Η ενδομητρίωση και τα γενετικά χαρακτηριστικά της νόσου
Από την πρώτη κλινική περιγραφή της ενδομητρίωσης, έχει επιτευχθεί σημαντική πρόοδος για την κατανόηση των αιτιών αυτής της εξουθενωτικής νόσου. Παρόλο που καμία απο τις θεωρίες για την παθογένεια της ενδομητρίωσης δεν μπορεί να εξηγήσει όλες τις περιγραφείσες εκδηλώσεις της, η θεωρία της παλίνδρομη εμμηνόρροιας έχει αποκτήσει ευρεία αποδοχή ως εξήγηση για τη διάδοση των ενδομητρικών κυττάρων. Ο ακριβής παράγοντας ή οι παράγοντες που ευνοούν την επιβίωση και την επακόλουθη εμφύτευση του εκτοπισμένου ενδομητρίου σε άλλους ιστούς, παραμένουν άγνωστοι. Οι έμφυτες ή επίκτητες ιδιότητες του ενδομητρίου και η ελαττωματική ανοσολογική κάθαρση είναι συστήματα που παρουσιάζουν ενδιαφέρον για την κατανόηση του μηχανισμού της εγκατάστασης ενδομητριωτικών εμφυτευμάτων στους ιστούς. Οι αιτίες για τα παρατηρούμενα χαρακτηριστικά γνωρίσματα της φλεγμονής, της εξάρτησης από τα οιστρογόνα και της ανθεκτικότητας στην προγεστερόνη στην παθοφυσιολογία του πόνου και της στειρότητας που συνδέονται με την ενδομητρίωση αποτελούν περιοχές της σύγχρονης έρευνας.
Στην παρούσα διπλωματική εργασία γίνεται αναφορά στις πιο πρόσφατες μεθόδους ανάλυσης του γονιδιώματος και οι εφαρμογές τους στην διερεύνηση των γενετικών χαρακτηριστικών της ενδομητρίωσης. Μερικά από τα πιο ενδιαφέροντα αποτελέσματα αφορούν την ταυτοποίηση γονιδιακών πολυμορφισμών (SNPs) και η ποσοτική έκφραση χαρακτηριστικού-γονιδιώματος (eQTL) σε δείγματα από ασθενείς με ενδομητρίωση. Τα ερευνητικά αποτελέσματα δείχνουν οτι η αλληλεπίδραση μεταξύ των γονιδίων παίζει καθοριστικό ρόλο στην έκφρασή τους. Επιπλέον, γενετικές μεταλλάξεις που έχουν ανιχνευτεί σε δείγματα απο ενδομητριωτικό ιστό δείχνουν μια πιθανή σύνδεση της ασθένειας με την εμφάνιση καρκίνου στους ασθενείς με ενδομητρίωση.
Η πρόοδος στην έρευνα για τα αίτια της ενδομητρίωσης, αποτελεί σημαντικό παράγοντα για την ανάπτυξη της πρόληψης, της έγκαιρης διάγνωσης με μη επεμβατικές μεθόδους και της αποτελεσματικής αντιμετώπισης της νόσου.Since the original clinical description of endometriosis, much has been accomplished in furthering our understanding of this debilitating disease. Though no theory of pathogenesis can account for all of the described manifestations of endometriosis, the retrograde menstruation theory has gained widespread acceptance as an explanation for the dissemination of endometrial cells. The exact factor or factors that orchestrate the survival and subsequent implantation of the displaced endometrium remain unknown. Innate or acquired properties of the endometrium and defective immune clearance are systems of interest in elucidating the establishment of endometriotic implants. Disease heterogeneity, particularly in lesional phenotype, requires adherence to histopathologic confirmation of implants in clinical and molecular research. The underpinnings for the observed hallmarks of inflammation, estrogen dependence, and progesterone resistance in the pathophysiology of endometriosis associated pain and infertility are areas of active research. With further advances in our understanding of endometriosis, preventive strategies, novel non-surgical diagnostic modalities and targeted therapeutics hold great promise of becoming realities.
In this diploma thesis, reference is made to the most recent methods of genome analysis and their applications in the investigation of endometriosis genetic features. Some of the most interesting results are related to the identification of genomic polymorphisms (SNPs) and quantitative expression of the genome (eQTL) in samples from endometriosis patients. The research results show that the interaction between genes plays a key role in their expression. In addition, genetic mutations detected in samples from endometriotic tissue show a possible association of the disease with cancer in endometriosis patients
Endometriosis of the lung: report of a case and literature review
This paper reports a case of endometriosis of the lung in a 29-year-old woman with long-term periodic catamenial hemoptysis. A chest computed tomography image obtained during menstruation revealed a radiographic opaque lesion in the lingular segment of the left superior lobe. During bronchoscopy, bleeding in the mucosa of the distal bronchus of the lingular segment of the left superior lobe was observed. Histopathology subsequent to an exploratory thoracotomy confirmed the diagnosis of endometriosis of the left lung. The 2-year follow-up after lingular lobectomy of the left superior lobe showed no recurrence or complications
Management of Sigmoid Volvulus Avoiding Sigmoid Resection
Acute sigmoid volvulus is typically caused by an excessively mobile and redundant segment of colon with a stretched mesenteric pedicle. When this segment twists on its pedicle, the result can be obstruction, ischemia and perforation. A healthy, 18-year-old Caucasian woman presented to the emergency department complaining of cramping abdominal pain, distention, constipation and obstipation for the last 72 h, accompanied by nausea, vomiting and abdominal tenderness. The patient had tympanitic percussion tones and no bowel sounds. She was diagnosed with acute sigmoid volvulus. Although urgent resective surgery seems to be the appropriate treatment for those who present with acute abdominal pain, intestinal perforation or ischemic necrosis of the intestinal mucosa, the first therapeutic choice for clinically stable patients in good general condition is considered, by many institutions, to be endoscopic decompression. Controversy exists on the decision of the time, the type of definitive treatment, the strategy and the most appropriate surgical technique, especially for teenagers for whom sigmoid resection can be avoided
Inflammatory Mediators and Pain in Endometriosis: A Systematic Review
Background: pain is one of the main symptoms of endometriosis and it has a deleterious effect on a patients’ personal and social life. To date, the clinical management of pain includes prolonged medication use and, in some cases, surgery, both of which are disruptive events for patients. Hence, there is an urgency for the development of a sufficient non-invasive medical treatment. Inflammation is one of the causative factors of pain in endometriosis. It is well established that inflammatory mediators promote angiogenesis and interact with the sensory neurons inducing the pain signal; the threshold of pain varies and it depends on the state and location of the disease. The inhibition of inflammatory mediators’ synthesis might offer a novel and effective treatment of the pain that is caused by inflammation in endometriosis. Objectives: patients with endometriosis experience chronic pelvic pain, which is moderate to severe in terms of intensity. The objective of this systematic review is to highlight the inflammatory mediators that contribute to the induction of pain in endometriosis and present their biological mechanism of action. In addition, the authors aim to identify new targets for the development of novel treatments for chronic pelvic pain in patients with endometriosis. Data Sources: three databases (PubMed, Scopus, and Europe PMC) were searched in order to retrieve articles with the keywords ‘inflammation, pain, and endometriosis’ between the review period of 1 January 2016 to 31 December 2020. This review has been registered with PROSPERO (registry number: CRD42020171018). Eligibility Criteria: only original articles that presented the regulation of inflammatory mediators and related biological molecules in endometriosis and their contribution in the stimulation of pain signal were included. Data Extraction: two authors independently extracted data from articles, using predefined criteria. Results: the database search yielded 1871 articles, which were narrowed down to 56 relevant articles of interest according to the eligibility criteria. Conclusions: inflammatory factors that promote angiogenesis and neuroangiogenesis are promising targets for the treatment of inflammatory pain in endometriosis. Specifically, CXC chemokine family, chemokine fractalkine, and PGE2 have an active role in the induction of pain. Additionally, IL-1β appears to be the primary interleukin (IL), which stimulates the majority of the inflammatory factors that contribute to neuroangiogenesis along with IL-6. Finally, the role of Ninj1 and BDNF proteins needs further investigation
Προσδιορισμός των επιπέδων των Νευροτροφινών NGF και NT-3 στο αμνιακό υγρό δευτέρου τριμήνου κυήσεων και συσχέτιση με διαταραχές της ενδομήτριας εμβρυϊκής ανάπτυξης
Η ενδομήτρια ανάπτυξη του εμβρύου αποτελεί συνισταμένη γενετικού δυναμικού
και ενδομήτριων επιδράσεων. Διαταραχή αυτής της διεργασίας μπορεί να οδηγήσει
σε έμβρυο που η ανάπτυξή του υπολείπεται σημαντικά των εβδομάδων κύησης
(SGA) ή αντίθετα υπερβαίνει κατά πολύ αυτές (LGA). Στις περιπτώσεις ανεπάρκειας
της εμβρυο-πλακουντιακής μονάδας, η υπολειπόμενη ανάπτυξη του εμβρύου
αποτελεί αναπόφευκτη συνέπεια και ίσως προσαρμοστικό μηχανισμό στην ελλιπή
παροχή θρεπτικών συστατικών και οξυγόνου. Μάλιστα όσο πιο σοβαρή είναι η
ανεπάρκεια αυτή, τόσο περισσότερος είναι ο περιορισμός της ανάπτυξης και
εκδηλώνεται πιο νωρίς. Η παρατηρούμενη ανακατανομή της εμβρυικής
κυκλοφορίας προς τα ευγενή όργανα, όπως ο εγκέφαλος, η καρδιά και τα
επινεφρίδια, έχει σκοπό να προστατέψει το έμβρυο, λειτουργώντας ως
αντιρροπιστικός μηχανισμός. Από την άλλη, όσο μεγαλύτερο είναι ένα έμβρυο τόσο
πιθανότερο είναι επίσης να υποκρύπτεται κάποια υποκείμενη παθολογία. Στην
περίπτωση αυτή ο σακχαρώδης διαβήτης της κύησης αποτελεί τη συχνότερη αιτία,
ενώ άλλα αίτια, όπως γενετικά σύνδρομα, είναι πολύ πιο σπάνια.
Η μελέτη των SGA και LGA εμβρύων είναι σημαντική, γιατί αμφότερες αυτές οι
κατηγορίες παρουσιάζουν αυξημένο κίνδυνο για εμφάνιση τόσο περιγεννητικών
όσο και μακροπρόθεσμων επιπλοκών. Οι παθοφυσιολογικοί μηχανισμοί, σε
κυτταρικό επίπεδο, που οδηγούν σε διαταραχή της ανάπτυξης του εμβρύου,
παραμένουν στο μεγαλύτερο ποσοστό τους άγνωστοι, ενώ η ανάπτυξη κάποιου
μοριακού προγνωστικού δείκτη δεν έχει καταστεί δυνατή, παρά τις προσπάθειες
πολλών ερευνητών.
Οι νευροτροφίνες αποτελούν μία οικογένεια παρόμοιων δομικά μορίων και
περιλαμβάνουν τον αυξητικό παράγοντα των νεύρων (NGF), τον νευροτροφικό
παράγοντα τον προερχόμενο από τον εγκέφαλο (BDNF) και τις νευροτροφίνες 3
και 4 (NT-3, NT-4). Οι ανωτέρω νευροτροφίνες, σύμφωνα με μελέτες, ανιχνεύονται
στο αμνιακό υγρό και η προέλευσή τους είναι κυρίως εμβρυική, ωστόσο υπάρχουν
ενδείξεις για διαπλακουντιακή μεταφορά από τη μητέρα, καθώς και για παραγωγή
από τον πλακούντα. Οι νευροτροφίνες φαίνεται να έχουν διπλό ρόλο, ανιχνευτή και
τροποποιητή της εμβρυο-μητρο-πλακουντιακής μονάδας. Ελλιπής θρέψη της
μητέρας επηρεάζει τη σηματοδότηση των νευροτροφινών στο επίπεδο του
πλακούντα, ενώ έμβρυα με εγγενή έλλειψη νευροτροφινών παρουσιάζουν
διαταραχές ανάπτυξης. Σύμφωνα με μελέτες, οι νευροτροφίνες δρουν στον
εμβρυικό μεταβολισμό, με αλληλεπίδραση με μόρια όπως η ινσουλίνη. Ο κύριος
ρόλος των νευροτροφινών είναι νευροπροστατευτικός μειώνοντας τις διεργασίες
απόπτωσης και επάγοντας διεργασίες επιβίωσης και διαφοροποίησης των νευρικών
κυττάρων, τόσο στον εγκέφαλο όσο και στον περιφερικό νευρικό ιστό.
Συμμετέχουν επίσης σε λεπτές διεργασίες διάπλασης του νευρικού συστήματος,
όπως αυτές της αξονικής ανάπτυξης και της διασύνδεσης-συναπτογένεσης των
νευρώνων, σε διεργασίες μεταγωγής ερεθισμάτων και σε ανώτερες διεργασίες
ωρίμανσης και πλαστικότητας του νευρικού ιστού, ιδιαίτερα στον εγκέφαλο, τόσο
στην εμβρυική όσο και στη νεογνική περίοδο.
Στις περιπτώσεις υπολειπόμενης ανάπτυξης, η ανακατανομή κυκλοφορίας
προστατεύει την ανάπτυξη και λειτουργία του εγκεφάλου και εμφανίζει ενδιαφέρον
ο προσδιορισμός των επιπέδων των νευροτροφινών σε τέτοιες καταστάσεις, καθώς
και η πιθανή συσχέτιση τους με τη βαρύτητα της διαταραχής. Στις περιπτώσεις
μακροσωμίας, επίσης παρουσιάζει ενδιαφέρον η αξιολόγηση των επιπέδων των
νευροτροφινών, καθώς υπάρχουν μελέτες που δείχνουν αλληλεπίδραση των
νευροτροφινών με άλλους αυξητικούς παράγοντες, καθώς και επίδραση των
νευροτροφινών του εμβρύου στον πλακούντα, το όργανο που καθορίζει τη θρέψη
και οξυγόνωση του εμβρύου.
Η παρούσα μελέτη αποσκοπεί στην επιβεβαίωση της παρουσίας και τον ποσοτικό
προσδιορισμό των νευροτροφινών NGF και ΝΤ-3 στο αμνιακό υγρό κυήσεων
δευτέρου τριμήνου και η ανάδειξη πιθανής συσχέτισης των επιπέδων τους στο υγρό
αυτό με το ρυθμό ενδομήτριας ανάπτυξης του εμβρύου, ώστε να προκύψουν
πληρέστερα και κατανοητά συμπεράσματα για τους μηχανισμούς υπολειπόμενης
ανάπτυξης, που μπορεί να σχετίζεται επίσης με φτωχότερο περιγεννητικό
αποτέλεσμα. Επίσης να αποσαφηνιστεί η ύπαρξη τυχόν προγνωστικού ρόλου της
συγκέντρωσης των ουσιών αυτών στην έκβαση της κύησης. Ο NGF είναι μία
πολυπεπτιδική ορμόνη που συμμετέχει στην ανάπτυξη των συμπαθητικών και
κεντρικών νευρικών συστημάτων. Συμβάλλει στην επιβίωση των χολινεργικών
νευρώνων στο κεντρικό νευρικό σύστημα και στην ανάπτυξη των νευρώνων και
συντήρηση του φαινοτύπου των νευρώνων στο περιφερικό νευρικό σύστημα
(PNS). Επιπλέον, έχει ανευρεθεί στον ανθρώπινο πλακούντα, εύρημα που
υποδεικνύει ένα δυνητικό ρόλο στην ανάπτυξη του πλακούντα. Το NT-3 θεωρείται
ότι είναι ο πρόγονος όλων των νευροτροφινών. Παρουσιάζει μια δομή παρόμοια με
αυτή του NGF και του BDNF, αλλά σε αντίθεση με αυτά, τα επίπεδά του στο
κεντρικό νευρικό σύστημα είναι υψηλότερα κατά την ανάπτυξη του εμβρύου από
ότι στην ενήλικη ζωή. Η παρουσία του NT-3 στον φυσιολογικό και παθολογικό
πλακούντα κατά τη διάρκεια της εγκυμοσύνης έχει επιβεβαιωθεί από το 2009,
υποδηλώνοντας πιθανό ρόλο στην ανάπτυξη του πλακούντα αλλά και στην
ανάπτυξη του εμβρύου με βάση όσα προαναφέρθηκαν.
Τα δείγματα του αμνιακού υγρού της προοπτικής μελέτης συλλέχθηκαν από
γυναίκες οι οποίες είχαν υποβληθεί σε αμνιοπαρακέντηση στις πρώτες εβδομάδες
του 2ου τριμήνου της εγκυμοσύνης (15-22 εβδομάδες) για διάφορες ενδείξεις.
Εξαιρέθηκαν από τη μελέτη οι δίδυμες εγκυμοσύνες και οι εγκυμοσύνες με έμβρυα
με γενετικές ανωμαλίες ή σοβαρές συγγενείς ανωμαλίες. Όλες οι κυήσεις
παρακολουθήθηκαν έως και τον τοκετό. Τα πρότυπα ανάπτυξης των εμβρύων και
τα βάρη γέννησης τους καταγράφηκαν και χωρίστηκαν στη συνέχεια σε τρεις
ομάδες: SGA (μικρά για την εβδομάδα κύησης), AGA (κατάλληλο βάρος για την
εβδομάδα κύησης) και LGA (μεγάλα για την εβδομάδα κύησης). Το δείγμα της
μελέτης αποτέλεσαν 51 έμβρυα ως εξής: 12 SGA έμβρυα και 8 LGA έμβρυα που
κατηγοριοποιήθηκαν σε εκατοστιαία θέση με βάση την ηλικία της κύησης, το φύλο
του εμβρύου και το ύψος και το βάρος της μητέρας και 31 AGA έμβρυα που
αποτέλεσαν την ομάδα ελέγχου. Τα αντίστοιχα δείγματα αμνιακού υγρού
αντλήθηκαν από την τράπεζα δειγμάτων αμνιακού υγρού και οι δύο βιοδείκτες (ΝΤ3 και NGF) μετρήθηκαν ώστε να συγκριθούν τα επίπεδα τους ανάμεσα στην ομάδα
ελέγχου και στις 2 παθολογικές υποομάδες (SGA και LGA). Οι συγκεντρώσεις του
NGF και του NT-3 προσδιορίστηκαν με ανοσοενζυμική μέθοδο ELISA με κατάλληλα
kit (R&D Systems, Minneapolis, MN, USA).
Όσον αφορά τα αποτελέσματα που προέκυψαν, οι συγκεντρώσεις του NGF ήταν
τόσο παρόμοιες μεταξύ των μελετώμενων ομάδων που μας οδήγησαν στο
συμπέρασμα ότι τα επίπεδα του NGF δεν επηρεάζονται από την εμβρυϊκή
διατροφική κατάσταση ούτε επηρεάζουν το τελικό βάρος γέννησης. Μια άλλη
εξήγηση θα μπορούσε να είναι ότι ο ρόλος του NGF στην εμβρυϊκή ανάπτυξη
εκδηλώνεται κυρίως σε πιο προχωρημένο εμβρυϊκό στάδιο από το πρώιμο δεύτερο
τρίμηνο, που συνήθως πραγματοποιείται η αμνιοπαρακέντηση. Από την άλλη
πλευρά, οι συγκεντρώσεις του NT-3 διέφεραν μεταξύ των μελετώμενων ομάδων
και παρατηρήθηκε μια σαφής τάση προς αύξηση των επιπέδων του NT-3 όσο η
εμβρυϊκή ανάπτυξη μειωνόταν. Πιο συγκεκριμένα, η μέση συγκέντρωση NT-3 των
LGA εμβρύων ήταν χαμηλότερη από αυτήν των AGA εμβρύων (11,87 pg/ml έναντι
15,9 pg/ml, αντίστοιχα), ενώ η μέση συγκέντρωση NT-3 των SGA εμβρύων ήταν
υψηλότερη από αυτήν των AGA εμβρύων (23,5 pg/ml έναντι 15,9 pg/ml,
αντίστοιχα), οδηγώντας σε περίπου διπλάσια επίπεδα στην SGA υποομάδα σε σχέση
με αυτά της LGA υποομάδας. Παρά ταύτα η διαφορά αυτή δεν έφτασε το ορισθέν
όριο στατιστικής σημαντικότητας του 5%. Είναι δυνατόν πιο μεγάλες μελέτες να
επιβεβαιώσουν αυτήν την παρατηρούμενη τάση και να διερευνήσουν με
περισσότερη ακρίβεια τον ακριβή ρόλο αυτής της νευροτροφίνης στα άκρα του
φάσματος της εμβρυικής ανάπτυξης.
Στην παρούσα μελέτη ένας περιορισμός ήταν ο αριθμός των περιστατικών. Η
προοπτική φύση της μελέτης μαζί με τη σπανιότητα των επεμβατικών
προγεννητικών ελέγχων σήμερα μετά και την ευρεία πλέον χρήση των μη
επεμβατικών εξετάσεων δυσκόλεψε σημαντικά τη συγκέντρωση ενός μεγαλύτερου
δείγματος μελέτης. Η ίδια αιτία δεν επέτρεψε το χωρισμό του δείγματος σε
υποομάδες, δηλαδή σε περιπτώσεις βαριάς υπολειπόμενης εμβρυικής ανάπτυξης
ή/και μακροσωμίας, όπου τα αποτελέσματα θα μπορούσαν να είναι στατιστικώς
σημαντικά. Γίνεται αντιληπτή η ανάγκη πραγματοποίησης μεγαλύτερων
πολυκεντρικών μελετών, ώστε να αποσαφηνισθεί περαιτέρω η συσχέτιση των NGF
και NT-3 με την παθολογική ενδομήτρια ανάπτυξη.Fetal intrauterine growth is the combination of genetically predetermined potential
and intrauterine environment effects. Failure of this process can result in a fetus
whose development falls significantly short of gestational weeks (SGA fetus) or, on
the contrary, greatly exceeds them (LGA fetus). In cases of feto-placental unit
insufficiency, restricted fetal growth is an inevitable consequence and perhaps an
adaptive mechanism to the insufficient supply of nutrients and oxygen. In fact, the
more significant this deficiency is, the more critical is the restriction of growth and
the earlier it manifests itself. The observed fetal blood redistribution to vital organs,
such as the brain, heart, and adrenal glands, acts as a compensatory mechanism
in order to protect the fetus. On the other hand, the bigger the fetus, the more
likely it is that an underlying pathology is also present, with gestational diabetes
being the most common cause, other causes, such as genetic syndromes, being
rare.
The study of SGA and LGA fetuses is important because both groups are at
increased risk for both perinatal and long-term complications. The underlying
pathophysiological mechanisms remain to date largely unknown, while the
development of a molecular prognostic indicator is still pending, despite the efforts
of many researchers.
Neurotrophins are a family of structurally similar molecules and include nerve
growth factor (NGF), brain-derived neurotrophic factor (BDNF), and neurotrophins
3 and 4 (NT-3, NT-4). According to previous research, the above neurotrophins are
detected in amniotic fluid and their origin is predominantly fetal. However, there
are indications of transplacental transfer of maternal neurotrophins, as well as local
placental production. Neurotrophins seem to have a dual role, being detectors and
modifiers of the feto-placental unit. Maternal undernutrition affects neurotrophin
signaling at the placental level, while fetuses with a congenital lack of
neurotrophins manifest developmental disorders. Published research shows that
neurotrophins act on the fetal metabolism by interacting with molecules such as
insulin. The main role of neurotrophins is neuroprotective by reducing apoptosis as
well as inducing survival and differentiation processes of the nerve cells, in both
the brain and the peripheral nervous system. They are also involved in subtle
processes of neuronal development, such as those of axonal growth and
interconnection (synaptogenesis) of neurons, in processes of transduction of
impulses, and in higher processes of maturation and neuroplasticity, especially in
the brain, both during the fetal and the neonatal periods.
In cases of residual development, fetal blood redistribution protects brain
development and function; it is thus important to determine the levels of
neurotrophins in such conditions, as well as their possible correlation with the
severity of the SGA and LGA fetal centiles. In cases of macrosomia, the evaluation
of neurotrophin levels is also of interest, as there are studies showing interaction
of neurotrophins with other growth factors, as well as an effect of fetal
neurotrophins on the placenta, the tissue that mediates nutrition and oxygenation
of the fetus.
The present study aimed to confirm the presence of neurotrophins NGF and NT-3
and to quantify them in second-trimester amniotic fluid samples in an attempt to
determine a possible correlation of their levels with fetal intrauterine growth
velocity. This may lead to insights into the mechanisms of abnormal fetal growth,
which which is frequently associated with poor perinatal outcome. We thus sought
to determine whether these proteins possessed any prognostic value regarding
pregnancy outcomes. NGF is a polypeptide hormone that plays a crucial role in the
development of the sympathetic and central nervous systems. It contributes to the
survival of cholinergic neurons in the central nervous system as well as to the
growth and maintenance of neuronal phenotype in the peripheral nervous system
(PNS). Additionally, it is present in the human placenta, a finding that indicates a
potential role also in placental development. NT-3 is considered to be the
progenitor of all neurotrophins. Its structure resembles that of NGF and BDNF;
however, it differs from them in that it shows higher levels within the central
nervous system during fetal development than during adult life. The presence of
NT-3 in both normal and pathological placenta during pregnancy was confirmed in
2009.
Amniotic fluid samples were collected from women who had undergone
amniocentesis in the early weeks of the 2nd trimester of pregnancy (15-22 weeks)
for various indications. Twin pregnancies and pregnancies with fetuses with
aneuploides or congenital abnormalities were excluded from the study. All
pregnancies were followed up to delivery. Fetal birth weights were recorded, and
the cases were divided into three groups: SGA (small for gestational week), AGA
(appropriate weight for gestational week), and LGA (large for gestational week).
The study sample consisted of 51 fetuses: 12 SGA fetuses and 8 LGA fetuses were
compared with 31 AGA fetuses that constituted the control group. The respective
amniotic fluid samples were drawn from the amniotic fluid samples bank and the
two biomarkers (NT-3 and NGF) were measured so as to compare the levels
between the control group and the two pathological subgroups (SGA and LGA).
NGF and NT-3 concentrations were determined by enzyme-linked immunosorbent
assay (ELISA) kits (R&D Systems, Minneapolis, MN, USA), according to the
manufacturer’s instructions.
Regarding the results obtained, NGF concentrations were similar among the studied
groups; this led us to conclude that NGF levels are not affected by fetal nutritional
status nor do they affect birth weight. Another explanation may be that NGF action
on fetal development is manifested at a more advanced gestational stage than the
early second trimester, when the amniocenteses were performed. On the other
hand, NT-3 concentrations differed between the studied groups, and a clear trend
towards increased NT-3 levels was observed as fetal growth decreased.
Specifically, mean NT-3 concentration of LGA fetuses was lower than that of AGA
fetuses (11.87 pg/ml vs. 15.9 pg/ml, respectively), while the mean NT-3
concentration of SGA fetuses was higher than that of AGA fetuses (23.5 pg/ml vs.
15.9 pg/ml, respectively), leading to approximately twice the levels in the SGA
subgroup compared to those in the LGA subgroup. Nevertheless, this difference
failed to reach the statistical significance threshold of 5%. We hypothesize that
larger studies may confirm the observed trend and characterize the exact role of
NT-3 at the extremes of the fetal development spectrum.
Limitations apply to most studies; the main limitation here was the number of cases
included. The prospective nature of the study along with the rarity of invasive
prenatal procedures today and the widespread use of noninvasive testing made it
extremely difficult to gather a larger study sample. The same limitation prevented
us from dividing the extreme growth centile samples into subgroups, i.e., cases of
severe FGR and/or macrosomia, where the results might have been statistically
significant. Larger multicenter studies are needed in order to clarify the association
of NGF and NT-3 with abnormal fetal growth velocities
The Formidable yet Unresolved Interplay between Endometriosis and Obesity
Obesity and endometriosis are two very common entities, yet there is uncertainty on their exact relationship. Observational studies have repeatedly shown an inverse correlation between endometriosis and a low body mass index (BMI). However, obesity does not protect against endometriosis and on the contrary an increased BMI may lead to more severe forms of the disease. Besides, BMI is not accurate in all cases of obesity. Consequently, other anthropometric and phenomic traits have been studied, including body adiposity content, as well as the effect of BMI early in life on the manifestation of endometriosis in adulthood. Some studies have shown that the phenotypic inverse correlation between the two entities has a genetic background; however, others have indicated that certain polymorphisms are linked with endometriosis in females with increased BMI. The advent of metabolic bariatric surgery and pertinent research have led to the emergence of biomolecules that may be pivotal in understanding the pathophysiological interaction of the two entities, especially in the context of angiogenesis and inflammation. Future research should focus on three objectives: detection and interpretation of obesity-related biomarkers in experimental models with endometriosis; integration of endometriosis-related queries into bariatric registries; and multidisciplinary approach and collaboration among specialists
Rare Distant Metastatic Disease of Ovarian and Peritoneal Carcinomatosis: A Review of the Literature
Background: Although metastases of ovarian and peritoneal carcinomatosis are most commonly found within the peritoneal cavity, there is a number of other rare distant sites that have been reported. Our goal is to provide an evidence-based summary of the available literature considering the rare distant metastatic sites of ovarian and peritoneal carcinomatosis. Methods: A comprehensive search of the literature was conducted, with Medline/PubMed being searched for cases of rare metastatic disease originated from primary ovarian and peritoneal cancer with related articles up to 2019 including terms such as “ovarian cancer”, “metastases”, “peritoneal” and others. Results: The most common mechanism of ovarian cancer metastases consists of primarily dissemination within the peritoneal cavity, while, rare and distant sites can either occur at the beginning or during the course of the disease and they are usually associated with hematogenous route and lymphatic invasion, having poor prognosis, with the least common sites being skin, bone, CNS, eye, placenta, central airways, rare lymph nodes, intra-abdominal organs, heart and breast. Conclusions: The occurrence of metastatic sites described in this review represents the most common rare distant metastatic sites, and even though their patterns of metastases are still not fully clarified due to the rarity of the reports, they offer valuable information considering the pathophysiology of the disease
Advances of Laparoscopy for the Diagnosis of Pelvic Congestion Syndrome
The objective of this review is to describe the effectiveness of laparoscopy in the diagnosis and treatment of pelvic congestion syndrome (PCS). PCS is a cause of chronic pelvic pain (CPP) and is associated with dysfunction of the pelvic venous system. PCS is more common in women of reproductive age, and hormonal changes are associated with its development along with other reasons (e.g., working and living habits). There is an urgent need to establish an effective algorithm for the diagnosis and treatment of CPP, which could have a dramatic effect in patients’ everyday life. This algorithm should be able to overcome known issues that lead to the underdiagnosis of PCS, such as the overlap of its symptoms with other diseases. Here, we present our findings from literature articles about the methods used in practice today for the diagnosis of this syndrome. We also compare the methods to propose the most promising technique for providing a diagnosis with high accuracy. In our understanding, laparoscopy is superior when compared to other methods. It can provide a diagnosis of PCS while excluding or identifying other comorbidities and can also lead toward the next steps for the treatment of PCS
