Tedaviye Dirençli Depresyon Tedavisi
Depresyon en sık gözlenen psikiyatrik hastalık olup tedavi edilmediğinde çok ciddi toplumsal
ve ekonomik sorunlara yol açmaktadır. Depresyon tanısı ile takip edilen hastaların neredeyse yarısı
ilk başlanan antidepresan ilaç tedavisine yanıt vermemektedir. Hastaların %20’si hiçbir tedavi
yönteminden fayda görmeyerek kronikleşmektedir. Tedaviye Dirençli Depresyon (TDD), en az
2 farklı antidepresan ilacın yeterli süre ve yeterli dozda kullanılmasına rağmen depresif semptomların
devam etmesi olarak tanımlanmaktadır. Depresyon süresinin ve başarısızlıkla sonuçlanan
tedavi girişimlerinin sayısının gelecekteki tedaviden yanıt alma oranını düşürdüğü
değerlendirildiğinde TDD tedavisinde hedef en erken evrede klinik olarak düzelmeyi sağlamak
olmalıdır. TDD olgularında halen fikir birliğine varılmış standart bir tedavi yaklaşımı bulunmamaktadır.
Bu makalede tedaviye dirençli olarak değerlendirilen olguların tedavi seçenekleri, antidepresan
optimizasyonu, başka bir antidepresana geçiş, diğer farmakolojik ajanlar ile tedaviyi
güçlendirme ya da kombinasyon, psikoterapi uygulamaları, somatik tedaviler ve psikocerrahi tedavi
seçenekleri üzerinden gözden geçirilecektir

Aim of the study is to investigate the referral type of dependent patients and its effect on treatment outcomes. This retrospectively
designed study was conducted at a private hospital. The sample included 323 patients, and all patients’ records were evaluated
according to their referral type, sociodemographic features, criminal history, relapse rates and accompanying axis II disorders. Patients
were reassessed six months after their discharge by semi-structured face-to-face or phone interviews with the patient or a family
member. There were significant differences in some of sociodemographic characteristics, presence of criminal records, substance use
patterns and relapse rates between voluntary inpatients and coerced inpatients. These results indicate a benefit in family participation at
the very early stages of dependency treatment. Prospective studies are needed to evaluate whether family participation at the beginning
of dependency treatment contributes to prognosis and patient’s motivatio
Assessment of the effects of CNR1, FAAH and MGLL gene variations on the synthetic cannabinoid use disorder
Given that drug addiction occurs as a result of complex gene-environment interaction, a number of studies claimed that cannabinoid receptor 1 (CNR1), fatty acid amide hydrolase (FAAH), and monoacylglycerol lipase (MGLL) single nucleotide polymorphisms (SNPs) are associated with the risk of substance use disorders such as cannabis, opioids, and, methamphetamine. However, scientific research on genetic susceptibility to synthetic cannabinoid addiction is limited. In this population-based case-control study, we aimed to evaluate the genetic susceptibility to synthetic cannabinoid use disorder in terms of these three endocannabinoid system genes in the Turkish population
