    The Influence of market focus planning strategies on competitiveness of private universities in Nairobi County, Kenya

    In the last three decades, the republic of Kenya has witnessed a tremendous increase in the number of chartered universities and a stiff competition for students. The student enrolment base coupled with the emergence of private university education providers turned the university arena in Kenya into a student enrolment market, leading to intense competition between Public and Private Universities. The researcher realizes that, the existing studies relate competitiveness with performance. Thus, the purpose of this study was to assess the influence of market focus planning strategies on competitiveness of private universities in Nairobi County, Kenya. Resource Based Theory, Competitive Advantage Theory and Generic Framework Theory guided this study. The study applied mixed method approach and thus adopted concurrent triangulation design. Target population comprised 66 Registrar Academics, 66 Registrar Admissions and 33 Directors of Marketing all totalling to 165. Using the Central Limit Theorem, 36 Registrars of Academics, 36 Registrars of Admissions and 18 Directors of Marketing were purposively sampled. Questionnaires were used to collect data from Registrar Academic and Admissions whereas interview guide was used to gather data from Directors of Marketing. Data analysis began by identifying common themes from the respondents’ description of their experiences. Qualitative data were analyzed thematically along the objectives and were presented in narrative forms. Quantitative data were analyzed using descriptive statistics and inferentially using Chi-Square through Statistical Packages for Social Science and presented using tables. The study established that many private universities have not fully adopted market focus planning strategies to enhance their competitiveness. The study recommends that private universities design and market their academic programmes affordable to students from all socio-economic backgrounds

    Youth Engagement in Agriculture in Kenya: Challenges and Prospects

    The Kenyan population is to a large extent comprised of a high and increasing cohort of young people, close to 78 percent of the population is below the age of thirty. Evidence reveals that youth engagement in agriculture is declining amidst rising youth unemployment yet the services and industrial sectors, despite growing at considerably faster rates have not created enough jobs for the burgeoning youthful labor force. This may have implications on food security, unemployment, and underemployment and may undermine the government efforts to drive economic growth through agriculture. Using data from the Kenya National Panel Survey data of 2005/6 and 2009/10, we examine youth employment dynamics across the different sectors and further provide insights into the determinants of youth participation in agriculture. Using the Kenya Census of 2009, we further document the challenges and constraints inherent to the youth in agricultural production relative to adults. The findings reveal that youthful farmers are concentrated more in agricultural production. Furthermore, a relatively lower percentage of youth use improved inputs (such as improved seeds, fertilizers, agricultural chemicals and veterinary drugs). With this poor rate of adoption of appropriate inputs, productivity is likely to remain low and constrain the youth to subsistence farming. The youth are also disenfranchised in the ownership and management of critical assets in agricultural production, especially land. Land tenure issues continue to impede many youths from engaging in agriculture, with the majority of youth using land without exclusive ownership rights. In addition, the results point to the fact that the youth are less likely to access credit, extension services and social capital (farmer group membership), all key factors in agricultural transformation. The in-depth analysis results seem to suggest that the youth with at least secondary education, males (both married and unmarried) and those youth residing in households with a large share of adults are less likely to engage in agriculture. It was recommended that a robust relationship between agencies interested in encouraging youth involvement in agriculture should be evolved through legislation and implementation of policies to guarantee training programmes, credit facilities and land accessibility to youths at the identified rural youth organizations.     The prevalence of retinopathy in prediabetes: A systematic review.

    Diabetic retinopathy is a leading cause of vision loss globally. The current diagnostic thresholds for diabetes are still based on historic data correlating glycemic parameters with retinopathy; however, an excess prevalence of retinopathy has also been reported in prediabetes. We aimed to determine the reported prevalence of retinopathy in adults with prediabetes. We performed searches using MEDLINE, EMBASE, PubMed, Web of Science, CINAHL, Google Scholar and the Cochrane databases from inception to August 1, 2020. We evaluated methodological quality and certainty of the evidence using a validated risk of bias tool and GRADE, respectively. Twenty-four studies (8,759 participants with prediabetes) were included after screening 5,155 abstracts and reviewing 98 full-text records. Nineteen studies (79%) reported population-based data. Retinopathy prevalence estimates ranged between 0.3-14.1% (median 7.1%, interquartile range 2.4-9.7%), with high variance in estimates due to differing screening methods, retinopathy grading protocols and study populations. We judged this as low-certainty evidence using GRADE, downgrading for risk of bias and inconsistency. From studies that compared both populations, post hoc analysis revealed a lower median retinopathy prevalence in normal glucose tolerance (3.2%, interquartile range 0.3-7.3%) than prediabetes (6.6%, interquartile range 1.9-9.8%). These data suggest an excess prevalence of retinopathy in prediabetes

    Detection of the spotted wing drosophila, Drosophila suzukii, in continental sub-Saharan Africa

    The spotted wing drosophila, Drosophila suzukii Matsumura, is an insect pest of soft-skinned fruit, native to Eastern Asia. Since 2008, a world-wide dispersal of D. suzukii is seen, characterized by the establishment of the pest in many Asian, American and European countries. While the potential for invasion of continental Africa by D. suzukii has been predicted, its presence has only been shown for Morocco in Northern Africa. Knowledge about a possible establishment in other parts of the continent is needed as a basis for pest management. In 2019, we carried out a first survey in three counties in Kenya to monitor for the presence of D. suzukii using traps baited with a blend of apple cider vinegar and red wine. A total of 389 D. suzukii flies were captured in a fruit farm at Nakuru county, with more female flies being trapped than males. We confirmed the morphological identification of D. suzukii using DNA barcoding. In 2020, we performed a follow-up survey at 14 locations in six counties to delimit the distribution of D. suzukii in the main berry growing zones in Kenya. The survey indicated that so far D. suzukii is restricted to Nakuru county where it was initially detected. This is the first study to provide empirical evidence of D. suzukii in continental sub-Saharan Africa, confirming that the pest is expanding its geographic range intercontinentally. Given the high dispersal potential of D. suzukii, a concerted effort to develop management strategies is a necessity for containment of the pest

    Multicenter Evaluation of Diagnostic Circulating Biomarkers to Detect Sight-Threatening Diabetic Retinopathy

    Importance: It is a global challenge to provide regular retinal screening for all people with diabetes to detect sight-threatening diabetic retinopathy (STDR). Objective: To determine if circulating biomarkers could be used to prioritize people with type 2 diabetes for retinal screening to detect STDR. Design, Setting, and Participants: This cross-sectional study collected data from October 22, 2018, to December 31, 2021. All laboratory staff were masked to the clinical diagnosis, assigned a study cohort, and provided with the database containing the clinical data. This was a multicenter study conducted in parallel in 3 outpatient ophthalmology clinics in the UK and 2 centers in India. Adults 40 years and older were categorized into 4 groups: (1) no history of diabetes, (2) type 2 diabetes of at least 5 years' duration with no evidence of DR, (3) nonproliferative DR with diabetic macular edema (DME), or (4) proliferative DR. STDR comprised groups 3 and 4. Exposures: Thirteen previously verified biomarkers were measured using enzyme-linked immunosorbent assay. Main Outcomes and Measures: Severity of DR and presence of DME were diagnosed using fundus photographs and optical coherence tomography. Weighted logistic regression and receiver operating characteristic curve analysis (ROC) were performed to identify biomarkers that discriminate STDR from no DR beyond the standard clinical parameters of age, disease duration, ethnicity (in the UK) and hemoglobin A1c. Results: A total of 538 participants (mean [SD] age, 60.8 [9.8] years; 319 men [59.3%]) were recruited into the study. A total of 264 participants (49.1%) were from India (group 1, 54 [20.5%]; group 2, 53 [20.1%]; group 3, 52 [19.7%]; group 4, 105 [39.8%]), and 274 participants (50.9%) were from the UK (group 1, 50 [18.2%]; group 2, 70 [25.5%]; group 3, 55 [20.1%]; group 4, 99 [36.1%]). ROC analysis (no DR vs STDR) showed that in addition to age, disease duration, ethnicity (in the UK) and hemoglobin A1c, inclusion of cystatin C had near-acceptable discrimination power in both countries (area under the receiver operating characteristic curve [AUC], 0.779; 95% CI, 0.700-0.857 in 215 patients in the UK with complete data; AUC, 0.696; 95% CI, 0.602-0.791 in 208 patients in India with complete data). Conclusions and Relevance: Results of this cross-sectional study suggest that serum cystatin C had good discrimination power in the UK and India. Circulating cystatin-C levels may be considered as a test to identify those who require prioritization for retinal screening for STDR

    Artificial Intelligence and Corneal Confocal Microscopy: The Start of a Beautiful Relationship.

    Corneal confocal microscopy (CCM) is a rapid non-invasive in vivo ophthalmic imaging technique that images the cornea. Historically, it was utilised in the diagnosis and clinical management of corneal epithelial and stromal disorders. However, over the past 20 years, CCM has been increasingly used to image sub-basal small nerve fibres in a variety of peripheral neuropathies and central neurodegenerative diseases. CCM has been used to identify subclinical nerve damage and to predict the development of diabetic peripheral neuropathy (DPN). The complex structure of the corneal sub-basal nerve plexus can be readily analysed through nerve segmentation with manual or automated quantification of parameters such as corneal nerve fibre length (CNFL), nerve fibre density (CNFD), and nerve branch density (CNBD). Large quantities of 2D corneal nerve images lend themselves to the application of artificial intelligence (AI)-based deep learning algorithms (DLA). Indeed, DLA have demonstrated performance comparable to manual but superior to automated quantification of corneal nerve morphology. Recently, our end-to-end classification with a 3 class AI model demonstrated high sensitivity and specificity in differentiating healthy volunteers from people with and without peripheral neuropathy. We believe there is significant scope and need to apply AI to help differentiate between peripheral neuropathies and also central neurodegenerative disorders. AI has significant potential to enhance the diagnostic and prognostic utility of CCM in the management of both peripheral and central neurodegenerative diseases

    Board characteristics and earnings management. Does firm size matter?

    The purpose of this paper is to extend the existing literature by empirically examining the effect of board characteristics on earnings management (EM) from a developing region perspective. The study further adds literature by examining whether firm size moderates the relationship between board characteristics and EM. This study employs data drawn from 88 listed firms in the East African Community (EAC) for the period between 2011 and 2020. The study used the system generalized method of moments (SGMM) estimation model to take care of potential endogeneity and reverse causality. The findings revealed a positive and significant relationship between board size and EM. The findings further indicated that board independence, board gender diversity, and board financial expertise had a negative and significant effect on EM. In addition, the findings confirmed that firm size moderated the relationship between board size, board independence, board gender diversity, and EM. The insights of this study may provide useful information for shareholders and regulators in evaluating board attributes that are effective in mitigating earnings management practices from a developing region. Further, board effectiveness in deterring EM should be evaluated with regard to firm size. Just a few empirical studies have examined the relationship between board characteristics and EM in developing regions. Thus, this study contributes to the existing literature by empirically examining the topic in the EAC.
