Isoforms of Retinol binding protein 4 (RBP4) are increased in chronic diseases of the kidney but not of the liver
<p>Abstract</p> <p>Background</p> <p>The levels of retinol-binding protein 4 (RBP4) ā the carrier protein for Vitamin A in plasma ā are tightly regulated under healthy circumstances. The kidney, the main site of RBP4 catabolism, contributes to an elevation of RBP4 levels during chronic kidney disease (CKD) whereas during chronic liver disease (CLD) RBP4 levels decrease. Little is known about RBP4 isoforms including apo-RBP4, holo-RBP4 as well as RBP4 truncated at the C-terminus (RBP4-L and RBP4-LL) except that RBP4 isoforms have been reported to be increased in hemodialysis patients. Since it is not known whether CLD influence RBP4 isoforms, we investigated RBP4 levels, apo- and holo-RBP4 as well as RBP4-L and RBP4-LL in plasma of 36 patients suffering from CKD, in 55 CLD patients and in 50 control subjects. RBP4 was determined by ELISA and apo- and holo-RBP4 by native polyacrylamide gel electrophoresis (PAGE). RBP4-L and RBP4-LL were analyzed after immunoprecipitation by mass spectrometry (MALDI-TOF-MS).</p> <p>Results</p> <p>RBP4 isoforms and levels were highly increased in CKD patients compared to controls (P < 0.05) whereas in CLD patients RBP4 isoforms were not different from controls. In addition, in hepatic dysfunction RBP4 levels were decreased whereas the amount of isoforms was not affected.</p> <p>Conclusion</p> <p>The occurrence of RBP4 isoforms is not influenced by liver function but seems to be strongly related to kidney function and may therefore be important in investigating kidney function and related disorders.</p
The importance of the kidneys for the hemostasis of retinol-binding protein 4 (RBP4) in plasma
Retinol-Bindungsprotein 4 (RBP4) fungiert als spezifisches Transportprotein
von Retinol (ROH) im Blut. Im Komplex mit Transthyretin (TTR) gewƤhrleistet es
die kontinuierliche Verteilung von Vitamin A (VA) in konstanter Konzentration
an Zielzellen und Gewebe. Die Leber speichert VA und synthetisiert RBP4 und
TTR. Im proximalen Tubulus der Nieren erfolgt die RĆ¼ckresorption von RBP4.
Bisherige Studien konnten - neben dem vermehrten Auftreten von quantitativen -
auch qualitative Modifikationen von RBP4 im Serum von Nierenkranken zeigen.
Die klinische Bedeutung dieser qualitativen Modifikationen, VerkĆ¼rzungen des
VolllƤngen-RBP4 am C-terminalen Ende um ein (RBP4-L) bzw. zwei Leucin
(RBP4-LL), ist jedoch unklar. Hinreichende Patientendaten dazu fehlten bisher.
Andererseits wird bei preterm geborenen Kindern mit sehr geringem
Geburtsgewicht (VLBW) die natĆ¼rliche Organreifung der Nieren im Uterus
vorzeitig abgebrochen und wirkt damit dysregulatorisch auf den VA-
Metabolismus. Das Ziel war es daher, den Effekt von chronischen Leber- und
Nierenerkrankungen bei adulten Individuen sowie einer Nierenunreife bei frĆ¼h
geborenen Kindern auf den ROH-RBP4-TTR-Transportkomplex umfassend zu
charakterisieren. Dazu wurden verschiedene Parameter des
Vitamin-A-Metabolismus im Serum und Urin auf quantitativer und qualitativer
Ebene untersucht. In den ersten beiden Studien wurden die Serumkonzentrationen
von RBP4, ROH und TTR sowie die relativen Anteile der qualitativen
RBP4-Modifikationen (apo-RBP4, holo-RBP4, RBP4-L, RBP4-LL) von Patienten mit
chronischer Nierenerkrankung (CKD), von Patienten mit chronischen
Lebererkrankungen (CLD), von HƤmodialyse-Patienten (HD) und von Patienten nach
Nierentransplantation (NTX) sowie von gesunden Probanden als Kontrollgruppe
analysiert. In der dritten Studie erfolgte die Analyse der Konzentration von
RBP4, apo- und holo-RBP4, ROH und TTR im Harn von frĆ¼h geborenen Kindern mit
sehr geringem Geburtsgewicht (VLBW) und von gesunden termgeborenen Kindern.
Die Ergebnisse dieser Studien zeigen sowohl eine quantitative Zunahme der
RBP4-Konzentration als auch eine relative Zunahme von qualitativen
RBP4-Modifikationen (RBP4-L, RBP4-LL) im Serum von CKD-, NTX- und HD-Patienten
im Vergleich zu Kontrollseren von gesunden Probanden sowie von CLD-Patienten.
Ebenso nimmt der Anteil des apo-RBP (ROH-ungebundenes RBP4) im Serum zu. Eine
Zunahme von qualitativen RBP4-Modifikationen im Serum ist assoziiert mit einer
eingeschrƤnkten Nierenfunktion nicht aber mit chronischen Lebererkrankungen
und unterstreicht die wichtige physiologische Funktion der Nieren fĆ¼r den
Katabolismus von ROH und RBP4. Im Harn der frĆ¼h geborenen Kinder konnte eine
etwa 5-fach hƶhere Retinolausscheidung sowie eine um das 16-fache bzw. 5-fache
erhƶhte RBP4- bzw. TTR-Exkretion im Vergleich zu termgeborenen Kindern
nachgewiesen werden. Die Ergebnisse zeigen, dass die renale Ausscheidung von
ROH, RBP4 und TTR in einem Zusammenhang mit der reduzierten tubulƤren
Resorption steht.Retinol binding protein 4 (RBP4) acts as the specific carrier protein for
retinol (ROH) in the blood. In complex with transthyretin (TTR), RBP4 provides
the continuous distribution of vitamin A (VA) to peripheral target cells and
tissues. The liver stores VA and synthesizes RBP4 as well as TTR. Nearly all
of the RBP4 is reabsorbed into the proximal tubular cells of the kidneys, the
main site of RBP4 catabolism. Importantly, dysfunctions of the liver and
kidneys are known to influence RBP4 homeostasis. Previous studies have
indicated an increased occurrence of RBP4 modifications in serum of patients
with chronic kidney disease on a quantitative level as well as on a
qualitative level. The clinical importance of these structural modifications
of the RBP4 molecule, including apo-RBP4, holo-RBP4 as well as RBP4 truncated
at the C-terminus by one leucine (RBP4-L) and, second at the C-terminal end by
two leucine molecules (RBP4-LL) not cleared up yet. However, sufficient data
for these patients are lacking. On the other hand, as the maturity of the
kidneys is interrupted by premature birth the impaired renal function may
affect the VA metabolism. Therefore, the aim of the research was to
characterize the RBP4-TTR-ROH complex in relation to chronic liver and kidney
disease in adult patients and furthermore in relation to renal immaturity in
preterm neonates with very low birth weight (VLBW). Various parameters of VA
metabolism in serum and urine were investigated. In the first two studies the
impact of kidney and liver dysfunctions were analyzed on both the quantitative
level (RBP4, ROH and TTR concentrations) and the qualitative level (apo RBP4,
holo RBP4, RBP4-L, RBP4-LL) in serum of patients with chronic kidney disease
(CKD), in patients with kidney dysfunction undergoing haemodialysis (HD), in
patients after renal transplantation (NTX) and in patients suffering from
chronic liver disease (CLD). The data were compared with the results obtained
from healthy controls. Relative amounts of RBP4-L, RBP4-LL, apo RBP4 and
levels of RBP4 were highly increased in CKD, NTX and HD patients compared to
controls whereas in CLD patients RBP4-L and RBP4-LL were not different from
controls. Summing up, the occurrence of truncated RBP4 forms is not influenced
by liver dysfunction but seems to be strongly related to kidney dysfunction.
Therefore the truncated RBP4 forms can probably be useful as biomarker for
kidney disease and related disorders. The third study investigated the
excretion of RBP4, apo- and holo-RBP4, ROH as well as TTR in urine from VLBW
infants compared with term infants. In the group of term infants, 17 of the 20
excreted ROH, but at an approximately five-times lower concentration.
Excretion of RBP4 and TTR was also much higher in VLBW infants. The increased
urinary ROH, RBP4 and TTR excretion in VLBW infants may be strongly related to
the impaired tubular function due to the renal immaturity
