    Patient safety and estimation of renal function in patients prescribed new oral anticoagulants for stroke prevention in atrial fibrillation

    OBJECTIVE: In clinical trials of dabigatran and rivaroxaban for stroke prevention in atrial fibrillation (AF), drug eligibility and dosing were determined using the Cockcroft-Gault equation to estimate creatine clearance as a measure of renal function. This cross-sectional study aimed to compare whether using estimated glomerular filtration rate (eGFR) by the widely available and widely used Modified Diet in Renal Disease (MDRD) equation would alter prescribing or dosing of the renally excreted new oral anticoagulants. PARTICIPANTS: Of 4712 patients with known AF within a general practitioner-registered population of 930 079 in east London, data were available enabling renal function to be calculated by both Cockcroft-Gault and MDRD methods in 4120 (87.4%). RESULTS: Of 4120 patients, 2706 were <80 years and 1414 were ≥80 years of age. Among those ≥80 years, 14.9% were ineligible for dabigatran according to Cockcroft-Gault equation but would have been judged eligible applying MDRD method. For those <80 years, 0.8% would have been incorrectly judged eligible for dabigatran and 5.3% would have received too high a dose. For rivaroxaban, 0.3% would have been incorrectly judged eligible for treatment and 13.5% would have received too high a dose. CONCLUSIONS: Were the MDRD-derived eGFR to be used instead of Cockcroft-Gault in prescribing these new agents, many elderly patients with AF would either incorrectly become eligible for them or would receive too high a dose. Safety has not been established using the MDRD equation, a concern since the risk of major bleeding would be increased in patients with unsuspected renal impairment. Given the potentially widespread use of these agents, particularly in primary care, regulatory authorities and drug companies should alert UK doctors of the need to use the Cockcroft-Gault formula to calculate eligibility for and dosing of the new oral anticoagulants in elderly patients with AF and not rely on the MDRD-derived eGFR

In order to obtain a successful flotation of slimy sulphide ores, two types of chemical auxilliaries are basically in use i. e. ( i ) a hydrocolloid acting as flocculent for gangue material, commonly referred as a depressor and (ii) a collector or a metal reagent, which conditions the mineral surface ( by adsorption and making it more hydrophobic ) so that it will rise to the surface when a frother is added.     The polarization of the resonance lines of 6Li, 7Li and Na, excited by electron impact, are calculated using the Glauber approximation. The present results are in good agreement with experimental values and with other theoretical calculations in the energy range from 2 eV to 5 eV

    Natural coir fibers, subjectd to thermal treatments in the range of 0oC to 200oC and alkali treatment with 5% to 30% concentration w/w, have been used in the present investigation to determine the interlayer variability of the cellulose planes (020), (110) and (110). Among the equatorial reflections (110), (110) and (020), the extent of variability is found to be more with the proportion of such affected planes less for (020) reflection in the native cellulose at lower temperature while at higher temperature (110) and (110) become more affected by variability defect.     Searching for tetraquarks on the lattice

    We address the question whether the lightest scalar mesons sigma and kappa are tetraquarks. We present a search for possible light tetraquark states with J^PC=0^++ and I=0, 1/2, 3/2, 2 in the dynamical and the quenched lattice simulations using tetraquark interpolators. In all the channels, we unavoidably find lowest scattering states pi(k)pi(-k) or K(k)pi(-k) with back-to-back momentum k=0,2*pi/L,.. . However, we find an additional light state in the I=0 and I=1/2 channels, which may be related to the observed resonances sigma and kappa with a strong tetraquark component. In the exotic repulsive channels I=2 and I=3/2, where no resonance is observed, we find no light state in addition to the scattering states.Comment: 3 pages, 1 figure, proceedings of Lepton-Photon 2009, Hambur

    Beneficiation and Utilisation of Low Grade Refractory Materials

    The present production of refractories in India is of the order of 0.75 million tons. These comprise fireclay, alumino silixates, high alumina and basic refractories and refractory cements, mortars, mortars, plastics and castables. Besides these, large tonnages of dead burnt dolomite are produced in many steel plants for fettling and repair of open-hearth and electric arc furnaces or lining L-D vessels. Of the total production of refrac-tories, steel plants consume almost 80%.By and large the refractory manufactures were coping up with the requi-rements of user industries except for the demand of some speciality refractories. However with the changing techn-ology, particularly in steel making, and greater emphasis on purity of refractories the refractory manufacturers find it exceedingly difficult to meet the stringent specifications laid down by the users
