    Concept and challenges of delivering preventive and care services in prehospital emergency medical service: A qualitative study

    Background and purpose: Prehospital emergency medical service (EMS) is an important part of the health care system. Today, the need for integrated emergency care services and delivering qualified preventive and care services felt more than before in accidents and emergencies. This survey aimed to investigate the views of emergency medical personnel on the concept and nature of preventive and care services and current challenges in delivering these services in prehospital EMS centers in Golestan province, Iran. Materials and methods: A qualitative study was done with 16 emergency medical personnel working in EMS sites in Golestan province using purposeful sampling. Data was collected by semistructured interview guide and framework analysis was implemented to analyze the data. Results: Two general themes were identified including 1) the concept of preventive and care services in the EMS and 2) the challenges in providing qualified services in the EMS sites in Golestan province. Also, five sub-themes and 12 subclasses were determined. Conclusion: To enhance and promote services, the EMS system of Golestan province need reforms in organizational structure, laws and administrative regulations, training programs, the system of monitoring and evaluation of personnel, also provision of equipment and manpower and provision of personnel amenities to improve the staff performance. Furthermore, it is necessary to boost emergency services at the community level through training people and enhancing collaboration and participation with other organizations. © 2015, Mazandaran University of Medical Sciences.     Hydrobiological Study of Algae of an Urban Freshwater River

    Biological assessment is a useful alternative for assessing the ecological quality of aquatic ecosystems since biological communities integrate the environmental effects of water chemistry. Algal and water samples were collected at monthly intervals from three sites of River Mutha (Pune, India). The most pollution tolerant genera and species of four groups of algae were recorded from three sites of River. Nygaard's and Palmer's biotic indices have been used for the assessment of quality of the River. Phytoplankton encountered in the water body reflects the average ecological condition and therefore, they may be used as indicator of water quality. The algal flora of polluted water bodies shows the dominance of blue green and diatoms like, Oscillatoria , Lyngbya , Anabena , Microcystis , Navicula , Nitzschia , Synedra , Gomphonema through out the investigation many green algae like, Pandorina , Scenedesmus , Stigeoclonium , Ankistrodesmus , Chlamydomonas , also occur abundantly and frequently. @JASE

    Burn patient care lost in good manufacturing practices?

    Application of cell therapies in burn care started in the early 80s in specialized hospital centers world-wide. Since 2007, cell therapies have been considered as "Advanced Therapy Medicinal Products" (ATMP), so classified by European Directives along with associated Regulations by the European Parliament. Consequently, regulatory changes have transformed the standard linear clinical care pathway into a more complex one. It is important to ensure the safety of cellular therapies used for burn patients and to standardize as much as possible the cell sources and products developed using cell culture procedures. However, we can definitely affirm that concentrating the bulk of energy and resources on the implementation of Good Manufacturing Practice (GMP) alone will have a major negative impact on the care of severely burned patients world-wide. Developing fully accredited infrastructures and training personnel (required by the new directives), along with obtaining approval for clinical trials to go ahead, can be a lengthy process.We discuss whether or not these patients could benefit from cell therapies provided by standard in-hospital laboratories, thus avoiding having to meet rigid regulations concerning the use of industrial pharmaceutical products. "Hospital Exemption" could be a preferred means to offer burn patients a customized and safe product, as many adaptations may be required throughout their treatment pathway. Patients who are in need of rapid treatment will be the ones to suffer the most from regulations intended to help them

    Two-Scale Kirchhoff Theory: Comparison of Experimental Observations With Theoretical Prediction

    We introduce a non-perturbative two scale Kirchhoff theory, in the context of light scattering by a rough surface. This is a two scale theory which considers the roughness both in the wavelength scale (small scale) and in the scales much larger than the wavelength of the incident light (large scale). The theory can precisely explain the small peaks which appear at certain scattering angles. These peaks can not be explained by one scale theories. The theory was assessed by calculating the light scattering profiles using the Atomic Force Microscope (AFM) images, as well as surface profilometer scans of a rough surface, and comparing the results with experiments.     Phase Diagram and Entanglement of Ising Model With Dzyaloshinskii-Moriya Interaction

    We have studied the phase diagram and entanglement of the one dimensional Ising model with Dzyaloshinskii-Moriya (DM) interaction. We have applied the quantum renormalization group (QRG) approach to get the stable fixed points, critical point and the scaling of coupling constants. This model has two phases, antiferromagnetic and saturated chiral ones. We have shown that the staggered magnetization is the order parameter of the system and DM interaction produces the chiral order in both phases. We have also implemented the exact diagonalization (Lanczos) method to calculate the static structure factors. The divergence of structure factor at the ordering momentum as the size of systems goes to infinity defines the critical point of the model. Moreover, we have analyzed the relevance of the entanglement in the model which allows us to shed insight on how the critical point is touched as the size of the system becomes large. Nonanalytic behavior of entanglement and finite size scaling have been analyzed which is tightly connected to the critical properties of the model. It is also suggested that a spin-fluid phase has a chiral order in terms of new spin operators which are defined by a nonlocal transformation.Comment: 11page, 15 figures, Accepted in Physical Review

    A Declarative Framework for Specifying and Enforcing Purpose-aware Policies

    Purpose is crucial for privacy protection as it makes users confident that their personal data are processed as intended. Available proposals for the specification and enforcement of purpose-aware policies are unsatisfactory for their ambiguous semantics of purposes and/or lack of support to the run-time enforcement of policies. In this paper, we propose a declarative framework based on a first-order temporal logic that allows us to give a precise semantics to purpose-aware policies and to reuse algorithms for the design of a run-time monitor enforcing purpose-aware policies. The basic ingredient is the NCQED counterpart of Yang-Mills recursion relations by means of Berends and Giele.     Excitation Spectrum of One-dimensional Extended Ionic Hubbard Model

    We use Perturbative Continuous Unitary Transformations (PCUT) to study the one dimensional Extended Ionic Hubbard Model (EIHM) at half-filling in the band insulator region. The extended ionic Hubbard model, in addition to the usual ionic Hubbard model, includes an inter-site nearest-neighbor (n.n.) repulsion, VV. We consider the ionic potential as unperturbed part of the Hamiltonian, while the hopping and interaction (quartic) terms are treated as perturbation. We calculate total energy and ionicity in the ground state. Above the ground state, (i) we calculate the single particle excitation spectrum by adding an electron or a hole to the system. (ii) the coherence-length and spectrum of electron-hole excitation are obtained. Our calculations reveal that for V=0, there are two triplet bound state modes and three singlet modes, two anti-bound states and one bound state, while for finite values of VV there are four excitonic bound states corresponding to two singlet and two triplet modes. The major role of on-site Coulomb repulsion UU is to split singlet and triplet collective excitation branches, while VV tends to pull the singlet branches below the continuum to make them bound states.Comment: 10 eps figure

    Long-range correlation and multifractality in Bach's Inventions pitches

    We show that it can be considered some of Bach pitches series as a stochastic process with scaling behavior. Using multifractal deterend fluctuation analysis (MF-DFA) method, frequency series of Bach pitches have been analyzed. In this view we find same second moment exponents (after double profiling) in ranges (1.7-1.8) in his works. Comparing MF-DFA results of original series to those for shuffled and surrogate series we can distinguish multifractality due to long-range correlations and a broad probability density function. Finally we determine the scaling exponents and singularity spectrum. We conclude fat tail has more effect in its multifractality nature than long-range correlations.Comment: 18 page, 6 figures, to appear in JSTA
