    This study focused on the narratives of individuals who stutter to examine the process by which their self-image is improved in a self-help group. According to previous sociological studies, individuals who stutter tend to remain silent to protect their dignity, as stuttering violates the turn-taking rules of natural conversation. However, the participants in the group spoke freely with one another at meetings. It appeared to the observer as if the participants were not self-conscious about their stuttering. We analyzed the content of conversations at meetings as well as the life histories of 13 participants who had been enduring the stigmatization of their stuttering since childhood. Their conversations appeared to focus on practical issues regarding how to cope with their suffering rather than on functional questions about how to correct stuttering. Participants tried to speak continuously in the meeting despite their stuttering, and such conversations enabled them to practice coping tactics and fight stigma

    Back muscle activity and sagittal spinal alignment during quadruped upper and lower extremity lift in young men with low back pain history

    [Background]Quadruped upper and lower extremity lift (QULEL) is performed for selective training of the lumbar multifidus muscle in patients with low back pain (LBP) or individuals with LBP history (LBPH). However, the activities of the back muscles and sagittal spinal alignment during QULEL are not clarified in individuals with LBPH.[Research question]This study aimed to analyze the activities of the back muscles and sagittal spinal alignment during QULEL in young male with LBPH.[Methods]The study comprised 9 asymptomatic young men and 8 young men with LBPH. The activities of the lumbar multifidus, latissimus dorsi and thoracic erector spinae, and lumbar erector spinae muscles were measured using surface electromyography. The flexion angles of the upper and lower thoracic spine, and extension angle of the lumbar spine were measured using a 6-DF electromagnetic motion tracking system. The association with LBPH was investigated using multiple logistic regression analysis with a forward selection method, with the activities of the back muscles, sagittal spinal alignment, age, body height, and body weight as independent variables.[Results]Multiple logistic regression analysis (p = 0.0002) showed that the activity of the latissimus dorsi and thoracic erector spinae muscles in the side on which the lower extremity was lifted and body height were significant and independent determinants of LBPH, but other factors were not.[Significance]The results of this study suggest that the activity of the latissimus dorsi and thoracic erector spinae muscles increases while there are no decrease in activity of the lumbar multifidus muscle and excessive extension of the lumbar spine during QULEL in young men with LBPH

    Effects of Antibiotics Administration on the Incidence of Wound Infection in Percutaneous Dilatational Tracheostomy

    The effect of antibiotics during the perioperative period of percutaneous dilatational tracheostomy (PDT) is still controversial. A total of 297 patients who underwent the PDT procedure were divided into 2 groups:those administered antibiotics perioperatively and those not administered antibiotics. Wound infections were noted in 7 cases (incidence rate, 2.36%) and no death was recorded. Of the 69 patients without antibiotics, 5 developed wound infections (incidence rate, 7.25%), while only 2 of the 228 patients with antibiotics developed wound infections (incidence rate, 0.88%) (p=0.002;risk ratio, 8.82;95% confidence interval, 1.67-46.6). Of the 7 cases of wound infection, 5 cases occurred during the early period after PDT (within 7 days). Collectively, the present results suggest that prophylactic administration of antibiotics may prevent the incidence of PDT-induced wound infection, especially in the early phase after the PDT procedures. The need for antibiotics in PDT should be reconsidered

    Usefulness of the SNR Using the Subtraction Method and Image Visibility Using the Howlett Chart Method in X-ray Fluoroscopic System

    X-ray fluoroscopy has been used not only for diagnoses but also for treatment, as in interventional radiology. The aim of this study is to assess the usefulness of the signal-to-noise ratio (SNR) using the subtraction method, which is used in the quality assurance/quality control (QA/QC) of magnetic resonance imaging (MRI) equipment, and image visibility using the Howlett chart method in the fluoroscopic examination. The fluoroscopy unit has a detector with an image intensifier and a color liquid crystal display. A 2620 dosimeter and an ionization chamber were selected for the dose measurement. Fluoroscopic images for visual and physical evaluations were collected as dynamic data. The skin surface absorbed dose rates for each tube current were measured using a dosimeter. The SNR using the subtraction method and image visibility using the Howlett chart method were examined. The results from both indicate that as the SNR improves, visual evaluation increases, but the rate of increase gradually saturates. Therefore, physical and visual evaluations are possible with the SNR using the subtraction method and the image visibility using the Howlett chart method
