    A heart teamโ€™s perspective on interventional mitral valve repair: Percutaneous clip implantation as an important adjunct to a surgical mitral valve program for treatment of high-risk patients

    ObjectiveSurgical mitral valve repair carries an elevated perioperative risk in the presence of severely reduced ventricular function and relevant comorbidities. We sought to assess the feasibility of catheter-based mitral valve repair using a clip-based percutaneous edge-to-edge repair system in selected patients at high surgical risk with mitral regurgitation grade 3 or worse.MethodsBetween 2002 and January 2011, 202 consecutive patients without prior mitral valve surgery (age 75 ยฑ 9 years; 63% were male) with symptomatic functional (65%), degenerative (27%), or mixed (8%) mitral regurgitation were treated with a percutaneous clip system for approximation of the anterior and posterior mitral leaflets. Risk for mitral valve surgery was considered high in terms of a mean logistic European System for Cardiac Operative Risk Evaluation of 44% (range, 21%โ€“54%). Preprocedural left ventricular ejection fraction was 35% or less in 36% of patients. An interdisciplinary heart team of cardiologists and cardiac surgeons discussed all patients.ResultsPercutaneous clip implantation was successful in 186 patients (92%). Patients were treated with 1 clip (nย =ย 125; 62%), 2 clips (nย =ย 64; 32%), or 3 or more clips (nย =ย 7; 3%). Reduction in mitral regurgitation from pre- to postprocedure was significant (Pย <ย .0001) and remained stable within the first 12 months in the majority of patients. Thirty-day mortality was 3.5% (7/202 patients). Hospital stay was 12 ยฑ 10 days, and median intensive care unit stay was 1 day (range, 0โ€“45 days). Eleven patients required surgical valve repair/replacement at a median of 38 days (0โ€“468 days) after percutaneous clip implantation.ConclusionsClip-based percutaneous mitral valve repair is a safe, low-risk, and effective therapeutic option in symptomatic patients with a high risk for surgery and does not exclude later surgical repair

    Increased B-cell activity with consumption of activated monocytes in severe COVID-19 patients

    The pathogenesis of autoimmune complications triggered by SARS-CoV2 has not been completely elucidated. Here, we performed an analysis of the cellular immune status, cell ratios, and monocyte populations of patients with COVID-19 treated in the intensive care unit (ICU) (cohort 1, N = 23) and normal care unit (NCU) (cohort 2, n = 10) compared with control groups: patients treated in ICU for noninfectious reasons (cohort 3, n = 30) and patients treated in NCU for infections other than COVID-19 (cohort 4, n = 21). Patients in cohort 1 presented significant differences in comparison with the other cohorts, including reduced frequencies of lymphocytes, reduced CD8+T-cell count, reduced percentage of activated and intermediate monocytes and an increased B/T8 cell ratio. Over time, patients in cohort 1 who died presented with lower counts of B, T, CD4+T, CD8+T-lymphocytes, NK cells, and activated monocytes. The B/T8 ratio was significantly lower in the group of survivors. In cohort 1, significantly higher levels of IgG1 and IgG3 were found, whereas cohort 3 presented higher levels of IgG3 compared to controls. Among many immune changes, an elevated B/T8-cell ratio and a reduced rate of activated monocytes were mainly observed in patients with severe COVID-19. Both parameters were associated with death in cohort 1

    Genome-Wide Association Analysis for Severity of Coronary Artery Disease Using the Gensini Scoring System

    Coronary artery disease (CAD) has a complex etiology involving numerous environmental and genetic factors of disease risk. To date, the genetic 9p21 locus represents the most robust genetic finding for prevalent and incident CAD. However, limited information is available on the genetic background of the severity and distribution of CAD. CAD manifests itself as stable CAD or acute coronary syndrome. The Gensini score quantifies the extent CAD but requires coronary angiography. Here, we aimed to identify novel genetic variants associated with Gensini score severity and distribution of CAD. A two-stage approach including a discovery and a replication stage was used to assess genetic variants. In the discovery phase, a meta-analysis of genome-wide association data of 4,930 CAD-subjects assessed by the Gensini score was performed. Selected single nucleotide polymorphisms (SNPs) were replicated in 2,283 CAD-subjects by de novo genotyping. We identified genetic loci located on chromosome 2 and 9 to be associated with Gensini score severity and distribution of CAD in the discovery stage. Although the loci on chromosome 2 could not be replicated in the second stage, the known CAD-locus on chromosome 9p21, represented by rs133349, was identified and, thus, was confirmed as risk locus for CAD severity

    The role of tenascin-C in tissue injury and tumorigenesis

    The extracellular matrix molecule tenascin-C is highly expressed during embryonic development, tissue repair and in pathological situations such as chronic inflammation and cancer. Tenascin-C interacts with several other extracellular matrix molecules and cell-surface receptors, thus affecting tissue architecture, tissue resilience and cell responses. Tenascin-C modulates cell migration, proliferation and cellular signaling through induction of pro-inflammatory cytokines and oncogenic signaling molecules amongst other mechanisms. Given the causal role of inflammation in cancer progression, common mechanisms might be controlled by tenascin-C during both events. Drugs targeting the expression or function of tenascin-C or the tenascin-C protein itself are currently being developed and some drugs have already reached advanced clinical trials. This generates hope that increased knowledge about tenascin-C will further improve management of diseases with high tenascin-C expression such as chronic inflammation, heart failure, artheriosclerosis and cancer

    Background!#!Outcome data about the use of tranexamic acid (TXA) in civilian patients in mature trauma systems are scarce. The aim of this study was to determine how severely injured patients are affected by the widespread prehospital use of TXA in Germany.!##!Methods!#!The international TraumaRegister DGUยฎ was retrospectively analyzed for severely injured patients with risk of bleeding (2015 until 2019) treated with at least one dose of TXA in the prehospital phase (TXA group). These were matched with patients who had not received prehospital TXA (control group), applying propensity score-based matching. Adult patients (โ‰ฅโ€‰16) admitted to a trauma center in Germany with an Injury Severity Score (ISS)โ€‰โ‰ฅโ€‰9 points were included.!##!Results!#!The matching yielded two comparable cohorts (nโ€‰=โ€‰2275 in each group), and the mean ISS was 32.4โ€‰ยฑโ€‰14.7 in TXA group vs. 32.0โ€‰ยฑโ€‰14.5 in control group (pโ€‰=โ€‰0.378). Around a third in both groups received one dose of TXA after hospital admission. TXA patients were significantly more transfused (pโ€‰=โ€‰0.022), but needed significantly less packed red blood cells (pโ€‰โ‰คโ€‰0.001) and fresh frozen plasma (pโ€‰=โ€‰0.023), when transfused. Massive transfusion rate was significantly lower in the TXA group (5.5% versus 7.2%, pโ€‰=โ€‰0.015). Mortality was similar except for early mortality after 6ย h (pโ€‰=โ€‰0.004) and 12ย h (pโ€‰=โ€‰0.045). Among non-survivors hemorrhage as leading cause of death was less in the TXA group (3.0% vs. 4.3%, pโ€‰=โ€‰0.021). Thromboembolic events were not significantly different between both groups (TXA 6.1%, control 4.9%, pโ€‰=โ€‰0.080).!##!Conclusion!#!This is the largest civilian study in which the effect of prehospital TXA use in a mature trauma system has been examined. TXA use in severely injured patients was associated with a significantly lower risk of massive transfusion and lower mortality in the early in-hospital treatment period. Due to repetitive administration, a dose-dependent effect of TXA must be discussed
