαφέρον για την πιθανή μελλοντική χρήση τους ως βιοδεικτών ή νέων θεραπευτικών στόχων. Σκοπός της παρούσας μελέτης ήταν να διερευνήσει τις μεταβολές στα επίπεδα των miR-146a, miR-29a, miR-181c, miR-199a, miR-208a και miR-208b στο πλάσμα ασθενών με υπερτροφική μυοκαρδιοπάθεια σε σχέση με υγιείς καθώς και να διερευνήσει την ύπαρξη συσχετίσεων μεταξύ των μεταβολών αυτών με κλινικά και εργαστηριακά δεδομένα. Στη μελέτη συμπεριελήφθησαν 50 ασθενείς με υπερτροφική μυοκαρδιοπάθεια καθώς και 30 υγιείς μάρτυρες. H μέτρηση των επιπέδων των miRNA έγινε με αλυσιδωτή αντίδραση πολυμεράσης πραγματικού χρόνου με τη χρήση ειδικών ιχνηθετών Taqman. Για τη σχετική ποσοτικοποίηση ακολουθήθηκε η μέθοδος 2-ΔΔCt. Τα αποτελέσματα της μελέτης αυτής έδειξαν ότι οι ασθενείς με υπερτροφική μυοκαρδιοπάθεια παρουσιάζουν αύξηση των επιπέδων του miR-146a και μάλιστα η αύξηση αυτή σχετίζεται με την παρουσία απόφραξης στο χώρο εξόδου της αριστερής κοιλίας. Τέλος η μελέτη του miR-29a αναδεικνύει ότι η ύπαρξη αιμόλυσης αποτελεί ένα σημαντικό παράγοντα που μπορεί να επηρεάσει τη μέτρηση των επιπέδων των miRNA στο πλάσμα και να οδηγήσει σε εσφαλμένα αποτελέσματα
Routine histopathology of septal myectomy for hypertrophic obstructive cardiomyopathy in a greek cohort
Hypertrophic cardiomyopathy (HCM) is a
diverse inherited disease affecting 1 in 500 individuals
irrespective of gender and ethnicity. A fraction of HCM
patients will eventually develop drug refractory dynamic
obstruction of the left ventricular outflow tract. For such
patients, septal myectomy is the procedure of choice to
alleviate their symptoms and improve their quality of
life. The current histopathological study, the first from
the Greek region, aims to examine the hallmark
histopathological characteristics of Hypertrophic
Obstructive Cardiomyopathy in a population of patients
undergoing septal myectomy at a single center over a ten
year period. Medical records and histopathology
specimens of thirty nine (n=39) patients were evaluated.
The sample comprised 22 males (56.4%) and 17 females
(43.6%). Mean patient age at myectomy was 53.9±16.7
years, ranging from 12 to 79 years. Maximal IVS
thickness on echocardiography was available for 35
patients with a median value of 2.08cm. Peak resting
LVOT Pressure Gradient was available for 33 patients
with a mean value of 104.88±44.20 mmHg. Central
tendency of each histopathological attribute expressed as
the median value was: moderate for myocyte
hypertrophy, mild for cytoplasmic vacuolization,
moderate for subendocardial fibrosis, moderate for
interstitial fibrosis, mild for replacement fibrosis,
moderate for myofibrillar disarray and mild for capillary
stenosis. Myocyte hypertrophy, present in all specimens,
was positively correlated with maximal IVS thickness
(tau-b=0.43, p=0.002). Replacement fibrosis was
positively correlated with the grade of microvascular
stenosis (tau-b=0.45, p=0.004). LVEF was negatively
correlated with the grade of interstitial fibrosis (taub=−0.43, p=0.035) and with the extent of myocardial
fiber disarray (tau-b=−0.42, p=0.034). Histopathological
attributes were not correlated with patient gender or age
thus proving that HCM has a histological phenotype
unique to each patient, mainly depending on each
specific sarcomeric mutatio
