    We report a case of a rapidly progressing giant retroperitoneal liposarcoma weighing 22 kg in a 41-year-old Japanese man, successfully treated with surgical excision. To our knowledge, this is the largest liposarcoma in the Japanese population reported in the literature

    Kidney Transplantation at Hirosaki Kidney Transplant Unit-Initial 5-Year Experience

    Introduction Kidney transplantation has been widely accepted as a definitive therapy for patients with renal failure. The Hirosaki Kidney Transplant Unit was organized in June 2006 in conjunction with the Departments of Urology, Cardiology, Respiratory Medicine and Nephrology, and Gastrointestinal Surgery, from the Hirosaki University School of Medicine. Herein, we introduce our current results and discuss our future strategies. Patients and Methods From June 2006 to December 2011, 36 kidney transplants were performed with 31 living donors and 5 deceased donors. Immunosuppression therapy included an inductor treatment of anti-CD25 antibody and triple therapy with calcineurin inhibitor, mycophenolate mofetil, and steroids. Results Recipients included 25 males and 11 females. The patients’ average age was 41.8 years. Nine living–pairs were ABO incompatible. Deceased donors were performed at Oyokyo Hospital. Median follow-up period was 27.6 months. Acute cellular rejection occurred in 8.3% of patients. Positive antigenemia for cytomegalovirus happened in 16.7% of patients, but none developed invasive diseases. All recipients are currently surviving. Graft survival rates at 1, 3, and 5 years are 100%, 94.7%, and 94.7%, respectively. Conclusion Successful kidney transplantations have been performed by a multidisciplinary unit at Hirosaki University.     Efficacy of SMART Stent Placement for Salvage Angioplasty in Hemodialysis Patients with Recurrent Vascular Access Stenosis

    Vascular access stenosis is a major complication in hemodialysis patients. We prospectively observed 50 patients in whom 50 nitinol shape-memory alloy-recoverable technology (SMART) stents were used as salvage therapy for recurrent peripheral venous stenosis. Twenty-five stents each were deployed in native arteriovenous fistula (AVF) and synthetic arteriovenous polyurethane graft (AVG) cases. Vascular access patency rates were calculated by Kaplan-Meier analysis. The primary patency rates in AVF versus AVG at 3, 6, and 12 months were 80.3% versus 75.6%, 64.9% versus 28.3%, and 32.3% versus 18.9%, respectively. The secondary patency rates in AVF versus AVG at 3, 6, and 12 months were 88.5% versus 75.5%, 82.6% versus 61.8%, and 74.4% versus 61.8%, respectively. Although there were no statistically significant difference in patency between AVF and AVG, AVG showed poor tendency in primary and secondary patency.     Effects of treadmill running on anxiety- and craniofacial pain-like behaviors with histone H3 acetylation in the brain of mice subjected to social defeat stress

    This study examined the effects of treadmill running (TR) regimens on craniofacial pain- and anxiety-like behaviors, as well as their effects on neural changes in specific brain regions of male mice subjected to repeated social defeat stress (SDS) for 10 days. Behavioral and immunohistochemical experiments were conducted to evaluate the impact of TR regimens on SDS-related those behaviors, as well as epigenetic and neural activity markers in the anterior cingulate cortex (ACC), insular cortex (IC), rostral ventromedial medulla (RVM), and cervical spinal dorsal horn (C2). Behavioral responses were quantified using multiple tests, while immunohistochemistry measured histone H3 acetylation, histone deacetylases (HDAC1, HDAC2), and neural activity markers (FosB and phosphorylated cAMP response element-binding protein (pCREB). The effects of both short-term TR (2 days, TR2) and long-term TR (10 days, TR10) regimens were conducted. TR10 significantly reduced anxiety- and formalin-evoked craniofacial pain-like behaviors in SDS mice. It normalized SDS-induced increases in histone H3 acetylation in both the anterior and posterior portions of the ACC, as well as the anterior portion of the IC. These inhibitory effects were also observed in SDS-related increases in HDAC1, FosB, and pCREB expression. Additionally, TR10 normalized increased histone H3 acetylation in the RVM and C2 regions, with specific effects on FosB and pCREB expression observed in the C2 region. In contrast, TR2 showed limited effects on craniofacial pain-like behaviors but reduced anxiety-like behaviors in SDS mice. Under sham conditions, TR2 had minimal impact on histone H3 acetylation. Paradoxically, TR2 increased formalin-evoked craniofacial pain-like behaviors during the early phase despite not altering acetylated histone H3 expression.     Comparison of Three Alternatives for Living Donor Nephrectomy

    Background. We re-started living kidney transplant in June 2006 after 20 years of interval at Hirosaki University Hospital. We sequentially changed procedure of living donor nephrectomy. First, donor nephrectomy was carried out by open surgery with a small incision (10 cm). We then introduced laparoscopic retroperitoneal approach followed by laparoscopic intraperitoneal approach. Here, operative and clinical outcome of the three alternatives of donor nephrectomy was evaluated. Methods. Twenty-seven cases who underwent living donor nephrectomy between June 2006 and July 2011 at Hirosaki University Hospital were enrolled. There were 12 males and 15 females with a mean age of 54.3 years. We performed 7 open donor nephrectomy with a small incision, 5 laparoscopic retroperitoneal donor nephrectomy, and 15 laparoscopic intraperitoneal donor nephrectomy. Results. Operation time was 155 min., 250 min., and 214 min., respectively. Estimated blood loss was 310 g, 100g, and 95g, respectively. Warm ischemic time was 2.0 min., 3.0 min., and 3.0 min., respectively. Patients who underwent laparoscopic donor nephrectomy cloud start to walk significantly earlier than those who underwent open donor nephrectomy. There were no surgical complications, nor conversion to open donor nephrectomy from laparoscopic donor nephrectomy. There were no significant difference with post-operative serum creatinine concentration in the donors among the three operation procedures. Conclusions. Safety procedure is the most important for living donor nephrectomy. Laparoscopic donor nephrectomy is safe and associated with early recovery of the donors.     Thimerosal modulates the agonist-specific cytosolic Ca2+ oscillatory patterns in single pancreatic acinar cells of mouse

    AbstractModulation of the agonist-specific cytosolic Ca2+ oscillatory pattern by thimerosal has been investigated in single pancreatic acinar cells using patch-clamp perforated whole-cell recording to measure the calcium-dependent chloride current (IC1(Ca2+)). 1 μM thimerosal, which fails to evoke Ca2+ oscillation f alone, clearly changed the pattern of Ca2+ oscillation from pulsatile spikes (evoked by low concentrations of activators) to sinusoidal or transient oscillations. The mimetic action of thimerosal was independent of extracellular Ca2+, was blocked by extracellular application of dithiothreitol or 10 mM caffeine, as well as by internal perfusion with heparin; but was unaffected by ruthenium red. We conclude that thimerosal modulates the agonist-specific cytosolic Ca2+ oscillatory patterns mediated by sensitizing the InsP3-induced Ca2+ release
