    International audienceIn high mountainous catchments, the spatial precipitation and therefore the overall water balance is generally difficult to estimate. The present paper describes the structure and calibration of a semi-lumped conceptual glacio-hydrological model for the joint simulation of daily discharge and annual glacier mass balance that represents a better integrator of the water balance. The model has been developed for climate change impact studies and has therefore a parsimonious structure; it requires three input times series ? precipitation, temperature and potential evapotranspiration ? and has 7 parameters to calibrate. A multi-signal approach considering daily discharge and ? if available ? annual glacier mass balance has been developed for the calibration of these parameters. The model has been calibrated for three different catchments in the Swiss Alps having glaciation rates between 37% and 52%. It simulates well the observed daily discharge, the hydrological regime and some basic glaciological features, such as the annual mass balance

    DEUTSCH Über den EHEC-Erreger, der in Deutschland zu ĂŒber dreissig tragischen TodesfĂ€llen gefĂŒhrt hat, ist viel spekuliert worden. Unterdessen konnten die Verbreitungswege nahezu aufgeklĂ€rt werden, wĂ€hrend die Entstehung des Krankheitserregers noch weitgehend unklar ist. Im Zusammenhang mit dem EHEC-Ausbruch wurde auch die Frage erörtert, ob bestimmte Landwirtschaftsmethoden (zum Beispiel gemischte Betriebe mit Tierhaltung und Pflanzenbau oder die organische DĂŒngung von GemĂŒse) oder ob gewisse Hygienisierungsmethoden fĂŒr frische, konsumfĂ€hige Produkte(wie zum Beispiel die Verwendung von schonenden, weniger RĂŒckstĂ€nde verursachenden organischen SĂ€uren) das Risiko erhöhen könnten. Diese Dokumentation geht nicht auf die aktuellen FĂ€lle von EHEC in Norddeutschland ein, sondern liefert an diesem Beispiel Hintergrundinformationen zur Frage, wie nachhaltige, in KreislĂ€ufen arbeitende Landwirtschaftsmethoden mit vom Tier auf den Menschen ĂŒbertragbare Krankheitserreger (Zoonosen) umgehen. ENGLISH There has been much speculation about the pathogenic EHEC bacterial strain that has tragically claimed the lives of more than 30 people so far in Germany. Meanwhile, the transmission pathways have for the most part been identified. The origin of the pathogen, however, is largely unclear. In the context of the EHEC outbreak there have been debates as to whether certain agriculturalmethods (e.g. mixed holdings including both livestock and crop production, or the use of organic fertilizers for vegetable production) or certain sanitization methods for fresh consumable products (such as the use of organic acids, a mild treatment that leaves fewer residues) may increase the risk of infection. This document does not address the current epidemiological situation of the outbreak in northern Germany. Its purpose is rather to provide background information on the question of how sustainable agricultural methods based on nutrient cycling deal with pathogens that can be transmitted from animals to humans (zoonoses). Hasta ahora casi se ha podido esclarecer la via de su propagaciĂłn, mientras que sigue estando poco claro como ha aparecido el agente patĂłgeno. Con el brote de la ECEH se ha cuestionado si determinadas prĂĄcticas agrĂ­colas (por ejemplo, mantener explotaciones mixtas con ganado y cultivos, o la fertilizaciĂłn orgĂĄnica de los vegetales) o si los mĂ©todos de higiene para productos frescos, listos para el consumo (como por ejemplo, el uso de ĂĄcidos orgĂĄnicos menos agresivos y con menor riesgo de dejar residudos), pudieran incrementar los riesgos.     Comparative efficacy of long-acting bronchodilators for COPD - a network meta-analysis

    Background: Clinicians are faced with an increasingly difficult choice regarding the optimal bronchodilator for patients with chronic obstructive pulmonary disease (COPD) given the number of new treatments. The objective of this study is to evaluate the comparative efficacy of indacaterol 75/150/300 ÎŒg once daily (OD), glycopyrronium bromide 50 ÎŒg OD, tiotropium bromide 18 ÎŒg/5 ÎŒg OD, salmeterol 50 ÎŒg twice daily (BID), formoterol 12 ÎŒg BID, and placebo for moderate to severe COPD. Methods: Forty randomized controlled trials were combined in a Bayesian network meta-analysis. Outcomes of interest were trough and post-dose forced expiratory volume in 1 second (FEV1), St. George’s Respiratory Questionnaire (SGRQ) score and responders (≄4 points), and Transition Dyspnea Index (TDI) score and responders (≄1 point) at 6 months. Results: Indacaterol was associated with a higher trough FEV1 than other active treatments (difference for indacaterol 150 ÎŒg and 300 ÎŒg versus placebo: 152 mL (95% credible interval (CrI): 126, 179); 160 mL (95% CrI: 133, 187)) and the greatest improvement in SGRQ score (difference for indacaterol 150 ÎŒg and 300 ÎŒg versus placebo: -3.9 (95% CrI -5.2, -2.6); -3.6 (95% CrI -4.8, -2.3)). Glycopyrronium and tiotropium 18 ÎŒg resulted in the next best estimates for both outcomes with minor differences (difference for glycopyrronium versus tiotropium for trough FEV1 and SGRQ: 18 mL (95% CrI: -16, 51); -0.55 (95% CrI: -2.04, 0.92). Conclusion: In terms of trough FEV1 and SGRQ score indacaterol, glycopyrronium, and tiotropium are expected to be the most effective bronchodilators

    Flotillins Interact with PSGL-1 in Neutrophils and, upon Stimulation, Rapidly Organize into Membrane Domains Subsequently Accumulating in the Uropod

    BACKGROUND: Neutrophils polarize and migrate in response to chemokines. Different types of membrane microdomains (rafts) have been postulated to be present in rear and front of polarized leukocytes and disruption of rafts by cholesterol sequestration prevents leukocyte polarization. Reggie/flotillin-1 and -2 are two highly homologous proteins that are ubiquitously enriched in detergent resistant membranes and are thought to shape membrane microdomains by forming homo- and hetero-oligomers. It was the goal of this study to investigate dynamic membrane microdomain reorganization during neutrophil activation. METHODOLOGY/PRINCIPAL FINDINGS: We show now, using immunofluorescence staining and co-immunoprecipitation, that endogenous flotillin-1 and -2 colocalize and associate in resting spherical and polarized primary neutrophils. Flotillins redistribute very early after chemoattractant stimulation, and form distinct caps in more than 90% of the neutrophils. At later time points flotillins accumulate in the uropod of polarized cells. Chemotactic peptide-induced redistribution and capping of flotillins requires integrity and dynamics of the actin cytoskeleton, but does not involve Rho-kinase dependent signaling related to formation of the uropod. Both flotillin isoforms are involved in the formation of this membrane domain, as uropod location of exogenously expressed flotillins is dramatically enhanced by co-overexpression of tagged flotillin-1 and -2 in differentiated HL-60 cells as compared to cells expressing only one tagged isoform. Flotillin-1 and -2 associate with P-selectin glycoprotein ligand 1 (PSGL-1) in resting and in stimulated neutrophils as shown by colocalization and co-immunoprecipitation. Neutrophils isolated from PSGL-1-deficient mice exhibit flotillin caps to the same extent as cells isolated from wild type animals, implying that PSGL-1 is not required for the formation of the flotillin caps. Finally we show that stimulus-dependent redistribution of other uropod-located proteins, CD43 and ezrin/radixin/moesin, occurs much slower than that of flotillins and PSGL-1. CONCLUSIONS/SIGNIFICANCE: These results suggest that flotillin-rich actin-dependent membrane microdomains are importantly involved in neutrophil uropod formation and/or stabilization and organize uropod localization of PSGL-1

    Consumers are increasingly aware of the health- and safety-related implications of the food which they can buy in the market. At the same time, households have become more aware of their environmental responsibilities. Regarding the production of food, a crucial and multifunctional role is played by agriculture. The way vegetables, fruits, and other crops are grown and how livestock is raised has an impact on the environment and landscape. Operations performed by farmers, such as water management, can be dangerous for the soil and the whole ecosystem. Consequently, there is a search for natural ways of sustaining the impact of agriculture on the environment. In this context, one of the most popular ideas is organic agriculture. In the literature on the subject, there are many concepts that some authors consider to be synonymous even as others argue that these terms are not interchangeable. There is, for example, "organic agriculture," "alternative agriculture," "sustainable agriculture," "ecological agriculture," "biological agriculture," "niche farming," "community-supported agriculture," and "integrated pest management." Very often, techniques and products related to organic agriculture are described by marketing experts with the use of abbreviations such as "bio" and "eco." Products with such markings and labels are increasingly popular in stores that often give them separate shelves for their sale. Despite the higher price compared to conventional products, they are increasingly sought by consumers. The entry examines the various impacts of organic agriculture with a view to these trends

    A Prospective Multicenter SPOG 2003 FN Study of Microbiologically Defined Infections in Pediatric Cancer Patients with Fever and Neutropenia.

    BACKGROUND: Fever and neutropenia (FN) often complicate anticancer treatment and can be caused by potentially fatal infections. Knowledge of pathogen distribution is paramount for optimal patient management. METHODS: Microbiologically defined infections (MDI) in pediatric cancer patients presenting with FN by nonmyeloablative chemotherapy enrolled in a prospective multi-center study were analyzed. Effectiveness of empiric antibiotic therapy in FN episodes with bacteremia was assessed taking into consideration recently published treatment guidelines for pediatric patients with FN. RESULTS: MDI were identified in a minority (22%) of pediatric cancer patients with FN. In patients with, compared to without MDI, fever (median, 5 [IQR 3-8] vs. 2 [IQR1-3] days, p < 0.001) and hospitalization (10 [6-14] vs. 5 [3-8] days, p < 0.001) lasted longer, transfer to the intensive care unit was more likely (13 of 95 [14%] vs. 7 of 346 [2.0%], p < 0.001), and antibiotics were given longer (10 [7-14] vs. 5 [4-7], p < 0.001). Empiric antibiotic therapy in FN episodes with bacteremia was highly effective if not only intrinsic and reported antimicrobial susceptibilities were considered but the purposeful omission of coverage for coagulase negative staphylococci and enterococci was also taken into account (81% [95%CI 68 - 90] vs. 96.6% [95%CI 87 - 99.4], p = 0.004) CONCLUSIONS: MDI were identified in a minority of FN episodes but they significantly affected management and the clinical course of pediatric cancer patients. Compliance with published guidelines was associated with effectiveness of empiric antibiotic therapy in FN episodes with bacteremia

    Gesellschaftliche Leistungen der biologischen Landwirtschaft

    Zusammenfassuung: In diesem Bericht werden die wissenschaftlichen Ergebnisse nationaler und internationaler Studien zu den gesellschaftlichen Leistungen des Biolandbaus zusammengefasst. Als Referenzen werden dabei sowohl die konventionelle wie auch die integrierte Landwirtschaft herangezogen. Es ist den Autoren bewusst, dass alle Landwirtschaftsmethoden in der Schweiz und im Ausland eine grosse Streubreite aufweisen, da der Einfluss der regionalen sozioökonomischen Rahmenbedingungen, der Standortfaktoren, der Betriebsleiter und der unterschiedlichen staatlichen und privaten Regulierungen gross ist. Trotzdem ergeben sich fĂŒr die verschiedenen Landwirtschaftsmethoden klare Unterschiede, die bei den agrarpolitischen Massnahmen berĂŒcksichtigt werden mĂŒssen. BezĂŒglich der positiven ExternalitĂ€ten weist der Biolandbau klare Mehrleistungen in der BiodiversitĂ€t auf. Dies sowohl auf der Ebene der Genetik, der Arten und besonders auch der Habitate. Deutlich sind die Mehrleistungen des Biolandbaus auch im Bereich des Humusaufbaus, was eine höhere CO2-Sequestrierung zur Folge hat. Obwohl eine dauerhafte Honorierung der Vermeidung negativer ExternalitĂ€ten im bisherigen Konzept der Weiterentwicklung des Direktzahlungssystems (WDZ) nicht vorgesehen ist, sind hier die Vorteile der biologischen Landwirtschaft betrĂ€chtlich. Dies betrifft die NĂ€hrstoff- und Pflanzenschutzmittelverluste in Grund- und OberflĂ€chengewĂ€sser sowie in andere Umweltkompartimente, der Verbrauch von nicht erneuerbarer Energie, die Emissionen von Klimagasen und die Belastungen mit Tiermedikamenten. Die nationalen und internationalen Studien zeigen auf, dass der Biolandbau sich besonders durch Systemwirkungen auszeichnet: erhöhte BiodiversitĂ€t, verbesserter Schutz des Bodens, wirksame Vermeidung von Umweltbelastungen, effiziente Nutzung von natĂŒrlichen Ressourcen, geringer Verbrauch nicht erneuerbarer Energie und verbessertes Tierwohl. Dies sind die Effekte von komplexen Response-Massnahmen der Betriebsleiter auf einfach zu kontrollierende EinschrĂ€nkungen und Verbote. Dadurch entstehen nicht zu unterschĂ€tzende Zusatzleistungen und Synergien bei der Erreichung der vom Bund definierten Ziele. Um den Mehrleistungen des Biolandbaus gerecht zu werden, schlagen die Autoren zwei neue Massnahmen vor: 1) BiodiversitĂ€tsbeitrag fĂŒr Acker, Sonderkulturen und intensive Wiesen und 2) CO2-SequestrationsbeitrĂ€ge. Damit sind aber die gesellschaftlich wichtigen Mehrleistungen des Biolandbaus noch nicht abgegolten. Bei der Vermeidung negativer ExternalitĂ€ten werden höhere umweltschutzrelevante Effekte erzielt. Diese können i) durch die EinfĂŒhrung eines negativen Anreizsystems nach dem Polluter-Pays-Principle (z.B. Stickstoffsteuer, CO2 - Abgabe), ii) durch ein höheres Einstiegsniveau bei den Cross-Compliance-Massnahmen fĂŒr alle Betriebe (ÖLNplus) oder iii) durch einen finanziellen Ausgleich fĂŒr die Mehrleistungen des Biolandbaus abgegolten werden. Die „Förderung des Biolandbaus“ als systemorientierte Massnahme macht auch nach dem Grundsatz des effizienten Einsatzes von Steuermitteln Sinn und stellt eine kosteneffektive Massnahme dar. Einerseits können damit die deutlich höheren positiven ExternalitĂ€ten abgegolten werden, andererseits wird die stĂ€rkere Vermeidung von negativen ExternalitĂ€ten genĂŒgend berĂŒcksichtigt

    Dexamethasone vs prednisone in induction treatment of pediatric ALL: results of the randomized trial AIEOP-BFM ALL 2000

    Induction therapy for childhood acute lymphoblastic leukemia (ALL) traditionally includes prednisone; yet, dexamethasone may have higher antileukemic potency, leading to fewer relapses and improved survival. After a 7-day prednisone prephase, 3720 patients enrolled on trial Associazione Italiana di Ematologia e Oncologia Pediatrica and Berlin-Frankfurt-MĂŒnster (AIEOP-BFM) ALL 2000 were randomly selected to receive either dexamethasone (10 mg/m(2) per day) or prednisone (60 mg/m(2) per day) for 3 weeks plus tapering in induction. The 5-year cumulative incidence of relapse (± standard error) was 10.8 ± 0.7% in the dexamethasone and 15.6 ± 0.8% in the prednisone group (P < .0001), showing the largest effect on extramedullary relapses. The benefit of dexamethasone was partially counterbalanced by a significantly higher induction-related death rate (2.5% vs 0.9%, P = .00013), resulting in 5-year event-free survival rates of 83.9 ± 0.9% for dexamethasone and 80.8 ± 0.9% for prednisone (P = .024). No difference was seen in 5-year overall survival (OS) in the total cohort (dexamethasone, 90.3 ± 0.7%; prednisone, 90.5 ± 0.7%). Retrospective analyses of predefined subgroups revealed a significant survival benefit from dexamethasone only for patients with T-cell ALL and good response to the prednisone prephase (prednisone good-response [PGR]) (dexamethasone, 91.4 ± 2.4%; prednisone, 82.6 ± 3.2%; P = .036). In patients with precursor B-cell ALL and PGR, survival after relapse was found to be significantly worse if patients were previously assigned to the dexamethasone arm. We conclude that, for patients with PGR in the large subgroup of precursor B-cell ALL, dexamethasone especially reduced the incidence of better salvageable relapses, resulting in inferior survival after relapse. This explains the lack of benefit from dexamethasone in overall survival that we observed in the total cohort except in the subset of T-cell ALL patients with PGR.
