    Surgical options for oro-antral communications closure

    An oro-antral communication is defined as a permanent pathological connection between the maxillary sinus and the septic oral cavity. This unfavorable event is usually associated with odontogenic maxillary sinusitis that can be caused by tooth extractions, infection, osteomyelitis, trauma, failed maxillary sinus lifts, bone grafts, poor positioning of dental implant fixtures, and other iatrogenic complications. Oroantral fistula (OAF) often develops after extraction of posterior maxillary teeth and requires surgical closure. Nevertheless, even gentle extraction does not always guarantee against OAF formation, so a surgeon should be capable of treating this complication. This lecture considers the main concepts and methods of plastic surgery directed at reconstructing the tightness of the mouth. Moreover, the advantages and disadvantages of presented methods among themselves and appreciated long-term results and aftermath of different ways of surgical techniques. The purpose of this lecture is to introduce the approach algorithm for the treatment of OAF; and review the fundamental surgical techniques for fistula closure, a comparative analysis of various methods for the plastic correction of the oroantral fistula with the use of the mucosal flap (buccal flap, palatal flap, combination techniques), buccal fat pad, tooth replantation, and the osteoplastic materials based on the data from the literature and online publications. The characteristics of an ideal material for the plastic correction of the oroantral fistula and the conditions for carrying out this surgery will be presented

    Managing pain and anxiety is of paramount importance in dentistry. 10 to 30% of adults and children may have some form of dental fear or anxiety. Patient anxiety has always been a major problem in dental offices. We, the dentists, are in lack of expertise in providing control over our patients' anxiety and pain. Although anxiety and pain can be managed with psychological techniques, in many cases a pharmacological approach is required. Today, there is a wide range of techniques and medications for sedation and anesthesia, which depending on the preference and experience of the anesthesia provider, can achieve the desired effect by oral, inhaler, intramuscular or intravenous administration. The technique in which low concentrations of nitrogen oxide (N2O) are titrated with oxygen has been used for over 130 years (since 1889) in many countries (USA, UK, Australia and Scandinavia) and is recog-nized as clinically successful, safe and cost effective technique com-pared to general anesthesia. Properly applied, by trained and accredited dentists, with well-maintained equipment, this technique has an extremely high success rate and must be included in the practice protocol as a basic tool for managing pain and anxiety in patients undergoing dental treatment in dental offices. In this lecture, we will focus on the evaluation of N2O in overcoming anxiety and pain in patients during the most common oral surgery procedures, as well as its advantages and disadvantages, indications and contraindications

    Sedation dentistry eases anxieties and phobias, helping patients remain calm and comfortable during dental procedures. Managing pain and anxiety is of paramount importance in dentistry. 10 to 30% of adults may have some form of dental fear or anxiety. Patient anxiety has always been a major problem in dental offices. We, the dentists, are in lack of expertise in providing control over our patients' anxiety and pain. Although anxiety and pain can be managed with psychological techniques, in many cases a pharmacological approach is required. With these many pharmacologic alternatives, many different dental patient populations can be sedated in a safe, effective manner, thus allowing the delivery of most dental treatments in a setting of reduced psychologic and physiologic stress. These pharmacologic sedatives have truly opened up a wonderful world of possibilities for the comfortable delivery of dental care, and should be integrated into every office's repertoire for delivery of care. Today, there is a wide range of techniques and medications for sedation and anesthesia, which depending on the preference and experience of the anesthesia provider, can achieve the desired effect by oral, inhaler, intramuscular or intravenous administration. In this lecture, we will focus on the interpretation on the present position of conscious sedation agents in adult dentistry and gerodontology as a valuable starting point for future perspectives

    Buccal fat pad flap for closure of oroantral communication

    Oroantral communication is created as a consequence of some dental procedures, extractions, expanded cyst, iatrogenic trauma or progressive infection. There are many traditional techniques that are used for this intervention, such as nasolabial flap, palatal transposition flaps, standard buccal flap, and latest technique pedicled buccal fat pad flap (BFP). For this technique is characteristic that the size of the buccal fat pad remains the same, no matter the body weight of the person and its fat distribution in the body. It is also easy to prepare and this tissue has a good blood supply. For this study were analyzed totally 43 articles, while 31 of them were with the inclusion criteria. It was based on narrative review on published articles in English language that reported results about the studied topic. The research was done with the use of data bases PubMed, Medscape, NCBI, and Cochrane Library. From the gathered results, it has been shown that patients treated with the BFP technique have had 81,75% successful outcome, and 18,25% resulted with failure. Pain was registered more intense, than the pain in the control group, same with postoperative edema, however there was not statistical significant difference in the maximum mouth opening in both techniques. From this study was concluded that the usage of BFP technique results with more intense post-operative consequences, however the high blood supply makes it good choice for closing OAC

    Background: Numerous surgical grafting procedures designed to reconstruct a partially toothless ridge or ridge defects have been described in the literature over the years. The procedures can be grouped according to the means used to increase the ridge such as soft and hard tissue augmentation procedures. To illustrate the different approaches to utilizing soft tissue augmentation, the following procedures will be discussed: Pedicle graft, Roll graft and Free graft procedures (Pouch graft, Interposition graft and Onlay graft procedure). Description of the procedure: In interpositional graft procedures, there is no need to remove the epithelium from the surface of the donor tissue. If augmentation is required in both buccolingual and apico-coronary direction, part of the graft must be placed above the surface of the tissue around the recipient site. Some of the grafted connective tissue surfaces will be exposed in the oral cavity. "Envelope" or partial thickness flap with relaxing incisions, is prepared on the vestibular surface of the defective area. An appropriate donor site is selected at the palate or in the area of the maxillary tubercle, and a free epithelial-connective tissue graft is harvested. If enlargement of the ridge height is not required, the epithelial surface of the graft is placed with the surrounding epithelium. The graft is sutured all over the tissue at the recipient site. The temporary bridge is positioned to serve as a reference when estimating the amount of tissue needed to fill the defect. Outcomes: The newly formed granulation tissue during healing will make a border between the graft and the adjacent tissue, smooth and properly epithelialized. Edema, which occurs postoperatively, will help contour the ridge. Conclusions: Class III ridge defects are a major challenge for the dentists, as the ridge needs to be enlarged in both vertical and horizontal dimensions.     Schwartz appliance and lip bumper as treatment modalities for mandibular crowding in mixed dentitions

    Introduction: Dental crowding is the most common form of malocclusion. It is more pronounced in the mandible than in the maxilla and tends to increase with age. Depending on the degree of space deficiency and the age of the children the prevalence of dental crowding can be as high as 50%. Schwartz appliance and lip bumper have been widely used as an early treatment for increasing lower dental arch dimensions. Aim: The aim of this review is to evaluate the scientific research about the efficacy of Schwartz appliance and lip bumper and compare the changes caused by the two appliances in addressing mandibular crowding in the mixed dentition. Materials and Methods: A comprehensive literature search on the electronic databases: PubMed, Cochrane Library and Google Scholar was performed. Also, a search of the reference lists of included studies was conducted, to ensure comprehensive coverage of the literature. Search terms included "Schwartz appliance" OR "removable appliance" AND "lip bumper" OR "fixed appliance" AND "mandibular crowding" AND "mixed dentition" OR "early dentition". The search resulted in identifying several studies that met the inclusion criteria. Results: Results show that both Schwartz appliance and lip bumper can effectively improve mandibular crowding in mixed dentition, although the extent and type of changes produced by these appliances may vary. Schwartz appliance is more effective in increasing inter canine width, while the lip bumper has more effect on increasing the arch length. Patient compliance as well as treatment duration are important factors that can have an effect on the success of orthodontic treatment. Conclusion: Schwartz appliance and lip bumper are effective in alleviating mandibular crowding, thus providing cost-effective and simple treatment in mixed dentition. The choice of the appliance should be based on the individual patient’s needs and treatment goals. Further research is needed to evaluate and compare the stability and long-term effects of these two appliances as well as the need for further fixed orthodontic treatment.
