Functional Lagrange formalism for time-non-local Lagrangians
We develop a time-non-local (TNL) formalism based on variational calculus,
which allows for the analysis of TNL Lagrangians. We derive the generalized
Euler-Lagrange equations starting from the Hamilton's principle and, by
defining a generalized momentum, we introduce the corresponding Hamiltonian
formalism. We apply the formalism to second order TNL Lagrangians and we show
that it reproduces standard results in the time-local limit. An example will
show how the formalism works, and will provide an interesting insight on the

Mammography screening: views from women and primary care physicians in Crete
Background: Breast cancer is the most commonly diagnosed cancer among women and a leading cause of death from cancer in women in Europe. Although breast cancer incidence is on the rise worldwide, breast cancer mortality over the past 25 years has been stable or decreasing in some countries and a fall in breast cancer mortality rates in most European countries in the 1990s was reported by several studies, in contrast, in Greece have not reported these favourable trends. In Greece, the age-standardised incidence and mortality rate for breast cancer per 100.000 in 2006 was 81,8 and 21,7 and although it is lower than most other countries in Europe, the fall in breast cancer mortality that observed has not been as great as in other European countries. There is no national strategy for screening in this country. This study reports on the use of mammography among middleaged women in rural Crete and investigates barriers to mammography screening encountered by women and their primary care physicians.
Methods: Design: Semi-structured individual interviews. Setting and participants: Thirty women between 45–65
years of age, with a mean age of 54,6 years, and standard deviation 6,8 from rural areas of Crete and 28 qualified
primary care physicians, with a mean age of 44,7 years and standard deviation 7,0 serving this rural population.
Main outcome measure: Qualitative thematic analysis.
Results: Most women identified several reasons for not using mammography. These included poor knowledge
of the benefits and indications for mammography screening, fear of pain during the procedure, fear of a serious
diagnosis, embarrassment, stress while anticipating the results, cost and lack of physician recommendation.
Physicians identified difficulties in scheduling an appointment as one reason women did not use mammography
and both women and physicians identified distance from the screening site, transportation problems and the
absence of symptoms as reasons for non-use.
Conclusion: Women are inhibited from participating in mammography screening in rural Crete. The provision
of more accessible screening services may improve this. However physician recommendation is important in
overcoming women's inhibitions. Primary care physicians serving rural areas need to be aware of barriers
preventing women from attending mammography screening and provide women with information and advice in a sensitive way so women can make informed decisions regarding breast caner screening
Surgical versus nonsurgical treatment for lumbar degenerative spondylolisthesis.
BACKGROUND: Management of degenerative spondylolisthesis with spinal stenosis is controversial. Surgery is widely used, but its effectiveness in comparison with that of nonsurgical treatment has not been demonstrated in controlled trials.
METHODS: Surgical candidates from 13 centers in 11 U.S. states who had at least 12 weeks of symptoms and image-confirmed degenerative spondylolisthesis were offered enrollment in a randomized cohort or an observational cohort. Treatment was standard decompressive laminectomy (with or without fusion) or usual nonsurgical care. The primary outcome measures were the Medical Outcomes Study 36-Item Short-Form General Health Survey (SF-36) bodily pain and physical function scores (100-point scales, with higher scores indicating less severe symptoms) and the modified Oswestry Disability Index (100-point scale, with lower scores indicating less severe symptoms) at 6 weeks, 3 months, 6 months, 1 year, and 2 years.
RESULTS: We enrolled 304 patients in the randomized cohort and 303 in the observational cohort. The baseline characteristics of the two cohorts were similar. The one-year crossover rates were high in the randomized cohort (approximately 40% in each direction) but moderate in the observational cohort (17% crossover to surgery and 3% crossover to nonsurgical care). The intention-to-treat analysis for the randomized cohort showed no statistically significant effects for the primary outcomes. The as-treated analysis for both cohorts combined showed a significant advantage for surgery at 3 months that increased at 1 year and diminished only slightly at 2 years. The treatment effects at 2 years were 18.1 for bodily pain (95% confidence interval [CI], 14.5 to 21.7), 18.3 for physical function (95% CI, 14.6 to 21.9), and -16.7 for the Oswestry Disability Index (95% CI, -19.5 to -13.9). There was little evidence of harm from either treatment.
CONCLUSIONS: In nonrandomized as-treated comparisons with careful control for potentially confounding baseline factors, patients with degenerative spondylolisthesis and spinal stenosis treated surgically showed substantially greater improvement in pain and function during a period of 2 years than patients treated nonsurgically. Knowledge, attitudes and practice of breast cancer screening among female health workers in a Nigerian urban city
<p>Abstract</p> <p>Background</p> <p>Late presentation has been observed as the hallmark of breast cancer in Nigerian women and an earlier onset has been reported in this population. This study was designed to assess the awareness of female health workers about risk factors and screening methods for early detection of breast cancer.</p> <p>Methods</p> <p>A cross-sectional descriptive study was carried out among female health workers in the two major government health institutions in Benin City, Edo State capital in Nigeria.</p> <p>Data analysis was by SPSS version 10 and test of significance was done with differences considered significant at p < 0.05.</p> <p>Results</p> <p>Three hundred and ninety-three (393) female health workers out of five hundred and five eligible subjects completed and returned the questionnaires, giving a response rate of 77.8%. One hundred and two (26%) were Doctors, two hundred and fifty-four (64.6%) Nurses, and thirty-seven (9.4%) were Radiographers, Laboratory Scientists and Pharmacists. A high proportion of our respondents had very poor knowledge about risk factors for breast cancer (55%). The awareness of mammography as a diagnostic method was very high (80.7%), but an extremely low knowledge of mammography as a screening method was found. Mammography practice of only 3.1% was found among those above 40 years of age who qualify for routine annual screening. Relatively low knowledge (45.5%) about Breast Self Examination (BSE) as a screening method was found.</p> <p>Conclusion</p> <p>These female health workers who are expected to act as role models and educate the public had poor knowledge of risk factors for breast cancer and practice of breast cancer screening. There is very urgent need for regular update courses for health workers concerning breast cancer education including screening methods.</p

The title compound, {(C2H10N2)[Cd2(HPO4)3]·H2O}n, was synthesized under hydrothermal conditions. The structure of this hybrid compound consists of CdO6, CdO5 and PO4 polyhedra arranged so as to build an anionic inorganic layer, namely [Cd2(HPO4)3]2−, parallel to the ab plane. The edge-sharing CdO6 octahedra form infinite chains running along the a axis and are linked by CdO5 and PO4 polyhedra. The ethylenediammonium cation and the water molecule are located between two adjacent inorganic layers and ensure the cohesion of the structure via N—H⋯O and O—H⋯O hydrogen bonds
Effective Lagrangian Approach to the Theory of Eta Photoproduction in the Region
We investigate eta photoproduction in the resonance region
within the effective Lagrangian approach (ELA), wherein leading contributions
to the amplitude at the tree level are taken into account. These include the
nucleon Born terms and the leading -channel vector meson exchanges as the
non-resonant pieces. In addition, we consider five resonance contributions in
the - and - channel; besides the dominant , these are:
and . The amplitudes for the
and the photoproduction near threshold have significant
differences, even as they share common contributions, such as those of the
nucleon Born terms. Among these differences, the contribution to the
photoproduction of the -channel excitation of the is the most
significant. We find the off-shell properties of the spin-3/2 resonances to be
important in determining the background contributions. Fitting our effective
amplitude to the available data base allows us to extract the quantity
, characteristic of the
photoexcitation of the resonance and its decay into the
-nucleon channel, of interest to precise tests of hadron models. At the
photon point, we determine it to be from
the old data base, and from a
combination of old data base and new Bates data. We obtain the helicity
amplitude for to be from the old data base, and from the combination of the old data base and new Bates
data, compared with the results of the analysis of pion photoproduction
