    Interannual variability of heat waves in South Korea and their connection with large-scale atmospheric circulation patterns

    This study investigates the interannual variation of heat wave frequency (HWF) in South Korea during the past 42 years (1973-2014) and examines its connection with large-scale atmospheric circulation changes. Korean heat waves tend to develop most frequently in late summer during July and August. The leading Empirical Orthogonal Function accounting for 50% of the total variance shows a mono-signed pattern over South Korea, suggesting that the dominant mechanisms responsible for the heat wave are linked in a spatial scale much larger than the nation. It also exhibits a regional variation with more occurrences in the southeastern inland area. The regression of the leading principal component (PC) time series of HWF with large-scale atmospheric circulation identifies a north-south dipole pattern between the South China Sea and Northeast Asia. When this large-scale circulation mode facilitates deep convection in South China Sea, it tends to weaken moisture transport from the South China Sea to Northeast Asia. Enhanced deep convection in the South China Sea triggers a source of Rossby wave train along southerly wind that generates positive geopotential height anomalies around Korea. The anomalous high pressure pattern is accompanied by large-scale subsidence in Korea, thereby providing a favourable condition for extreme hot and dry days in Korea. This study highlights that there is a decadal change of the relationship between Korean heat waves and large-scale atmospheric circulation patterns.     Observational Evidence of EHP Effects on the Melting of Snowpack over the Tibetan Plateau

    Observational evidences are presented showing that the Indo-Gangetic Plain (IGP) regions, bounded by the high altitude Himalayan mountains, are subject to heavy loading of absorbing aerosols, i.e., black carbon and dust, which can lead to widespread enhancement warming over the Tibetan Plateau and accelerated snowmelt in the western Tibetan Plateau (WTP) and Himalayas. The two pre-monsoon seasons of 2004 and 2005 were strikingly contrasting in terms of the aerosol loading over IGP. The warming of the TP in 2004 relative to 2005 was widespread, covering most of the WTP and Himalayas. This warming is closely linked to patterns of the snow melt. Consistent with the Elevated Heat Pump hypothesis, we find that increased loading of absorbing aerosols over IGP in the pre-monsoon season is associated with increased heating of the upper troposphere by dynamical feedback induced by aerosol heating, and enhances the rate of snowmelt over Himalayas and the WTP in April-May. Composite analysis with more contrasting years also shows that the heating of the troposphere by elevated dust and black carbon aerosols in the boreal spring can lead to widespread enhanced land-atmosphere warming, and accelerated snow melt in the Himalayas and Tibetan Plateau

    Vitamin D3 Supplementation Reduces the Symptoms of Upper Respiratory Tract Infection during Winter Training in Vitamin D-Insufficient Taekwondo Athletes: A Randomized Controlled Trial

    Vitamin D insufficiency may be associated with increased risk of upper respiratory tract infection (URTI) in athletes. This study examined the effects of vitamin D3 supplementation on salivary immune functions and symptoms of URTI in vitamin D-insufficient taekwondo athletes. Twenty-five male taekwondo athletes, aged 19–22 years with vitamin D insufficiency [serum 25-hydroxyvitamin-D concentrations (25(OH)D, 31.3 ± 1.39 nmol/L)], participated in this study. They were randomized to receive 5000 IU/day of vitamin D3 (n = 13) or placebo capsule (n = 12) during 4 weeks of winter training. Blood samples were collected two times (pre- and post-tests) for analyzing serum 25(OH)D concentration while salivary samples were obtained three times (pre-, mid-, and post-tests) for secretory immunoglobulin A (SIgA) and lactoferrin analyses. The symptoms of URTI were reported daily during the intervention. Serum 25(OH)D concentration significantly increased by 255.6% in the vitamin D group, whereas in the placebo group it did not change (p \u3c 0.001). While the significant increase in SIgA was observed in both groups (p \u3c 0.001), elevated salivary lactoferrin level in response to winter training was found only in the placebo group (p = 0.011). The change in serum 25(OH)D concentration was negatively associated with total URTI symptoms (r = −0.435, p = 0.015). Vitamin D3 supplementation may be effective in reducing the symptoms of URTI during winter training in vitamin D-insufficient taekwondo athletes

    Development of Korean Academy of Medical Sciences Guideline on the Skin and Related System: Impairment Evaluation of Disfigurement in Skin and Appearance

    The purpose of this study is to develop new standards for the disability evaluation with reference to existing laws and other study reports regarding disabilities for the rational evaluation of the diverse kinds of disfigurement in appearance and skin. Three plastic surgery specialists and 3 dermatology specialists developed a new standard for the disability evaluation which is appropriate for circumstances in Korea. Disability rate does not take into account the social occupation, gender or age of the patient, but instead, evaluate the Activity of Daily Living and the social adaptability of the appearance and skin disfigurement regardless of the balance between different disabilities. We tried to include most cutaneous disorders and categorized them into 3 types; congenital (Type 1), acquired (Type 2) as well as any permanent skin impairment sequelae of disease, trauma or treatment process (Type 3). For type 3 disorders, we tried to rate the score according to the size of involved skin lesion. The disability rate is determined by dividing the disability class into 8 steps based on the seriousness of each type of disability

    Factors associated with cervical cancer screening behaviors among young married female (aged 20–29) immigrants in South Korea

    Objective To identify factors that affect the participation of female immigrants in their 20s in the national cervical cancer screening programs. Methods Data were obtained from the National Health Insurance Services from 2016 to 2017. A total of 17,730 women who agreed to undergo cervical cancer screening during 2016–2017 were included in the study. Results Of the 17,730 women, 8,149 (46%) participated in cervical cancer screening, whereas, 9,581 (54%) did not. Logistic regression analysis of factors related to cervical cancer screening showed that the odds ratio (OR) of screening was higher in short duration of stay (OR, 1.18; 95% confidence interval [CI], 1.03–1.35), Chinese nationality (OR, 1.43; 95% CI, 1.28–1.59), unemployment (OR, 1; 95% CI, reference), participation in general health screening (OR, 4.16; 95% CI, 3.24–5.33), and comorbidities (OR, 1.16; 95% CI, 1.09–1.24) when compared to the other populations. The highest OR was associated with participation in general health screening. Conclusion Appropriate programs should be developed to increase participation of socially vulnerable groups in cervical cancer screening. Such programs will improve awareness regarding cervical cancer screening and reduce disparities in healthcare

    Psychometric Properties of the Hypomania Checklist-32 in Korean Patients with Mood Disorders

    OBJECTIVE The aim of this study was to examine the validity of the Korean version of the Hypomania Checklist-32, second revision (HCL-32-R2) in mood disorder patients. METHODS A total of 454 patients who diagnosed as mood disorder according to Structured Clinical Interview for DSM-IV Axis I Disorders, clinician version (SCID-CV) (bipolar disorder [BD] I, n=190; BD-II, n=72; and major depressive disorder [MDD], n=192) completed the Korean module of the HCL-32-R2 (KHCL-32-R2). RESULTS The KHCL-32-R2 showed a three-factorial structure (eigenvalue >2) that accounted for 43.26% of the total variance. Factor 1 was labeled "active/elated" and included 16 items; factor 2, "irritable/distractible" and included 9 items; and factor 3 was labeled "risk-taking/indulging" and included 9 items. A score of 16 or more on the KHCL-32-R2 total scale score distinguished between BD and MDD, which yielded a sensitivity of 70% and a specificity of 70%. MDD and BD-II also could be differentiated at a cut-off of 15 with maximized sensitivity (0.67) and specificity (0.66). Cronbach's alpha of KHCL-32-R2 and its subsets (factors 1, 2, and 3) were 0.91, 0.89, 0.81 and 0.79, respectively. Correlations between KHCL-32-R2 and Montgomery- Asberg Depression Rating Scale, Young Mania Rating Scale and Korean version of Mood Disorder Questionnaire were -0.66 (p=0.41), -0.14 (p=0.9), and 0.61 (p<0.001), respectively. CONCLUSION The KHCL-32-R2 may be a useful tool in distinguishing between bipolar and depressive patients in clinical settings

    The Origin of Systematic Forecast Errors of Extreme 2020 East Asian Summer Monsoon Rainfall in GloSea5

    This study examined the origin of the systematic underestimation of rainfall anomalies over East Asia during July-August 2020 in operational forecasts. Through partial nudging experiments, we found that the East Asian rainfall anomalies were successfully predicted in GloSea5 with corrected tropical sea surface temperature (SST) forcing. Once the observed SST is applied over the Indian Ocean and tropical central-eastern Pacific, a low-level anticyclonic anomaly over the subtropical western Pacific, which transports warm-moist air from the tropics to increase the East Asian precipitation, is well reproduced as observed. By further separating the SST into climatological and anomalous components, we revealed that the cold and dry mean state bias over the Indian Ocean and central-eastern Pacific is responsible for the weak anomalous atmospheric teleconnection patterns from the tropics to East Asia. This implies that correcting the model mean climatological fields can directly impact the operational seasonal forecast skill

    The cascade of care for latent tuberculosis infection in congregate settings:a national cohort 1 analysis, Korea, 2017-18

    BACKGROUND: In 2017, Korea implemented a nationwide project to screen and treat latent tuberculosis infection (LTBI) in high-risk for transmission public congregate settings. We aimed to assess programme success using a cascade of care framework. MATERIALS AND METHODS: We undertook a cohort study of people from three congregate settings screened between March 2017 and December 2018: (1) first-grade high school students, (2) employees of educational institutions, (3) employees of social welfare facilities. We report percentages of participants with LTBI completing each step in the cascade of care model. Poisson regression models were used to determine factors associated with not visiting clinics, not initiating treatment, and not completing treatment. RESULTS: Among the 96,439 participants who had a positive interferon-gamma release assay result, the percentage visiting clinics for further assessment, to initiate treatment, and who then completed treatment were 50.7, 34.7, and 28.9%, respectively. Compared to those aged 20-34 years, individuals aged < 20 years and aged ≥ 65 years were less likely to visit clinics, though more likely to complete treatment once initiated. Using public health centres rather than private hospitals was associated with people "not initiating treatment" (adjusted risk ratio [aRR], 3.72; 95% confidence interval [CI], 3.95-3.86). Nine-month isoniazid monotherapy therapy was associated with "not completing treatment," compared to 3-month isoniazid and rifampin therapy (aRR, 1.28; 95% CI, 1.16-1.41). CONCLUSION: Among participants with LTBI from three congregate settings, less than one third completed treatment. Age, treatment centre, and initial regimen were important determinants of losses to care through the cascade

    Risk factors for prolonged hospitalization and delayed treatment completion after laparoscopic appendectomy in patients with uncomplicated acute appendicitis

    Purpose We sought to identify the risk factors for prolonged hospitalization and delayed treatment completion after laparoscopic appendectomy in patients with uncomplicated acute appendicitis. Methods The study retrospectively analyzed 497 patients who underwent laparoscopic appendectomies for uncomplicated appendicitis between January 2018 and December 2020. The patients were divided into an early discharge group (≤2 days) and a late discharge group (>2 days) based on the length of hospital stay (LOS). The patients were also divided into uneventful and complicated groups according to the need for additional treatment after standard follow-up. Results Thirty-seven patients (7.4%) were included in the late discharge group. The mean LOS of the late discharge groups was 3.9 days. There were significant differences according to age, preoperative C-reactive protein (CRP), and operative time between the 2 groups. Only operative time was significantly associated with prolonged LOS in multivariate analysis. Thirty-five patients (7.0%) were included in the complicated group. The mean duration of treatment in the uneventful and complicated groups was 7.4 and 25.3 days, respectively. Significant differences existed between the uneventful and complicated groups in preoperative body temperature, preoperative CRP levels, maximal appendix diameter, and the presence of appendicoliths. In multivariate analysis, preoperative CRP levels and maximal appendix diameter were independent predictors of delayed treatment completion. Conclusion Shorter operative time is desirable to ensure minimal hospital stay in patients with uncomplicated appendicitis. Further efforts are needed to ensure that patients with uncomplicated appendicitis do not experience delayed treatment completion after laparoscopic appendectomies
