Kontrola glikemii i jej zmienność głównymi czynnikami ryzyka mikroalbuminurii u dzieci chorych na cukrzycę typu 1
Introduction: To assess in a prospective study the course and the predictors of microalbuminuria in children and adolescents with type 1 diabetes.Material and methods: 438 children and adolescents who developed diabetes in the years 1985–2004 were followed for 9.2 ± 3.4 years from the diagnosis. Microalbuminuria was assessed on the basis of timed overnight urine collections performed once per year. Variability of glycated haemoglobin was expressed as a coefficient of variation (%) calculated by dividing standard deviation (adjusted for the number of measurements) by mean of HbA1c.Results: Microalbuminuria was noted in 99 patients (22.6%) after 8.27 ± 3.3 years of diabetes. In 29 individuals (6.6%), microalbuminuria was persistent. The prevalence of microalbuminuria was not dependent on the period of diabetes diagnosis. During followup, 17 (58.6%) patients with persistent MA reverted to normoalbuminuria. Children without any episodes of microalbuminuria had significantly lower HbA1c variability (8.44%; 95% CI 7.81–9.08%) than those with one (10.28% 95% CI 9.10–11.47%; p = 0.007). The difference of HbA1c variability between patients with and without microalbuminuria persisted after correction by mean HbA1c (p = 0.04). Risk factors for ever developing microalbuminuria during the observation period in multivariate analysis included: mean HbA1c (HR [95% CI]: 1.17 [1.00–1.37; p = 0.05]) and its variability (1.04 [1.00–1.07]; p = 0.05), insulin dose (HR per 0.1 unit*kg- 1*day–1: 0.87 [0.79–0.96]; p = 0.005), presence of arterial hypertension (1.63 [1.07–2.49]; p = 0.02), and age at onset of diabetes (1.15 [1.08–1.21]; p < 0.0001).Conclusions: Children who develop microalbuminuria are characterised by poorer and more variable metabolic control, hinting at the importance of interventions aimed at both improvement and stabilisation of HbA1c levels. Increased incidence of type 1 diabetes in children and no change in the age of diagnosis and BMI-SDS at the onset : is the accelerator hypothesis not working?
Objective: One of the hypothesized reasons for the observed increase in type 1 diabetes incidence in children is weight gain, causing accelerated disease development in predisposed individuals. This so-called accelerator hypothesis is, however, controversial. The aim was to analyze whether, in the ethnically homogeneous population of Lesser Poland, an increase in the number of cases of diabetes among children was associated with younger age and higher body mass index-standard deviation score (BMI-SDS) at the time of diagnosis.
Methods: Retrospective data analysis from medical records of all patients <14 years (n=559; 50.6% male), with newly diagnosed type 1 diabetes, in Lesser Poland between 1st January 2006 and 31st December 2017 (11 years).
Results: The incidence ratio ranged significantly (p<0.001) from the lowest in 2006 (11.2/100,000/year) to the highest in 2012 (21.9/100,000/year). The mean age of diagnosis was 8.2±3.5 years. There was no trend in decreasing diagnosis age (p=0.43). The mean BMI-SDS was -0.4±1.2. Almost all children (91.6%) presented with BMI-SDS within the normal range at the time of diagnosis, with only 2.7% of cases being obese and 5.7% underweight at the moment of diagnosis. There was no clear trend at all in BMI-SDS over the study period.
Conclusion: These results do not corroborate an increase of type 1 incidence in paediatric population being associated with younger age of diagnosis and higher BMI-SDS. This implies that the accelerator hypothesis does not hold true in the study population
Growing and sexual development and IGF-1, its binding proteins IGFBP-1 and IGFBP-3 in children and adolescents with type 1 diabetes mellitus
1 (IGF-1, insulin-like growth factor-1) i jego białek wiążących IGFBP-1
i IGFBP-3 (insulin-like growth factor binding protein) w trakcie trwania
choroby. Oceniano też wpływ sposobu leczenia na stężenia wyżej wymienionych czynników
wzrostowych i parametry auksologiczne.
MATERIAŁ I METODY. Do badań włączono 148 chorych na cukrzycę typu 1, leczonych
metodą konwencjonalną i metodami wielokrotnych wstrzyknięć insuliny, w wieku 2–22
lat, u których choroba trwała 0–21 lat, i 54 zdrowych rówieśników. Oceniano u
nich parametry auksologiczne i dojrzewanie płciowe. Krew do badań pobierano w
godzinach 7.30 –8.30 rano, na czczo. Surowicze stężenia IGF-1 oznaczano metodą
radioimmunologiczną (RIA, radioimmunoassay), stężenia IGFBP-1 i IGFBP-3
metodą immunoradiometryczną (IRMA, immunoradiometric assay) a HbA1c
przy użyciu wysoko rozdzielczej chromatografii cieczowej (HPLC, high performance
liquid chromathography). Do analizy statystycznej wykorzystano test t-Studenta,
ANOVA, regresję liniową i wieloczynnikową.
WYNIKI.
niż w grupie kontrolnej, w czasie jej trwania stopniowo się obniżał, by osiągnąć
Wzrost dzieci i młodzieży chorych
i IGFBP-3, u wszystkich chorych na cukrzycę korelował on z dobowym zapotrzebowaniem
IGFBP-1. Warunkują one odpowiednią bioaktywność IGF-1.INTRODUCTION. The aim of the study was evaluation of the somatic development
and sexual maturation of children and adolescents with type 1 diabetes mellitus
and estimation of their IGF-IGFBP system at the on-set of diabetes as well as
during the progress of the disease. It was also analysed how the methods of insulinotherapy
influence estimated growth factors and auxology parameters.
MATERIAL AND METHODS. 148 diabetic patients without other diseases treated
with conventional and intensive insulinotherapy with diabetes duration 0-21 years
and 54 age-matched healthy children and adolescents 2–22 years old were included
into the study. Their somatic and pubertal development were estimated by experienced
physician and anthropologist. Blood samples were taken at 7.30–8.30 a.m., after
8h fasting. The serum concentrations of IGF-I were measured by RIA; IGFBP-1 and
IGFBP-3 concentrations by IRMA and HbA1c by HPLC method. The data were
analysed using test t-Student, ANOVA, linear and multiple regression analyses.
RESULTS. The diabetic patients’ height did not differ from the height of
healthy children and adolescents. At the onset of diabetes patients were not significantly
taller than their healthy contemporaries but with the disease duration progress
their growth slowly decreased and finally they reached lower level of height than
in the control group (the difference didn’t reach statistical significance). Sexual
maturation of diabetic patients did not differ from age related healthy polish
population independently of diabetes duration. All diabetic and healthy children’s
z score of height were higher than z score of mid-parental height. IGF-I concentrations
were lower and IGFBP-3 concentrations were higher in diabetic patients than in
the control group. IGFBP-1 levels did not differ in both groups. Before the onset
of puberty patients’ height correlated with IGF-I, IGFBP-1, and IGFBP-3. In all
diabetic patients height correlated with insulin requirement. IGF-1 concentrations
correlated also with insulin requirement and HbA1c. Diabetic patients treated
with intensive insulinotherapy were slightly taller and had higher IGF-1 and lower
IGFBP-1 concentrations than patients treated with other methods of insulinotherapy.
CONCLUSIONS. Somatic development of diabetic children depends not only
on genetic influence, but also on adequate insulin substitution which could be
achieved by intensive insulinotherapy. Despite decreased IGF-I levels, height
within normal ranges was attained, probably because of adequate insulin compensation
leading to normal IGFBP-1 levels, thus adequate IGF-I bioavailability

metodą HPLC (high performance liquid chromatography) (BioRad).
WYNIKI.
0,69 j./kg/d.). Częstość ciężkich niedocukrzeń przed
leczeniem CSII i w trakcie terapii była jednakowa
(1 epizod przed i 1 w czasie CSII).

Mutation screening in Kir6.2 gene in subjects with permanent neonatal diabetes : clinical characteristics and the modification of treatment method
WSTĘP. W ostatnim czasie opisano aktywujące mutacje w genie KCJN11 kodującym Kir6.2, ATP-wrażliwą podjednostkę kanału potasowego, występujące u pacjentów z trwałą cukrzycą noworodków (PND,
permanent neonatal diabetes).
MATERIAŁ I METODY. Celem badania była ocena znaczenia mutacji w genie KCJN11 w powstawaniu PND
w populacji polskiej.
przeprowadzono sekwencjonowanie genu KCJN11.
WYNIKI. Zidentyfikowano 3 pacjentów z heterozygotycznymi mutacjami zmiany sensu. Dwóch chłopców (Pol1 i Pol2) było nosicielami uprzednio opisanej mutacji R201H, natomiast jedna dziewczynka
(Pol3), u której diagnozę postawiono w wieku
26 tygodni, była nosicielem nowej mutacji R50Q.
W chwili zachorowania wszyscy trzej nosiciele mutacji
w genie Kir6.2 charakteryzowali się znaczną hiperglikemią (30-50 mmol/l). Warte odnotowania były
cechy klinicznej heterogenności: wiek rozpoznania
wynosił 2, 3 i 26 tygodni odpowiednio w badaniach
Pol1, Pol2, Pol3; wszyscy troje byli urodzeni o czasie, urodzeniowa masa ciała wynosiła odpowiednio
2450, 2700 i 3000 g, a zapotrzebowanie na insulinę
w chwili badania - 0,25, 0,68 oraz 0,1 U/kg. Wszystkich trzech pacjentów z powodzeniem przestawiono
na terapię pochodną sulfonylomocznika. W przypadku
Pol1 i Pol2 zastosowano glipizyd o zmodyfikowanym
uwalnianiu (GITS) odpowiednio w dawce 5 i 30 mg,
w przypadku Pol3 użyto 0,5 mg glimepirydu.
WNIOSKI.

Activating mutations in the KCJN11
gene encoding Kir6.2, the ATP-sensitive potassiumchannel subunit have been recently described in patients with permanent neonatal diabetes (PND).
MATERIAL AND METHODS. We examined the contribution of KCJN11 mutations to PND in patients from
Poland. We sequenced the KCJN11 gene in 7 insulin
treated, diabetic patients diagnosed before 6 months (range 1-26 weeks).
RESULTS. We identified three patients with de novo
heterozygous missense mutations. Two males, (Pol1
and Pol2) carried the previously described R201H mutation and one female (Pol3) diagnosed at 26 weeks
was a carrier of novel mutation R50Q. All three subjects with Kir6.2 mutations presented with severe hyperglycemia at the diagnosis (30-50 mmol/l). There
was evidence of clinical heterogeneity: the age of
diagnosis was 2, 3 and 26 weeks in Pol1, Pol2, Pol3
respectively; all were born at term with birth weights
of 2450, 2700 & 3000 g and insulin requirements varied 0.25, 0.68, 0.1 U/kg. All three patients were successfully transferred to sulphonylurea therapy: Pol1
and Pol2 to sustained-release Glipizide 5 and 30 mg
daily, respectively, Pol3 to 0.5 mg glimepiride.
CONCLUSION. We conclude that Kir6.2 mutations are
a common cause of PND in European Caucasians and
provided the evidence of clinical heterogeneity between mutation carriers. The clinical results are consistent with the novel mutation R50Q being less severe than R201H although variation between patients
with R201H suggest there may be other genetic or
environmental moderators of phenotyp
Nieinsulinowa hipoglikemia trzustkowa u dorosłych — przegląd genetyki
Hyperinsulinaemic hypoglycaemia (HH) is also classically referred to as “nesidioblastosis”. Heterogeneous clinical manifestation of the disease causes risk of late diagnosis or even misdiagnosis. In infants and children, it can lead to serious and permanent damage to the central nervous system, which leads to the manifesting mental retardation. HH is characterised by unregulated insulin secretion from pancreatic β-cells. This effect has been correlated with nine genes: ABCC8, KCNJ11, GCK, GLUD-1, HADH1, SLC16A1, HNF4A, HNF1A, and UCP2. Mutations in these genes were found in approximately 48% of cases. The genetic background of the remaining cases is unknown. Understanding the genetic basis of familial hyperinsulinism has changed the early look at the disease. It has allowed for the differentiation of specific types of the disease. Depending on which of the nine disease-associated loci bears a pathogenic mutation, they differ in phenotype and pattern of inheritance. This review provides a brief overview of the genetic mechanisms of HH and its possible clinical presentations. (Endokrynol Pol 2015; 66 (4): 344–354)
(Endokrynol Pol 2015; 66 (4): 344–354)
Mutation screening in Kir6.2 gene in subjects with permanent neonatal diabetes. Clinical characteristics and the modification of treatment method
WSTĘP . W ostatnim czasie opisano aktywujące mutacje
w genie KCJN11 kodującym Kir6.2, ATP-wrażliwą
podjednostkę kanału potasowego, występujące u pacjentów z trwałą cukrzycą noworodków (PND,
permanent neonatal diabetes).MATERIAŁ I METODY. Celem badania była ocena znaczenia
mutacji w genie KCJN11 w powstawaniu PND
w populacji polskiej.
przeprowadzono sekwencjonowanie genu KCJN11.WYNIKI. Zidentyfikowano 3 pacjentów z heterozygotycznymi
mutacjami zmiany sensu. Dwóch chłopców (Pol1 i Pol2) było nosicielami uprzednio opisanej
mutacji R201H, natomiast jedna dziewczynka
(Pol3), u której diagnozę postawiono w wieku 26 tygodni, była nosicielem nowej mutacji R50Q.
W chwili zachorowania wszyscy trzej nosiciele mutacji
w genie Kir6.2 charakteryzowali się znaczną hiperglikemią
(30–50 mmol/l). Warte odnotowania były
cechy klinicznej heterogenności: wiek rozpoznania wynosił 2, 3 i 26 tygodni odpowiednio w badaniach
Pol1, Pol2, Pol3; wszyscy troje byli urodzeni o czasie,
urodzeniowa masa ciała wynosiła odpowiednio
2450, 2700 i 3000 g, a zapotrzebowanie na insulinę
w chwili badania — 0,25, 0,68 oraz 0,1 U/kg. Wszystkich
trzech pacjentów z powodzeniem przestawiono na terapię pochodną sulfonylomocznika. W przypadku
Pol1 i Pol2 zastosowano glipizyd o zmodyfikowanym
uwalnianiu (GITS) odpowiednio w dawce 5 i 30 mg,
w przypadku Pol3 użyto 0,5 mg glimepirydu. WNIOSKI. Podsumowując, mutacje genu Kir6.2 są powszechną
przyczyną PND w populacjach europejskich.
Na uwagę zasługuje kliniczna heterogenność
cechująca nosicieli mutacji. Mutacja R50Q wydaje się
łagodniejsza niż R201H, jednak różnice między nosicielami
mutacji R201H dowodzą, że inne czynniki genetyczne
i środowiskowe mogą wpływać na fenotyp choroby.INTRODUCTION. Activating mutations in the KCJN11
