The FK506 binding protein 13 kDa (FKBP13) interacts with the C-chain of complement C1q
BACKGROUND: The pharmacological action of specific immunosuppressants is mediated by immunophilins. While cyclosporin A binds to cyclophilins, FK506/tacrolimus, rapamycin, and others bind to FK506 binding proteins (FKBPs). Different physiological actions of immunophilins were described but their genuine function, however, remains elusive and is still under investigation. A yeast two-hybrid screen was performed using the FK506 binding protein 13 kDa (FKBP13) as a bait and a fetal liver expression library as a prey. RESULTS: The C-chain of complement C1q (C1q-C) was detected to interact with FKBP13 in the yeast two-hybrid system and in a protein complementation assay. Neither FKBP12, FKBP25, FKBP52 nor the unrelated immunophilin CypA did react with C1q-C in the yeast system stressing the specificity of the interaction. Binding of C1q-C to FKBP13 could not be prevented in the presence of FK506, demonstrating that possibly other regions than the binding pocket of the drug are responsible for the interaction of the two proteins. CONCLUSION: It is concluded that exclusively FKBP13 but no other FKBPs tested so far interact with the C-chain of complement C1q in the two different assays and further work will be initiated to investigate the physiological relevance of the interaction

A prospective comparison of power Doppler sonography and ileocolonoscopy in the evaluation of activity in patients with Crohn's disease
In der Literatur finden sich bisher nur wenige prospektive Daten zum
klinischen Wert der Power-Dopplersonographie bei Morbus Crohn. Der
Goldstandard zur Bewertung der Aktivität ist die Ileokoloskopie. Ziel dieser
prospektiven Studie war die Evaluierung der Power-Dopplersonographie mit
Beantwortung folgender Fragen: 1\. Führt die Einschätzung der entzündlichen
Aktivität einzelner Darmsegmente durch die Sonographie unter Einbeziehung der
neuen Dopplermethoden im Ver-gleich mit der Ileokoloskopie zu konkordanten
oder zu divergenten Ergebnissen? 2\. Kann die Sonographie die Ausdehnung der
Entzündung bei Patienten mit einem Morbus Crohn korrekt einschätzen? 31
Patienten mit gesichertem Morbus Crohn wurden prospektiv mit dem B-Mode sowie
der Power- Dopplersonographie und mit der Ileokoloskopie untersucht. Die
Sonogra-phie erfolgte binnen 3 Tagen vor der Endoskopie. Sämtliche
Untersuchungen fĂĽhrten erfahrene Diagnostiker durch, die zu den klinischen
Daten und den Ergebnissen der ĂĽbrigen Untersuchungsverfahren verblindet waren.
Mit Hilfe vor Studienbeginn definier-ter Kriterien wurde die Aktivität für den
Ultraschall und fĂĽr die Ileokoloskopie fĂĽr jedes einzelne Darmsegment
bestimmt. Die Aktivität wurde nach einem Score von 1 (keine) bis 4 (hohe)
eingeteilt. Bei den 31 Patienten wurden insgesamt 180 Darmsegmente so-
nographisch und endoskopisch untersucht. Die statistische Auswertung erfolgte
mit dem gewichteten Kappa-Test und mit der Berechung von Sensitivität,
Spezifität sowie den prädiktiven Werten. In den Ergebnissen der Studie zeigte
sich eine hohe Konkordanz in den Resultaten von Ultraschall und
Ileokoloskopie. Die Befunde im Colon descendens stimmten am stärks-ten
ĂĽberein. Diese prospektive Studie evaluierte die Power-Dopplersonographie und
beantwortete die Fragestellungen wie folgt: 1\. Die Einschätzung der
entzündlichen Aktivität einzelner Darmsegmente durch die Sonographie unter
Einbeziehung der neuen Dopplermethoden im verblindeten Vergleich mit der
Ileokoloskopie fĂĽhrt zu konkordanten Ergebnissen. 2\. Die Sonographie kann die
Ausdehnung der EntzĂĽndung bei Patienten mit einem Morbus Crohn korrekt
einschätzen, wobei ihre Spezifität höher als ihre Sensitivi-tät ist.
Insbesondere geringgradige EntzĂĽndungen der Mucosa, die sich im endo-
skopischen Bild mit einzelnen Aphten präsentieren, entziehen sich häufiger als
ausgeprägte entzündliche Veränderungen dem sonographischen Nachweis. Die-sen
Zusammenhang spiegeln auch die prädiktiven Werte wider. Der positive
prädiktive Wert ist höher als der negative, der Nachweis einer Entzündung
durch den Ultraschall ist sicherer als der sonographische Ausschluss. Die
Kombination von B-Mode und Power-Dopplersonographie besitzt eine hohe Genau-
igkeit in der Bewertung der Aktivität bei Morbus Crohn. Der Ultraschall als
Kombination von B-Mode und Power-Dopplersonographie in der Hand des erfahrenen
Untersuchers kann zu Verlaufsuntersuchungen bei Patienten mit einem Morbus
Crohn empfohlen werden.Introduction: Currently, endoscopy is the preferred technique for assessing
the activity of Crohn’s disease in the colon and the terminal ileum but this
is invasive. Since patients with Crohn’s disease may require frequent
evaluation, there is a high need for a non-invasive method. In recent years,
power Doppler sonography has been proposed as a method to assess Crohn’s
disease activity. The aim of this prospective study was to evaluate diagnostic
criteria for power Doppler sonography by blinded comparison with
ileocolonoscopy. Methods: Thirty-one patients with confirmed Crohn’s disease
were prospectively investigated with B-mode and power Doppler sonography (HDI
5000, Philips Ultrasound) as well as ileocolonoscopy. Sonography was performed
within 3 days before endoscopy. All procedures were performed by experienced
examiners who were blinded to the clinical data and other results. Defined
ultrasound parameters (bowel wall thickness, vascularization pattern) were
used to determine a sonographic score of the activity. The degree of activity
was scored from 1 (none) to 4 (high) by both ultrasound and ileocolonoscopy
(pattern, extent of typical lesions). For each patient all segments of the
colon and the terminal ileum were evaluated by both ultrasound and endoscopy.
The Weighted Kappa Test (StatXact, software) was used for statistical
analysis. Results: In total 180 bowel segments were evaluated by both
ultrasound and endoscopy. The study showed a high concordance of power Doppler
sonography and ileocolonoscopy (weighted kappa by region- sigmoid colon: 0.82;
transverse colon: 0.81; ascending colon: 0.79; cecum: 0.81; terminal ileum:
0.86). Highest concordance was found in the descending colon (weighted kappa:
0.90; 95% confidence interval: 0.83 – 0.98). Conclusions: The results
demonstrated a high concordance of the non-invasive combination of B-mode
ultrasound and power Doppler sonography with ileocolonoscopy, the current
diagnostic standard. The diagnostic criteria established in this study can be
a useful tool to monitor disease activity in patients with Crohn’s disease by
ultrasound
