    A donor-specific epigenetic classifier for acute graft-versus-host disease severity in hematopoietic stem cell

    Background Allogeneic hematopoietic stem cell transplantation (HSCT) is a curative treatment for many hematological conditions. Acute graft-versus-host disease (aGVHD) is a prevalent immune-mediated complication following HSCT. Current diagnostic biomarkers that correlate with aGVHD severity, progression, and therapy response in graft recipients are insufficient. Here, we investigated whether epigenetic marks measured in peripheral blood of healthy graft donors stratify aGVHD severity in human leukocyte antigen (HLA)-matched sibling recipients prior to T cell-depleted HSCT. Methods We measured DNA methylation levels genome-wide at single-nucleotide resolution in peripheral blood of 85 HSCT donors, matched to recipients with various transplant outcomes, with Illumina Infinium HumanMethylation450 BeadChips. Results Using genome-wide DNA methylation profiling, we showed that epigenetic signatures underlying aGVHD severity in recipients correspond to immune pathways relevant to aGVHD etiology. We discovered 31 DNA methylation marks in donors that associated with aGVHD severity status in recipients, and demonstrated strong predictive performance of these markers in internal cross-validation experiments (AUC = 0.98, 95 % CI = 0.96–0.99). We replicated the top-ranked CpG classifier using an alternative, clinical DNA methylation assay (P = 0.039). In an independent cohort of 32 HSCT donors, we demonstrated the utility of the epigenetic classifier in the context of a T cell-replete conditioning regimen (P = 0.050). Conclusions Our findings suggest that epigenetic typing of HSCT donors in a clinical setting may be used in conjunction with HLA genotyping to inform both donor selection and transplantation strategy, with the ultimate aim of improving patient outcome

    Executive Function in Very Preterm Children at Early School Age

    We examined whether very preterm (≤30 weeks gestation) children at early school age have impairments in executive function (EF) independent of IQ and processing speed, and whether demographic and neonatal risk factors were associated with EF impairments. A consecutive sample of 50 children (27 boys and 23 girls) born very preterm (mean age = 5.9 years, SD = 0.4, mean gestational age = 28.0 weeks, SD = 1.4) was compared to a sample of 50 age-matched full-term controls (23 girls and 27 boys, mean age = 6.0 years, SD = 0.6) with respect to performance on a comprehensive EF battery, assessing the domains of inhibition, working memory, switching, verbal fluency, and concept generation. The very preterm group demonstrated poor performance compared to the controls on all EF domains, even after partialing out the effects of IQ. Processing speed was marginally related to EF. Analyses with demographic and neonatal risk factors showed maternal education and gestational age to be related to EF. This study adds to the emerging body of literature showing that very preterm birth is associated with EF impairments

    Rapid-Response Low Infrared Emission Broadband Ultrathin Plasmonic Light Absorber

    Plasmonic nanostructures can significantly advance broadband visible-light absorption, with absorber thicknesses in the sub-wavelength regime, much thinner than conventional broadband coatings. Such absorbers have inherently very small heat capacity, hence a very rapid response time, and high light power-to-temperature sensitivity. Additionally, their surface emissivity can be spectrally tuned to suppress infrared thermal radiation. These capabilities make plasmonic absorbers promising candidates for fast light-to-heat applications, such as radiation sensors. Here we investigate the light-to-heat conversion properties of a metal-insulator-metal broadband plasmonic absorber, fabricated as a free-standing membrane. Using a fast IR camera, we show that the transient response of the absorber has a characteristic time below 13 ms, nearly one order of magnitude lower than a similar membrane coated with a commercial black spray. Concurrently, despite the small thickness, due to the large absorption capability, the achieved absorbed light power-to-temperature sensitivity is maintained at the level of a standard black spray. Finally, we show that while black spray has emissivity similar to a black body, the plasmonic absorber features a very low infra-red emissivity of almost 0.16, demonstrating its capability as selective coating for applications with operating temperatures up to 400°C, above which the nano-structure starts to deform

    Extended lymphadenectomy in patients with pancreatic cancer is debatable

    Lymph node staging is one of the most important factors in determining the prognosis after resection of pancreatic ductal adenocarcinoma. Despite ongoing efforts to further refine lymph node staging, the debate on the extent of lymphadenectomy during pancreaticoduodenectomy is still open. The purpose of this review was to summarize the evidence about performing standard lymphadenectomy during curative resection of pancreatic cancer. All four prospective randomized controlled trials published concluded that extended lymphadenectomy does not contribute to better oncologic outcome for patients with adenocarcinoma of the pancreatic head. Indeed, one major drawback of extended lymphadenectomy is the higher risk of persistent postoperative diarrhea. No prospective randomized studies could be found on the role of extended lymphadenectomy in patients with adenocarcinoma of the corpus and tail. Based on current evidence there is no indication that extended lymphadenectomy should be performed routinely during resection of pancreatic cancer
