Formation et évaluation en ligne des compétences en inspection visuelle du col utérin avec acide acétique (VIA) et Lugol (VILI) dans les pays à faibles et moyennes ressources économiques (LMIC)

Objectif: Malgré son interprétation subjective, l'inspection visuelle est une méthode prometteuse de dépistage du cancer du col utérin dans les pays à faibles ressources économiques. Le but de cette thèse est de valider un instrument basé sur le web qui permet d'évaluer les compétences diagnostiques et la gestion des cas pathologiques. Méthodes: Les professionnels de santé ont été invités à participer à une évaluation des compétences dans l'inspection visuelle du col. Nous avons proposé un cours théorique, suivi d'un test en ligne comprenant 70 questions à choix multiples, basées sur des cas cliniques provenant du dépistage. Résultats: Dans la période d'août à octobre 2014, 255 participants ont effectué le test avec un taux de réussite de 38,8%. Les participants avec de l'expérience en colposcopie et les médecins ont eu respectivement 3,6 et 2 fois plus de chance de réussir en comparaison aux autres professionnels de santé. Conclusions: Le test VIA/VILI en ligne est réalisable et permet d'identifier les participants expérimentés

Bilateral temporomandibular joint ankyloses after bilateral sagittal split osteotomy of the mandible: An uncommon clinical case with long-term outcome evaluation

Temporomandibular joint (TMJ) ankylosis is an extremely handicapping condition characterized by a permanent restriction in the ability to open the mouth, and it is mainly caused by traumatic and infectious events. It rarely occurs as a complication following orthognathic surgery. The treatment goals are the restoration of viable mandibular function and, in growing pediatric patients, symmetrical mandibular development. We are reporting here the unusual case of a bilateral bony TMJ ankylosis in a patient who underwent a bilateral sagittal split osteotomy of the mandible for correction of a mandibular deficiency. The patient was treated by bilateral arthrotomies with the interposition of a full-thickness skin graft as well as bilateral coronoidectomies with a favorable long-term (6 years) functional outcome. Only one similar case has been previously reported in the literature
