    Oligometastases and Oligo-recurrence: The New Era of Cancer Therapy

    Recurrence or metastasis of cancer has been considered to occur in the last stage of the patient's life. However, the new notions of oligometastases and oligo-recurrence have been proposed and the paradigm shift in the conceptualization of cancer metastasis or cancer recurrence. Oligometastases is the state in which the patient shows distant relapse in only a limited number of regions. Local therapy such as surgery, radiotherapy and radiofrequency ablation for the relapsed sites could thus improve patient's survival. On the other hand, oligo-recurrence is a notion similar to oligometastases. However, the conditions of oligo-recurrence has a primary site of the cancer controlled, meaning that all gross recurrent or metastatic sites could be treated using local therapy

    A call for the aggressive treatment of oligometastatic and oligo-recurrent non-small cell lung cancer.

    Metastatic non-small cell lung cancer (NSCLC) carries a dismal prognosis. Clinical evidence suggests the existence of an intermediate, or oligometastatic, state when metastases are limited in number and/or location. In addition, following initial curative therapy, many patients present with limited metastatic disease, or oligo-recurrence. Metastasis-directed, anti-cancer therapies may benefit these patients. A growing evidence-base supports the use of hypofractionated, image-guided radiotherapy (HIGRT) for a variety of malignant conditions including inoperable stage I NSCLC and many metastatic sites. When surgical resection is not possible, HIGRT offers an effective alternative for local treatment of limited metastatic disease. Early studies have produced promising results when HIGRT was delivered to all known sites of disease in patients with oligometastatic/oligo-recurrent NSCLC. In a population of patients formerly considered rapidly terminal, these studies report five year overall survival rates of 13-22%. HIGRT for metastatic NSCLC warrants further study. We call for large, intergroup, and even international randomized trials incorporating HIGRT and other metastasis-directed therapies into the treatment of patients with oligometastatic/oligo-recurrent NSCLC

    Photocatalytic Activity Enhancement of Anatase/Rutile-Mixed Phase TiO2 Nanoparticles Annealed with Low-Temperature O2 Plasma

    Photodecomposition and photobactericidal activities of anatase/rutile-mixed phase TiO2 nanoparticles annealed with low-temperature O2 plasma were clarified by comparing them with those annealed in ambient air. The photocatalytic activities of plasma-assisted-annealed sample greatly enhanced as compared with the untreated sample, under not only ultraviolet irradiation but also visible-light irradiation. The photocatalytic activities of air-annealed samples did not enhance under ultraviolet irradiation but enhanced under visible-light irradiation. The enhanced photocatalytic activities due to the plasma-assisted annealing (PAA) originated from the increased photoexcited carrier concentration. This enhancement was discussed from PAA-induced characteristic factors. PAA facilitated the phase transformation to anatase, contributing directly to extending the photoexcited carrier lifetime. PAA introduced more oxygen vacancies, contributing to trapping more photogenerated electrons. PAA also introduced more bridging/terminal oxygen groups adsorbed on the surface, increasing the upward band-bending, the depletion layer width at the surface, and the charge transfer from rutile to anatase. These two introductions contributed to facilitating the separation of photoexcited carriers. Furthermore, PAA reduced the aggregate size of TiO2 nanoparticles formed on the surface, contributing to increasing optical absorptions. More reactive oxygen species produced from the bridging/terminal oxygen groups by the photoexcited carriers would also enhance the photocatalytic activities

    Abscopal effect of radiation on lung metastases of hepatocellular carcinoma: a case report

    <p>Abstract</p> <p>Introduction</p> <p>The abscopal effect is the effect of radiation therapy at a site distant to the area of irradiation. This is not a common event and has not been clearly defined, resulting in few reported cases in the literature. We discuss this phenomenon in a patient with hepatocellular carcinoma.</p> <p>Case presentation</p> <p>A 63-year-old Japanese man underwent extended right hepatic lobectomy for hepatocellular carcinoma. During his follow-up examination, a single lung metastasis and a single mediastinal lymph node metastasis were found. Trans-catheter arterial embolization was initially attempted to treat the mediastinal tumor, however this approach failed to take effect and carried risks of spinal artery embolism. External-beam irradiation, with a dose of 2.25 Gy per fraction, was performed using an antero-posterior parallel-opposed technique (total dose, 60.75 Gy). A computed tomography scan performed one month after starting radiotherapy showed a remarkable reduction of the mediastinal lymph node metastasis. In addition to this, we observed spontaneous shrinking of the lung metastasis, which was located in the right lower lobe and out of the radiation field. No chemotherapy was given during the period. There has been no recurrence of either the lung metastasis or the mediastinal lymph node metastasis during a follow-up 10 years after the radiotherapy.</p> <p>Conclusion</p> <p>We observed a rare abscopal effect in a site distant from the area of irradiation. Irradiation of the mediastinum resulted in tumor mass regression in the untreated lung tumor.</p

    High efficiency carbon-based multilayers for LAMP at 250 eV

    X-ray reflection near the Brewster's angle by multilayer mirrors can be used to detect the polarization from X-ray sources. The photon emission spectra from some isolated neutron stars and AGN/blazars etc. show that their emission is peaked at low energies near 250eV, which is just below carbon K-absorption edge. The Lightweight Asymmetry and Magnetism Probe (LAMP) is proposed as a micro-satellite mission dedicated for astronomical X-ray polarimetry working at 250 eV and is currently under early phase study. Co/C multilayers are selected and designed at the energy near 250eV with a grazing incident angle of 45°. The carbon layer thickness ratio is optimized to get the highest integral reflectivity which means larger effective signals in the astrophysics observation. The multilayer coatings were manufactured by direct current magnetron sputtering on D263 glasses and electroformed nickels and characterized using Grazing incidence X-ray reflectometry at 8keV. Reactive sputtering with 4%, 6% and 8% nitrogen were used to improve the Co/C multilayer interfaces respectively. Reflectivity for s-polarization and p-polarization light was measured at BEAR beamline in Elettra synchrotron facility. Co/C multilayer deposited with 6% nitrogen exhibits the best performance comparing to other multilayers with different nitrogen content

    Effects of nonequilibrium atmospheric-pressure O2 plasma-assisted annealing on anatase TiO2 nanoparticles

    Anatase TiO2 nanoparticles (NPs) immobilized on glass substrates were annealed with the assistance of nonequilibrium atmospheric-pressure O2 plasma. The plasma-assisted annealing greatly enhanced the photodecomposition and photobactericidal activity as compared with electric-furnace annealing. The plasma-assisted annealing reduced the TiO2 NP agglomerate size and increased the optical absorption, the photoinduced electrical conductivity, the amounts of bridging and terminal oxygen groups, and the (112)/(101) plane intensity ratio, causing the lattice oxygen deficiency that formed partially Ti-rich surface portions. The enhanced photobactericidal activity would arise from the bridging and terminal oxygen groups. The enhanced photodecomposition would arise from the increased concentration of photogenerated carriers due to the following three factors. The first is the optical absorption increased by the agglomerate size reduction and the (112) plane growth or appearance, which exert scattering more incident photons. The second is the charge separation of photogenerated carriers facilitated by the bridging and terminal oxygen groups, which originate from oxygen vacancies via oxygen ion impact from the plasma. The third is the charge transfer of plasmon-excited electrons from the partially Ti-rich portions to TiO2. The enhanced photodecomposition would also arise from more reactive oxygen species generated from the bridging and terminal oxygen groups by the photogenerated carriers

    Pretreatment carcinoembryonic antigen level is a risk factor for para-aortic lymph node recurrence in addition to squamous cell carcinoma antigen following definitive concurrent chemoradiotherapy for squamous cell carcinoma of the uterine cervix

    <p>Abstract</p> <p>Background</p> <p>To identify pretreatment carcinoembryonic antigen (CEA) levels as a risk factor for para-aortic lymph node (PALN) recurrence following concurrent chemoradiotherapy (CCRT) for cervical cancer.</p> <p>Methods</p> <p>From March 1995 to January 2008, 188 patients with squamous cell carcinoma (SCC) of the uterine cervix were analyzed retrospectively. No patient received PALN irradiation as the initial treatment. CEA and squamous cell carcinoma antigen (SCC-Ag) were measured before and after radiotherapy. PALN recurrence was detected by computer tomography (CT) scans. We analyzed the actuarial rates of PALN recurrence by using Kaplan-Meier curves. Multivariate analyses were carried out with Cox regression models. We stratified the risk groups based on the hazard ratios (HR).</p> <p>Results</p> <p>Both pretreatment CEA levels ≥ 10 ng/mL and SCC-Ag levels < 10 ng/mL (<it>p </it>< 0.001, HR = 8.838), SCC-Ag levels ≥ 40 ng/mL (<it>p </it>< 0.001, HR = 12.551), and SCC-Ag levels of 10-40 ng/mL (<it>p </it>< 0.001, HR = 4.2464) were significant factors for PALN recurrence. The corresponding 5-year PALN recurrence rates were 51.5%, 84.8%, and 27.5%, respectively. The 5-year PALN recurrence rate for patients with both low (< 10 ng/mL) SCC and CEA was only 9.6%. CEA levels ≥ 10 ng/mL or SCC-Ag levels ≥ 10 ng/mL at PALN recurrence were associated with overall survival after an isolated PALN recurrence. Pretreatment CEA levels ≥ 10 ng/mL were also associated with survival after an isolated PALN recurrence.</p> <p>Conclusions</p> <p>Pretreatment CEA ≥ 10 ng/mL is an additional risk factor of PALN relapse following definitive CCRT for SCC of the uterine cervix in patients with pretreatment SCC-Ag levels < 10 ng/mL. More comprehensive examinations before CCRT and intensive follow-up schedules are suggested for early detection and salvage in patients with SCC-Ag or CEA levels ≥ 10 ng/mL.</p
