    Background: Measurements of maximum urethral closure pressure (MUCP) are a part of urodynamic investigations preceding an incontinence surgery and a part of urethral function tests. Objectives: The aim of the study was to compare maximum urethral closure pressure determined by a microtip catheter with those measured by an air-charged catheter. Material and methods: A prospective randomized study in a tertiary referral centre. 122 female patients with urodynamic stress incontinence were randomly assigned to have their urethral pressure profi les measured at rest by both microtip and air-charged catheters. Intervention and Measurements: Each patient had three measurements taken by each catheter type. Means of the measurements were compared with regard to correlation and repeatability. For statistical analysis, an approach proposed by Bland-Altman was applied to assess the agreement between the two techniques. Results: Correlation coeffi cient between MUCP by the air-charged and the microtip catheter was r=0.8507(95% CI 0.7928 – 0.8934; pCel: Pomiar maksymalnego ciśnienia zamknięcia cewki moczowej (MUCP) jest częścią badania urodynamicznego poprzedzającego operacyjne leczenie nietrzymania moczu i częścią testow czynnościowych cewki moczowej. Do badania losowo włączono 122 kobiety z nietrzymaniem moczu, u których wykonywano profi l ciśnień w cewce moczowej w spoczynku przy pomocy zarówno cewnika microtip jak i cewnika wypełnionego powietrzem. Każda pacjentka miała wykonane trzy pomiary przy pomocy obu cewników. Średnie z pomiarów porównano w odniesieniu do korelacji i powtarzalności. Do celów statystycznych użyto metody Bland-Altmana oceniającej zgodność obu technik pomiarów. Wyniki: Współczynnik korelacji pomiędzy MUCP mierzonym cewnikiem powietrznym a cewnikiem microtip wynosił r=0,8507(95% CI 0,7928–0,8934;

    Criticality of natural absorbing states

    We study a recently introduced ladder model which undergoes a transition between an active and an infinitely degenerate absorbing phase. In some cases the critical behaviour of the model is the same as that of the branching annihilating random walk with N2N\geq 2 species both with and without hard-core interaction. We show that certain static characteristics of the so-called natural absorbing states develop power law singularities which signal the approach of the critical point. These results are also explained using random walk arguments. In addition to that we show that when dynamics of our model is considered as a minimum finding procedure, it has the best efficiency very close to the critical point.Comment: 6 page

    Poisson distribution of male mating success in laboratory populations of Drosophila melanogaster

    Variation among males and females in reproductive success is a major determinant of effective population size. Most studies of male mating success in Drosophila, however, have been done under conditions very different from those in typical cultures. We determined the distribution of male mating success in five laboratory populations of D. melanogaster maintained on a 14 d, discrete generation cycle fairly representative of standard Drosophila cultures. Mating success was measured as the number of matings a male could achieve under conditions closely approximating a regular culture vial of these populations. Preliminary studies determined that most mating in these populations occurred within 14 h of the flies attaining sexual maturity. Consequently, individual virgin males were marked with white paint on their thorax, put into vials with varying numbers of unmarked virgin flies of both sexes, and monitored continuously for matings over a period of up to 14 h. At various times during the assay, virgin males and females were added to these vials in proportions simulating the pattern of eclosion in culture vials. The observed variation in the number of matings per male in the five populations was, by and large, consistent with a Poisson distribution, suggesting that male mating success in short-generation-time, discrete-generation laboratory cultures of D. melanogaster may fulfil a fundamental assumption of the Wright-Fisher model of genetic drift in finite populations

    Parallel targeted and non-targeted quantitative analysis of steroids in human serum and peritoneal fluid by liquid chromatography high-resolution mass spectrometry

    We developed and validated a liquid chromatography high-resolution mass spectrometry method for the absolute quantification of 51 steroids for clinical analysis of human serum and, for the first time, peritoneal fluid. Data acquisition was performed in both targeted and untargeted mode simultaneously, thus allowing the accurate and precise quantification of the main components of the classical steroid pathways (17 steroids) as well as the analysis of 34 additional non-classical steroids. For targeted analysis, validation was performed according to FDA guidelines, resulting, among other parameters, in accuracy < 13% RSD and precision < 10% relative error, for both inter- and intra-day validation runs. By establishing steroid-specific response factors, the calibration curves of the targeted analytes can be extended to untargeted analytes. This approach opens novel possibilities for the post hoc analysis of clinical samples as the data can be examined for virtually any steroid even after data acquisition, enabling facile absolute quantification once a standard becomes available. We demonstrate the applicability of the approach to evaluate the differences in steroid content between peripheral serum and peritoneal fluid across the menstrual cycle phases, as well as the effect of the synthetic gestagen dienogest on the steroid metabolome.     Cognitive changes in older women after urogynaecological surgery

    Objectives: The need for pelvic floor surgery will increase with an aging population in the future. Aim of this prospectivestudy was to evaluate the evolution of cognitive function in elderly women after urogynaecological surgery.Material and methods: Between 2010 and 2014, 51 female patients 70 years and older who underwent urogynaecologicalsurgery participated in this study. Geriatric and urogynaecological assessment were performed before and six weeks after surgery, including the Mini-COG test, the clock-drawing test, a depression scale, an activities of daily living questionnaire, and the German pelvic floor questionnaire.Results: Mean age was 77 years (range 70–91). Overall, 15 women were operated for incontinence, 31 for prolapse, andfive for miscellaneous reasons. Only two (3.9%) of the 51 women developed postoperative delirium. Abnormal cognitivefindings increased from preoperatively 15.7% to 39.2% six weeks after surgery (odds ratio 3.4, 95% confidence interval1.3 to 8.7, p &lt; 0.001). There were no statistically significant pre-post differences in activities of daily living and depression scores. Pelvic floor function indices improved significantlyConclusion: This study shows an overall decline of cognitive function in a vulnerable group of elderly women during theshort-term postoperative period. Postoperative cognitive dysfunction (POCD) after surgery has been described for othertypes of non-cardiac surgery but has been understudied in urogynaecological surgery. These patients need interdisciplinary management to prevent or minimize adverse effects of surgery on cognitive function

    Laparoscopic sacrocolpopexy mesh excision step-by-step.

    INTRODUCTION AND HYPOTHESIS The objective was to demonstrate the surgical procedure of laparoscopic mesh removal after sacrocolpopexy to aid clinicians facing mesh complications. METHODS Video footage shows the laparoscopic management of mesh failure and mesh erosion after sacrocolpopexy with narrated video sequences of two patients. RESULTS Laparoscopic sacrocolpopexy represents the gold standard in advanced prolapse repair. Mesh complications occur infrequently but infections, failure of prolapse repair and mesh erosions necessitate mesh removal and repeat sacrocolpopexy if applicable. The video deals with two women referred to our tertiary referral urogynecology unit in the University Women's Hospital of Bern, Switzerland, after laparoscopic sacrocolpopexies that were carried out in remote hospitals. Both patients were asymptomatic more than 1 year after surgery. CONCLUSIONS Complete mesh removal after sacrocolpopexy and repeat prolapse surgery can be challenging but is feasible and is aimed at improving patients' complaints and symptoms

    The Future of Citizen Science

    There is an emerging trend of democratizing science and schooling within science education that can be characterized as citizen science. We explore the roots of this movement and some current projects to underscore the meaning of citizen science in science and schooling. We show that citizen science, as it is currently conceptualized, does not go far enough to resolve the concerns of communities and environments when considered holistically and when compared with more dynamic and multidimensional ideas for characterizing science. We use the examples of colony collapse disorder (CCD) and emerging trends of nanotechnology as cases in point. Then we justify three dialogical spheres of influence for future citizen science. As citizen science becomes more holistic, it embodies the responsibility of youths who are prepared to engage real concerns in their community
