    Determination and prioritization of eco-park components for sustainable urban development

    BACKGROUND AND OBJECTIVES: Environmental sustainability is an important program of the United Nations to achieve sustainable urban development. It is important to understand the complicated relationship between the urban ecosystem and the natural environment to solve the environmental problems of cities. Owing to its ecological origin, the establishment of ecological parks plays an essential role in sustainable urban development as it aims to support the spiritual needs of citizens, advocate the life of other species, and minimize the manipulation of nature. This study aims to identify and prioritize important ecological, physical, economic, and social criteria in the eco-park of north-central Iran. Due to its topographic features and specific climate, it provides different habitats for wildlife and has ability to protect biodiversity in the urban ecosystem.METHODS: There are currently intentions for biodiversity compensation in urban planning due to public opinion and species extinction problems. However, scarce studies are available to reinforce this fundamental concept in urban management. This study aimed to determine and prioritize Alborz eco-park components in north-central Iran. In this research, the indicators were selected by fuzzy Delphi, using multivariate decision-making, entropy technique, and TOPSIS. FINDINGS: Based on the TOPSIS method, among environmental-ecological criteria, the highest rank belonged to the ex situ conservation of animals in the urban climate and biodiversity enhancement (relative closeness to ideal solution = 0.621). Eco-park establishment along the Chalus road and Hemmat superhighway was the design and physical criterion with the highest score (relative closeness to ideal solution = 0.696). From the socio-economic view, increasing the understanding of human-nature unity was the highly scored criterion (relative closeness to ideal solution = 0.767).CONCLUSION: The eco-Park is not only about conservation but also about large-scale restoration of nature. The implementation of this new generation of parks in response to local environmental concerns regarding location, physical aspects, design, and wildlife conservation will help transition to sustainable urban development. The north-central eco-park of Iran can be used as a model for the development of sustainable urban ecosystems

    "nBackground: Preeclampsia, a specific syndrome in pregnancy, may summits mortality or morbidity in mother and fetus. Diagnostic methods are based on 24 hours urine protein measures which may be tedious, thus it is desirable to apply a faster and more applicable method for this goal. In this study we evaluate measurement of 8-hours urine protein in order to estimate 24-hours urine protein measure. "n"nMethods: Fifty pregnant women were entered in a cross sectional study in Vali-e-asr hospital located in Tehran- Iran, during 2008-2009. A urine sample was given after 8-hours and urine volume as well as protein levels were calculated and compared with the same values of 24-hours urine measures. Other necessary data was obtained by history taking and physical examination as well as using patient's medical records. "n"nResults: Mean of age was 27.5±5.4 years and mean of gestational age was 30.5±4 weeks. Mean of urine protein in 8 and 24 hours urine protein were 79±119 and 532±1218 mg respectively. Sensitivity, specificity, positive predictive value and negative predictive value of 8 hours urine protein were 61%, 98%, 88% and 90% respectively, in order to diagnosis of 24 hours urine protein to consider cut off point 105 mg for 8 hours urine protein. Mean protein levels were significantly higher in group with proteinuria ≥300 mg/24h in these two types of urine samples (p< 0.001). "n"nConclusion: Sensitivity of 8-hours urine protein is low but its specificity is suitable for normal mothers. We offer measuring of 8-hours urine protein as a valuable method for diagnosis of preeclampsia

The aim of this study was to compare the efficacy and safety of administration of a single dose of 25 mg vaginal Misoprostol (A) with 100 mg Misoprostol (B) for cervical ripening."n"nMethods: Sixty subjects with pregnancies of at least 40 gestation weeks, and an unfavorable cervix (Bishop Score <6) were randomly assigned to receive one dose oral misoprostol 100 mg or 25 mg vaginal misoprostol. The main dependent variables were mode of delivery, time interval from induction to cervical ripening, need for additional oxytocin, perinatal results and maternal side effects. "n"nResults: The mean time from the indication of induction to cervical ripening was the same in the two groups (3.63 h in oral group and 3.9 h in vaginal group). Two groups did not differ statistically in the percent of patients who were delivered vaginally or with cesarean section during 24 hours (20% in group A versus 10% in group B)."n"nConclusion: Based on the results of our study, Misoprostol is an effective drug for the cervical ripening and labor induction in the term pregnancies. 25 mg vaginal tablet of Misoprostol is as effective as 100 mg oral misoprostol for cervical ripening

    Aims The European Society of Cardiology (ESC) EURObservational Research Programme (EORP) Non-ST-segment elevation myocardial infarction (NSTEMI) Registry aims to identify international patterns in NSTEMI management in clinical practice and outcomes against the 2015 ESC Guidelines for the management of acute coronary syndromes in patients presenting without ST-segment-elevation. Methods and results Consecutively hospitalised adult NSTEMI patients (n = 3620) were enrolled between 11 March 2019 and 6 March 2021, and individual patient data prospectively collected at 287 centres in 59 participating countries during a two-week enrolment period per centre. The registry collected data relating to baseline characteristics, major outcomes (inhospital death, acute heart failure, cardiogenic shock, bleeding, stroke/transient ischaemic attack, and 30-day mortality) and guideline-recommended NSTEMI care interventions: electrocardiogram pre- or in-hospital, prehospitalization receipt of aspirin, echocardiography, coronary angiography, referral to cardiac rehabilitation, smoking cessation advice, dietary advice, and prescription on discharge of aspirin, P2Y12 inhibition, angiotensin converting enzyme inhibitor (ACEi)/angiotensin receptor blocker (ARB), beta-blocker, and statin. Conclusion The EORP NSTEMI Registry is an international, prospective registry of care and outcomes of patients treated for NSTEMI, which will provide unique insights into the contemporary management of hospitalised NSTEMI patients, compliance with ESC 2015 NSTEMI Guidelines, and identify potential barriers to optimal management of this common clinical presentation associated with significant morbidity and mortality
