Cerebrospinal fluid CXCL13 in clinically isolated syndrome patients: Association with oligoclonal IgM bands and prediction of Multiple Sclerosis diagnosis
Cerebrospinal fluid (CSF) CXCL13 was shown to correlate with markers of intrathecal inflammation and CSF oligoclonal IgM bands (IgMOB) have been associated with a more severe Multiple Sclerosis (MS) course.We correlated CSF CXCL13 levels with clinical, MRI and CSF parameters, including CSF IgMOB, in 110 Clinically Isolated Syndrome (CIS) patients.CSF CXCL13 levels correlated with CSF cell count, total protein, IgG Index and with the presence of CSF IgGOB and IgMOB.CSF CXCL13 levels â„. 15.4. pg/ml showed a good positive predictive value and specificity for a MS diagnosis and for a clinical relapse within one year from onset

Dietary nitrate prevents progression of carotid subclinical atherosclerosis through blood pressure-independent mechanisms in patients with or at risk of type 2 diabetes mellitus
Aims
To test if 6 months' intervention with dietary nitrate and spironolactone could affect carotid subclinical atherosclerosis and stiffness, respectively, vs. placebo/doxazosin, to control for blood pressure (BP).
Methods
A subgroup of participants in our double-blind, randomized-controlled, factorial VaSera trial had carotid imaging. Patients with hypertension and with/at risk of type 2 diabetes were randomized to active nitrate-containing beetroot juice or placebo nitrate-depleted juice, and spironolactone or doxazosin. Vascular ultrasound for carotid diameter (CD, mm) and intimaâmedia thickness (CIMT, mm) was performed at baseline, 3- and 6-months. Carotid local stiffness (CS, m/s) was estimated from aortic pulse pressure (Arteriograph) and carotid lumen area. Data were analysed by modified intention to treat and using mixed-model effect, adjusted for confounders.
Results
In total, 93 subjects had a baseline evaluation and 86% had follow-up data. No statistical interactions occurred between the juice and drug arms and BP was similar between the juices and between the drugs. Nitrate-containing vs. placebo juice significantly lowered CIMT (â0.06 [95% confidence interval â0.12, â0.01], P =â.034), an overall difference of ~8% relative to baseline; but had no effect on CD or CS. Doxazosin appeared to reduce CS from baseline (â0.34 [â0.62, â0.06]) however, no difference was detected vs. spironolactone (â0.15 [â0.46, 0.16]). No differences were detected between spironolactone or doxazosin on CIMT and CD.
Conclusions
Our results show that 6 months' intervention with dietary nitrate influences vascular remodelling, but not carotid stiffness or diameter. Neither spironolactone nor doxazosin had a BP-independent effect on carotid structure and function
Safety and efficacy of the ChAdOx1 nCoV-19 vaccine (AZD1222) against SARS-CoV-2: an interim analysis of four randomised controlled trials in Brazil, South Africa, and the UK.
BACKGROUND: A safe and efficacious vaccine against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), if deployed with high coverage, could contribute to the control of the COVID-19 pandemic. We evaluated the safety and efficacy of the ChAdOx1 nCoV-19 vaccine in a pooled interim analysis of four trials. METHODS: This analysis includes data from four ongoing blinded, randomised, controlled trials done across the UK, Brazil, and South Africa. Participants aged 18 years and older were randomly assigned (1:1) to ChAdOx1 nCoV-19 vaccine or control (meningococcal group A, C, W, and Y conjugate vaccine or saline). Participants in the ChAdOx1 nCoV-19 group received two doses containing 5âĂâ1010 viral particles (standard dose; SD/SD cohort); a subset in the UK trial received a half dose as their first dose (low dose) and a standard dose as their second dose (LD/SD cohort). The primary efficacy analysis included symptomatic COVID-19 in seronegative participants with a nucleic acid amplification test-positive swab more than 14 days after a second dose of vaccine. Participants were analysed according to treatment received, with data cutoff on Nov 4, 2020. Vaccine efficacy was calculated as 1â-ârelative risk derived from a robust Poisson regression model adjusted for age. Studies are registered at ISRCTN89951424 and ClinicalTrials.gov, NCT04324606, NCT04400838, and NCT04444674. FINDINGS: Between April 23 and Nov 4, 2020, 23â848 participants were enrolled and 11â636 participants (7548 in the UK, 4088 in Brazil) were included in the interim primary efficacy analysis. In participants who received two standard doses, vaccine efficacy was 62·1% (95% CI 41·0-75·7; 27 [0·6%] of 4440 in the ChAdOx1 nCoV-19 group vs71 [1·6%] of 4455 in the control group) and in participants who received a low dose followed by a standard dose, efficacy was 90·0% (67·4-97·0; three [0·2%] of 1367 vs 30 [2·2%] of 1374; pinteraction=0·010). Overall vaccine efficacy across both groups was 70·4% (95·8% CI 54·8-80·6; 30 [0·5%] of 5807 vs 101 [1·7%] of 5829). From 21 days after the first dose, there were ten cases hospitalised for COVID-19, all in the control arm; two were classified as severe COVID-19, including one death. There were 74â341 person-months of safety follow-up (median 3·4 months, IQR 1·3-4·8): 175 severe adverse events occurred in 168 participants, 84 events in the ChAdOx1 nCoV-19 group and 91 in the control group. Three events were classified as possibly related to a vaccine: one in the ChAdOx1 nCoV-19 group, one in the control group, and one in a participant who remains masked to group allocation. INTERPRETATION: ChAdOx1 nCoV-19 has an acceptable safety profile and has been found to be efficacious against symptomatic COVID-19 in this interim analysis of ongoing clinical trials. Necropoli dellâIsola Sacra. Le ricerche 1968-89Â : ripercorrendo unâesperienza
Lâarticolo ripercorre metodi, problematiche e risultati delle ricerche condotte nella necropoli dellâIsola Sacra (1968-1989) sotto la direzione di I. Baldassarre, con lo scopo di giungere a una lettura in chiave antropologica di una necropoli romana di etĂ imperiale a partire dalla documentazione archeologica. La necropoli si sviluppa ai due lati di una strada (seconda metĂ del I sec. d.C.) alla quale le tombe fanno riferimento con una costante ricerca di visibilitĂ . Lo studio ha permesso di ricostruire modi e tempi dellâoccupazione dello spazio funerario da parte delle singole tombe che si addossano le une alle altre creando lâimmagine di edificazioni per gruppi unitari. Si tratta di tombe familiari, la cui funzione risulta dallâapparato epigrafico, dai dispositivi per i rituali funerari, dalla scarsitĂ di materiali di corredo, che appaiono piuttosto in relazione a sepolture individuali, piĂč « povere », che affollano tutti gli spazi liberi tra le tombe a cella. La ricerca ha anche messo in luce il ruolo chiave degli apparati figurativi e lo strutturarsi di un repertorio di decorazione funeraria, tema di particolare interesse per il ceto libertino che seppellisce nella necropoli. Nel quadro del crescente interesse per lâideologia funeraria romana e riproponendo la storia di una ricerca nata molti anni fa in una UniversitĂ piena di stimoli, lâarticolo intende evidenziare quanto un contesto affrontato nella sua globalitĂ possa contribuire ad approfondire la discussione relativa a questi temi.The article reviews methods and results of the research conducted at the Isola Sacra necropolis (1968-1989) under the direction of I. Baldassarre, with the aim of producing an anthropological reading of a Roman necropolis based on the archaeological record. The necropolis extends on both sides of the road (second half of the 1st c. AD) and the clusters of tombs are the result of the dynamics of the necropolis system, with a constant striving for visibility by the buyers of family tombs, in some cases incorporating older burials. The function of family tombs is evident from the epigraphs, the arrangements for funeral rites and the rarity of grave goods, which are found rather in the « poorer » tombs, crowded into the spaces between the chamber tombs. The research also shed light on the structuring of a repertoire of funerary ornaments, a theme of great interest for the freedman class buried here. As part of the growing interest in issues related to Roman funerary ideology and by going back over on the history of a project devised many years ago in a highly stimulating university, the paper seeks to show that a context studied as a whole can contribute to discussion in depth of these issues
