State-of-the-art approach towards magnetic resonance imaging of the nervous system structures in patients with cardiac implantable electronic devices
Introduction: MRI generated forces are the source of potential complications in patients with cardiac implantable electronic devices (CIED). The technological progress, and growing clinical evidence concerning the operation of the contemporary MR non-conditional CIEDs during MRI, have started to significantly change our every-day clinical practice. Nevertheless, a lot of patients who could have an MRI performed safely, still have been refused the examination.
State-of-the-art: In many clinical situations, an MRI examination in a patient with a CIED is reasonable, and is linked to a negligible risk of complications if performed under strict precautions. The MagnaSave Registry that evaluated the influence of nonthoracic MRI on the function of MR non-conditional CIEDs, and numerous studies involving thoracic and nonthoracicMRIs in patients with legacy CIEDs, have confirmed the feasibility and safety of such examinations. In this article, practical tips aimed towards improving the safety of MRI in MR conditional and non-conditional CIED patients are largely based on the very recently released (2017) HRS expert consensus statement.
Clinical implications: Clinical data emphasize the necessity of making the MRI more accessible to CIED patients, also in the case of MR non-conditional systems or when the thorax MR imaging is clinically reasonable. This goal should be achieved by increasing the number of centers complying with respective recommendations and applying protocols that would guarantee the highest safety level.
Future directions: Further studies are warranted to assess safety issues related to the main current contraindication to MRI, i.e., the presence of abandoned leads
Histone dependent signalization during pharmacotherapy of chronic obstructive pulmonary disease
Przewlekła obturacyjna choroba płuc (POChP) jest czwartą co do częstości przyczyną zgonów na świecie. Molekularne
mechanizmy odpowiadające za rozwój choroby nie zostały dotychczas poznane, ale wiadomo, że u podłoża POChP leży
przewlekły proces zapalny i stres oksydacyjny/nitrozacyjny spowodowany paleniem tytoniu. Wyniki ostatnich badań wskazują,
że zaburzenia funkcji histonów, które prowadzą, poprzez wzrost transkrypcji genów, do syntezy cytokin prozapalnych,
mogą odgrywać zasadniczą rolę w patomechanizmie choroby. Reakcje acetylacji i deacetylacji histonów wpływają na ich
aktywność transkrypcyjną, a zmiany intensywności tych reakcji, wywołane przez leki, mogą stanowić interesujące pole do
dalszych badań. W artykule autorzy prezentują aktualne opinie na temat roli steroidów jako inhibitorów stanu zapalnego, jak
również zjawiska steroidooporności. Wyniki badań, w których oceniano aktywność przeciwzapalną i zdolność do hamowania
stresu oksydacyjnego takich leków, jak teofilina i N-acetylocysteina, mogą prowadzić do zastosowania tych leków
w leczeniu przyczynowym POChP, jednakże obserwacje te wymagają potwierdzenia w przebiegu dalszych badań.Chronic obstructive pulmonary disease (COPD) is a 4th major cause of morbidity and mortality worldwide. Cigarette smoking
and oxidative/nitrosative stress leading to chronic inflammation is considered as a major cause of COPD but up to now,
details of molecular pathways responsible for development of disease are unknown. Recent reports indicate the role of
disruption in histone function in promoting synthesis of inflammatory cytokines through increased gene transcription which
underlies disease development. Core histone acetylation/deacetylation regulate their transcription activity and drug induced
changes of its intensity may be an interesting field of further research. In this article the opinions about the role of steroids as
inhibitors of the inflammatory process as well as resistance to steroids have been presented. Findings from studies which
aimed to explore the anti-inflammatory activity of drugs such as theophylline and N-acetylcysteine and their ability to
suppress oxidative stress may suggest the usefullness of these drugs in causative treatment of COPD. However, further
studies are necessary to confirm these findings
Posttranslational p53 phosphorylation in non small cell lung cancer cells after radio/chemotherapy
Wstęp: Jednym z najważniejszych regulatorów cyklu komórkowego i apoptozy jest białko p53. Wiele czynników zmienia
ekspresję tego białka i modyfikuje jego funkcje. Celem badań autorów było określenie stabilności i wybranych potranslacyjnych
modyfikacji białka p53 w przebiegu terapii raka płuca.
Materiał i metody: Oceniano zmiany ilości markera uszkodzenia DNA - poli-ADP-rybozy, ploidię DNA, ekspresję antygenu
proliferacyjnego Ki-67, ekspresję natywnego i zmutowanego p53 oraz intensywność reakcji fosforylacji wybranych reszt
serynowych tego białka, C-końcowej Ser392 i N-końcowych Ser15 i Ser20 w próbkach pobranych w trakcie bronchoskopii
chorych na nawrotowego niedrobnokomórkowego raka płuca przed radio- i chemioterapią i po niej. Badania objęły
23 pacjentów będących po zabiegu chirurgicznym w stadiach I-IIIA.Wyniki: Terapia obniżyła liczbę komórek będących w fazie G2/M, ale zwiększyła frakcję komórek w fazie S o około 50%.
Zwiększyła także ekspresję białka p53 oraz ilość białka fosforylowanego w pozycjach Ser392 i Ser20, a zmiany te korelowały
ze zmianami ilości poli-ADP-rybozy i ekspresją Ki-67.
Wnioski: Uzyskane przez autorów wyniki wskazują, że oprócz zmian ekspresji białka p53 jego potranslacyjna fosforylacja
uczestniczy w regulacji proliferacji komórek nowotworowych pod wpływem leków.Introduction: p53 protein is a critical regulator of cell cycle and apoptosis. Many stimuli change its expression and modify
its functions. The aim of our work was to determine stability and chosen posttranslational modification of p53 protein during
the treatment of lung cancer.
Material and methods: We investigated levels of poly-ADP-ribose- a marker of cellular DNA damage, DNA ploidy, Ki-67
expression, wild type and mutated p53 protein expression and intensity of phosphorylation of chosen p53 serine sites:
C-terminal Ser392, and N-terminal Ser15, and Ser20 in fiberoptic bronchoscopy biopsy samples taken from patients suffering
from recurrent squamous cell lung cancer before and after radio/chemotherapy. Analysis was based on results obtained
from 23 patients after surgery in I-IIIA clinical stage of the disease.
Results: Therapy lowered the number of G2/M cells, but increased S fraction cell population in about 50%. Therapy
increased p53 expression and p53 phosphorylation at Ser392 and Ser20, and these changes correlated with poly-ADP-ribose
levels and Ki-67 expression.
Conclusions: Our results indicate that apart from changes in p53 quantity, p53 posttranslational phosphorylation play a role
in regulation of neoplastic cells proliferation in response to drugs
Temperature distribution around thin electroconductive layers created on composite textile substrates
In this paper, the authors describe the distribution of temperatures around electroconductive pathways created by a physical vacuum deposition process on flexible textile substrates used in elastic electronics and textronics. Cordura material was chosen as the substrate. Silver with 99.99% purity was used as the deposited metal. This research was based on thermographic photographs of the produced samples. Analysis of the temperature field around the electroconductive layer was carried out using Image ThermaBase EU software. The analysis of the temperature distribution highlights the software’s usefulness in determining the homogeneity of the created metal layer. Higher local temperatures and non-uniform distributions at the same time can negatively influence the work of the textronic system
Application of thermography in the diagnosis of the algodystrophic syndrome type I in 42-year-old patient
Algodystrophic syndrome entity (type I) has not fully known pathogenesis and course. It is characterized by severe pain of the distal part of limb, oedema, vasomotor dysfunction and impaired efficiency. These symptoms occur after trauma, thoracic surgeries, myocardial infarction, stroke, peripheral nerve damage, less common in the course of venous or arterial thrombosis. Pathomechanism of the disease is unclear, but the most important factor of its pathogenesis seems to be a disorder of the autonomic nervous system. Limb pain and swelling, in the typical cases, are accompanied by vasomotor disturbances, restricted mobility, increased sensitivity to pressure and temperature changes. The course of the disease can be divided into three periods: I – acute period, II – dystrophic period, III – atrophic period. Apart from the vasomotor form, we can distinguish paralytic form (after a stroke) and toxic (druginduced). The majority of patients has emotional lability, hyperreactivity and a tendency to anxiety and depression. Imaging studies are useful in the diagnosis. The effectiveness of treatment depends on the period in which the diagnosis is made. The sooner patients are treated, the outcomes are better. Analgesics and sympatholytics areused in this therapy. Good effects of decongestants, anti-inflammatory medications and stimulating bone calcification treatment are obtained after the application of an alternating magnetic field of low frequency, low-energy laser, whirlpool limbs massage, affected limb exercises. We present a case of 42-year-old man with algodystrophic syndrome, in which the diagnosis was made in the advanced second stage of the disease. Pharmacological treatment and physiotherapy had been used for 3 months and led to a significant improvement, which is well illustrated by the thermographic examination.Wielomiejscowy zespół odruchowy typu I jest jednostką chorobową o nie w pełni poznanym patomechanizmie i przebiegu. Charakteryzuje się silnym bólem dystalnej części kończyny, obrzękiem, dysfunkcją naczynioruchową i upośledzoną sprawnością. Objawy te występują po urazach, operacjach na klatce piersiowej, po zawale serca, po udarze, uszkodzeniu nerwów obwodowych, rzadziej w przebiegu zakrzepicy żylnej lub tętniczej. Mechanizm powstawania choroby jest niejasny. Na pierwszy plan wysuwają się zaburzenia funkcji autonomicznego układu nerwowego. Bólowi i obrzękowi kończyny w przypadkach o typowym przebiegu towarzyszą: zaburzenia naczynioruchowe, ograniczona bólem ruchomość oraz wzmożona wrażliwość na ucisk i zmiany temperatury otoczenia. W przebiegu zespołu wyróżnia się trzy okresy: I – ostry, II – dystroficzny, III – atroficzny. Poza postacią odruchową zespołu występuje postać porażenna (po udarze) i toksyczna (polekowa). U większości chorych zwracają uwagę labilność emocjonalna, hiperreaktywność oraz tendencja do stanów lękowych i depresji. Przydatne w rozpoznaniu są badania obrazowe. Skuteczność leczenia zależy od okresu, w którym ustalono rozpoznanie i podjęto terapię. W leczeniu farmakologicznym stosuje się leki przeciwbólowe oraz hamujące układ współczulny. Dobre efekty przeciwobrzękowe i przeciwzapalne, a także stymulację uwapnienia kości uzyskuje się po zastosowaniu: zmiennego pola magnetycznego niskiej częstotliwości, lasera niskoenergetycznego, masażu wirowego kończyn, ćwiczeń indywidualnych zajętej kończyny. W pracy przedstawiono przypadek 42-letniego mężczyzny z wielomiejscowym zespołem odruchowym typu I, u którego diagnozę postawiono dopiero w zaawansowanym II okresie choroby. Zastosowane leczenie farmakologiczne i fizykoterapeutyczne po 3 miesiącach doprowadziło do znaczącej poprawy, którą dobrze obrazuje badanie termograficzne
