New aspects of pharmacotherapy in acute heart failure
Acute heart failure is an important clinical problem. In the USA approximately one million
patients are hospitalised annually for acute heart failure, and about 50% of these require
readmission within 6–12 months of discharge. Acute heart failure accounts for most hospital
admissions among patients over 65 years of age in the USA.
The prognosis in heart failure patients is poor. Mortality at five years is 25% and is higher
than in patients with myocardial infarction and some cancers. Despite major progress in
chronic heart failure management (Evidence Based Medicine) the treatment of acute heart
failure is still empirical. Large multi-centre studies on the pharmacotherapy of acute heart
failure are still lacking. The authors discuss management in acute heart failure concerning
guidelines of the European Society of Cardiology from 2005 and modern pharmacotherapy

Niewydolność serca (HF) jest istotnym problemem zdrowotnym ze względu na wysoką śmiertelność oraz częste hospitalizacje. W wielu krajach, w tym także w Polsce, ostatnie lata wskazują, że hospitalizacje z powodu HF są najczęstszym powodem przyjęć do szpitali. Dlatego zapobieganie hospitalizacjom jest niewątpliwie wyzwaniem współczesnej medycyny. Chorzy po przebytym epizodzie zaostrzenia HF charakteryzują się wysoką śmiertelnością i częstymi ponownymi hospitalizacjami szczególnie w okresie od 60 do 90 dni po opuszczeniu szpitala (odpowiednio 15% i 30%), mimo że u większości z nich udaje się uzyskać istotną poprawę w odpowiedzi na zastosowane leczenie. Świadomość najwyższego ryzyka zgonu oraz ponownej hospitalizacji chorych z HF w okresie bezpośrednio po wypisaniu ze szpitala ma implikacje kliniczne i narzuca konieczność odpowiedniego postępowania. Najważniejsza jest optymalizacja terapii. W niniejszym artykule przedstawiono praktyczne wskazówki dotyczące postępowania w okresie przedwypisowym oraz we wczesnej fazie po wypisaniu ze szpitala
Atrial fibrillation as a prognostic factor in patients with systolic heart failure
Background: Atrial fibrillation (AF) decreases left ventricular stroke volume, impaires the
mechanical function of heart valves, and disturbes neurohumoral activity of the miocardium.
This study evaluated the influence of AF on clinical presentation and long-term follow-up of the
patients with systolic heart failure.
Methods: A retrospective analysis of the data on clinical presentation of 152 patients (38 women
and 114 men) with systolic heart failure (EF < 40%) was performed. The following parameters
were compared between patients with and without AF: heart rate and presence of LBBB, several
echocardiographic parameters (EF, LVEDV, size of LA, LVEDd, LVESd), oxygen consumption
per minute, and frequency of end-point’s (death, stroke, rehospitalization, CABG) during mean
936-day follow-up.
Results: The 53 (35%) AF patients in comparison with 99 non-AF were characterized by:
lower exercise performance (VO2max = 10.9 vs. 17.2 ml/kg/min), greater diameter of the left
atrium (49.9 vs. 45.9 mm) and decreased end-diastolic volume (LVEDV = 159.5 vs. 183.7 ml),
less frequent occurrence of LBBB (7.5% vs. 13.0%), higher overall mortality (28.3% vs. 17.1%),
and more frequent rehospitalizations (15% vs. 8%). In patients with persistent AF (40%) with
HR < 75/min the mortality was 29% and was statistically significantly lower than in patients
with HR > 75/min in whom it was 54%.
Conclusions: Atrial fibrillation in patients with systolic heart failure is a disadvantageous, but
not independent risk factor. In patients with persistent atrial fibrillation the control of heart rate
< 75/min was associated with significant decrease in mortality
Restrictive left ventricular filling pattern and its effect on the clinical course of systolic heart failure in patients receiving carvedilol
Background: To analyze differences in brain natriuretic peptide (BNP) levels depending on
mitral flow pattern (MFP) and to assess the effects of carvedilol on changes in MFP, left
ventricular function and exercise capacity.
Methods and results: The study population consisted of 73 patients with symptomatic heart
failure in NYHA classes II and III and LVEF < 40% without prior beta-blockade. In all
patients at baseline, before carvedilol, and then at 3 and 12 months after initiation of treatment,
the following parameters were assessed: HRs, serum BNP, echocardiographic parameters, and
exercise capacity with gas monitoring during cardiopulmonary stress test. Before carvedilol there
was a positive correlation between BNP and E/A (r = 0.17; p = 0.05). BNP was significantly
higher in patients with restrictive MFP (rMFP) as compared with nonrestrictive MFP (nrMFP)
(541.5 ± 206.7 vs. 412.6 ± 207.2; p = 0.009), and lower VO2peak in rMFP as compared with nrMFP
(12.5 ± 3.7 vs. 16.5 ± 4.7; p = 0.001). After initiation of carvedilol, the patients with rMFP had
reduced E/A (2.9 vs. 1.4; p = 0.003), and rMFP was changed to nrMFP in 60.8% of patients.
Respective BNP concentrations were 342.16 ± 284.31 vs. 326.40 ± 264.6; NS. In patients with
rMFP VO2peak , %N increased significantly from 42.4 ± 10.2 to 52.4 ± 14.4; p = 0.012.
Conclusions: In patients with systolic congestive heart failure, the presence of rMFP is
related to higher BNP levels and reduced VO2peak. Chronic treatment with carvedilol replaces
rMFP with nrMFP and improves exercise capacity in some patients
Leczenie reperfuzyjne zawału wspomagane endogennymi mechanizmami kardioprotekcyjnymi
A prompt primary percutaneous coronary intervention with aspiration thrombectomy and subsequent stent implantation at
the culprit lesion combined with optimal anti-platelet and anti-thrombotic pharmacotherapy is the safest and most effective
reperfusion strategy. Unfortunately, this therapy does not guarantee always a good, long-term clinical outcome and left
ventricular function recovery. Despite the unquestionable benefit of reperfusion, we have evidence that its initial phase leads
to additional damage at the area at risk, so the final effect is the compromise between benefits and destruction, which come
with the reperfusion wavefront. Experimental studies suggest that postconditioning with several very brief cycles of ischaemia
alternating with reperfusion applied immediately after relief of a prolonged epicardial occlusion is associated with the infarct
size limitation. The cellular protective effect of postconditioning seems to be related to prevention of mitochondrial permeability
transition pore activation. We have still to wait for confirmation of the clinical effectiveness of both postconditionig as
well as pharmacotherapy that mimics postconditioning effects (e.g. with cyclosporine) in a randomised, multicenter clinical
trial with properly designed endpoint. To date, most clinically tested agents that induced endogenous cardioprotection such
as adenosine, erythropoietin, protein kinase C-δ inhibitor, atrial natriuretic peptide, atorvastatin and nicorandil were failed to
reduce infarct size. Time will tell whether postconditioning and therapy that it mimics meet expectation and find place in the
recommendations concerning management of acute myocardial infarction.

pierwotnej angioplastyki wieńcowej połączonej z odessaniem skrzepliny z nasierdziowego odcinka i implantacji stentu
w miejsce odpowiedzialne za incydent w osłonie leków przeciwpłytkowych i przeciwzakrzepowych. Niestety, u dużego
odsetka pacjentów model ten nie gwarantuje dobrego odległego wyniku klinicznego ani poprawy funkcji lewej komory.
Mimo bezsprzecznych korzyści związanych z reperfuzją, naukowcy dysponują dowodami na to, że jej początkowa faza
prowadzi do dodatkowego uszkodzenia wcześniej niedokrwionego obszaru, a zatem końcowy efekt stanowi wypadkową
korzyści i zniszczenia, jakie wiążą się z falą reperfuzji. Badania doświadczalne wskazują, że hartowanie reperfuzją polegające
na kilku cyklach naprzemiennie występujących krótkich epizodów reperfuzji i niedokrwienia bezpośrednio po zakończeniu
fazy niedokrwienia, a przed rozpoczęciem fazy reperfuzji, ogranicza strefę zawału. Komórkowym efektorem tego procesu
odpowiedzialnym za działanie ochronne są mitochondrialne megakanały. Wciąż brak klinicznego potwierdzenia skuteczności
zarówno procesu hartownia reperfuzją, jak i farmakoterapii naśladującej hartowanie z udziałem cyklosporyny w randomizowanych,
wieloośrodkowych badaniach klinicznych z odpowiednio zaprojektowanym punktem końcowym. Dotychczas
większość klinicznie przebadanych preparatów naśladujących endogenną kardioprotekcję, takich jak m.in. adenozyna, erytropoetyna,
inhibitor kinazy białkowej C-δ, przedsionkowy peptyd natriuretyczny, atorwastatyna czy nikorandil, okazała się
nieskuteczna. Czas pokaże, czy hartowanie reperfuzją i jego naśladowanie spełnią pokładane nadzieje i znajdą uznanie
w zaleceniach dotyczących leczenia zawału.
