    Self-medication with antibiotics among non-medical university students of Karachi: a cross-sectional study

    Background: The prevalence of self -medication with antibiotics is quite high in developing countries as opposed to developed countries. Antibiotics are often taken erroneously for certain ailments, without having the appropriate knowledge of their use. This carries potential risks for the individual as well as the community, in form of several side effects such as antibiotic resistance. Therefore the prevalence of self-medicated antibiotics in developing countries needs to be studied.Methods: A descriptive cross-sectional study was carried out at six different non-medical universities of Karachi. 431 students were included in the study. Data was collected using self-administered questionnaires and analyzed using SPSS version 19.Results: 50.1% students reported having self-medicated themselves in the past 6months and 205 (47.6%) reported self-medication with antibiotics. Amoxicillin was the most self-prescribed antibiotic (41.4%). Awareness of the adverse effects of antibiotics was demonstrated by 77.3% of the students and sleep disturbance was the most commonly known (46.5%) side effect. 63.1% denied having any knowledge about antibiotic resistance and only 19.9% correctly knew that indiscriminate use of antibiotics can lead to increased antibiotic resistance.CONCLUSION: The prevalence of self-medication with antibiotics among the non-medical university students was high despite the awareness of adverse effects.     Production system and challenges of saline aquaculture in Punjab and Sindh provinces of Pakistan

    IntroductionLack of data about saline aquaculture in Pakistan has curtailed informed policy and investment decision making resulting in undervaluation of and underinvestment in the sector. Therefore, saline aquaculture in Pakistan is today an underdeveloped sector despite its potential as an alternative livelihood for the country's rural farmers. This study represents one of the initial exploratory investigations into saline aquaculture systems in Pakistan, aiming to comprehend the existing sectoral landscape, production challenges, post-harvest constraints, and the associated capacity and investment needs.MethodsThe study employed purposive sampling to survey 121 low-income saline aquaculture farmers across five districts of Southern Punjab and Sindh provinces. The analysis was carried out using descriptive statistics.ResultsThe results revealed that the saline aquaculture sector is predominantly male-centric, with agricultural land utilized for both agriculture and aquaculture purposes. Ponds, which usually serve multiple functions, focus primarily on carp production, but adherence to good management practices remains limited. Farmers face various challenges, including the high costs of feed and seeds, freshwater scarcity, inadequate technical knowledge, and marketing issues.Discussion and conclusionThis study serves as a foundational assessment, addressing data and information gaps crucial for supporting the sustainable development of saline aquaculture in Pakistan. To facilitate such development, the study recommends initiating programs to strengthen technical skills in saline aquaculture, together with the establishment of hatcheries and breeding stations for saline-tolerant species, aiming to reduce dependence on freshwater species in saline pond environments

    Effects of antibiotic resistance, drug target attainment, bacterial pathogenicity and virulence, and antibiotic access and affordability on outcomes in neonatal sepsis: an international microbiology and drug evaluation prospective substudy (BARNARDS)

    Background Sepsis is a major contributor to neonatal mortality, particularly in low-income and middle-income countries (LMICs). WHO advocates ampicillin–gentamicin as first-line therapy for the management of neonatal sepsis. In the BARNARDS observational cohort study of neonatal sepsis and antimicrobial resistance in LMICs, common sepsis pathogens were characterised via whole genome sequencing (WGS) and antimicrobial resistance profiles. In this substudy of BARNARDS, we aimed to assess the use and efficacy of empirical antibiotic therapies commonly used in LMICs for neonatal sepsis. Methods In BARNARDS, consenting mother–neonates aged 0–60 days dyads were enrolled on delivery or neonatal presentation with suspected sepsis at 12 BARNARDS clinical sites in Bangladesh, Ethiopia, India, Pakistan, Nigeria, Rwanda, and South Africa. Stillborn babies were excluded from the study. Blood samples were collected from neonates presenting with clinical signs of sepsis, and WGS and minimum inhibitory concentrations for antibiotic treatment were determined for bacterial isolates from culture-confirmed sepsis. Neonatal outcome data were collected following enrolment until 60 days of life. Antibiotic usage and neonatal outcome data were assessed. Survival analyses were adjusted to take into account potential clinical confounding variables related to the birth and pathogen. Additionally, resistance profiles, pharmacokinetic–pharmacodynamic probability of target attainment, and frequency of resistance (ie, resistance defined by in-vitro growth of isolates when challenged by antibiotics) were assessed. Questionnaires on health structures and antibiotic costs evaluated accessibility and affordability. Findings Between Nov 12, 2015, and Feb 1, 2018, 36 285 neonates were enrolled into the main BARNARDS study, of whom 9874 had clinically diagnosed sepsis and 5749 had available antibiotic data. The four most commonly prescribed antibiotic combinations given to 4451 neonates (77·42%) of 5749 were ampicillin–gentamicin, ceftazidime–amikacin, piperacillin–tazobactam–amikacin, and amoxicillin clavulanate–amikacin. This dataset assessed 476 prescriptions for 442 neonates treated with one of these antibiotic combinations with WGS data (all BARNARDS countries were represented in this subset except India). Multiple pathogens were isolated, totalling 457 isolates. Reported mortality was lower for neonates treated with ceftazidime–amikacin than for neonates treated with ampicillin–gentamicin (hazard ratio [adjusted for clinical variables considered potential confounders to outcomes] 0·32, 95% CI 0·14–0·72; p=0·0060). Of 390 Gram-negative isolates, 379 (97·2%) were resistant to ampicillin and 274 (70·3%) were resistant to gentamicin. Susceptibility of Gram-negative isolates to at least one antibiotic in a treatment combination was noted in 111 (28·5%) to ampicillin–gentamicin; 286 (73·3%) to amoxicillin clavulanate–amikacin; 301 (77·2%) to ceftazidime–amikacin; and 312 (80·0%) to piperacillin–tazobactam–amikacin. A probability of target attainment of 80% or more was noted in 26 neonates (33·7% [SD 0·59]) of 78 with ampicillin–gentamicin; 15 (68·0% [3·84]) of 27 with amoxicillin clavulanate–amikacin; 93 (92·7% [0·24]) of 109 with ceftazidime–amikacin; and 70 (85·3% [0·47]) of 76 with piperacillin–tazobactam–amikacin. However, antibiotic and country effects could not be distinguished. Frequency of resistance was recorded most frequently with fosfomycin (in 78 isolates [68·4%] of 114), followed by colistin (55 isolates [57·3%] of 96), and gentamicin (62 isolates [53·0%] of 117).     In this study, off-centered stagnation flow of three dimensional Burger’s fluid over an infinite rotating disk in a porous medium with a uniform magnetic field, which is applying normal to the disk, is investigated. A uniform suction/injection is applied through the surface of the porous disk. The structure has been modeled in the form of ordinary differential equations, which are reduced from partial differential equations by using the similarity transformation. Analytical solution is obtained by non-perturbation technique of homotopy analysis method (HAM). The influence of non-dimensional parameters on velocity profile is presented in graphical form and the numerical comparison is made with the viscous fluid as a special case

    Impact of Age, Gender, post infection and post vaccination status on antibody response in COVID 19 patients

    Objective: To evaluate severe acute respiratory syndrome coronavirus-2 spike protein antibodies against coronavirus disease-2019 in post-infection and post-vaccinated individuals. Method: The cross-sectional study was conducted June, 1 to July 31, 2021, at the Rehman Medical Institute, Peshawar, Pakistan, and comprised subjects of either gender in whom immunogenicity was checked 35 days post-vaccination and 90 days post-infection. Correlation with age and gender was checked. Specimens were collected and investigated for severe acute respiratory syndrome coronavirus-2 spike protein antibodies by consuming electro-chemiluminescence immunoassay. Data was analysed using SPSS 23. Results: Of the total 256 patients enrolled, 70(27.34%) were included; 49(69%) males and 21(29.6%) females. The overall mean age was 44±7.75 years. Among 30(42.8%) patients with positive polymerase chain reaction test, the mean time between positive the test and antibody screening was 90±30 days. Among the 40(57.2%) vaccinated individuals, the time between vaccination and antibody screening was 35±9.74 days. Overall, 68(97%) patients revealed robust positive findings to severe acute respiratory syndrome coronavirus-2 spike proteins antibodies >50IU/mL. Male subjects had significantly higher immunogenic response compared to females (p=0.001), and immunogenicity decreased with advancing age (p<0.001). Also, post-vaccinated patients’ antibody response was significant compared to post-infection patients’ response (p=0.001). Conclusion: Majority of the patients had significantly higher antibody titers against severe acute respiratory syndrome coronavirus-2 post-infection and post-vaccination. Males and younger individuals developed a significant humoral immunity compared to females and the elderly. Key Words: Antibodies, COVID-19, SARS-CoV-2, Vaccination

    Scutellaria petiolata Hemsl. ex Lace &amp; Prain (Lamiaceae).: A New Insight in Biomedical Therapies

    The recent investigation was designed to explore Scutellaria petiolata Hemsl. ex Lace &amp; Prain (Lamiaceae) whole plant in various extracts (methanol (SPM), dichloromethane (SPDCM), n-Hexane (SPNH), and aqueous (SPAQ) for a phytochemicals assessment, ESI-LC-MS chemical analysis, in vitro antimicrobials, and antioxidants and in vivo anti-inflammatory and analgesic potential. The qualitative detection shows that all the representative groups were present in the analyzed samples. The examined samples display the greatest amount of total flavonoid content (TFC, 78.2 &plusmn; 0.22 mg QE/mg) and total phenolic contents (TPC, 66.2 &plusmn; 0.33 mg GAE/g) in the SPM extract. The SPM extract proceeded to the ESI-LC-MS to identify the chemical constituents that presented nineteen bioactive ingredients, depicted for the first time from S. petiolata mainly contributed by flavonoids. The analyzed samples produced considerable capability to defy the microbes. The SPM extract was observed effective and offered an appreciable zone of inhibition (ZOI), 17.8 &plusmn; 0.04 mm against the bacterial strain Salmonellatyphi and 18.8 &plusmn; 0.04 mm against Klebsiella pneumonia. Moreover, the SPM extract also exhibited 19.4 &plusmn; 0.01 mm against the bacterial strains Bacillus atrophaeus and 18.8 &plusmn; 0.04 mm against Bacillus subtilis in comparison to the standard levofloxacin (Gram-negative) and erythromycin (Gram-positive) bacterial strains that displayed 23.6 &plusmn; 0.02 mm and 23.2 &plusmn; 0.05 mm ZOI, correspondingly. In addition to that, the SPD fraction was noticed efficiently against the fungal strains used with ZOI 19.07 &plusmn; 0.02 mm against Aspergillus parasiticus and 18.87 &plusmn; 0.04 mm against the Aspergillus niger as equated to the standard with 21.5 &plusmn; 0.02 mm ZOI. In the DPPH (2,2-diphenyl-1-picrylhydrazyl) analysis, the SPM extract had the maximum scavenging capacity with IC50 of 78.75 &plusmn; 0.19 &micro;g/mL succeeded by the SPDCM fraction with an IC50 of 140.50 &plusmn; 0.20 &micro;g/mL free radicals scavenging potential. Through the ABTS (2,2&prime;-azino-bis (3-ethylbenzothiazoline-6-sulfonic acid) assay, the similar extract (SPM) presented an IC50 = 85.91 &plusmn; 0.24 &micro;g/mL followed by the SPDCM fractions with IC50 = 182.50 &plusmn; 0.35 &micro;g/mL, and n-Hexane fractions were reported to be the least active between the tested samples in comparison to ascorbic acid of IC50 = 67.14 &plusmn; 0.25 &micro;g/mL for DPPH and IC50 of 69.96 &plusmn; 0.18 &micro;g/mL for ABTS assay. In the in vivo activities, the SPM extract was the most effective with 55.14% inhibition as compared to diclofenac sodium with 70.58% inhibition against animals. The same SPM crude extract with 50.88% inhibition had the most analgesic efficacy as compared to aspirin having 62.19% inhibition. Hence, it was assumed from our results that all the tested samples, especially the SPM and SPDCM extracts, have significant capabilities for the investigated activities that could be due to the presence of the bioactive compounds. Further research is needed to isolate the responsible chemical constituents to produce innovative medications

    Scutellaria (Lamiaceae) comprises over 360 species. Based on its morphological structure of calyx, also known as Skullcap, it is herbaceous by habit and cosmopolitan by habitat. The species of Scutellaria are widely used in local communities as a natural remedy. The genus contributed over three hundred bioactive compounds mainly represented by flavonoids and phenols, chemical ingredients which serve as potential candidates for the therapy of various biological activities. Thus, the current review is an attempt to highlight the biological significance and its correlation to various isolated bioactive ingredients including flavonoids, terpenoids, phenols, alkaloids, and steroids. However, flavonoids were the dominant group observed. The findings of the Scutellaria reveal that due to its affluent basis of numerous chemical ingredients it has a diverse range of pharmacological potentials, such as antimicrobial, antioxidant, antifeedant, enzyme inhibition, anti-inflammatory, and analgesic significance. Currently, various bioactive ingredients have been investigated for various biological activities from the genus Scutellaria in vitro and in vivo. Furthermore, these data help us to highlight its biomedical application and to isolate the responsible compounds to produce innovative medications as an alternative to synthetic drugs
