    Evaluating the effects of nano fluids based MQL milling of IN718 associated to sustainable productions

    The aeronautical industry is constantly striving for goals related to lesser production/maintenance time and cost. In this regard, aero-engines made up of Nickel-based alloy are preferred for high performance to improve the burning efficiency. However, the processing of the Nickel-based alloys remain challenging in manufacturing industry with the aim of sustainable production. This research investigated the manufacturing progress of face milling of Inconel 718 by using two different lubrication conditions; minimum quantity lubrication (MQL) and nanofluids based minimum quantity lubrication (NF-MQL). A high degree of sustainability was achieved through increasing productivity (material removal rate) and quality (surface roughness) enhancement while minimizing the power and temperature. The impacts of four most influencing parameters including feed rate, speed, flow rate and depth of cut were investigated on sustainable production performance measures. Empirical models of surface roughness, temperature, material removal rate and power were developed using response surface methodology. Analysis of the developed empirical models and validation were executed through analysis of variance and confirmatory experiments results. Finally, a multi objective optimization was implemented to attain maximum sustainability effect by generating a compromise between lowest surface roughness and cost, and highest material removal rate. The results revealed that the depth of cut is the most significant process parameter for both lubrication environments. The results show the NF-MQL as the better alternative which resulted in 20.1%, 14.7% and 13.3% percentage reduction for surface roughness, temperature and power, respectively. Furthermore, the results revealed that NF-MQL resulted in better desirability achievement (71.3%) as compared to MQL (70.1%)

    Study protocol of DIVERGE, the first genetic epidemiological study of major depressive disorder in Pakistan

    INTRODUCTION: Globally, 80% of the burdenof major depressive disorder (MDD) pertains to low- and middle-income countries. Research into genetic and environmental risk factors has the potential to uncover disease mechanisms that may contribute to better diagnosis and treatment of mental illness, yet has so far been largely limited to participants with European ancestry from high-income countries. The DIVERGE study was established to help overcome this gap and investigate genetic and environmental risk factors for MDD in Pakistan. METHODS: DIVERGE aims to enrol 9000 cases and 4000 controls in hospitals across the country. Here, we provide the rationale for DIVERGE, describe the study protocol and characterise the sample using data from the first 500cases. Exploratory data analysis is performed to describe demographics, socioeconomic status, environmental risk factors, family history of mental illness and psychopathology. RESULTS AND DISCUSSION: Many participants had severe depression with 74% of patients who experienced multiple depressive episodes. It was a common practice to seek help for mental health struggles from faith healers and religious leaders. Socioeconomic variables reflected the local context with a large proportion of women not having access to any education and the majority of participants reporting no savings. CONCLUSION: DIVERGE is a carefully designed case-control study of MDD in Pakistan that captures diverse risk factors. As the largest genetic study in Pakistan, DIVERGE helps address the severe underrepresentation of people from South Asian countries in genetic as well as psychiatric research

    High-Sensitivity Troponin I and the Risk of Flow Limiting Coronary Artery Disease in Non-St Elevation Acute Coronary Syndrome (Nste-Acs)

    Background: In acute coronary syndrome, elevated troponins are associated with worse clinical outcomes. We examined the relationship between the level of troponin elevation and the presence of a flow-limiting lesion for patients with no history of coronary disease admitted with NSTE-ACS. Methods: From January of 2010 until April of 2013, 561 patients received coronary angiography for new-onset NSTE-ACS. The Mann-Whitney Test, chi-square test, and Spearman correlation were used to examine relationships. Inferences were made at the 0.05 level of significance. The independent samples t test and the chi square test were used to identify predictors of LV systolic dysfunction- LVSD. Results: The 430 patients with a flow-limiting coronary lesions had a higher troponin I level than the 131 patients without obstructive coronary disease (5.69 ng/ml vs. 2.85 ng/ml, p=0.002). Further, within troponin categories, those in the greater than 5.0 ng/ml group were more likely to have angiographically significant CAD than those in the less than 0.5 ng/ml group (p=0.012). Elevated troponins were also associated with increased thrombus burden, worse systolic function, higher complexity of the lesions, and worse post intervention TIMI flow. Cardiac troponin \u3e5ng/ml [odds ratio=2.13 (95%CI=1.22 to 3.70) p=0.008] and DM [odds ratio=1.74 (95%CI=1.02 to 2.97) p=0.042] were independent predictors of LVSD. Advanced LM disease and age were marginally significant. Conclusion: The degree of cardiac troponin I elevation should be incorporated into the risk stratification models of NSTE-ACS to promptly triage high-risk patients to early invasive strategies and tailored anticoagulant therapy to reduce troponin elevation and improve myocardial perfusion

    Psychosocial responses to disaster: An Asian perspective

    The psychological and psychiatric impact of great natural disasters are beginning to be understood leading to new methods of prevention, intervention and mitigation. There is limited data from the Asian continent, however, which has been the location of some of the greatest disasters of recent times. In this paper, we outline the psychosocial intervention efforts from nine Asian nations when confronted with large-scale natural catastrophic events. These include reports from situations where local services have some capacity to respond as well as those where services are destroyed or overwhelmed. From this it is possible to draw some general principles of psychosocial disaster intervention: (1) Assessment of disaster, extant service systems and incoming resources. (2) Assessment of help-seeking pathways and cultural models of illness. (3) Facilitation and support for family reunion, identification of the dead and cultural and religious practices to address death and grief. (4) Foster and bolster community group activities where possible. (5) Psychosocial training of community, aid and health workers using a train the trainer model to promote case identification, psychoeducation and intervention, with specific emphasis on vulnerable groups, especially children. (6) Promote general community psychoeducation. (7) Train medical and health staff in basic psychiatric and psychological assessment and intervention for post-traumatic stress, mood and anxiety disorders. (8) Minimise risk factors for psychiatric morbidity such as displacement and loss of gainful activity. (9) Reshape mental health systems recognising the long-term psychiatric sequelae of disaster. The collective learnt experience from Asian natural disasters may be constructively used to plan strategies to respond appropriately to the psychosocial consequences of disaster both within Asia and in the rest of the world. © 2008.Suresh Sundram...Daya Somasundaram...et al
